    FATIGUE ASSESSMENT OF CRACK GROWTH BASED ON FAILURE ASSESSMENT DIAGRAMS FOR A SEMI-SUBMERSIBLE PLATFORM

    This paper deals with the assessment of fatigue crack propagation on the connection between column and brace for a semi-submersible. The analysis of global and local structural responses under different sea states are performed to acquire the transfer functions of stresses. Based on an existing crack the Failure Assessment Diagrams (FAD) are applied as criterion of acceptance for the safety of crack and structure during the crack growth calculation cycle. The crack growth rate considering threshold stress intensity factor and stress ratio is used. During safety assessment the stress response from ultimate sea state is outlined. A comparison of fatigue crack growth using ultimate stress and normal stress data with different crack growth rate is presented. The results show the reliability of fatigue assessment using FAD as a measurement of acceptability of crack propagation

    Race and Ethnicity-Adjusted Age Recommendation for Initiating Breast Cancer Screening.

    IMPORTANCE Breast cancer (BC) is the second leading cause of cancer death in women, and there is a substantial disparity in BC mortality by race, especially for early-onset BC in Black women. Many guidelines recommend starting BC screening from age 50 years; however, the current one-size-fits-all policy to start screening all women from a certain age may not be fair, equitable, or optimal. OBJECTIVE To provide race and ethnicity-adapted starting ages of BC screening based on data on current racial and ethnic disparities in BC mortality. DESIGN, SETTING, AND PARTICIPANTS This nationwide population-based cross-sectional study was conducted using data on BC mortality in female patients in the US who died of BC in 2011 to 2020. EXPOSURES Proxy-reported race and ethnicity information was used. The risk-adapted starting age of BC screening by race and ethnicity was measured based on 10-year cumulative risk of BC-specific death. Age-specific 10-year cumulative risk was calculated based on age group-specific mortality data without modeling or adjustment. MAIN OUTCOMES AND MEASURES Disease-specific mortality due to invasive BC in female patients. RESULTS There were BC-specific deaths among 415 277 female patients (1880 American Indian or Alaska Native [0.5%], 12 086 Asian or Pacific Islander [2.9%], 62 695 Black [15.1%], 28 747 Hispanic [6.9%], and 309 869 White [74.6%]; 115 214 patients died before age 60 years [27.7%]) of any age in the US in 2011 to 2020. BC mortality per 100 000 person-years for ages 40 to 49 years was 27 deaths in Black females, 15 deaths in White females, and 11 deaths in American Indian or Alaska Native, Hispanic, and Asian or Pacific Islander females. When BC screening was recommended to start at age 50 years for all females with a 10-year cumulative risk of BC death of 0.329%, Black females reached this risk threshold level 8 years earlier, at age 42 years, whereas White females reached it at age 51 years, American Indian or Alaska Native and Hispanic females at age 57 years, and Asian or Pacific Islander females 11 years later, at age 61 years. Race and ethnicity-adapted starting ages for Black females were 6 years earlier for mass screening at age 40 years and 7 years earlier for mass screening at age 45 years. CONCLUSIONS AND RELEVANCE This study provides evidence-based race-adapted starting ages for BC screening. These findings suggest that health policy makers may consider a risk-adapted approach to BC screening in which individuals who are at high risk are screened earlier to address mortality due to early-onset BC before the recommended age of mass screening

    Incidence, mortality and survival in malignant pleural mesothelioma before and after asbestos in Denmark, Finland, Norway and Sweden

    Publisher Copyright: © 2021, The Author(s).Background: Malignant pleural mesothelioma (MPM) is a rare but fatal cancer, which is largely caused by exposure to asbestos. Reliable information about the incidence of MPM prior the influence of asbestos is lacking. The nationwide regional incidence trends for MPM remain poorly characterized. We use nationwide MPM data for Denmark (DK), Finland (FI), Norway (NO) and Sweden (SE) to assess incidence, mortality and survival trends for MPM in these countries. Methods: We use the NORDCAN database for the analyses: incidence data were available from 1943 in DK, 1953 in FI and NO and 1958 in SE, through 2016. Survival data were available from 1967 through 2016. World standard population was used in age standardization. Results: The lowest incidence that we recorded for MPM was 0.02/100,000 for NO women and 0.05/100,000 for FI men in 1953–57, marking the incidence before the influence of asbestos. The highest rate of 1.9/100,000 was recorded for DK in 1997. Female incidence was much lower than male incidence. In each country, the male incidence trend for MPM culminated, first in SE around 1990. The regional incidence trends matched with earlier asbestos-related industrial activity, shipbuilding in FI and SE, cement manufacturing and shipbuilding in DK and seafaring in NO. Relative 1-year survival increased from about 20 to 50% but 5-year survival remained at or below 10%. Conclusion: In the Nordic countries, the male incidence trends for MPM climaxed and started to decrease, indicating that the prevention of exposure was beneficial. Survival in MPM has improved for both sexes but long-term survival remains dismal.Peer reviewe

