Competing Technology Options and Stakeholder Interests for Tracking Freight Railcars in Indian Railways
This teaching case examines the implementation of a new technology for tracking individual freight railcars (wagons) by Indian Railways. After exploring multiple ‘technological options,’ the Indian Railways decided to undertake a pilot project based on time-tested Automatic Equipment Identification system using Radio Frequency Identification (RFID) technology. However, a number of other technological options are now available, which include EPC Gen2-based RFID systems, Global Positioning System solutions, Optical Character Recognition (OCR)-based systems, and manual hand-held data collection devices integrated with the current Freight Operations System. Each of these systems has its own advantages and limitations. Although Indian Railways officials are going ahead with the pilot project, they are uncertain as to the appropriate technological choice, given the wide range of available technology options. Further, they are faced with competing interests from different stakeholder groups (departments), who favor different technologies
Modernization of Passenger Reservation System: Indian Railways’ Dilemma
This teaching case discusses the challenges being faced by the technology managers at Indian Railways (IR) in the current scenario of a resurgent national economy coupled with increasing customer expectations. In the face of growing competition from road and low-cost airlines, to retain its customers, IR has responded by changing its business rules. The Railway Ministry expects a rapid response from Centre for Railway Information Systems (CRIS) to incorporate all these changes in the passenger reservation system (PRS). The old PRS, which is time-tested and reliable, and has been serving the customers’ needs for nearly two decades, is now proving to be relatively inflexible to match the rapidly changing business requirements. Although the current scenario of a constant need to change the programming logic of PRS has been making maintenance tougher for CRIS officials, they have realized that PRS is a time-tested, proven, and reliable technology. Though they would be happy to replace the old PRS with a new state-of-art system that would provide them greater maintenance flexibility, the repercussions associated with possible failure of the new system are far too serious. The case exhibits the current dilemma being faced by the head of CRIS, the umbrella agency for information technology (IT) implementation in IR: whether IR should continue using the old PRS technology with its inherent shortcomings, or should it take the risk and go in for a wholesale replacement with a new state-of-art technology which would provide greater maintenance flexibility
Neurofilament light chain and tau concentrations are markedly increased in the serum of patients with sporadic Creutzfeldt-Jakob disease, and tau correlates with rate of disease progression
OBJECTIVES: A blood-based biomarker of neuronal damage in sporadic Creutzfeldt-Jakob disease (sCJD) will be extremely valuable for both clinical practice and research aiming to develop effective therapies. METHODS: We used an ultrasensitive immunoassay to measure two candidate biomarkers, tau and neurofilament light (NfL), in serum from patients with sCJD and healthy controls. We tested longitudinal sample sets from six patients to investigate changes over time, and examined correlations with rate of disease progression and associations with known phenotype modifiers. RESULTS: Serum concentrations of both tau and NfL were increased in patients with sCJD. NfL distinguished patients from controls with 100% sensitivity and 100% specificity. Tau did so with 91% sensitivity and 83% specificity. Both tau and NfL appeared to increase over time in individual patients, particularly in those with several samples tested late in their disease. Tau, but not NfL, was positively correlated with rate of disease progression, and was particularly increased in patients homozygous for methionine at codon 129 ofPRNP. CONCLUSIONS: These findings independently replicate other recent studies using similar methods and offer novel insights. They show clear promise for these blood-based biomarkers in prion disease. Future work should aim to fully establish their potential roles for monitoring disease progression and response to therapies
A review of clinical decision-making: Models and current research
Aims and objectives: The aim of this paper was to review the current literature with respect to clinical decision-making models and the educational application of models to clinical practice. This was achieved by exploring the function and related research of the three available models of clinical decision making: information processing model, the intuitive-humanist model and the clinical decision making model.
