Pelvic inflammatory disease during the post-partum year.
OBJECTIVE: To investigate the occurrence of, and risk factors for, pelvic inflammatory disease (PID) occurring during the post-partum year. METHODS: Demographic and clinical data for women who delivered a term infant with 5-minute Apgar score > or = 8 from 1992 through 1999 at a large urban hospital were extracted from an electronic medical record system. RESULTS: During the study period, 15 206 deliveries occurred among 12 549 women. PID was diagnosed during the post-partum year of 148 (1.0%) deliveries. In univariate analysis, young age, black race, and both pre-delivery history and post-partum diagnosis of chlamydial and gonococcal infection were associated with PID. In multivariate analysis, only young age and a positive test for gonorrhea before delivery or post-partum were independent predictors of PID. CONCLUSIONS: Pelvic inflammatory disease was diagnosed during the post-partum year in 1% of women studied. Young maternal age was an important demographic risk factor. Further investigation of post-partum STD acquisition and progression to PID is needed to determine whether women are at increased risk following delivery
Daily mood, partner support, sexual interest, and sexual activity among adolescent women

Methods: Women (ages 14 – 17 at enrollment; N=146) enrolled from one of three adolescent primary care clinics completed up to five 84-day diaries over a 27-month period. The diaries assessed partner interactions, sexual activity, substance use and mood. Partner-specific measures assessed on each day included partner emotional support (4 items; alpha = 0.94), argument with a partner (no/yes) and coitus (no/yes). Within-day measures assessed marijuana use (no/yes), Positive Mood (3-items; alpha = 0. 86); Negative Mood (3-items; alpha = 0.82) and Sexual Interest (1-item). Lagged measures of mood and sexual activity were included in multivariate models to control for recent mood and sexual behavior effects on current day mood and coitus. Two main analyses were conducted: coitus as a predictor of positive and negative mood; and the role of positive and negative mood as predictors of coitus. Analyses were conducted by multivariate mixed effect regression and mixed effect logistic regression models.
Results: Data represent 28,376 days from 146 participants. The average number of diary days was 194 days per participant. Sexual activity was reported on 8.3% of days, with condoms used for 27.0% of these coital events. Marijuana was used on 11% of days. Significant predictors of positive mood on a given day included partner support, marijuana use, and coitus. Negative mood was associated with having an argument with a partner and with prior day coitus. Predictors of coitus on a given day included age (Odds ratio = 1.22), increased coital frequency in previous week (OR = 1.49), coitus on the previous day (1.21), increased same-day sexual interest (OR = 2.8) and decreased same-day negative mood (OR = 0.92).
Conclusions: The data demonstrate complex associations of sexual interest, mood, partner interactions and sexual activity

Partitioning and Scaling Signed Bipartite Graphs for Polarized Political Blogosphere
Blogosphere plays an increasingly important role as a forum for public debate. In this paper, given a mixed set of blogs debating a set of political issues from opposing camps, we use signed bipartite graphs for modeling debates, and we propose an algorithm for partitioning both the blogs, and the issues (i.e. topics, leaders, etc.) comprising the debate into binary opposing camps. Simultaneously, our algorithm scales both the blogs and the underlying issues on a univariate scale. Using this scale, a researcher can identify moderate and extreme blogs within each camp, and polarizing vs. unifying issues. Through performance evaluations we show that our proposed algorithm provides an effective solution to the problem, and performs much better than existing baseline algorithms adapted to solve this new problem. In our experiments, we used both real data from political blogosphere and US Congress records, as well as synthetic data which were obtained by varying polarization and degree distribution of the vertices of the graph to show the robustness of our algorithm
Preoperative radiographic valgus alignment predicts the extent of lateral soft tissue release and need for constraint in valgus total knee arthroplasty
Background: In total knee arthroplasty (TKA) for valgus knees, the decision to use a constrained implant is often made intraoperatively depending on the extent of soft tissue releases performed and residual soft tissue imbalance. The purpose of this study is to determine if preoperative radiographic criteria of valgus knees can predict the extent of soft tissue releases required and the level of constraint needed to balance the knee during TKA. Methods: A single surgeon's 807 consecutive TKA standing hip-knee-ankle radiographs from 2007-2012 were analyzed. One hundred eighty-seven valgus knees were identified and annotated. Statistical univariate and multivariate analyses were performed for both outcomes, lateral release and articulation, to assess the association with risk factors of gender, age, and preoperative radiographic markers of valgus deformity. A P-value .05). A preoperative anatomic tibiofemoral valgus angle of >16.8° was associated with the use of a constrained articulation during surgery. Conclusions: Our data demonstrate that preoperative radiographic characteristics of the valgus knee can be utilized to predict the extent of lateral soft tissue release and whether a constrained articulation will be required in TKA. This will provide surgeons with useful information to offer accurate preoperative counseling to patients and to ensure that the appropriate prosthetic parts are available during surgery.
