    Background: High-dose isotretinoin has been reported to have adverse effects on bone mineral density (BMD); however, studies evaluating changes in BMD with isotretinoin therapy at different dosages and with varying treatment durations have produced conflicting results. Objective: To investigate the effect of a standard, single course of isotretinoin therapy on BMD and bone turnover markers in patients with nodulocystic acne. Methods: Thirty-six patients (15 male, 21 female) with severe, recalcitrant, nodulocystic acne and 36 healthy controls (16 male, 20 female) were enrolled in the study. Patients received isotretinoin treatment for 4-6.months until a cumulative dose of 120 mg/kg had been achieved. BMD in the lumbar spine and femur was measured at baseline and at the end of therapy by dual-energy X-ray absorptiometry. Serum calcium, phosphate, parathormone, total alkaline phosphatase, osteocalcin, free deoxypyridinoline, and urinary calcium were also measured before and at the end of treatment. Results: No significant differences were found in lumbar spine and femoral BMD between the patient and control groups at the beginning of the study (P >0.05), and no statistically significant difference was observed between the BMD values in patients at the beginning vs. the end of treatment (P >0.05). No statistically significant difference in bone turnover markers was found between patients and controls at the beginning of the study (P >0.05), and no statistically significant changes in bone turnover markers were observed in patients at the beginning vs. the end of treatment (P >0.05). Conclusion: A single course of isotretinoin therapy has no clinically significant effect on bone metabolism. © 2008 International Society of Dermatology

    Dissecting cellulitis in a white male: Response to isotretinoin

    A 22-year-old white male with a one-year history of dissecting cellulitis of the scalp was successfully treated with oral isotretinoin. Complete healing was seen after 2 months and the treatment was discontinued after a course of 9 months. The patient is still in remission 3 months after cessation of the therapy

    Hereditary angioedema is a rare autosomal dominant disorder resulting from the congenital deficiency of functional C1 esterase inhibitor protein. Patients with hereditary angioedema are clinically characterized by recurrent episodes of swelling of the extremities, face, trunk, airways and abdominal viscera. Attacks may occur either spontaneously or following stress or trauma. The disease is usually associated with attacks of abdominal pain. So, patients may apply for this complaint to other clinics rather than dermatology, and may be misdiagnosed and followed for a long time. Therefore hereditary angioedema should be thought in differential diagnosis of patients suffering from abdominal pain. Here in this writing, we describe a family with hereditary angioedema who has been followed as Familial Mediterranean Fever for a long time. The family members complained from swellings which have been occuring in various regions of the body and disappearing spontaneously, and complained from severe abdominal pain, since childhood. These patients have been followed and tried to be treated with the misdiagnosis of Familial Mediterranean Fever for many years. These patients were diagnosed as hereditary angioedema in our clinic, and benefited from danazol treatment

    Topical pimecrolimus 1 cream for resistant seborrheic dermatitis of the face: An open-label study

    Background: Treatment options for seborrheic dermatitis are numerous, including both topical and systemic agents (e.g. topical corticosteroids, oral antifungals, and psoralen plus UVA). However, long-term use of topical corticosteroids may lead to adverse effects. Pimecrolimus 1% cream is an effective and well tolerated treatment for seborrheic dermatitis. Objective: To explore the efficacy of pimecrolimus 1% cream for the treatment of seborrheic dermatitis lesions resistant to conventional treatments. Methods: Sixteen patients with resistant seborrheic dermatitis of the face applied pimecrolimus 1% cream twice daily for 2 weeks. The lesions were assessed clinically and the severity of the signs were assessed using a 4-point score. Additionally, the scores of all affected regions (paranasal, forehead, and eyebrows) were evaluated separately to assess whether different results would be obtained in different regions of the face. Also, patients completed self-assessments on a 100mm Visual Analogue Scale (VAS) at each visit. Results: Statistically significant reductions in the scores of all parameters were observed at day 7 and day 14 of the study. There were no significant differences between the responses on the three regions of the face. No adverse effects were reported except for temporary pruritus immediately after the application of pimecrolimus 1% cream in one patient. Conclusion: Although the interpretation of efficacy was limited by the open-label, non-controlled study design and the small number of patients, this trial suggests that pimecrolimus 1% cream may be a successful treatment choice for patients with resistant seborrheic dermatitis of the face. © 2010 Adis Data Information BV.     Psoriasiform drug eruption associated with metformin hydrochloride: A case report

    An 18 year-old-woman presented with a 1-week history of a psoriasiform eruption on her limbs and trunk that began 1 week after starting metformin hydrochloride. She had taken no other medications. She had no personal or family history of psoriasis. The lesions disappeared within 5 weeks after discontinuation of the drug. In the 4 months following the cessation of metformin hydrochloride, no relapse was observed, but rechallenge with oral metformin again produced the eruption. Metformin hydrochloride should be added to the list of drugs that can cause a psoriasiform eruption

    Myocardial functional and textural findings of the right and left ventricles and their association with cellular adhesion molecules in Behçet's disease

    Aim: This study was designed to assess whether ultrasonic reflectivity, evaluated by integrated backscatter analysis was associated with the severity of inflammation and diastolic dysfunction of the left (LV) and right ventricles (RV) in Behçet's disease (BD). Methods: The study consisted of 20 patients with BD and 18 healthy controls. The expression of CD11b and CD63 on the surface of granulocytes and monocytes were measured by flow cytometry. RV and LV diastolic functions were assessed by tricuspid and mitral annular tissue Doppler recordings, respectively. Backscatter cyclic variation (CV) and maximal intensity (IB) were measured in RV lateral wall, interventricular septum, and posterior LV wall from parasternal long-axis view, apex from apical four-chamber view, anterior, inferior, and lateral LV walls from parasternal short axis view. Results: The mean fluorescence intensity (MFI) of CD11b on granulocytes and CD63 on monocytes in BD patients was higher than those of controls. Patients with BD had smaller mitral and tricuspid annular early diastolic wave velocities and early/late diastolic wave velocity ratios (E/A) than controls. MFI of CD11b and CD63 was inversely correlated with tricuspid and mitral annular E/A in BD patients. CV of RV and global CV of LV were found to be diminished in BD patients than controls. CV of RV and global CV of LV showed a direct correlation with tricuspid and mitral annular E/A ratio, respectively. Conclusion: There is an association between the levels of cellular adhesion molecules, deterioration of diastolic function, and altered myocardial ultrasonic reflectivity in BD. © 2007, the Authors