    Effects of narrow gap wave resonance on a dual-floater WEC-breakwater hybrid system

    The effects of gap wave resonance on the performance of a dual-floater hybrid system consisting of an oscillating-buoy type wave energy converter (WEC) and a floating breakwater are important for the design of such a hybrid system. This paper investigates the gap wave resonance by employing a two-dimensional numerical wave flume developed using the Star-CCM + software. The maximum wave elevation in the WEC-breakwater gap and the effects of the gap wave resonance on the performance of the dual-floater hybrid system were studied. The influence of the WEC motion and the geometrical parameters of the hybrid system on the maximum wave elevation were analyzed. The maximum gap wave elevation is essentially controlled by the vertical velocity of the free surface in the WEC-breakwater gap. The gap wave resonance was found to significantly improve the wave energy extraction performance of the hybrid system. This allowed the maximum conversion efficiency to exceed the well-known limit of 0.50 for a symmetric body in single degree-of-freedom motion. The wave resonance frequencies in the WEC-breakwater gap decreased with the increase of the gap width and the WEC draft. Due to the energy extraction of the WEC, the horizontal and vertical forces on the breakwater were reduced by up to 0.79 and 0.59, respectively

    PPAR Ligands Function as Suppressors That Target Biological Actions of HMGB1

    High mobility group box 1 (HMGB1), which has become one of the most intriguing molecules in inflammatory disorders and cancers and with which ligand-activated peroxisome proliferator-activated receptors (PPARs) are highly associated, is considered as a therapeutic target. Of particular interest is the fact that certain PPAR ligands have demonstrated their potent anti-inflammatory activities and potential anticancer effects. In this review article we summarize recent experimental evidence that PPAR ligands function as suppressors that target biological actions of HMGB1, including intracellular expression, receptor signaling cascades, and extracellular secretion of HMGB1 in cell lines and/or animal models. We also propose the possible mechanisms underlying PPAR involvement in inflammatory disorders and discuss the future therapeutic value of PPAR ligands targeting HMGB1 molecule for cancer prevention and treatment

    Second Primary Cancers After Liver, Gallbladder and Bile Duct Cancers, and These Cancers as Second Primary Cancers

    Background: Second primary cancers (SPCs) are important clinically as they may negatively influence patient survival and they may tell about therapeutic side effects and general causes of cancer. Population-based literature concerning SPCs after hepatobiliary cancers is limited and here we assess risks of SPCs after hepatocellular cancer (HCC), and cancers of the gallbladder, bile ducts and ampulla of Vater. In reverse order, we consider the risk of hepatobiliary cancers as SPCs after any cancer. Methods: We used standardized incidence ratios (SIRs) to estimate bidirectional relative risks of subsequent cancers associated with hepatobiliary cancers. Cancer diagnoses were obtained from the Swedish Cancer Registry from years 1990 through 2015. Results: We identified 9997 primary HCCs, 1365 gallbladder cancers and 4721 bile duct cancers. After HCC, risks of four SPCs were increased: gallbladder (SIR = 4.38; 95% confidence interval 1.87-8.67), thyroid (4.13; 1.30-9.70), kidney (2.92; 1.66-4.47) and squamous cell skin (1.55; 1.02-2.26) cancers. In reverse order, HCC as SPC, in addition to the above cancers, associations included upper aerodigestive tract, esophageal, small intestinal and bladder cancers and non-Hodgkin lymphoma. For gallbladder and bile duct cancers, associations were found with small intestinal and pancreatic cancers. Conclusion: The results suggested that HCC is associated with two types of SPC, one related to shared environmental risk factors, such as alcohol, exemplified by upper aerodigestive tract and esophageal cancer, and the other related to immune dysfunction, exemplified by squamous cell skin cancer. SPCs associated with gallbladder and bile duct cancers suggest predisposition to mutations in the mismatch repair gene MLH1.Peer reviewe

