Sensitivity of the Cherenkov Telescope Array to a dark matter signal from the Galactic centre
We provide an updated assessment of the power of the Cherenkov Telescope Array (CTA) to search for thermally produced dark matter at the TeV scale, via the associated gamma-ray signal from pair-annihilating dark matter particles in the region around the Galactic centre. We find that CTA will open a new window of discovery potential, significantly extending the range of robustly testable models given a standard cuspy profile of the dark matter density distribution. Importantly, even for a cored profile, the projected sensitivity of CTA will be sufficient to probe various well-motivated models of thermally produced dark matter at the TeV scale. This is due to CTA's unprecedented sensitivity, angular and energy resolutions, and the planned observational strategy. The survey of the inner Galaxy will cover a much larger region than corresponding previous observational campaigns with imaging atmospheric Cherenkov telescopes. CTA will map with unprecedented precision the large-scale diffuse emission in high-energy gamma rays, constituting a background for dark matter searches for which we adopt state-of-the-art models based on current data. Throughout our analysis, we use up-to-date event reconstruction Monte Carlo tools developed by the CTA consortium, and pay special attention to quantifying the level of instrumental systematic uncertainties, as well as background template systematic errors, required to probe thermally produced dark matter at these energies
Interobserver Variability in the Treatment of Little Finger Metacarpal Neck Fractures
Purpose To address the null hypothesis that surgeons shown radiographs of little finger metacarpal neck fractures with measured fracture angulation would recommend surgery as often as surgeons shown unmarked radiographs. Methods Members of the Science of Variation Group, an international collaboration of fully trained orthopedic and trauma surgeons, were asked to review 20 little finger metacarpal neck fracture cases, which included a vignette and 3 high-quality radiographs. Members were then randomized to review radiographs with or without measured fracture angulation on the lateral view and select operative or nonoperative management. Results Surgeons shown radiographs with measured angulation were more likely to recommend surgery, and there was less variability among these surgeons, particularly for fractures with less angular deformity. Conclusions Measured fracture angulation has a small but significant influence on treatment recommendations for little finger metacarpal neck fractures
Central venous catheterization in pediatric patients affected by hematological malignancies.
The objective of this retrospective study was to evaluate the significance and complications of percutaneous central venous catheterization in pediatric patients affected by hematologic malignancies. One hundred and fifty-eight central venous catheters were inserted in 125 pediatric patients (male/female 67/58; median age: 4 years; range 10 m - 6 y.) affected by hematological malignancies. Venous access was obtained by means of a tunnelled silicone rubber Groshong catheter inserted percutaneously in the subclavian vein (91.1%), the internal jugular vein or in the femoral vein. The medial duration of catheterization was 231.8 days (range 8-1014 days). The total number of catheter days was 33,792 (92.6 years). There were no complications related to catheter insertion. Only one patient developed significant post-operative bleeding. One hundred and nine catheters (68.9%) were removed when they were no longer needed and 49 (31.1%) were removed due to complications: 6 catheter occlusions (12.2%), 7 were accidentally withdrawn (14.3%), 3 for local infections (6.1%) and 33 for catheter-related infection (67.3%). A Groshong catheter seems to provide good access to the blood stream for a long period of time with a low incidence of complications in children with acute hematological malignancie
Taxane-induced nail changes: incidence, clinical presentation and outcome
The clinical characteristics of nail changes in seven patients receiving taxane-containing chemotherapy are described. They include nail pigmentation, subungual hematoma, Beau's lines and onycholysis and subungual suppuration. The incidence of such changes (ranging from 0% to 44%) is reviewed from a Medline search of the literature
PET-CT in oncological patients: Analysis of informal care costs in cost-benefit assessment
PURPOSE:
The authors analysed the impact of nonmedical costs (travel, loss of productivity) in an economic analysis of PET-CT (positron-emission tomography-computed tomography) performed with standard contrast-enhanced CT protocols (CECT).
MATERIALS AND METHODS:
From October to November 2009, a total of 100 patients referred to our institute were administered a questionnaire to evaluate the nonmedical costs of PET-CT. In addition, the medical costs (equipment maintenance and depreciation, consumables and staff) related to PET-CT performed with CECT and PET-CT with low-dose nonenhanced CT and separate CECT were also estimated.
RESULTS:
The medical costs were 919.3 euro for PET-CT with separate CECT, and 801.3 euro for PET-CT with CECT. Therefore, savings of approximately 13% are possible. Moreover, savings in nonmedical costs can be achieved by reducing the number of hospital visits required by patients undergoing diagnostic imaging.
CONCLUSIONS:
Nonmedical costs heavily affect patients' finances as well as having an indirect impact on national health expenditure. Our results show that PET-CT performed with standard dose CECT in a single session provides benefits in terms of both medical and nonmedical costs
