Connatalis syphilis = Congenital syphilis
A syphilis az elmúlt évtizedekben ismét egyre gyakrabban diagnosztizált betegség. Előretörésével számítanunk kell a congenitalis syphilis megjelenésére is. Syphilis állhat spontán, illetve habituális vetélés, halvaszülés hátterében. Az intrauterin élet során szerzett syphilisszel világra jövő újszülöttek gyakran tünetmentesek, és a fertőzés által okozott szervi károsodások csak hónapokkal vagy akár évekkel később válnak nyilvánvalóvá. A betegség megelőzhető a terhes nők szűrésével és kezelésével. Minden újszülött, aki potenciálisan syphilisszel fertőződhetett, kezelendő a megszületés napjától kezdve. A syphilis elleni kezelés szuverén szere ma is a penicillin.

Syphilis has been a re-emerging disease in the past few decades. As a consequence, the prevalence of congenital syphilis is expected to be on the rise. Maternal syphilis may be related to several pathologies, such as miscarriage, stillbirth, or congenital syphilis in the child. Infants that acquire syphilis in utero are frequently asymptomatic, and the organ damage caused by the infection may be apparent only years later. Syphilis is a curable disease, and most of its complications in the infant can be prevented by screening and treating the mother. Every newborn potentially infected should be treated with penicillin immediately starting on the day of birth
A normoglykaemia elérésének korlátai inzulinkezelt 2-es tipusú cukorbetegekben
Insulin therapy is the most effective treatment of diabetes. It is proven to prevent microvascular disease and likely to decrease the risk of cardiovascular complications. However, these benefits are associated with a 2-3 times increased risk of hypoglycaemia and a faster weight gain compared to other antidiabetic medications. In addition, one study found elevated all-cause mortality among patients on intensive therapy (requiring more frequent insulinisation). Insulin has growth factor properties that may translate to increased mitogenicity. These factors could prevent the medical team or the patient from initiation or intensification of insulin therapy. The authors describe evidence on long-term remission related to transient intensified insulin therapy at diabetes diagnosis. The currently recommended method of insulin initiation is once daily basal insulin treatment that offers different schedules for intensification. The authors review the pharmacokinetics of analogue insulins that translate to similar efficacy to human insulins with a 20-30% lower risk of hypoglycaemia. Orv. Hetil., 2015, 156(36), 1443-1450
Relationship between serum calcium and CA 19-9 levels in colorectal cancer
AIM: To examine the calcium metabolism of colorectal cancer (CRC) in patients with colorectal cancer and control patients.
METHODS: Seventy newly diagnosed CRC patients were included. The healthy control group was age and gender matched (n=32). Particular attention was devoted to the relationship between serum calcium of patients, and levels of AFP, CEA, carbohydrate antigen 19-9 (CA 19-9) (that could be considered as prognostic factors). Furthermore, the Ca-sensing receptor (CaSR) gene A986S polymorphism was investigated in these patients, as well as the relationship between different CaSR genotypes and the data stated above.
RESULTS: A lower level of ionized calcium (also corrected for albumin) was found in the serum of CRC patients with normal 25(OH) vitamin D levels. The ionized calcium concentration was inversely correlated with the serum level of CA 19-9. There was no difference in the distribution of CaSR genotypes, between CRC patients and general population. The genotypes did not correlate with other data examined.