Background: Clinical decision-making is a unique process that involves the interplay between knowledge of pre-existing pathological conditions, explicit patient information, nursing care and experiential learning. Historically, two models of clinical decision making are recognised from the literature; the information processing model and the intuitive-humanist model. The usefulness and application of both models has been examined in relation the provision of nursing care and care related outcomes. More recently a third model of clinical decision making has been proposed. This new multidimensional model contains elements of the information processing model but also examines patient specific elements that are necessary for cue and pattern recognition.

The diversification of Heliconius butterflies: what have we learned in 150 years?
Research into Heliconius butterflies has made a significant contribution to evolutionary biology. Here, we review our understanding of the diversification of these butterflies, covering recent advances and a vast foundation of earlier work. Whereas no single group of organisms can be sufficient for understanding life's diversity, after years of intensive study, research into Heliconius has addressed a wide variety of evolutionary questions. We first discuss evidence for widespread gene flow between Heliconius species and what this reveals about the nature of species. We then address the evolution and diversity of warning patterns, both as the target of selection and with respect to their underlying genetic basis. The identification of major genes involved in mimetic shifts, and homology at these loci between distantly related taxa, has revealed a surprising predictability in the genetic basis of evolution. In the final sections, we consider the evolution of warning patterns, and Heliconius diversity more generally, within a broader context of ecological and sexual selection. We consider how different traits and modes of selection can interact and influence the evolution of reproductive isolation.RMM is funded by a Junior Research Fellowship at King’s College, Cambridge. Economic and other barriers to adopting recommendations to prevent childhood obesity: results of a focus group study with parents
Abstract Background Parents are integral to the implementation of obesity prevention and management recommendations for children. Exploration of barriers to and facilitators of parental decisions to adopt obesity prevention recommendations will inform future efforts to reduce childhood obesity. Methods We conducted 4 focus groups (2 English, 2 Spanish) among a total of 19 parents of overweight (BMI ≥ 85th percentile) children aged 5-17 years. The main discussion focused on 7 common obesity prevention recommendations: reducing television (TV) watching, removing TV from child's bedroom, increasing physically active games, participating in community or school-based athletics, walking to school, walking more in general, and eating less fast food. Parents were asked to discuss what factors would make each recommendation more difficult (barriers) or easier (facilitators) to follow. Participants were also asked about the relative importance of economic (time and dollar costs/savings) barriers and facilitators if these were not brought into the discussion unprompted. Results Parents identified many barriers but few facilitators to adopting obesity prevention recommendations for their children. Members of all groups identified economic barriers (time and dollar costs) among a variety of pertinent barriers, although the discussion of dollar costs often required prompting. Parents cited other barriers including child preference, difficulty with changing habits, lack of information, lack of transportation, difficulty with monitoring child behavior, need for assistance from family members, parity with other family members, and neighborhood walking safety. Facilitators identified included access to physical activity programs, availability of alternatives to fast food and TV which are acceptable to the child, enlisting outside support, dietary information, involving the child, setting limits, making behavior changes gradually, and parental change in shopping behaviors and own eating behaviors. Conclusions Parents identify numerous barriers to adopting obesity prevention recommendations, most notably child and family preferences and resistance to change, but also economic barriers. Computational Method for Phase Space Transport with Applications to Lobe Dynamics and Rate of Escape
Lobe dynamics and escape from a potential well are general frameworks
introduced to study phase space transport in chaotic dynamical systems. While
the former approach studies how regions of phase space are transported by
reducing the flow to a two-dimensional map, the latter approach studies the
phase space structures that lead to critical events by crossing periodic orbit
around saddles. Both of these frameworks require computation with curves
represented by millions of points-computing intersection points between these
curves and area bounded by the segments of these curves-for quantifying the
transport and escape rate. We present a theory for computing these intersection
points and the area bounded between the segments of these curves based on a
classification of the intersection points using equivalence class. We also
present an alternate theory for curves with nontransverse intersections and a
method to increase the density of points on the curves for locating the
intersection points accurately.The numerical implementation of the theory
presented herein is available as an open source software called Lober. We used
this package to demonstrate the application of the theory to lobe dynamics that
arises in fluid mechanics, and rate of escape from a potential well that arises
