    Exploring sustainable healthcare waste management implementation in teaching hospitals in Malaysia

    There are growing expectations for healthcare organizations to take active steps and introduce sustainable approaches to healthcare waste management (HCWM). This study investigated sustainable healthcare waste management (SHCWM) in two university teaching hospitals in Malaysia. The objectives of the study were to identify current practices and SHCWM initiatives as well as examine the training and knowledge of hospital staff on sustainable healthcare waste management. Following a questionnaire survey of 243 senior hospital staff, the study found that sustainable healthcare waste management initiatives were being implemented in the healthcare facilities (HCFs) surveyed. Descriptive statistics using frequency distributions and mean rank analysis were used to analyze data from the 243 respondents. The findings revealed that proper segregation and sorting, purchasing supplies that generate less hazardous waste, education/awareness about material reuse and recycling, purchasing environmentally friendly products, and encouraging material reuse and recycling through reward were the five initiatives perceived to be most implemented at the HCFs. The study also found that majority of the respondents received training and were highly knowledgeable about the health and environmental risks of HCW as well as SHCWM aspects such as waste minimization. The findings from this study provide an overview of SHCWM in particularly large teaching hospitals in Malaysia and suggests further research for a more comprehensive and wider understanding of SHCWM in HCFs in Malaysia

    Systematic literature review on green maintenance principles and maintenance performance indicators for green buildings design

    The UNEP Emissions Gap Report 2020 highlighted buildings and cities as one among six sector solutions for climate change through enhanced energy efficiency and carbon reduction. A good performing green building is seen as a solution for energy efficiency and environmental protection, but performance of green building is largely affected by maintenance management. Studies have suggested that green buildings might not be performing as intended. While maintenance management has been widely researched, green maintenance performance has rarely been studied. Hence, a systematic literature review was conducted on the integration of green maintenance principles into maintenance performance indicators for green buildings. This review was based on the publication standard, namely RepOrting Standards for Systematic Evidence Syntheses (ROSES) and integrated multiple research designs. Two leading databases (Web of Science and Scopus) were utilized. Based on the data, for green maintenance principles and performance indicators, 4 main aspects were established, namely environment, economy, technical and social with 5 green maintenance principles. For green maintenance performance, 11 green maintenance indicators and 25 sub-indicators were identified. This study has offered significant contribution to the body of knowledge on this topic. The findings explained the importance of integrating green maintenance principles into maintenance performance for green building to ensure desired maintenance performance, as follows: 1) to encourage green building managers to realise their role in maintenance performance; 2) to plan integration strategies according to the desired maintenance performance; and 3) to inform on the knowledge gap existence on specific areas and future studies suggestions

    Long-term myocardial recovery after mitral valve replacement in noncompaction cardiomyopathy

    Isolated noncompaction of the left ventricle is a congenital cardiomyopathy, which has been described recently, with literature limited to case reports and case series. Even though various complications have been reported with noncompaction cardiomyopathy, among them severe mitral regurgitation has been reported recently in a few cases. There is no great evidence in the literature about its management, apart from some cases of mitral valve repair and replacement in young patients. We are reporting a case of an elderly lady with isolated left ventricular noncompaction cardiomyopathy associated with severe mitral regurgitation treated with mitral valve replacement with one and half year of follow up demonstrating significant myocardial recovery

    Topological Surface States Protected From Backscattering by Chiral Spin Texture

    Topological insulators are a new class of insulators in which a bulk gap for electronic excitations is generated by strong spin orbit coupling. These novel materials are distinguished from ordinary insulators by the presence of gapless metallic boundary states, akin to the chiral edge modes in quantum Hall systems, but with unconventional spin textures. Recently, experiments and theoretical efforts have provided strong evidence for both two- and three-dimensional topological insulators and their novel edge and surface states in semiconductor quantum well structures and several Bi-based compounds. A key characteristic of these spin-textured boundary states is their insensitivity to spin-independent scattering, which protects them from backscattering and localization. These chiral states are potentially useful for spin-based electronics, in which long spin coherence is critical, and also for quantum computing applications, where topological protection can enable fault-tolerant information processing. Here we use a scanning tunneling microscope (STM) to visualize the gapless surface states of the three-dimensional topological insulator BiSb and to examine their scattering behavior from disorder caused by random alloying in this compound. Combining STM and angle-resolved photoemission spectroscopy, we show that despite strong atomic scale disorder, backscattering between states of opposite momentum and opposite spin is absent. Our observation of spin-selective scattering demonstrates that the chiral nature of these states protects the spin of the carriers; they therefore have the potential to be used for coherent spin transport in spintronic devices.Comment: to be appear in Nature on August 9, 200

    The melanoma-specific graded prognostic assessment does not adequately discriminate prognosis in a modern population with brain metastases from malignant melanoma

    The melanoma-specific graded prognostic assessment (msGPA) assigns patients with brain metastases from malignant melanoma to 1 of 4 prognostic groups. It was largely derived using clinical data from patients treated in the era that preceded the development of newer therapies such as BRAF, MEK and immune checkpoint inhibitors. Therefore, its current relevance to patients diagnosed with brain metastases from malignant melanoma is unclear. This study is an external validation of the msGPA in two temporally distinct British populations.Performance of the msGPA was assessed in Cohort I (1997-2008, n=231) and Cohort II (2008-2013, n=162) using Kaplan-Meier methods and Harrell's c-index of concordance. Cox regression was used to explore additional factors that may have prognostic relevance.The msGPA does not perform well as a prognostic score outside of the derivation cohort, with suboptimal statistical calibration and discrimination, particularly in those patients with an intermediate prognosis. Extra-cerebral metastases, leptomeningeal disease, age and potential use of novel targeted agents after brain metastases are diagnosed, should be incorporated into future prognostic models.An improved prognostic score is required to underpin high-quality randomised controlled trials in an area with a wide disparity in clinical care

