    Impact of Self and Therapists’ Evaluation of Responses to Miracle Question and Goal: Survey of Japanese Undergraduate and Graduate Students

    La Terapia Breve Centrada en Soluciones es una psicoterapia que no se centra en el problema, sino en la clarificación de los objetivos terapéuticos de la persona y en la extensión de la excepción. El propósito de este estudio fue examinar los efectos de las evaluaciones de la clarificación de objetivos tanto de los clientes como de los terapeutas sobre las variables que se consideran importantes en la SFBT. Un total de 223 participantes, quienes respondieron a todos los cuestionarios, fueron los sujetos de análisis. Los resultados de este estudio mostraron una correlación entre las evaluaciones propias y las de los terapeutas de la respuesta a la pregunta por el milagro, la concreción del objetivo terapéutico y la realidad de dicho objetivo. Por otro lado, ninguna de las relaciones estuvieron fuertemente correlacionadas, lo que confirma que la autoevaluación y la evaluación de los terapeutas diferían en cierta medida. La evaluación relacionada con los objetivos del terapeuta tuvo un impacto positivo en la construcción de soluciones, la orientación de soluciones y el análisis causal. Además, la autoevaluación relacionada con la pregunta por el milagro y los objetivos terapéuticos tuvo un impacto positivo en la autoeficacia y el grado de resolución de problemas. Estos hallazgos indican que las autoevaluaciones y las evaluaciones de los terapeutas, relacionadas con la pregunta por el milagro y el objetivo terapéutico, tienen un efecto diverso en la resolución de problemas

    درمان فشرده راه حل محور نوعی روان درمانی است که بر مشکل تمرکز نمی‌کند بلکه بر شفاف‌سازی وضعیت هدف و گسترش استثنائات متمرکز است. هدف این مطالعه، بررسی تاثیرات ارزیابی مراجعین و درمانگران درباره شفاف‌سازی هدف درباره متغیرهایی است که در SFBT مهم تلقی می‌شوند. به طور کلی، 223 شرکت‌کننده به همه پرسشنامه‌ها پاسخ دادند و در این پژوهش شرکت کردند. نتایج این مطالعه همبستگی بین ارزیابی‌های فردی و درمانگران را برای تصورات مربوط به پرسش معجزه، ملموس بودن هدف آن و واقعیت هدف آن، نشان می‌دهد. از طرف دیگر، هیچ یک از روابط، همبستگی قدرتمندی نداشتند و این موضوع تایید می‌کند که ارزیابی فردی و ارزیابی درمان‌گر تا حد معینی با هم تفاوت دارند. ارزیابی هدف محور از سوی درمانگر، تاثیر مثبتی روی راه حل سازی، جهت‌گیری راه حل‌ها و تحلیل‌های علّی داشت. به علاوه‌، ارزیابی فردی مراجع از پرسش و هدف چینی پرسش معجزه، تاثیر مثبتی روی خودکارآمدی و میزان حل مسئله داشت. این یافته‌ها نشان دادند که ارزیابی‌های مراجع و درمانگران مربوط به پرسش معجزه و هدف آن، تاثیرات متفاوتی روی حل مسئله دارد

    Lösungsfokussierte Kurztherapie ist eine Psychotherapie, die nicht auf das Problem konzentriert, sondern auf die Klärung des Zielzustands und die Erweiterung der Ausnahme. Zielsetzung dieser Studie war es, die Auswirkungen der Bewertung der Zielklärung durch Klient:innen und Therapeut:innen auf die in der SFBT als wichtig erachteten Variablen zu untersuchen. Insgesamt 223 Teilnehmer:innen, die alle Fragebögen bearbeiteten, waren Gegenstand der Analyse. Die Ergebnisse dieser Studie zeigten eine Korrelation zwischen Selbst- und Therapeut:inneneinschätzungen für die Vorstellung der Wunderfrage, die Konkretheit des Ziels und die Realität des Ziels. Andererseits war keine der Beziehungen stark korreliert, was bestätigt, dass sich die Selbsteinschätzung und die Einschätzung der Therapeut:innen in gewissem Ausmaß unterscheiden. Die zielbezogene Bewertung durch die Therapeut:innen hatte einen positiven Einfluss auf die Lösungsentwicklung, die Lösungsorientierung und die Ursachenanalyse. Außerdem hatte die Selbsteinschätzung in Bezug auf die Wunderfrage und das Ziel einen positiven Einfluss auf die Selbstwirksamkeit und den Grad der Problemlösung. Diese Ergebnisse deuten darauf hin, dass die Einschätzungen von Klient:innen und Therapeut:innen in Bezug auf die Wunderfrage und das Ziel einen vielfältigen Einfluss auf die Problemlösung haben

    La thérapie brève orientée sur les solutions est une psychothérapie qui ne se concentre pas sur le problème, mais sur la clarification de l\u27état désiré (l’objectif) et l\u27extension de l\u27exception. Le but de cette étude était d\u27examiner les effets des évaluations de la clarification des objectifs par les patients et les thérapeutes sur les variables considérées comme importantes dans la SFBT. Au total, 223 participants ont répondu à tous les questionnaires et ont fait l\u27objet de l\u27analyse. Les résultats de cette étude ont montré une corrélation entre les évaluations des clients et des thérapeuteset la formulation de la Question Miracle, le caractère concret de l\u27objectif et la réalité de l\u27objectif. En revanche, aucune des relations n\u27était fortement corrélée, confirmant que l\u27auto-évaluation et l\u27évaluation des thérapeutes différaient dans une certaine mesure. L\u27évaluation liée aux objectifs par le thérapeute a eu un impact positif sur la construction de solutions, l\u27orientation solution et l\u27analyse causale. En outre, l\u27auto-évaluation des patients liée à la question miracle et à l\u27objectif a eu un impact positif sur l\u27auto-efficacité et le degré de résolution de problèmes. Ces résultats indiquent que les évaluations des patients et des thérapeutes liées à la question miracle et à l\u27objectif ont un effet diversifié sur la résolution de problèmes

