    Negotiating Public and Legal Spaces: The Emergence of an LGBT Movement in Vietnam

    Vietnam’s laws, policies and decrees do not explicitly discriminate against lesbian, gay, bisexual and transgender (LGBT) individuals but their rights are not legally protected and they are socially marginalised. The state promotes a model of a married heterosexual couple with two children in the media and through its public policy campaigns. Families that comply are able to obtain membership of the Communist Party and run for office; women are eligible for micro-credit programmes. Same-sex couples cannot marry and are thus ineligible for the benefits that married couples enjoy. Family laws, with regard to child custody, inheritance and property, do not protect same-sex couples. In order to understand how LGBT civil society organisations can affect legal and social change with regard to the laws that regulate sexual norms and unions, this empirical study explores the following two examples of collective action in Vietnam: 1.     Deletion of apolipoprotein E gene modifies the rate of depletion of alpha tocopherol (vitamin E) from mice brains

    AbstractOur previous reports show that apolipoprotein E (apoE) influences the dynamics of alpha tocopherol (vitamin E) in brain. In this investigation, the patterns of depletion of alpha tocopherol from tissues of apoE deficient and wild type mice were compared after the animals were fed vitamin E deficient diets. Alpha tocopherol concentrations in specific regions of the brain and peripheral tissues at different times were determined by HPLC with electrochemical detection. ApoE deficiency significantly retarded the rate of depletion of alpha tocopherol from all regions of the brain. In addition, comparison of the rates of depletion of alpha tocopherol in both apoE deficient and wild type animals showed that cerebellum behaved differently from other areas such as cortex, hippocampus and striatum. This reinforces the uniqueness of cerebellum with regard to vitamin E biology. Patterns of depletion of tocopherol from peripheral tissues were different from brain. Serum tocopherol was higher in apoE deficient animals and remained higher than wild type during E deficiency. Depletion of liver tocopherol also tended to be unaffected by apoE deficiency. Our current and previous observations strongly suggest that apoE has an important role in modulating tocopherol concentrations in brain, probably acting in concert with other proteins as well

    Reconfigurable quantum metamaterials

    By coupling controllable quantum systems into larger structures we introduce the concept of a quantum metamaterial. Conventional metamaterials represent one of the most important frontiers in optical design, with applications in diverse fields ranging from medicine to aerospace. Up until now however, metamaterials have themselves been classical structures and interact only with the classical properties of light. Here we describe a class of dynamic metamaterials, based on the quantum properties of coupled atom-cavity arrays, which are intrinsically lossless, reconfigurable, and operate fundamentally at the quantum level. We show how this new class of metamaterial could be used to create a reconfigurable quantum superlens possessing a negative index gradient for single photon imaging. With the inherent features of quantum superposition and entanglement of metamaterial properties, this new class of dynamic quantum metamaterial, opens a new vista for quantum science and technolog

    Interactive Random Forests Plots

    Random Forests is a useful data mining tool that is quite popular in finding variable importance. However, many people don’t make use of the Random Forests results in interactive graphs. Partly, this is because software packages that can do interactive graphs can’t handle large data sets and those that use Random Forests have large data sets or many variables. A new software package in R, known as iPlots eXtreme, that is still in development makes it simple to explore large data sets interactively. I have created a function, called irfplot (interactive random forests plot) that specifically uses Random Forests to produce interactive graphs that are more informative than using raw values. I will use the interactive Random Forests plot that I’ve created to explore the nutrition data set from the Cache County Memory Study

    Clinical competence in performing and recognising a mediolateral episiotomy of protective angle and length: a systematic review