    Entanglement distribution and quantum discord

    Establishing entanglement between distant parties is one of the most important problems of quantum technology, since long-distance entanglement is an essential part of such fundamental tasks as quantum cryptography or quantum teleportation. In this lecture we review basic properties of entanglement and quantum discord, and discuss recent results on entanglement distribution and the role of quantum discord therein. We also review entanglement distribution with separable states, and discuss important problems which still remain open.     Loss of Receptor on Tuberculin-Reactive T-Cells Marks Active Pulmonary Tuberculosis

    BACKGROUND: Tuberculin-specific T-cell responses have low diagnostic specificity in BCG vaccinated populations. While subunit-antigen (e.g. ESAT-6, CFP-10) based tests are useful for diagnosing latent tuberculosis infection, there is no reliable immunological test for active pulmonary tuberculosis. Notably, all existing immunological tuberculosis-tests are based on T-cell response size, whereas the diagnostic potential of T-cell response quality has never been explored. This includes surface marker expression and functionality of mycobacterial antigen specific T-cells. METHODOLOGY/PRINCIPAL FINDINGS: Flow-cytometry was used to examine over-night antigen-stimulated T-cells from tuberculosis patients and controls. Tuberculin and/or the relatively M. tuberculosis specific ESAT-6 protein were used as stimulants. A set of classic surface markers of T-cell naive/memory differentiation was selected and IFN-gamma production was used to identify T-cells recognizing these antigens. The percentage of tuberculin-specific T-helper-cells lacking the surface receptor CD27, a state associated with advanced differentiation, varied considerably between individuals (from less than 5% to more than 95%). Healthy BCG vaccinated individuals had significantly fewer CD27-negative tuberculin-reactive CD4 T-cells than patients with smear and/or culture positive pulmonary tuberculosis, discriminating these groups with high sensitivity and specificity, whereas individuals with latent tuberculosis infection exhibited levels in between. CONCLUSIONS/SIGNIFICANCE: Smear and/or culture positive pulmonary tuberculosis can be diagnosed by a rapid and reliable immunological test based on the distribution of CD27 expression on peripheral blood tuberculin specific T-cells. This test works very well even in a BCG vaccinated population. It is simple and will be of great utility in situations where sputum specimens are difficult to obtain or sputum-smear is negative. It will also help avoid unnecessary hospitalization and patient isolation

    Deep learning to detect optical coherence tomography-derived diabetic macular edema from retinal photographs: a multicenter validation study

    PURPOSE: To validate the generalizability of a deep learning system (DLS) that detects diabetic macular edema (DME) from two-dimensional color fundus photography (CFP), where the reference standard for retinal thickness and fluid presence is derived from three-dimensional optical coherence tomography (OCT). DESIGN: Retrospective validation of a DLS across international datasets. PARTICIPANTS: Paired CFP and OCT of patients from diabetic retinopathy (DR) screening programs or retina clinics. The DLS was developed using datasets from Thailand, the United Kingdom (UK) and the United States and validated using 3,060 unique eyes from 1,582 patients across screening populations in Australia, India and Thailand. The DLS was separately validated in 698 eyes from 537 screened patients in the UK with mild DR and suspicion of DME based on CFP. METHODS: The DLS was trained using DME labels from OCT. Presence of DME was based on retinal thickening or intraretinal fluid. The DLS's performance was compared to expert grades of maculopathy and to a previous proof-of-concept version of the DLS. We further simulated integration of the current DLS into an algorithm trained to detect DR from CFPs. MAIN OUTCOME MEASURES: Superiority of specificity and non-inferiority of sensitivity of the DLS for the detection of center-involving DME, using device specific thresholds, compared to experts. RESULTS: Primary analysis in a combined dataset spanning Australia, India, and Thailand showed the DLS had 80% specificity and 81% sensitivity compared to expert graders who had 59% specificity and 70% sensitivity. Relative to human experts, the DLS had significantly higher specificity (p=0.008) and non-inferior sensitivity (p 50%) and a sensitivity of 100% (p=0.02 for sensitivity > 90%). CONCLUSIONS: The DLS can generalize to multiple international populations with an accuracy exceeding experts. The clinical value of this DLS to reduce false positive referrals, thus decreasing the burden on specialist eye care, warrants prospective evaluation

    In this Viewpoint, we discuss how the identification of oral antibiotics and their distinction from other commonly used medicines can be challenging for consumers, suppliers, and health-care professionals. There is a large variation in the names that people use to refer to antibiotics and these often relate to their physical appearance, although antibiotics come in many different physical presentations. We also reflect on how the physical appearance of medicine influences health care and public health by affecting communication between patients and health-care professionals, dispensing , medicine use, and the public understanding of health campaigns. Furthermore, we report expert and stakeholder consultations on improving the identification of oral antibiotics and discuss next steps towards a new identification system for antibiotics. We propose to use the physical appearance as a tool to support and nudge awareness about antibiotics and their responsible use
