The Effectiveness of Screening Applications in Breast Cancer
The effectiveness of screening practices in breast cancer is known. However, are these screening applications effective in our
country? In order to study this issue, our hospital was selected as the screening hospital. The cases that had been operated before and after the selection were compared in our hospital.
A total of 288 patients were included. Of these 288 patients, 285 (99%) were female and 3 were male. The average age of the
patients who were admitted was 56.70±13.23 (55,17-58,24). When the breast with the involvement was examined; it was seen
that 153 were left breast and 135 were right breast. When the tumor size was examined, it was found that 159 (55.2%) patients
had a tumor size of<2 cm, 116 (40.3%) patients had 2-5 cm, and 13 (4.5) patients had a tumor size of>5 cm. 101 patients (35.1%
4) had stage 1, 78 patients (27.1%) had stage 2A, 45 patients (15.6%) had stage 2B, 38 patients (13.2%) had stage 3A, 23 patients
(8%) had stage 3C, and 1 patient (0.3%) had stage 4.
203 (70%) patients were pre-screening patient group and were referred to as Group A. 85 (30%) were post-screening group and
were called Group B.
Of the 203 patients in group A, the mass in 100 patients was left-localized. When the size of the tumor in group A was examined,
it was determined that 107 (52.7%) patients had tumor sizes of<2 cm, 86 (42.4%) patients 2-5 cm and 10 (4.9%) patients with a
tumor size of>5 cm. It was detected that 67 patients (33%) had stage 1, 55 patients (27.1%) had stage 2A, 31 patients (15.3%) had
stage 2B, 29 patients (14.3%) had stage 3A, 21 patients (10.3%) had stage 3C, and 1 patient (0.04%) had stage 4.
Of the 85 patients in group B, 53 (62.4%) patients had left breast localized mass. When tumor size was examined, it was found
that 52 (61.2%) patients had a size of 2 cm, 30 (34.2%) patients had 2-5 cm and 3 (3.5%) patients had a size of>5 cm. It was found
that 36 (42.4%) patients were stage 1, 23 (27.1%) were stage 2A, 14 (16.5%) patients were stage 2B, 9 (10.6%) patients were stage
3A and 3 (3.5%) patients were stage 3C.
There was no difference between the two groups in terms of all data
An extra-adrenal paraganglioma treated with radiotherapy
Abdominal paragangliomas (PG) are usually misdiagnosed as retroperitoneal masses originated from pancreas, kidney or adrenal gland. Although the optimum treatment of the disease is surgical excision, resection may not be proper secondary to advanced disease or close relationship with the main vessels.Case:A 72 year old female patient was suffering from right lumbar pain and feeling of fullness for last 1 year. In another medical center a computed abdomen tomography (CT) had been carried out which was showing a pancreatic mass. For further evaluation she was referred to our clinic. Physical examination revealed bilateral grade 2 lower extremity edema and an appendectomy scar. We repeated CT and performed three-dimensional reconstruction revealing of a 35x52x42 mm sized hypervascular mass which was separate from the pancreas. This mass was partially surrounding inferior vena cava and entering the aorta-caval groove. CT-guided trucut biopsy was performed. Pathology resulted as benign, low mitotic activity PG. She was diagnosed as extra-adrenal paraganglioma (EAPG). Being close to the main vessels, instead of surgical excision radiotherapy (RT) was decided. After completion of the treatment, abdominal pain and lower extremity edema disappeared. 6 months after RT on control CT, there was no progression in the size of the mass.PG is a rare tumor. Because of the risk of malignant transformation, decided treatment should be surgery. After confirmation of the diagnosis, for paragangliomas which has close relationship to main vessels RT is an alternative treatment
Comparison of Efficiencies of Laparoscopic Transversus Abdominis Plane Block and Local Anesthesia at Trocar Site After Laparoscopic Appendectomy
Objective: The most common operation performed in the general surgery practice is appendectomy. With the spread of laparoscopy, minimally invasive procedures have become more preferable and less pain, early mobilization, early return to work and
better cosmetic results have been obtained in the postoperative period. A method for reducing postoperative pain is the ultrasound-guided transversus abdominis plane (TAP) block. The aim of this study was to compare the efficacy of local anesthetic (LA)
infiltration in the trocar site with transversus abdominis plane block performed under direct laparoscopic view without using
ultrasound in patients undergoing laparoscopic appendectomy for acute appendicitis.