    Objective: It is assumed that all doctors and midwives understand and apply evidence‐based principles in performing episiotomies in their everyday practice. However, remarkable discrepancies between even the most reputable literature sources in defining and describing the technique of performing mediolateral episiotomy (MLE) suggest that there is much ambiguity and confusion for both researchers and clinicians alike. Design: The systematic review protocol was written prior to starting the review and registered in the international prospective register of systematic reviews (PROSPERO/ID CRD42017070523) last updated on December 15, 2017. The review is reported using the Preferred Reporting Items for Systematic Reviews and Meta‐Analysis (PRISMA) guidelines. Methods: A database search was performed using: Medline, CINAHL, Scopus, Informit, the Cochrane Library and PubMed from database inception to 17 September 2017, with a final search on 10 February 2017. Studies were included if they examined clinicians' competency in performing an 'ideal' or 'correct' mediolateral episiotomy, as well as those studies that compared the performance of different professional roles. Studies usually defined an 'ideal' incision as one that met the criteria of an acceptable angle of incision from the midline, starting incision point distance from the midline and in terms of the length of the incision created. Results: While many of the studies included in this review were not of high quality (author self‐assessment) and had their own study criteria for a MLE, the literature suggests clinicians are generally unable to perform or simulate episiotomies within such standards. Overall, most of the literature reported doctors were performing more 'ideal', lateral and longer incisions compared to midwives; however, there were studies that found the opposite, showing statistically significant results in favour of midwives performing more protective episiotomies. There was no association between clinicians' participation in formal training courses and their ability to perform the 'ideal' incision, though one study did find an increased number of episiotomies performed under supervision improved clinicians competency. Conclusion: The obvious lack of understanding around defining and performing MLE for clinicians of various professional roles suggests the need to produce a uniform set of guidelines, and to develop a universal, low‐cost approach for teaching and performing the MLE technique in any clinical environment around the world

    The Vietnamese Version of the Brief Illness Perception Questionnaire and the Beliefs about Medicines Questionnaire:Translation and Cross-cultural Adaptation

    OBJECTIVE: To translate and cross-culturally adapt the Brief Illness Perception Questionnaire (BIPQ) and the Beliefs about Medicines Questionnaire (BMQ) into Vietnamese. METHODS: We followed the guideline by Beaton et al. (2000 & 2007). Stage I: two translators (informed and uninformed) translated the questionnaires. Stage II: the translations were synthesized. Stage III: back translation was performed by two translators fluent in both Vietnamese and English but naïve to the outcome measurement. Stage IV: seven experts reached consensus on the pre-final Vietnamese version (BIPQ-V and BMQ-V). Stage V: field test of the questionnaires on 16 twelve-year-old students and 31 Vietnamese patients. In addition, we determined the internal consistency and test-retest reliability of the questionnaires in 34 Vietnamese patients with acute coronary syndrome. RESULTS: All experts agreed that there was semantic, idiomatic, experiential, and conceptual equivalence between the original and pre-final Vietnamese versions of the BIPQ and BMQ. Cronbach's alpha coefficients of the internal consistency were acceptable for the BMQ-V Specific-Necessity (0.64), BMQ-V Specific-Concerns (0.62), and BMQ-V General-Harm (0.60), with the exception of BMQ-V General-Overuse (0.27). Intra-class correlation coefficients of the test-retest reliability was acceptable for the subscales of BMQ-V (range: 0.77-0.86), and BIPQ-V items (range: 0.62-0.85) with the exception of BIPQ-V 1 (0.44, 95% CI -014-0.72) and BIPQ-V 4 (0.57, 95% CI 0.22-0.81). CONCLUSIONS: The Vietnamese version of BIPQ and BMQ are reliable tools to assess illness perceptions and beliefs about medicines of patients with acute coronary syndrome. Psychometric properties of these questionnaires should be tested in different patient populations

    Belimumab promotes negative selection of activated autoreactive B cells in systemic lupus erythematosus patients.

    Belimumab has therapeutic benefit in active systemic lupus erythematosus (SLE), especially in patients with high-titer anti-dsDNA antibodies. We asked whether the profound B cell loss in belimumab-treated SLE patients is accompanied by shifts in the immunoglobulin repertoire. We enrolled 15 patients who had been continuously treated with belimumab for more than 7 years, 17 matched controls, and 5 patients who were studied before and after drug initiation. VH genes of sort-purified mature B cells and plasmablasts were subjected to next-generation sequencing. We found that B cell-activating factor (BAFF) regulates the transitional B cell checkpoint, with conservation of transitional 1 (T1) cells and approximately 90% loss of T3 and naive B cells after chronic belimumab treatment. Class-switched memory B cells, B1 B cells, and plasmablasts were also substantially depleted. Next-generation sequencing revealed no redistribution of VH, DH, or JH family usage and no effect of belimumab on representation of the autoreactive VH4-34 gene or CDR3 composition in unmutated IgM sequences, suggesting a minimal effect on selection of the naive B cell repertoire. Interestingly, a significantly greater loss of VH4-34 was observed among mutated IgM and plasmablast sequences in chronic belimumab-treated subjects than in controls, suggesting that belimumab promotes negative selection of activated autoreactive B cells