CONCLUSION: Based on these results, lower levels of serum calcium might be a pathogenic and prognostic factor in colorectal cancer

1-es és a 2-es típusú cukorbetegek terhessége során romolhatnak a diabéteszes szövődmények. Kevéssé ismert azonban a terhesség hatása a betegség hosszútávú kimenetelére. Nem tisztázottak a gesztációs diabétesz (GDM) késői anyai következményei sem. Jelen vizsgálatban hasonló módszerekkel pregesztációs (1-es és 2-es típusú) diabéteszben és korábban GDM-ben szenvedő (kGDM) asszonyokat vizsgáltunk évekkel szülésük után. A terhességük előtt is cukorbeteg asszonyokban gyakori a túlsúly, a hypertonia és a dohányzás. A kGDM kohorszban összefüggést találtunk a glukóz intolerancia (GI) súlyossága és a metabolikus szindróma gyakorisága között. GI asszonyokban magasabb osteoprotegerin szinteket, az oGTT során károsodott leptinválaszt észleltünk. kGDM betegeinkben magasabb resistin és alacsonyabb adiponectin értékeket találtunk. Az adiponectin negatívan korrelált ismert CV kockázati tényezőkkel. kGDM asszonyokban fokozott CV kockázatot igazoltunk a Framingham rizikópontszám alkalmazásával kontrollokhoz képest. Károsodott volt a centrális érfali rugalmasságot jellemző carotis-femoralis pulzushullám sebesség, ill. az endothelfüggő vazodilatációt jellemző postocclusiv reaktív hyperaemia index is kGDM-ben. Független kapcsolatot találtunk a D3 vitamin szint, az inzulinérzékenység és a beta-sejt működés között. Eredményeink alapján mind a pregesztációs, mind a gesztációs diabétesz szülést követő szoros követése javasolt a szövődmények kialakulásának kivédésére, késleltetésére. | Diabetic complications can progress during pregnancy of type 1 or type 2 diabetic women. However the long term consequences of a diabetic pregnancy are not well described. Similarly there is uncertainty regarding the late maternal consequences of gestational diabetes (GDM). This study aimed to investigate women with pregestational (type 1 and type 2) diabetes and with prior gestational diabetes (pGDM) using similar methodology years after delivery. In women with pregestational diabetes high frequency of obesity, hypertension, and smoking was found. In the pGDM cohort we found a dose-response relationship between the severity of glucose intolerance (GI) and the prevalence of metabolic syndrome (MS). Among women with GI we found elevated osteoprotegerin levels and an abnormal leptin response during an oGTT. In pGDM women resistin levels were elevated, adiponectin levels decreased. Adiponectin negatively correlated with several CV risk factors. In pGDM women an elevated CV risk was proven using the Framingham risk score compared to controls. pGDM women had abnormal carotid-femoral pulse wave velocity and postocclusive reactive hyperemia index that measures endothel-dependent vasodilation. There was an independent relationship between levels of vitamin D3 and insulin sensitivity and beta-cell function. Our results support the recommendation for the follow-up of women with gestational or pregestational diabetes to prevent or delay its late complications
A diabetes és a depresszió együttes előfordulása, lehetséges kétirányú kapcsolata és szűrésük jelentősége
Absztrakt:
A cukorbetegség és a depresszió világszerte népbetegség. Mindkettő kialakulása
multifaktoriális, amelyben szerepet játszhatnak külső, környezeti tényezők,
valamint belső, a szervezetben játszódó folyamatok, többek között különböző
gének nem megfelelő működése. Bár a diabetes és a depresszió komorbiditása jól
ismert, a két betegség hátterében zajló folyamatok, illetve ezen folyamatok
egymásra hatása nem minden részletet illetően tisztázott. A szerzők összefoglaló
közleményének célja bemutatni a 2-es típusú cukorbetegség és a depresszió
közötti kétirányú kapcsolat hátterében álló ismert és még csak feltételezett
mechanizmusokat, illetve felhívni a figyelmet a két betegség egyidejű szűrésének
és kezelésének fontosságára, hiszen együttes fennállásukkor nőhet a morbiditás
