    Trends in sun exposure awareness and protection practices in Malta : 1999-2004

    Rising skin cancer incidence rates have led to sun awareness campaigns in Malta since the early 1990s. The aim of this study is to assess the impact of these campaigns by analysing trends in sun exposure-related knowledge and behaviour amongst the Maltese people. A total of 559 Maltese pedestrians aged 16-50 years were interviewed in 1999. The same questionnaire was used to interview 304 pedestrians in 2004. More people admitted spending leisure time outdoors during peak sunshine hours in 2004 than in 1999 (85.9% vs 62.4%, p<0.001). There was a drop in people regularly wearing a hat during outdoor leisure activities from 32.4% to 18.4% (p<0.001), and from 37.5% to 9.3% (p<0.001) during outdoor work activities. Rates of regular sunscreen use remained constant at about 50% using it for outdoor leisure activities, but dropped from 25.0% to 9.3% for outdoor work (p=0.02). In 2004, 96.1% of participants having children aged less than 12 years stated that they regularly used sunscreen on their children (87.0% in 1999, p=0.01), while 66.2% said that their children regularly wore a hat (78.4% in 1999, p=0.05). More people were regarding a suntan as unhealthy in 2004 than in 1999 (62.8% versus 37.0%, p<0.001). The mass media remained the most important source of health information.peer-reviewe

    Treatment of psoriasis with biologic agents in Malta

    Introduction: Biologic therapy has revolutionalised the treatment of moderate to severe psoriasis leading to improved clinical outcomes and quality of life scores. This study aims to determine current biologic use in psoriatic patients at our Dermatology department at Sir Paul Boffa hospital, Malta. Method: All patients who were administered biologic therapy for psoriasis in Malta until the end of 2014 were included. Data included demographic details, disease duration and severity, biologic use and duration, previously attempted treatments, side effects, early and late response to biologic using Psoriasis Area Severity Index (PASI) scores and Dermatology Life Quality index (DLQI) scores. Results: A total of 36 patients were started on a biologic between 2009 and 2014 for psoriasis (M:25, F:11) with a mean age of 46.9 years. These included etanercept (n=22), infliximab (n=8), adalimumab (n=4) and ustekinumab (n=2). Secondary failure was the main reason why biologics were stopped and switched. Most patients had an improvement in their PASI scores after 2 to 4 weeks of starting the biologic and had a PASI 90 score improvement. All patients had more than a 5 point improvement in DLQI score. Discussion: Biologic use in our department is on the increase. Our patients had considerable improvements in their PASI and DLQI scores. Secondary failures have occurred usually after 2 to 4 years and switching has yielded positive results. Biologics are expensive drugs and recently we have switched to cheaper biosimilars. Doctors should be aware of the treatment options available for psoriasis patients, their possible side effects and when to refer to our department. In most cases a satisfactory response can be achieved.peer-reviewe

    Representations of molecular force fields. I. Ethane: Ab initio and model, harmonic and anharmonic

