    Relationship between odontogenic bacteremia and orthodontic stripping

    Introduction: The aim of this study was to evaluate the prevalence of bacteremia associated with an orthodontic stripping procedure. Methods: The study included 29 orthodontic patients (mean age, 18.2 6 3.4 years). We used a standardized stripping procedure: a perforated stripping disk with a contra-angle hand piece was used at a low speed (\15,000 rpm; 10 seconds) on the mandibular anterior teeth. Blood samples were collected by inserting a cannula into the left antecubital fossa. A baseline sample was taken before treatment, and a second sample was taken after the stripping procedure. These samples were inoculated into aerobic and anaerobic blood culture bottles and incubated, and the bacterial cultures were identified; the samples collected before and after the stripping procedure were statistically analyzed. Results: Transient bacteremia was not detected in any pretreatment blood sample, but it was found in 1 postoperative blood sample; this sample tested positive for Streptococcus sanguis. Conclusions: The bacterial species in the positive postoperative blood sample was S sanguis, which might be associated with infective endocarditis. Clinicians should explain the level of risk to the patient and consult a concerned medical specialist

    Currently, aesthetics of the anterior teeth is a significant issue in general dentistry and the most frequently cited reason for patients seeking orthodontic treatment. For many years, lingual orthodontics was perceived as complex and problematic treatment procedures and therefore not widely used internationally. However, during the last decade, the percentage of patients treated with lingual orthodontics has increased, appliance systems have renewed, and the technique has developed to such an extent that in some cases, it is easier, quicker, and more accurate than traditional buccal orthodontics. The aim of this review is to evaluate and generally consider the usage of lingual orthodontics which commonly preferred by patients in routine orthodontic procedures and look through current applications. Additionally, special considerations regarding the contemporary diagnosis and treatment planning in lingual orthodontics are presented in this study. ÖZET Günümüzde, ön dişlerin estetik olarak dizilimi ve görünümü genel diş hekimliğinde önemli bir konu olmuş ve bu önem hastalar için lingual ortodonti tercihinde birinci sebep halini almıştır. Lingual ortodonti yıllardır karmaşık ve uygulanması zor tedavi yöntemlerinden kabul edilmiş ve uluslar arası alanda yaygın bir kullanım alanı bulamamıştır. Ancak son dönemlerde, bu yöntem aracılığıyla tedavi edilen hasta yüzdesi artmış, vakalarda uygulanan aygıtlar yenilenmiş, tedaviler kolaylaştırılmış, hızlandırılmış ve her geçen gün geleneksel ortodontik tedavilerle elde edilen başarılara ulaşılmaya başlanmıştır. Bu derlemenin amacı, ortodontide her geçen gün daha fazla hastanın tercih ettiği bu uygulamayı genel olarak değerlendirmek ve güncel uygulamalara genel bir bakış yapmaktır. Ayrıca lingual ortodontik uygulamalardaki güncel muayene ve tedavi planlamalarından, karşılaşılan özel durumlardan da bu çalışmada bahsedilecektir.     Rapid Maxillary Expansion and Facemask Therapy: Effects on Mandibular Condyle Position and Dentoskeletal Profile

    Aim:The aim of this study was to evaluate the effects of the combination of rapid maxillary expansion and facemask therapy on mandibular condyle position and dentoskeletal profile; during the treatment of class III patients.Subjects and Methods:Treatment group comprised 31 patients, 16 girls and 15 boys (mean age: 9.2±1.7 years) and control group comprised 27 subjects, 14 girls and 13 boys (mean age: 9.7±2.2 years); with class III maxillary retrusion. Mandibular position indicator (MPI®) measurements of the SAM®3 articulator and cephalometric changes were evaluated. Treatment and control changes within the groups and the differences between the groups were analyzed statistically. Wilcoxon's paired sample test and Mann-Whitney U-test were used for statistical evaluation, at p<0.05 level.Results:At the start, more than 70 per cent subjects (range: 71.40–83.70 per cent) showed asymmetrical condylar movement in every plane of space. Significant change was determined in control group, for left condyle AZZ measurement, during observation period. Left AX change was found statistically higher in treatment group than the control (p<0.05). In the treatment group, SNB, U1-NA (mm) and U1-NA (degrees) measurements were decreased (p<0.05) and N-Me (p<0.05); SNA, SN-MP and ANS-Me (p<0.01); ANB, A to N perp and UL to E plane (p<0.001) measurements were increased, when compare to control.Conclusions:During the study period, left condyle anteroposterior position changed more and became closer to the centric relation position in the treatment group than the control. Rapid maxillary expansion and facemask therapy resulted in a significant improvement in the maxillomandibular relationship. Mandible moved downward and backward and maxilla moved forward direction without rotation

