The Effects of Intervention Strategy on Self-Perception of Hearing Handicap
ABSTRACTThe purpose of this study was to determine whether participation in a counseling-based aural rehabilitation program would result in greater reduction of self-per-ceived hearing handicap than hearing aid use alone. Thirty-one postlingually hearing-impaired adults were placed into three groups after audiological evaluation. The first group received hearing aids and participated in a counseling-based aural rehabilitation (AR) program. The second group received hearing aids only. The third group received neither hearing aids nor counseling-based AR. The Hearing Handicap Inventory for the Elderly was ad-ministered to all subjects before audiological evaluation and again to all subjects 2 mo after receipt of hearing aids for the experimental groups. For both experimental groups, self-perception of hearing handicap was signifi-cantly reduced as a function of intervention when meas-ured on any of the three Hearing Handicap Inventory for the Elderly scales, whereas there was no change in self-perception of hearing handicap for the control group on any scale. In addition, there was weak but significant evidence that participating in the counseling-based AR program in addition to hearing aid use resulted in a greater reduction of self-perceived hearing handicap than did hearing aid use alone
A New Look at Performance on Theory-of-Mind Tasks by Adolescents With Autism Spectrum Disorder
Purpose: A hallmark of autistic spectrum disorder (ASD) is disruption in theory-of-mind development, including the understanding of false beliefs. Previous studies have typically assessed the development of first- and second-order false belief concepts in ASD, with tasks primarily emphasizing physical causality and logical inferencing. The present study investigated how preadolescents and adolescents with ASD performed on false belief tasks that included social inferencing of psychological states as well as logical inferencing of physical states.
Method: Two categories of tasks were administered: four traditional logical inferencing tasks and four social inferencing tasks specifically developed for this study. In addition, a prompt hierarchy was included to ascertain if performance on both task types improved. Participants were 45 children and adolescents primarily selected from three urban school districts: 15 adolescents with a previous diagnosis of ASD (ASD group); 15 typically developing children matched for age, gender, and ethnicity (CA group); and 15 typically developing children matched for language age, gender, and ethnicity (LA group).
Results: Three findings were pertinent. First, the CA group performed at higher levels than did the LA group and the ASD group on both task sets. Second, although the CA and the LA groups performed equally well on both the logical and the social inferencing tasks, the ASD group performed better on the social inferencing tasks. Finally, the prompt hierarchy significantly improved overall task performance for the ASD and LA groups.
Clinical Implications: These findings indicate that task type, variations in vocabulary ability, and the provision of support influenced performance on the false belief tasks

Learning to Listen Again: The Role of Compliance in Auditory Training for Adults With Hearing Loss
Purpose: To examine the role of compliance in the outcomes of computer-based auditory training with the Listening and Communication Enhancement (LACE) program in Veterans using hearing aids. Method: The authors examined available LACE training data for 5 tasks (i.e., speech-in-babble, time compression, competing speaker, auditory memory, missing word) from 50 hearing-aid users who participated in a larger, randomized controlled trial designed to examine the efficacy of LACE training. The goals were to determine: (a) whether there were changes in performance over 20 training sessions on trained tasks (i.e., on-task outcomes); and (b) whether compliance, defined as completing all 20 sessions, vs. noncompliance, defined as completing less than 20 sessions, influenced performance on parallel untrained tasks (i.e., off-task outcomes). Results: The majority, 84% of participants, completed 20 sessions, with maximum outcome occurring with at least 10 sessions of training for some tasks and up to 20 sessions of training for others. Comparison of baseline to posttest performance revealed statistically significant improvements for 4 of 7 off-task outcome measures for the compliant group, with at least small (0.2 \u3c d \u3c 0.3) Cohen\u27s d effect sizes for 3 of the 4. There were no statistically significant improvements observed for the noncompliant group. Conclusion: The high level of compliance in the present study may be attributable to use of systematized verbal and written instructions with telephone follow-up. Compliance, as expected, appears important for optimizing the outcomes of auditory training. Methods to improve compliance in clinical populations need to be developed, and compliance data are important to report in future studies of auditory training