Material and Methods: The patients who underwent laparoscopic appendectomy due to the diagnosis of appendicitis in the
Department of General Surgery at Health Sciences University, Fatih Sultan Mehmet Education and Research Hospital between
August 2017 and January 2018 were randomized into two groups. Group 1 was performed TAP block under laparoscopic view
and Group 2 was performed LA infiltration in the trocar site. The pain levels of the patients were calculated by using visual analogue score (VAS) at the postoeprative 0th, 2nd, 6th, 12th and 24th hours. The duration of operation, postoperative nausea and
vomiting complaints, the first times of gas-stool discharge, the first times of mobilization, duration of hospital stay, and the doses
of nonsteroidal analgesic drugs and opioids were recorded and compared. The statistical analysis was performed using SPSS version 20.0 software. The p values below 0.05 were evaluated as statistically significant.
Results: There was no significant difference between the groups in terms of age, gender, BMI and ASA scores (p> 0.05). The VAS
values recorded at 0th, 2nd, 6th, 12th and 24th hours were significantly lower in the TAP block group than in the trocar site group.
The duration of hospital stay in the TAP block group was detected to be shorter (p: 0.002). Moreover, analgesic consumption in
TAP block group was lower compared to the trocar site group (p: 0,023).
Conclusion: The TAP block method applied by surgeon during laparoscopic appendectomy is more effective than local anesthesia application in the trocar site. Due to decreased analgesic use, intestinal movements may be returned in the early period and
this may shorten the length of hospitalization
Wernicke ensefalopatisi: Bariatrik cerrahide nadir bir komplikasyon
GİRİŞ: Wernicke Ensefalopatisi (WE) tiamin eksikliğine bağlı gelişen bir hastalıktır. Bariatrik cerrahi
geçirmiş hastalarda nadiren görülebilmektedir. Bu çalışmamızda laparoskopik tüp mide ameliyatı
geçirmiş, proksimal mide mukozasında ödeme ikincil kusma nedeniyle beslenmesi kısıtlanmış, WE gelişen
bir hastayı sunmayı amaçladık.
OLGU: İki hafta önce sleeve gastrektomi yapılmış hasta şiddetli kusma nedeniyle yatırıldı. Kusma
etyolojisi araştırılmak üzere çekilen bilgisayarlı tomografide (BT) proksimal mide mukozasında kalınlaşma
dışında patoloji tespit edilmedi. Endoskopide lüminal patoloji saptanmadı. Takiplerinde konfüzyon
gelişen hastanın nörolojik muayenesinde horizontal nistagmus ve sola bakış kısıtlılığı tespit edildi. Görme
kaybı, baş dönmesi, ayağa kalkamama, yürüme güçlüğü ve ciltte döküntüleri klinik tabloya eşlik etti.
Beyin manyetik rezonans görüntülemede (MRG) iki taraf talamusta T2-hiperintens lezyonlar ve vazojenik
ödem ile uyumlu difüzyon artışı görülmesi üzerine hastada WE düşünüldü. Multivitamin desteği
başlanılmış hastaya 1000 mg/gün damar içi tiamin replasmanı başlandı. Tedaviyi takiben ertesi gün içinde
hastanın görme kaybı geriledi. İkinci günde bilinç normal hale geldi. Devam eden günlerde, bulantı ve
kusması azalmayan hastada, nistagmus ve uyku hali azaldı, ancak tamamen gerilemedi. MR-anjiografi ile
EEG tetkiklerinde ek patoloji saptanmadı. Bir hafta boyunca yüksek doz tiamin replasmanı alan hastada
anlamlı düzelme izlenmedi. Enteral beslenme desteğine nazojejunal beslenme sondası ile başlanan
hastanın üç haftada bulguları geriledi. Yatışının 6. haftasında hafif nistagmus ve ataksi dışında şikayet
kalmadı. Taburculuğunun üçüncü ayında nörolojik muayenesi tamamen normaldi.
SONUÇ: Bariatrik cerrahi sonrası WE nadir görülen ama tehlikeli bir komplikasyondur. Bizim hastamızda
damar içi replasman kısmi fayda sağlamış, nazojejunal beslenme daha etkili olmuştur. WE düşünülüp,
damar içi tiamin tedavisine dirençli olan hastalarda enteral beslenmenin etkili olabileceğini düşünüyoru
Dren parathormonu bize yol gösterir mi?
