    OBJECTIVE. To review the use of episiotomy during vaginal delivery in Hong Kong public hospitals. DESIGN. Prospective observational survey. SETTING. Public hospitals, Hong Kong. PARTICIPANTS. Women who underwent normal vaginal delivery of a singleton foetus with cephalic presentation. MAIN OUTCOME MEASURES. Number of women having an episiotomy, severe-degree (third- or fourth-degree) tear, other types of tear, blood loss at delivery, postpartum haemorrhage, need for blood transfusion, puerperal pyrexia, wound infection, gaping wound that required suture removal, and drainage or resuturing of a perineal wound. RESULTS. Between 1 January and 31 March 2003, there were 6222 singleton spontaneous normal vaginal deliveries in the public hospitals of Hong Kong. Of the 6167 women in whom the status of the perineum was known, episiotomy was performed in 5274 (85.5%). Primiparous women were more likely to undergo episiotomy at delivery than multiparous women (97.9% vs 71.4%). Women with episiotomy had significantly less perineal tearing of any kind than those without. The occurrence of any type of perineal tear and severe-degree (third- or fourth-degree) tear was significantly lower in primiparous women who had an episiotomy than those without (P<0.05). Women with episiotomy had increased mean blood loss at delivery but other complications were not significantly increased. CONCLUSIONS. In Hong Kong, episiotomy is routinely performed during normal vaginal delivery. It is associated with a significantly lower overall rate of perineal tearing. This study was observational, nonetheless the occurrence of other complications was likely to increase when episiotomy was performed. Firm evidence from several randomised controlled studies shows that routine episiotomy is unjustified and possibly harmful. Routine episiotomy should not be promoted in Hong Kong without further randomised controlled study.published_or_final_versio

    Depth resolution enhancement in optical scanning holography with a dual-wavelength laser source

    In this paper, we use two point sources to analyze the depth resolution of an optical scanning holography (OSH) system with a single-wavelength source. A dual-wavelength source is then employed to improve it, where this dual-wavelength OSH (DW-OSH) system is modeled with a linear system of equations. Object sectioning in DW-OSH is obtained with the Fourier domain conjugate gradient method. Simulation results show that, with the two source wavelengths at 543 nm and 633 nm, a depth resolution at 2.5 mum can be achieved. Furthermore, an OSH system emulator is provided to demonstrate the performance of DW-OSH compared with a conventional OSH system.published_or_final_versio

    Enhancement of quantum nondemolition measurements with an electro-optic feed-forward amplifier

    Methods for the enhancement of optical quantum nondemolition (QND) measurements are discussed. We review the use of meter squeezing; as a QND enhancement tool and present a method of QND enhancement using an electro-optic feed-forward amplifier. By applying a linearized theory it is shown that these techniques work very well together. The combined effect of these enhancement methods is modeled for two QND systems, a squeezed light beam splitter and an optical parametric amplifier. We also discuss the conflict between the normal QND criteria and QND systems that involve noiseless amplification. We use an additional parameter to quantify the problem. A method for correcting the effects of noiseless amplification is discussed and modeled. We also discuss a special case of QND that eliminates the optical interaction between the meter and signal input beams. This system is shown to be a very effective QND device.     Enhanced depth resolution in optical scanning holography using a configurable pupil

    The optical scanning holography (OSH) technique can capture all the three-dimensional volume information of an object in a hologram via a single raster scan. The digital hologram can then be processed to reconstruct individual sectional images of the object. In this paper, we present a scheme to reconstruct sectional images in OSH with enhanced depth resolution, where a spatial light modulator (SLM) is adopted as a configurable point pupil. By switching the SLM between two states, different Fresnel zone plates (FZPs) are generated based on the same optical system. With extra information provided by different FZPs, a depth resolution at 0.7 μm can be achieved.published_or_final_versio

    Noiseless electro-optic processing of optical signals generated with squeezed light

    We demonstrate an elegant way of handling optical signals which are generated using squeezed states of light without losing their improved signal to noise ratio. We do this by amplifying, without significant noise penalty, both signal and noise away from the quantum noise limit into the classical domain. This makes the information robust to losses. Our system achieves a signal transfer coefficient, T-s, close to unity. As a demonstration we amplify a small signal carried by 35% amplitude squeezed light and show that unlike the fragile squeezed input, the signal amplified output is robust to propagation losses. A signal transfer coefficient of T-s = 0.75 is achieved even in the presence of large introduced (86%) downstream losses. (C) 1998 Optical Society of America

    Blind sectional image reconstruction for optical scanning holography

    Optical scanning holography is a powerful holographic recording technique in which only a single twodimensional scan is needed to record three-dimensional information. As in standard digital holography, for the reconstruction of a sectional image, the resulting data must then be postprocessed to obtain sectional content. We propose a blind sectional image reconstruction technique to automate the data processing. This reconstruction uses edge information to determine the appropriate Fresnel zone plates automatically and applies inverse imaging to recover the sectional images with significant suppression of the defocus noise. The experimental data used to verify the algorithm are measured from a physical implementation of the optical scanning holography system. © 2009 Optical Society of America.published_or_final_versio

    Monsoons and habitat influence trophic pathways and the importance of terrestrial-marine linkages for estuary sharks

    Tropical estuaries often receive enhanced fluxes of terrestrial derived organic matter and phytoplankton during the wet season, and such monsoonal events may significantly influence the trophic dynamics of these systems. This study examined spatio-temporal terrestrial-marine linkages in a tropical estuary, the Pearl River Estuary (PRE), southern China, by investigating trophic pathways leading to estuary sharks. We investigated spatial (inshore vs. offshore) and seasonal (wet vs. dry season) variation in the relative importance of terrestrial- and marine-derived carbon, so as to assess the contribution of detrital pathways to the pelagic spadenose shark, Scoliodon laticaudus; ontogentic changes in shark diets were also documented. Stable isotope analyses (SIA) and fatty acid (FA) profiling indicated that spadenose sharks assimilated both marine and terrestrial carbon via consumption of zooplantivorous fish and shrimps. Detrital carbon sources were more important to juvenile and pre-mature sharks at inshore locations, especially during the wet season when river discharge increased and terrestrial detritus was more abundant. Ontogenetic dietary shifts were evident: juvenile and pre-mature sharks had significantly higher levels of bacterial (detrital) FA than adults which contained more animal-derived FA. Inshore sharks, with more depleted δ13C signatures, relied more on terrestrial carbon than sharks offshore. Comparison of spadenose shark FA profiles with those of the sympatric, white-spotted bamboo shark (Chiloscyllium plagiosum)—a benthic predator that acquires detrital carbon via consumption of polychaetes and crustaceans—revealed that they made greater use of detrital carbon sources. However, spadenose sharks in the inner estuary assimilated higher proportions of terrestrial detritus (44–56%) than bamboo sharks (31–45%). The importance of terrestrial detritus for both shark species demonstrated the important contribution of terrestrial detritus to both pelagic and benthic food webs in the PRE. Terrestrial-marine linkages are therefore of great significance, particularly during the wet season, in this estuarine system, which serves as feeding and nursery grounds for both shark species, and trophic subsidies from land are likely to be important for marine predators in other tropical estuaries.
