    Morbidity and related factors among elderly people in South Korea: results from the Ansan Geriatric (AGE) cohort study

    BACKGROUND: A thorough examination of the morbidity and comorbidity profiles among the elderly and an evaluation of the related factors are required to improve the delivery of health care to the elderly and to estimate the cost of that care. In South Korea where the aged population is rapidly increasing, however, to date only one study using a limited sample (84 subjects) has provided information on morbidity and related factors among the elderly. Using a large, stratified, random sample (2,767 subjects) from the population-based Ansan Geriatric study, the present study sought to assess the morbidity and comorbidity, and to determine the relationships of these variables with sociodemographic and health characteristics in elderly people in South Korea. METHODS: A total of 2,767 subjects (1,215 men and 1,552 women) aged 60–84 years were randomly selected from September 2002 to August 2003 in Ansan, South Korea. Data on sociodemographic and health characteristics, and clinical diagnosis were collected using questionnaires. When available, the medical records and medications taken by the subjects were also cross-checked. RESULTS: Of the total subjects, 78.0% reported diagnosed disease, 11.0% had been cured, and 46.8% had been diagnosed with more than two diseases. The mean number of morbidities per person among elderly Koreans was 1.62 ± 1.35 (mean ± standard deviation), and women had a greater number of diseases per person than did men. The most common morbidities were chronic diseases such as hypertension, arthritis, and diabetes mellitus. In women, osteoporosis and arthritis were the second and third most prevalent diseases, respectively. Morbidity was significantly associated with gender, employment, household income, alcohol intake, self-assessed health status, and worries about health. CONCLUSION: These data will enhance understanding of the patterns of health problems among elderly Koreans and will contribute to the application of appropriate intervention strategies

    Polyamide thin-film composite membranes based on carboxylated polysulfone microporous support membranes for forward osmosis

    Due to its simple process and low energy consumption, forward osmosis (FO) has gained significant attention in the fields of portable hydration bags, desalination, landfill leachate treatment, and brine concentration. However, current state-of-the-art reverse osmosis (RO) membranes show relatively low water fluxes in FO processes due to high internal concentration polarization (ICP) and high mass transfer resistance in commercially available microporous support membranes. In this study, carboxylated polysulfones (CPSFs) were synthesized via direct polysulfone (PSF) functionalization and considered as hydrophilic, mechanically stable microporous support membranes. The incorporation of hydrophilic groups into hydrophobic polymer backbones often reduces mechanical strength due to excessive water swelling. However, the mechanical properties of CPSFs (degree of substitution, DS=0.49\u20130.85) were similar to those of pristine PSF, and they retained their hydrophilic nature. Microporous CPSF membranes were prepared in various conditions, and FO water fluxes and salt passages of polyamide thin-film/CPSF composite membranes were measured and compared with each other. CPSF-based FO membranes showed significantly higher water fluxes (water flux in FO mode: 18 L/m2 h, salt passage: 2.2 g/m2 h under 1 M MgCl2 as a draw solution, active layer facing DI water) than PSF-based FO membranes (10.5 L/m2 h, 1.5 g/m2 h at the same conditions), which might be due to enhanced hydrophilicity and reduced ICP.Peer reviewed: YesNRC publication: Ye

    A Robust Scenario Approach for the Vehicle Routing Problem with Uncertain Travel Times

    International audienceWe consider a vehicle routing problem with uncertain travel times in which a penalty is incurred for each vehicle that exceeds a given time limit. A traditional stochastic programming approach would require precise knowledge of the underlying probability distributions of random data. In a novel approach presented here, we assume that only rough information on future travel times is available, leading to the multiple range forecasts of travel times and the probabilities of each range being realized. In this setting, we replace the point estimates of travel times on a scenario by range estimates. For each scenario, we then find the robust routes that protect the solution against the worst case within the given ranges, and finally we find the routes with the minimum expected cost. We propose a branch-and-cut algorithm to solve the problem and report computational results on both randomly generated and the well-known Solomon's instances. The results demonstrate that our approach is a favorable one when exact information of probability distributions is not available

    Can We Reboot the Role of Intraperitoneal Chemotherapy in the Treatment for Gastric Cancer with Peritoneal Carcinomatosis?: A Retrospective Cohort Study Regarding Minimally Invasive Surgery Conjoined with Intraperitoneal plus Systemic Chemotherapy

    Background: Peritoneal carcinomatosis (PC) is the most common form of metastasis in gastric cancer (GC) and is related with a poor prognosis. Several treatment modalities including systemic chemotherapy and intraperitoneal chemotherapy have been studied and adopted in treatment of GC patients with PC. Nevertheless, few studies have reported the comparison of the oncologic outcomes between minimally invasive surgery (MIS) with intraperitoneal (IP) chemotherapy and conventional chemotherapy for GC with PC. Methods: We retrospectively reviewed the clinical records of 74 patients who had been diagnosed as GC with PC via either intra-abdominal exploration or abdominopelvic computed tomography between January 2011 and April 2021. After performing propensity score-matching for this retrospective data, we compared the outcomes of 26 patients who underwent MIS followed by IP combined systemic chemotherapy (MIS-IP group) and 26 patients who underwent systemic chemotherapy only (SC-only group). Results: The 2-year progression free survival rate of the MIS-IP group was significantly higher than the SC-only groups (36.4% and 10.5%, respectively; p = 0.010). In multivariate analysis to detect relevant factors on PFS, IP chemotherapy (HR 0.213; p < 0.001), Eastern Cooperative Oncology Group performance status (HR 3.689; p = 0.002), and the amount of ascites (p = 0.011) were significant prognostic factors. Conclusions: This study demonstrated the therapeutic potential of MIS conjoined IP plus systemic chemotherapy for GC patients with PC. MIS conjoined by IP plus systemic chemotherapy can be adopted as a treatment option to reboot the role of IP chemotherapy in GC patients with PC

    Childhood Obesity: A Potential Key Factor in the Development of Glioblastoma Multiforme

    Glioblastoma multiforme (GBM) is a malignant primary tumor type of the central nervous system (CNS). This type of brain tumor is rare and is responsible for 12–15% of all brain tumors. The typical survival rate of GBM is only 12 to 14 months. GBM has a poor and unsatisfactory prognosis despite advances in research and therapeutic interventions via neurosurgery, radiation, and chemotherapy. The molecular heterogeneity, aggressive nature, and occurrence of drug-resistant cancer stem cells in GB restricts the therapeutic efficacy. Interestingly, the CNS tumors in children are the second most usual and persistent type of solid tumor. Since numerous research studies has shown the association between obesity and cancer, childhood obesity is one of the potential reasons behind the development of CNS tumors, including GBM. Obesity in children has almost reached epidemic rates in both developed and developing countries, harming children’s physical and mental health. Obese children are more likely to face obesity as adults and develop non-communicable diseases such as diabetes and cardiovascular disease as compared to adults with normal weight. However, the actual origin and cause of obesity are difficult to be pointed out, as it is assumed to be a disorder with numerous causes such as environmental factors, lifestyle, and cultural background. In this narrative review article, we discuss the various molecular and genetic drivers of obesity that can be targeted as potential contributing factors to fight the development of GBM in children
