    External iliac artery dissection secondary to endofibrosis in a cyclist

    Endofibrosis of the external iliac artery is an uncommon disease affecting primarily young, otherwise healthy, endurance athletes. Thigh pain during maximal exercise with quick resolution postexercise is characteristic of the so-called cyclist's iliac syndrome. We report an unusual case in which the typical endofibrotic plaque was accompanied by dissection of the external iliac artery. The patient was treated surgically with excision of the affected artery segment and placement of an interposition graft. This case highlights an unusual finding in association with external iliac artery endofibrosis and provides an opportunity to briefly review the literature on the subject

    Effects of Medially Wedged Foot Orthoses on Knee and Hip Joint Running Mechanics in Females With and Without Patellofemoral Pain Syndrome.

    We examined the effects of medially wedged foot orthoses on knee and hip joint mechanics during running in females with and without patellofemoral pain syndrome (PFPS). We also tested if these effects depend on standing calcaneal eversion angle. Twenty female runners with and without PFPS participated. Knee and hip joint transverse and frontal plane peak angle, excursion, and peak internal knee and hip abduction moment were calculated while running with and without a 6° full-length medially wedged foot orthoses. Separate 3-factor mixed ANOVAs (group [PFPS, control] x condition [medial wedge, no medial wedge] x standing calcaneal angle [everted, neutral, inverted]) were used to test the effect of medially wedged orthoses on each dependent variable. Knee abduction moment increased 3% (P = .03) and hip adduction excursion decreased 0.6° (P < .01) using medially wedged foot orthoses. No significant group x condition or calcaneal angle x condition effects were observed. The addition of medially wedged foot orthoses to standardized running shoes had minimal effect on knee and hip joint mechanics during running thought to be associated with the etiology or exacerbation of PFPS symptoms.     Ischemic Colitis Secondary to Ergotamine Use: A Case Study

    A 48-year-old woman with a history of chronic migraines, initially admitted for inpatient management of intractable migraine headaches, developed new onset abdominal pain, hypotension, and diarrhea on hospital day number ten. In our institution's headache unit, patients are treated by a multidisciplinary approach, including individualized drug therapy based on diagnosis and previous response to therapy. Given the patient's hypotension and clinical appearance, she was transferred to the intensive care unit and treated for septic shock and metabolic acidosis. A bedside colonscopy revealed diffuse ischemic colitis. Final pathology after colon resection showed widespread, transmural necrosis of the colonic wall. We review the pathophysiology of ergotamine use and its potential association with ischemic colitis

    Managing soil carbon and nitrogen for productivity and environmental quality

    Includes bibliographical references (pages 774-775).In this study, we investigated the impact of cropping system management on C and N pools, crop yield, and N leaching in a long-term agronomic experiment in Southwest Michigan. Four management types, conventional (CO), integrated fertilizer (IF), integrated compost (IC), and transitional organic (TO) were applied to two crop sequences, a corn (Zea mays L.)–corn–soybean [Glycine max (L.) Merr.]–wheat (Triticum aestivum L.) rotation and continuous corn, which were grown with and without cover crops in the IF, IC, and TO managements. Using compost as a fertility source and reducing the use of herbicides and other chemicals resulted in long-term changes in soil organic matter pools such TO ≥ IC > IF ≥ CO for total C and N and for the labile C and N measured through aerobic incubations at 70 and 150 d. Mineralizable N varied within the rotation, tending to increase after soybean and decrease after corn production in all systems. Corn yield was closely associated with 70-d N mineralization potential, being greatest for first-year corn with cover and least for continuous corn without cover under all management types. Although the TO and IC systems produced the lowest yield for second-year or continuous corn, the combination of soybean and wheat plus red clover (Trifolium pratense L.) always supported high yield for first-year corn. Clinically, amyloidosis may be categorized as localized or systemic. The gastrointestinal tract is among the most common places for deposition of amyloid, but large, localized amyloid deposits are an uncommon occurrence and rarely cause extraluminal bowel compression resulting in obstruction as was seen in the case presented in this clinical scenario

    A multi-instrument and multi-wavelength high angular resolution study of MWC614: quantum heated particles inside the disk cavity