    Prognosis Prediction of Colorectal Cancer Using Gene Expression Profiles

    Background: Investigation on prognostic markers for colorectal cancer (CRC) deserves efforts, but data from China are scarce. This study aimed to build a prognostic algorithm using differentially expressed gene (DEG) profiles and to compare it with the TNM staging system in their predictive accuracy for CRC prognosis in Chinese patients.Methods: DEGs in six paired tumor and corresponding normal tissues were determined using RNA-Sequencing. Subsequently, matched tumor and normal tissues from 127 Chinese patients were assayed for further validation. Univariate and multivariate Cox regressions were used to identify informative DEGs. A predictive index (PI) was derived as a linear combination of the products of the DEGs and their Cox regression coefficients. The combined predictive accuracy of the DEGs-based PI and tumors' TNM stages was also examined by a logistic regression model including the two predictors. The predictive performance was evaluated with the area under the receiver operating characteristics (AUCs).Results: Out of 75 candidate DEGs, we identified 10 DEGs showing statistically significant associations with CRC survival. A PI based on these 10 DEGs (PI-10) predicted CRC survival probability more accurately than the TNM staging system [AUCs for 3-year survival probability 0.73 (95% confidence interval: 0.64, 0.81) vs. 0.68 (0.59, 0.76)] but comparable to a simplified PI (PI-5) using five DEGs (LOC646627, BEST4, KLF9, ATP6V1A, and DNMT3B). The predictive accuracy was improved further by combining PI-5 and the TNM staging system [AUC for 3-year survival probability: 0.72 (0.63, 0.80)].Conclusion: Prognosis prediction based on informative DEGs might yield a higher predictive accuracy in CRC prognosis than the TNM staging system does

    Second cancers and causes of death in patients with testicular cancer in Sweden

    While treatment for testicular cancer (TC) has become standardized after the 1980s with an associated significant improvement in patient survival, this has been accompanied by an increased risk of second primary cancers (SPCs). Patients were identified from the Swedish Cancer Registry spanning the years from 1980 to 2015, including 8788 individuals with primary TC and their SPCs. Relative risks (RRs) for SPC were calculated using the generalized Poisson regression model. SPCs were diagnosed in 9.4% of patients with TC and half of them were late onset cancers not common in the population in their 40s. Overall RR of SPCs (excluding second TC) was 1.30 (95%CI: 1.20-1.40), including high risks for seven solid cancers, non-Hodgkin lymphoma and leukemia. Second TC was the most common SPC and the RR of 17.19 (95%CI: 14.89-19.85) was the highest recorded. Cancers known to be fatal as first primary cancers were also fatal as SPC in TC patients. Survival at 30 years of follow-up was approximately 80% for TC patients without SPC but it decreased to 40% for patients with SPC. The unexpected finding that half of the identified SPCs were typical late onset cancers in the middle-aged population raises concerns that therapy may facilitate premature aging. The risks of SPC are clinically important for the long-term management of TC patients and the high-mortality calls for a future management strategy.Peer reviewe

    An approach to urban system spatial planning in Chengdu Chongqing economic circle using geospatial big data

    Evidence suggests city grouping is an important way to implement urbanization in China. However, the Chengdu-Chongqing Economic Circle (CCEC) is a typical dual-core structure, and the development level of each city is different. If we do not focus on the key directions for urban development, it will not be conducive to the new-type urbanization process. Therefore, we use spatial analysis techniques and geographic big data sets to construct an approach for urban system layout optimization from a global perspective. It mainly includes urban extended trend analysis based on night light, multi-modal traffic network analysis, and spatial economic density analysis using Open Street Map (OSM) and Point of Interest (POI) data. The research results show the following interesting findings. Firstly, the historical relationship of cities has a significant impact on city grouping, and efficient transportation connections and prosperous enterprise distribution are key conditions for urban grouping during the acceleration period of urbanization. Secondly, the development of urban grouping should break through administrative restrictions and achieve a moderate separation of administrative divisions and economic divisions. It is beneficial to the rapid growth of the city group and the improvement of the internal structure. Thirdly, the urban group of Southern Sichuan and Western Chongqing (SSWC) is the region with the most potential for growth in CCEC. The urban expansion index (UEI) of the Yibin-Luzhou area is 2.16, and the spatial economic density has increased by 130/km2 in the past decade. Providing flexible development authority and focus on the construction of the Yibin-Luzhou Urban Belt along the Yangtze River is an important way to integrate southern Sichuan. Moreover, we found the UEI has a good universality and it can be used for studying urban expansion trends and city relationships in rapidly developing regions, especially in metropolitan areas or urban agglomeration