Synergism between two specialists should approve the results for favourable treatment outcomes. With collaboration, orthodontic therapy can enhance periodontal health and periodontal therapy can enhance the orthodontic outcomes. While technology provides us the interaction between specialties, biology establishes the basis of treatment. Orthodontist of the new century would consult experienced, responsible and enlightened periodontist. Patients undergoing orthodontic treatment must be informed about possible periodontal risks, gingival overgrowth and alveolar bone loss. In the present article conventional and current theraphies including acellular dermal matrix, enamel matrix proteins and periodontal plastic surgeries and a new orthognatic surgery alternative &ldquo;periodontally accelerated osteogenic orthodontics&rdquo; will be discussed. Also we will try to give answers to following questions: &ldquo;Does orthodontic treatment contribute to periodontal disease?&rdquo;, &ldquo;Could periodontally infected patiensts be treated with orthodontic tooth movements?&rdquo;, &ldquo;Which and when a surgical approach should be made to patients?&rdquo;, &ldquo;Which treatment options give best outcomes and less harm to periodontal tissues?&rdquo;, &ldquo;How frequent will orthodontic patients be followed by a periodontist?&rdquo;.     Prevalence of Maxillary Permanent Canine Impaction in Relation to Anomalous Lateral Incisors

    Objective:The aim of this study was to document the prevalence of maxillary permanent canine impaction in relation to anomalous adjacent lateral incisors in a Turkish population.Materials and MethodsPretreatment orthodontic records of 68 subjects (27 male, 41 female) with palatal or buccal impaction of one or both permanent maxillary canines were included in this study. Localization of the impacted permanent canines was determined by using cone-beam computed tomography images and classified as buccal or palatal impaction. Maxillary lateral incisors were recorded as normal, small, peg shaped, impacted, or missing using 3-dimensional digital models. The percentage of the total sample in each group was calculated, and the significance of the relationship between canine impaction and anomalous lateral incisors was examined with the Pearson v2 test at a significance level of 0.05.Results:The overall prevalence of peg-shaped maxillary permanent lateral incisors was found to be 18.3%. Impaction of the maxillary canine was 5.18 times more common in females than males, and palatal canine impaction was almost 1.27 times more common than the buccal impaction.Conclusion:Palatally impacted maxillary permanent canines were more common than buccally impacted canines and occurred more often in female subjects. Also, the prevalence of maxillary canine impaction in association with anomalous lateral incisors was different among Turkish males and females

    Consistency of Treatment Planning Decisions in Class II Malocclusions Using Digital and Plaster Models

    Aim:The aim of this study is to evaluate the treatment-planning value of digital models compared with plaster models and determine the consistency of treatment decisions.Materials and Method:Twenty-eight models of Class II malocclusion subjects were used for treatment planning by 3 orthodontists. The orthodontists decided on a treatment plan using digital models at the first session and a plaster model in the second session. Treatment decisions were classified and recorded as extraction, distalization, functional appliance, or auxiliary treatments. McNemar test and kappa statistics were used for statistical analysis. Statistical significance was set at p< 0.05.Results:No statistically significant difference was found between the treatment-plan decisions using digital or plaster models. Kappa statistics was fair for auxiliary (K=0.222), moderate for distalization (K=0.222), and substantial for extraction (K=0.634) and functional appliance (K=0.771) procedures.Conclusion:The results of this study revealed that digital models can be used for treatment planning as an alternative to plaster models
