    Airway response to respiratory syncytial virus has incidental antibacterial effects.

    RSV infection is typically associated with secondary bacterial infection. We hypothesise that the local airway immune response to RSV has incidental antibacterial effects. Using coordinated proteomics and metagenomics analysis we simultaneously analysed the microbiota and proteomes of the upper airway and determined direct antibacterial activity in airway secretions of RSV-infected children. Here, we report that the airway abundance of Streptococcus was higher in samples collected at the time of RSV infection compared with samples collected one month later. RSV infection is associated with neutrophil influx into the airway and degranulation and is marked by overexpression of proteins with known antibacterial activity including BPI, EPX, MPO and AZU1. Airway secretions of children infected with RSV, have significantly greater antibacterial activity compared to RSV-negative controls. This RSV-associated, neutrophil-mediated antibacterial response in the airway appears to act as a regulatory mechanism that modulates bacterial growth in the airways of RSV-infected children

    MRI compared to conventional diagnostic work-up in the detection and evaluation of invasive lobular carcinoma of the breast: a review of existing literature

    Item does not contain fulltextPURPOSE: The clinical diagnosis and management of invasive lobular carcinoma (ILC) of the breast presents difficulties. Magnetic resonance imaging (MRI) has been proposed as the imaging modality of choice for the evaluation of ILC. Small studies addressing different aspects of MRI in ILC have been presented but no large series to date. To address the usefulness of MRI in the work-up of ILC, we performed a review of the currently published literature. MATERIALS AND METHODS: We performed a literature search using the query "lobular AND (MRI OR MR OR MRT OR magnetic)" in the Cochrane library, PubMed and scholar.google.com, to retrieve all articles that dealt with the use of MRI in patients with ILC. We addressed sensitivity, morphologic appearance, correlation with pathology, detection of additional lesions, and impact of MRI on surgery as different endpoints. Whenever possible we performed meta-analysis of the pooled data. RESULTS: Sensitivity is 93.3% and equal to overall sensitivity of MRI for malignancy in the breast. Morphologic appearance is highly heterogeneous and probably heavily influenced by interreader variability. Correlation with pathology ranges from 0.81 to 0.97; overestimation of lesion size occurs but is rare. In 32% of patients, additional ipsilateral lesions are detected and in 7% contralateral lesions are only detected by MRI. Consequently, MRI induces change in surgical management in 28.3% of cases. CONCLUSION: This analysis indicates MRI to be valuable in the work-up of ILC. It provides additional knowledge that cannot be obtained by conventional imaging modalities which can be helpful in patient treatment

    A review of data needed to parameterize a dynamic model of measles in developing countries

    <p>Abstract</p> <p>Background</p> <p>Dynamic models of infection transmission can project future disease burden within a population. Few dynamic measles models have been developed for low-income countries, where measles disease burden is highest. Our objective was to review the literature on measles epidemiology in low-income countries, with a particular focus on data that are needed to parameterize dynamic models.</p> <p>Methods</p> <p>We included age-stratified case reporting and seroprevalence studies with fair to good sample sizes for mostly urban African and Indian populations. We emphasized studies conducted before widespread immunization. We summarized age-stratified attack rates and seroprevalence profiles across these populations. Using the study data, we fitted a "representative" seroprevalence profile for African and Indian settings. We also used a catalytic model to estimate the age-dependent force of infection for individual African and Indian studies where seroprevalence was surveyed. We used these data to quantify the effects of population density on the basic reproductive number <it>R</it><sub>0</sub>.</p> <p>Results</p> <p>The peak attack rate usually occurred at age 1 year in Africa, and 1 to 2 years in India, which is earlier than in developed countries before mass vaccination. Approximately 60% of children were seropositive for measles antibody by age 2 in Africa and India, according to the representative seroprevalence profiles. A statistically significant decline in the force of infection with age was found in 4 of 6 Indian seroprevalence studies, but not in 2 African studies. This implies that the classic threshold result describing the critical proportion immune (<it>p</it><sub>c</sub>) required to eradicate an infectious disease, <it>p</it><sub>c </sub>= 1-1/<it>R</it><sub>0</sub>, may overestimate the required proportion immune to eradicate measles in some developing country populations. A possible, though not statistically significant, positive relation between population density and <it>R</it><sub>0 </sub>for various Indian and African populations was also found. These populations also showed a similar pattern of waning of maternal antibodies. Attack rates in rural Indian populations show little dependence on vaccine coverage or population density compared to urban Indian populations. Estimated <it>R</it><sub>0 </sub>values varied widely across populations which has further implications for measles elimination.</p> <p>Conclusions</p> <p>It is possible to develop a broadly informative dynamic model of measles transmission in low-income country settings based on existing literature, though it may be difficult to develop a model that is closely tailored to any given country. Greater efforts to collect data specific to low-income countries would aid in control efforts by allowing highly population-specific models to be developed.</p