és a mortalitás. A cukorbetegek depresszióra irányuló szűrésével és a
depressziós betegek diabetes irányú kivizsgálásával csökkenthető lehet a
komorbiditásból származó komplikációk és szövődmények, illetve a kezelési
nehézségek előfordulása. A két betegség ellátásában fontos szerepe van a
családorvosnak, aki a korai diagnózissal és kezeléssel javíthatja a betegek
életminőségét, lassíthatja a betegségek progresszióját és a szövődmények

Abstract:
Diabetes and depression are considered global epidemics. Both have multifactorial
aetiologies, including external, environmental factors and internal factors in
connection with physiological processes as well as genetic variants triggering
disease onset. Although the co-occurrence of diabetes and depression is well
described, the mechanisms underlying these diseases and their interactions are
still not entirely revealed. The authors aimed to present known and potential
explanations of the co-occurrence of these diseases and to highlight the
importance of their timely diagnosis and effective treatment, as their
co-occurrence may increase morbidity and mortality. Screening for depression
among diabetes patients and for diabetes among patients with depression may
decrease the incidence of complications and consequences of this comorbidity and
may foster more effective treatment. General practitioners play a key role in
the care of both diseases by providing timely diagnosis and adequate treatment
potentially leading to a better quality of life, slower disease progression and

Real-world effectiveness of IDegLira compared with intensified conventional insulin therapy in adults with type 2 diabetes: a retrospective cohort study
Background:
IDegLira is a fixed-ratio combination of insulin degludec and liraglutide with proven efficacy against simpler regimens and non-inferiority against basal-bolus insulin therapy. However, the evaluation of its real-world effectiveness is hindered by technical issues and requires further exploration. Thus we aimed to compare effectiveness of insulin degludec/liraglutide (IDegLira) versus intensified conventional insulin therapy (ICT) for type 2 diabetes in a real-world setting.
Methods:
This retrospective cohort study from an outpatient clinic in Hungary included people who initiated IDegLira due to inadequate glycaemic control (HbA1c > 7.0% [53.0 mmol/mol]) with oral and/or injectable antidiabetic drugs. Data were compared with a historical cohort who initiated ICT. Outcomes included HbA1c, body weight, and hypoglycaemia differences over 18 months of follow-up.
Results:
Data were included from 227 and 72 people who initiated IDegLira and ICT, respectively. Estimated mean difference (MD) in HbA1c at 18 months favoured IDegLira versus ICT (MD 0.60, 95% CI 0.88–0.32 [MD 6.6 mmol/mol, 95% CI 9.6–3.5]). More people reached target HbA1c ≤7.0% (53.0 mmol/mol) with IDegLira than ICT (odds ratio 3.36, 95% CI 1.52–7.42). IDegLira treatment was associated with weight loss compared with gain for ICT (MD 6.7 kg, 95% CI 5.0–8.5). The hazard ratio for hypoglycaemia comparing IDegLira with ICT was 0.18 (95% CI 0.08–0.49).
Conclusions:
Treatment with IDegLira over 18 months resulted in greater HbA1c reductions, weight loss versus gain, and a lower rate of hypoglycaemia versus ICT in people with type 2 diabetes

Bevezetés és célkitűzés: Kutatási célunk a 2011 és 2015 közötti
magyarországi szülések feldolgozásának segítségével gesztációs hetekre, nemekre,
valamint egyes és többes terhességből született magzatokra bontott aktuális
percentilistáblázat megalkotása. Emellett vizsgáltuk, hogy 1996 és 2015 között
ötéves periódusokban hogyan változtak gesztációs hetenként az egyes és iker fiú-
és leánymagzatok átlag születési súlyai. Módszer: A
Magyarországon kötelezően kitöltendő és vezetett Tauffer-statisztika 2011 és
2015 közötti adatai alapján minden gesztációs héthez kiszámoltuk az 5, 10, 25,
50, 75, 90 és 95 percentilisértéket nemenként, egyes és ikerterhességek esetén.