    Solution-Focused Brief Therapy is a psychotherapy which does not focus on the problem, but on the clarification of the goal state and the extension of the exception. The purpose of this study was to examine the effects of both clients’ and therapists’ evaluations of goal clarification on the variables which is considered important in SFBT. A total of 223 participants who responded to all questionnaires were the subject of the analysis. The results of this study showed a correlation between self and therapists’ evaluations for the imagination of the Miracle Question, the concreteness of the goal, and the reality of the goal. On the other hand, none of the relationships were strongly correlated, confirming that the self evaluation and the therapists’ evaluation differed to a certain extent. Goal-related evaluation by the therapist had a positive impact on solution building, solution orientation, and causal analysis. Besides, self evaluation related to Miracle Question and goal had a positive impact on self-efficacy and degree of problem-solving. These findings indicate that self and therapists’ evaluations related to Miracle Question and goal have a diverse effect on problem-solving

    The Diagnosis and Treatment of Early-Stage Colorectal Cancer

    The introduction of colorectal endoscopic submucosal dissection (ESD) has expanded the applications for endoscopic treatment; as a result, lesions with low metastatic potential can be treated endoscopically regardless of the lesion size. The most attractive feature of ESD is the achievement of en bloc resection with a lower local recurrence rate in comparison to that of endoscopic piecemeal mucosal resection. However, in case of gastric cancers, ESD is not as widely applied to the treatment of colorectal neoplasms because of its technical difficulty, longer procedural time, and increased perforation risk. In the movement toward diversified endoscopic treatment strategies for superficial colorectal neoplasms, endoscopists who begin to perform ESD need to recognize the indications of ESD, as well as the technical issues and associated complications of this procedure

    Detectability of Colon Polyp Using Computed Virtual Chromoendoscopy with Flexible Spectral Imaging Color Enhancement

    The aim of this pilot study was to assess the feasibility of using computed virtual chromoendoscopy with the flexible spectral imaging color enhancement (FICE) for colon neoplasia screening. A modified back-to-back colonoscopy using FICE and white light in the right-sided colon was conducted prospectively for the consecutive patients attending for the postoperative (sigmoidectomy or anterior resection) follow-up colonoscopy. Histopathology of detected lesions was confirmed by evaluation of endoscopic resection or biopsy specimens. One-hundred and two patients were enrolled, and 100 patients (61 males and mean age 63 years) were finally analyzed. The total number of polyps detected by FICE and white light colonoscopy was 65 and 45, respectively. The miss rate for all polyps with FICE (24%) was significantly less than that with white light (46%) (P = 0.03). Colonoscopy using FICE could beneficially enhance the detection of neoplastic lesions in the right-sided colon compared to white light colonoscopy

    Visualization of Laterally Spreading Colorectal Tumors by Using Image-Enhanced Endoscopy

    Laterally spreading tumors may sometimes evade detection by colonoscopy. This study aimed to evaluate the use of image-enhanced endoscopy for visualizing laterally spreading tumors of the nongranular type. We reviewed consecutive patients with 47 non-granular-type laterally spreading tumors that had been examined using white-light imaging, autofluorescence imaging, narrow-band imaging, and chromoendoscopy with indigo carmine. The quality of visualization was evaluated using a 5-point scale by less- and more-experienced endoscopists. Autofluorescence imaging provided significantly better visualization than white-light imaging for both less-experienced and experienced endoscopists. On the other hand, no significant differences were observed between the quality of visualization provided by white-light imaging and narrow-band imaging for less-experienced endoscopists. Autofluorescence imaging provides high-quality visualization of non-granular-type laterally spreading tumors on still images. Multicenter trials should be conducted to confirm the usefulness of autofluorescence imaging in detecting laterally spreading colorectal tumors

    High-resolution mapping of in vivo genomic transcription factor binding sites using in situ DNase I footprinting and ChIP-seq

    Accurate identification of the DNA-binding sites of transcription factors and other DNA-binding proteins on the genome is crucial to understanding their molecular interactions with DNA. Here, we describe a new method: Genome Footprinting by high-throughput sequencing (GeF-seq), which combines in vivo DNase I digestion of genomic DNA with ChIP coupled with high-throughput sequencing. We have determined the in vivo binding sites of a Bacillus subtilis global regulator, AbrB, using GeF-seq. This method shows that exact DNA-binding sequences, which were protected from in vivo DNase I digestion, were resolved at a comparable resolution to that achieved by in vitro DNase I footprinting, and this was simply attained without the necessity of prediction by peak-calling programs. Moreover, DNase I digestion of the bacterial nucleoid resolved the closely positioned AbrB-binding sites, which had previously appeared as one peak in ChAP-chip and ChAP-seq experiments. The high-resolution determination of AbrB-binding sites using GeF-seq enabled us to identify bipartite TGGNA motifs in 96% of the AbrB-binding sites. Interestingly, in a thousand binding sites with very low-binding intensities, single TGGNA motifs were also identified. Thus, GeF-seq is a powerful method to elucidate the molecular mechanism of target protein binding to its cognate DNA sequences