    High angular resolution observations of young stellar objects are required to study the inner astronomical units of protoplanetary disks in which the majority of planets form. As they evolve, gaps open up in the inner disk regions and the disks are fully dispersed within ~10 Myrs. MWC 614 is a pre-transitional object with a ~10au radius gap. We present a set of high angular resolution observations of this object including SPHERE/ZIMPOL polarimetric and coronagraphic images in the visible, KECK/NIRC2 near-infrared aperture masking observations and VLTI (AMBER, MIDI, and PIONIER) and CHARA (CLASSIC and CLIMB) long-baseline interferometry at infrared wavelengths. We find that all the observations are compatible with an inclined disk (i ~55deg at a position angle of ~20-30deg). The mid-infrared dataset confirms the disk inner rim to be at 12.3+/-0.4 au from the central star. We determined an upper mass limit of 0.34 Msun for a companion inside the cavity. Within the cavity, the near-infrared emission, usually associated with the dust sublimation region, is unusually extended (~10 au, 30 times larger than the theoretical sublimation radius) and indicates a high dust temperature (T~1800 K). As a possible result of companion-induced dust segregation, quantum heated dust grains could explain the extended near-infrared emission with this high temperature. Our observations confirm the peculiar state of this object where the inner disk has already been accreted onto the star exposing small particles inside the cavity to direct stellar radiation.Comment: 24 pages.     High-throughput sequencing of the paired human immunoglobulin heavy and light chain repertoire.

    Each B-cell receptor consists of a pair of heavy and light chains. High-throughput sequencing can identify large numbers of heavy- and light-chain variable regions (VH and VL) in a given B-cell repertoire, but information about endogenous pairing of heavy and light chains is lost after bulk lysis of B-cell populations. Here we describe a way to retain this pairing information. In our approach, single B cells (>5 × 104 capacity per experiment) are deposited in a high-density microwell plate (125 pl/well) and lysed in situ. mRNA is then captured on magnetic beads, reverse transcribed and amplified by emulsion VH:VL linkage PCR. The linked transcripts are analyzed by Illumina high-throughput sequencing. We validated the fidelity of VH:VL pairs identified by this approach and used the method to sequence the repertoire of three human cell subsets-peripheral blood IgG+ B cells, peripheral plasmablasts isolated after tetanus toxoid immunization and memory B cells isolated after seasonal influenza vaccinatio

    Resist materials for 157-nm microlithography: an update

    Fluorocarbon polymers and siloxane-based polymers have been identified as promising resist candidates for 157 nm material design because of their relatively high transparency at this wavelength. This paper reports our recent progress toward developing 157 nm resist materials based on the first of these two polymer systems. In addition to the 2-hydroxyhexafluoropropyl group, (alpha) -trifluoromethyl carboxylic acids have been identified as surprisingly transparent acidic functional groups. Polymers based on these groups have been prepared and preliminary imaging studies at 157 nm are described. 2-Trifluoromethyl-bicyclo[2,2,1] heptane-2-carboxylic acid methyl ester derived from methyl 2-(trifluoromethyl)acrylate was also prepared and gas-phase VUV measurements showed substantially improved transparency over norbornane. This appears to be a general characteristic of norbornane-bearing geminal electron-withdrawing substituents on the 2 carbon bridge. Unfortunately, neither the NiII nor PdII catalysts polymerize these transparent norbornene monomers by vinyl addition. However, several new approaches to incorporating these transparent monomers into functional polymers have been investigated. The first involved the synthesis of tricyclononene (TCN) monomers that move the bulky electron withdrawing groups further away from the site of addition. The hydrogenated geminally substituted TCN monomer still has far better transparency at 157 nm than norbornane. The second approach involved copolymerizing the norbornene monomers with carbon monoxide. The third approach involved free-radical polymerization of norbornene monomers with tetrafluoroethylene and/or other electron-deficient comonomers. All these approaches provided new materials with encouraging absorbance at 157 nm. The lithographic performance of some of these polymers is discussed

    Particle-type dependence of azimuthal anisotropy and nuclear modification of particle production in Au+Au collisions at s(NN)**(1/2) = 200-GeV

    We present STAR measurements of the azimuthal anisotropy parameter v2v_2 and the binary-collision scaled centrality ratio RCPR_{CP} for kaons and lambdas (Λ+Λˉ\Lambda+\bar{\Lambda}) at mid-rapidity in Au+Au collisions at sNN=200\sqrt{s_{_{NN}}}=200 GeV. In combination, the v2v_2 and RCPR_{CP} particle-type dependencies contradict expectations from partonic energy loss followed by standard fragmentation in vacuum. We establish pT5p_T \approx 5 GeV/c as the value where the centrality dependent baryon enhancement ends. The KS0K_S^0 and Λ+Λˉ\Lambda+\bar{\Lambda} v2v_2 values are consistent with expectations of constituent-quark-number scaling from models of hadron fromation by parton coalescence or recombination.Comment: 6 pages, 4 figures, 1 table.