Vizsgáltuk továbbá a születési súlyok terhességi hetenkénti változását 1996 és
2015 között ötéves periódusokban. Eredmények: A 2011 és 2015
közötti élveszületések súly-percentilisértékeit grafikusan ábrázoltuk, és
táblázatokban is összefoglaltuk. A vizsgált 20 év alatt az egyes terhességekben
a 35–41. héten a születési súly 2011–2015-höz viszonyítva az 1996–2005-ig
terjedő időszakban alacsonyabb volt (a legalacsonyabb 1996–2000-ben), míg a
2006–2010-es periódusban magasabb vagy hasonló volt (például a 38. héten a
fiúknál 2011–2015-ben az átlagsúly 3249 g, 1996–2000-ben 34,3 [SE 3,0] g-mal,
2001–2005-ben 11,5 [2,9] g-mal kevesebb, 2006–2010-ben 18,1 [2,9] g-mal több).
Az ikerterhességekben hasonló, de nem egyértelmű tendenciát figyeltünk meg a
35–38. heti születési súlyokban. Következtetés: Tekintettel a
születési súlyok elmúlt 20 évben megfigyelt jelentős változására, szükséges az
általában használt percentilistáblázat megújítása. A születési súlyok 1996 és
2010 között növekedtek, elsősorban az érett magzatok esetén, az ezt követő

Abstract:
Introduction and aim: We aimed to provide a current birth weight
percentile table for singleton and twin pregnancies stratified by gestational
week at delivery and sex using data from all live births in Hungary between 2011
and 2015. In addition, we examined temporal trends in average birth weights in
singleton and twin pregnancies by sex in five-year periods between 1996 and
2015. Method: We calculated the 5th, 10th, 25th, 50th, 75th,
90th, and 95th centiles of birth weight for each gestational week by sex for
singleton and twin pregnancies using compulsory collected obstetrical data
(Tauffer Statistics) in Hungary in 2011–2015. Furthermore, we described changes
in birth weights by gestational week between 5-year periods from 1996 to 2015.
Results: We present birth weight centiles for live births
in both tabular and graphical forms using data from 2011 to 2015. In general,
live birth weights in gestational weeks 35–41 were lower in the period of
1996–2005 (the lowest in 1996–2000) and were higher in the period of 2006–2010
compared to the reference period of 2011–2015 (e.g., the average male newborn
weighed 3249 g at gestational week 38 in 2011–2015, which is 34.3 [SE at 3.0] g
less in 1996–2000, 11.5 [2.9] g less in 2001–2005, and 18.1 [2.9] g more in
2006–2010). Similar trends were not observed in birth weights of twin
pregnancies in gestational weeks 35–38. Conclusion: Given the
observed substantial change in birth weights during the past 20 years, renewal
of the commonly used percentile tables is necessary. Birth weights increased
from 1996 to 2010, mainly of mature newborns, followed by a stabilization or

Pregnancy outcomes of women with untreated ‘mild’ gestational diabetes (gestational diabetes by the WHO 2013 but not by the WHO-1999 diagnostic criteria) – A population-based cohort study
AIMS: We compared pregnancy outcomes of untreated 'mild' GDM (GDM by WHO 2013 but not by WHO-1999) to normal glucose tolerant women (NGT). METHODS: In a universal screening program 4333 pregnant women had a 3-point 75 g OGTT in Hungary in 2009-2013. By WHO-2013 untreated NGT was diagnosed in n = 3303, 'mild' GDM in n = 336 cases. RESULTS: 'Mild' GDM women were older (mean difference, SE: 1.4, 0.3 yrs), had higher fasting (1.0, 0.02), 60-minute (1.0, 0.09), and 120-minute (0.4, 0.06 mmol/l) blood glucose, and blood pressure (2.6, 0.5/2.0, 0.5 mmHg). Weight gain was similar in both groups (-0.3, 0.3 kg). GDM newborns were heavier (142, 50 g) and were more frequently macrosomic (>4000 g, OR 1.85, 95 %CI 1.35-2.54). Hypertension during pregnancy was more prevalent in the GDM group (OR 1.55, 95 %CI 1.05-2.28), as well as induced (OR 1.38, 95 %CI 1.10-1.74) and instrumental delivery (OR 1.34, 95 %CI 1.07-1.68), and acute caesarean section (OR 1.32, 95 %CI 1.04-1.64). Most of these differences substantially attenuated or became non-significant after adjustment for pre-pregnancy BMI. CONCLUSIONS: Pregnancy outcomes of 'mild' GDM were worse compared to normal glucose tolerant women however these differences were explained by the pre-pregnancy BMI difference between groups