GirişAmeliyat sonrası hipokalsemi, total tiroidektomi sonrası en sık görülen komplikasyondur. Buçalışmada total tiroidektomi uygulanan hastalarda dren sıvısı parathormon değerlerinin ameliyatsonrası hipokalsemiye etkisi değerlendirildi.Materyal&MetodEkim 2018- Mart 2019 tarihleri arasında total tiroidektomi uygulanan 23 hastanın D vitaminidüzeyleri, ameliyat öncesi ve sonrası kalsiyum ve parathormon değerleri, dren sıvısıparathormunu değerleri kaydedildi. Ameliyat sonrası hipokalsemi sınırı 8.5 gr/dl olarak belirlendi.Değerler normokalsemik (n:13) ve hipokalsemik (n:10) hastalar arasında karşılaştırıldı.Bulgular21 (% 91,3) kadın ve 2 (%8,7) erkek hastanın ortalama yaş değerleri 47.1±12.4. Gruplar arasındacinsiyet dağılımı eşitti, yaş karşılaştırıldığında normakalsemik hastaların istatistiksel olarak dahagenç olduğu izlendi (41,2±11,8 vs 51,6±11,2; p:0,042). Gruplar arasında D vitamini, ameliyatöncesi parathormon ve kalsiyum değerleri açısından istatistiksel olarak anlamlı fark saptanmadı(sırasıyla p:0.2, p:0.2, p:0.06). Dren parathormonları incelendiğinde iki grup arasında istatistikselfark izlenmedi (hipokalsemik vs normokalsemik, medyan 3153,7(123-12699) vs 2115,3(93-6487)ng/ml; p:0.9). Ameliyat sonrası parathormon değerlerinde iki grup arasında fark izlenmedi (p:0.09).TartışmaGüncel pratikte ameliyat sonrası hipokalsemi için risk faktörleri belirlemek ve erken tanı koymakönemlidir. Bu çalışmamızda dren sıvısı parathormon düzeylerinin ameliyat sonrası hipokalsemiyiile korele olmadığı izlendi
Laparoskopik batın eksplorasyonundan sonra görülen trokar yeri hernisi
Trokar yeri hernisi bir insizyonel herni çeşidi olup ameliyattan sonra farklı zaman dilimlerinde karşımıza
çıkabilemektedir.Ortalama insidansı %1.85 dir.Laparaskopik girişimler sonrası trokar yeri herniasyonuna
bağlı postoperatif mekanik barsak tıkanıklığı gelişen olgularda erken tanı ve tedavi hayat kurtarıcıdır.
OLGU
43 yaşında erkek hasta sol üst kadrana delici kesici alet yaralaması nedeniyle acil polikliniğimize
getirildi.Muayenede batına nafiz yaralanma ve batında serbest sıvı tespit edildi.Hemodinamik
parametrelerin bozulması üzerine olguya tanısal laparoskopi kararI verildi.Göbek altından girilen bir adet
10 mm lik trokar ve her iki alt kadranlardan girilen birer adet 5 mm lik torkar ile batın eksplarasyonu
yapıldı.Batında yaygın hemoraji olduğu saptanan olguda olası organ yaralanmaları ve damara
yaralanmaları açısından eksplarasyon laparoskopik olarak yapıldı.Batındaki yaygın kanama odağının kas
kaynaklı olduğu tespit edilip giriş yeri ve kas bölgesinde kanama kontrolü laparoskopik olarak sağlandı ve
ameliyata son verildi.Ameliyat sonrası dönemde olgunun gaz gaita deşarjını takibe 4. günde taburcu
edildi.Olgu ameliyattan sonra 6. günde karın ağrısı,şişkinlik,bulantı şikayetleri ile tekrar acil servise
başvurud.Muayenesinde ileus bulguları tespit edilen olguya yapılan batın tomografisinde umblikal trokar
bölgesinde herniasyon gösteren intestinal ans tespit edildi.Hasta acilen ameliyata alındı.umblikal
bölgedeki eski insizyon yerinden mini laparatomi yapıldı. Umblikal trokar bölgesinde hernie olmuş
beslenmesi normal ince barsak ansı gözlendi. Redüksiyon ve primer fasya onarımı uygulandı. Postoperatif
dönemde herhangi bir ek komplikasyon olmadı. Hasta postoperatif ikinci gün taburcu edildi.
SONUÇ
Trokar yeri hernileri laparoskopik cerrahide nadir görülmekle birlikte ciddi komplikasyonlardan
biridir.Laparoskopik ameliyatlardan sonra gelişen mekanik intestinal obstürüksiyonlarda trokar yeri
herniasyonu mutlaka akılda bulundurulmalıdır