    Spatially Distributed Instructions Improve Learning Outcomes and Efficiency

    Learning requires applying limited working memory and attentional resources to intrinsic, germane, and extraneous aspects of the learning task. To reduce the especially undesirable extraneous load aspects of learning environments, cognitive load theorists suggest that spatially integrated learning materials should be used instead of spatially separated materials, thereby reducing the split-attention effect (Sweller & Chandler, 1994). Recent work, however, has suggested a new distinction between two common formats of spatially separated displays: spatially distributed versus spatially stacked (Jang & Schunn, 2010). Moreover, a distinction between instructions and learning task materials has rarely been made. Across two studies with 106 college students (56 in Study 1 and 50 in Study 2), we compared spatially distributed (multiple sources of information are placed side by side) versus spatially stacked (only one at the top is visible) instructions, without changing the learning task materials, on both task performance and learning. With materials more typical of practice, Study 1 showed that the distributed-display instructions led learners to more efficient learning; learners finished the task faster and scored higher in the overall learning test. With materials more tightly controlled for spatial format per se, Study 2 replicated the effect and found that the benefit of the distributed instructions appeared to be associated with changes in cognitive load. Implications for educational practice are discussed. © 2011 American Psychological Association

    Learning Through Case Comparisons: A Meta-Analytic Review

    Over the past 20 years, there has been much research on how people learn from case comparisons. This work has implemented comparison activities in a variety of different ways across a wide range of laboratory and classroom contexts. In an effort to assess the overall effectiveness of case comparisons across this diversity of implementation and contexts and to explore what variables might moderate learning outcomes, we conducted a meta-analysis of 57 experiments with 336 tests. Random effects analyses of the 336 tests revealed that case comparison activities commonly led to greater learning outcomes than other forms of case study including sequential, single case, and nonanalogous, as well as traditional instruction and control (d = .50), 95% CI [.44, .56]. Of 15 potential moderators, four variables were found to reliably moderate the effectiveness of case comparisons: the objective of the comparison, the presentation of a principle, the content, and the lag between the comparison and testing. Asking learners to find similarities between cases, providing principles after the comparisons, using perceptual content, and testing learners immediately are all associated with greater learning.We conclude with a discussion of the theoretical and practical implications of these results for cognitive theory and classroom practice. © Division 15, American Psychological Association

    Economic Benefits Assessment of Accelerated Pavement Testing Research in California: Case Study

    A cost-benefit analysis method for determining the economic benefits of heavy vehicle simulator testing was evaluated. The University of California and its research partners began conducting accelerated pavement testing in 1994 on behalf of the California Department of Transportation. The authors present the findings of a pilot project intended to define a method suitable for measuring the direct economic benefits of heavy vehicle simulator testing in California. The chosen method was based on a cost-benefit analysis initially developed in Australia and later enhanced and applied in South Africa to determine economic benefits of their respective accelerated pavement testing programs. Results of a case study applying the Australian-South African method to heavy vehicle simulator tests conducted in California are presented. The case study evaluated benefits (in cost savings) from heavy vehicle simulator tests performed to validate innovative pavement mixes and designs proposed for the rehabilitation of a high-traffic urban Interstate route in the Los Angeles, California, area. Although local conditions in these countries differed significantly, the method was successfully applied and consistently showed positive results, presented as discounted net present value and benefit-cost ratio.