Urinary albumin-to-creatinine ratio and serum albumin are predictors of acute kidney injury in non-ventilated COVID-19 patients: a single-center prospective cohort study
PURPOSE: Acute kidney injury (AKI) is a frequent complication among COVID-19 patients in the intensive care unit, but it is less frequently investigated in general internal medicine wards. We aimed to examine the incidence, the predictors of AKI, and AKI-associated mortality in a prospective cohort of non-ventilated COVID-19 patients. We aimed to describe the natural history of AKI by describing trajectories of urinary markers of hemodynamic, glomerular, and tubular injury. METHODS: 141 COVID-19 patients were enrolled to the study. AKI was defined according to KDIGO guidelines. Urine and renal function parameters were followed twice a week. Multivariate logistic regression was used to determine the predictors of AKI and mortality. Trajectories of urinary markers were described by unadjusted linear mixed models. RESULTS: 19.7% patients developed AKI. According to multiple logistic regression, higher urinary albumin-to-creatinine ratio (OR 1.48, 95% CI 1.04-2.12/1 mg/mmol) and lower serum albumin (OR 0.86, 95% CI 0.77-0.94/1 g/L) were independent predictors of AKI. Mortality was 42.8% in the AKI and 8.8% in the group free from AKI (p < 0.0001). According to multiple logistic regression, older age, lower albumin, and AKI (OR 3.9, 95% CI 1.24-12.21) remained independent predictors of mortality. Urinary protein-to-creatinine trajectories were diverging with decreasing values in those without incident AKI. CONCLUSION: We found high incidence of AKI and mortality among moderately severe, non-ventilated COVID-19 patients. Its development is predicted by higher albuminuria suggesting that the originally damaged renal structure may be more susceptible for virus-associated effects. No clear relationship was found with a prerenal mechanism, and the higher proteinuria during follow-up may point toward tubular damage
The impact of COVID-19 infection before the vaccination era on the hospitalized patients requiring hemodialysis: a single-center retrospective cohort
Due to effective vaccinations, the COVID-19 (coronavirus disease 2019) infection that caused the pandemic has a milder clinical course. We aimed to assess the mortality of hospitalized COVID-19 patients before the vaccination era. We investigated the mortality in those patients between 1 October 2020 and 31 May 2021 who received hemodialysis treatment [patients with previously normal renal function (nCKD), patients with chronic kidney disease previously not requiring hemodialysis (CKDnonHD), chronic kidney disease (CKD), and patients on regular hemodialysis (pHD)]. In addition, participants were followed up for all-cause mortality in the National Health Service database until 1 December 2021. In our center, 83 of 108 (76.9%) were included in the analysis due to missing covariates. Over a median of 26 (interquartile range 11-266) days of follow-up, 20 of 22 (90.9%) of nCKD, 23 of 24 (95.8%) of CKDnonHD, and 17 of 37 (45.9%) pHD patients died (p < 0.001). In general, patients with nCKD had fewer comorbidities but more severe presentations. In contrast, the patients with pHD had the least severe symptoms (p < 0.001). In a model adjusted for independent predictors of all-cause mortality (C-reactive protein and serum albumin), CKDnonHD patients had increased mortality [hazard ratio (HR) 1.91, 95% confidence interval (CI), 1.02-3.60], while pHD patients had decreased mortality (HR 0.41, 95% CI 0.20-0.81) compared to nCKD patients. After further adjustment for the need for intensive care, the difference in mortality between the nCKD and pHD groups became non-significant.