    The quadratic and selected cubic force constants for ethane have been computed, using single determinant molecular orbital wavefunctions at the 4ā€31G level, with a view to testing and extending model consistent force fields (CFF) for ā€™ā€™molecular mechanicsā€™ā€™ calculations. Results agree semiquantitatively with experiment, but experimental force constants of sufficient reliability to provide a definitive comparison are not yet available. In a comparison with the most rational general CFF available, that of Ermer and Lifson, the most significant discrepancies found to occur are those for certain stretchā€“bend couplings assumed to be zero in the CFF but shown to be appreciable by quantum calculation. It is observed that these couplings, but not the stretchā€“stretch couplings, are well accounted for by a steric interaction model. The ab initio cubic constants examined display the same pattern of conformity with a steric model. Bendā€“bendā€“bend and bendā€“bendā€“stretch but not all stretchā€“stretchā€“stretch interactions agree with those of the steric model. The partial success of the steric model shows that it is possible to represent a large number of interaction constants, quadratic and higher order, by a small number of parameters in molecular mechanics. The failure of the steric model to account for predominantly stretching interactions confirms that ā€™ā€™classicalā€™ā€™ nonbonded interactions as embodied in conventional Ureyā€“Bradley fields are not the only major contributors to offā€diagonal force constants. The definitive version was published in Limnology and Oceanography 61 (2016): 1574ā€“1588, doi:10.1002/lno.10297.American eel (Anguilla rostrata) complete their life cycle by migrating from the east coast of North America to Sargasso Sea, where they spawn planktonic eggs and dye. Larvae that develop from eggs need to return to North American coastal waters within the first year of life and are influenced by the oceanic currents during this journey. A coupled physicalā€“biological model is used to investigate the extent to which inter-annual changes in the ocean circulation affect the success rates of larvae in reaching coastal nursery habitats. Our results suggest that natural oceanic variability can lead to changes in larval success rates by a factor of 2. Interannual variation in success rates are strongly affected by the Gulf Stream inertial overshoot events, with the largest success in years with an inertial overshoot and the smallest in years with a straighter and more southern configuration of the Gulf Stream downstream of Cape Hatteras.     In the course of a high-throughput search for optimal combinations of bidentate ligands with Pd(II) carboxylates to generate oxidation catalysts, we obtained and crystallographically characterized a number of crystalline products. While some combinations afforded the anticipated (L-L)Pd(OC(O)R)_2 structures (L-L = bipyridine, tmeda; R = CH_3, CF_3), many gave unusual oligometallic complexes resulting from reactions such as Cāˆ’H activation (L-L = sparteine), Pāˆ’C bond cleavage (L-L = 1,2-bis(diphenylphosphino)ethane, and Cāˆ’C bond formation between solvent (acetone) and ligand (L-L = 1,4-bis(2,6-diisopropylphenyl)-1,4-diaza-1,3-butadiene). These findings illustrate potential pitfalls of screening procedures based on assuming uniform, in situ catalyst self-assembly

    A stakeholder-based system dynamics model of return-to-work: a research protocol

    Background. Returning to work following a job-related injury or illness can be a complex process, influenced by a range of interrelated personal, psychosocial, and organizational components. System dynamics modelling (SDM) takes a sociotechnical systems perspective to view return-to-work (RTW) as a system made up of multiple feedback relationships between influential components. Design and Methods. To build the RTW SDM, a mixed-method approach will be used. The first stage, that has already been completed, involved creating a baseline model using key informant interviews. Second, in two manufacturing companies, stakeholder-based models will be developed through interviews and focus groups with senior management, frontline workers, and frontline supervisors. Participants will be asked about the RTW process in general and more targeted questions regarding influential components. Participants will also be led through a reference mode exercise where they will be asked to estimate the direction, shape and magnitude of relationships between influential components. Data will be entered into the software program Vensim that provides a platform for visualizing system-structure and simulating the effects of adapting components. Finally, preliminary model validity testing will be conducted to provide insights on model generalizability and sensitivity. Expected Impact of the study for Public Health. The proposed methodology will create a SDM of the RTW process using feedback relationships of influential components. It will also provide an important simulation tool to understand system behaviour that underlies complex RTW cases, and examine anticipated and unanticipated consequences of disability management policies

    Pathogenicity of a Very Virulent Strain of Marek's Disease Herpesvirus Cloned as Infectious Bacterial Artificial Chromosomes

    Bacterial artificial chromosome (BAC) vectors containing the full-length genomes of several herpesviruses have been used widely as tools to enable functional studies of viral genes. Marek's disease viruses (MDVs) are highly oncogenic alphaherpesviruses that induce rapid-onset T-cell lymphomas in chickens. Oncogenic strains of MDV reconstituted from BAC clones have been used to examine the role of viral genes in inducing tumours. Past studies have demonstrated continuous increase in virulence of MDV strains. We have previously reported on the UK isolate C12/130 that showed increased virulence features including lymphoid organ atrophy and enhanced tropism for the central nervous system. Here we report the construction of the BAC clones (pC12/130) of this strain. Chickens were infected with viruses reconstituted from the pC12/130 clones along with the wild-type virus for the comparison of the pathogenic properties. Our studies show that BAC-derived viruses induced disease similar to the wild-type virus, though there were differences in the levels of pathogenicity between individual viruses. Generation of BAC clones that differ in the potential to induce cytolytic disease provide the opportunity to identify the molecular determinants of increased virulence by direct sequence analysis as well as by using reverse genetics approaches on the infectious BAC clones
