    The new molecular markers DDIT3, STT3A, ARG2 and FAM129A are not useful in diagnosing thyroid follicular tumors

    Preoperative characterization of thyroid follicular lesions is challenging. Fine-needle aspiration specimens cannot differentiate follicular carcinomas from benign follicular neoplasias. Recently, promising markers have been detected using modern molecular techniques. We conducted a retrospective study to confirm the usefulness of immunohistochemical staining for the protein markers, DDIT3, STT3A (ITM1), ARG2 and FAM129A (C1orf24) in separating benign and malignant thyroid follicular lesions. Formalin-fixed, paraffin-embedded thyroid tissue from 30 in-house cases (15 follicular carcinomas and 15 follicular adenomas), as well as 8 follicular carcinomas and 21 follicular adenomas on tissue microarray slides were stained immunohistochemically for DDIT3, STT3A, ARG2 and FAM129A expression. Control tissue consisted of thyroid parenchyma adjacent to the tumors and 11 separate cases of normal thyroid parenchyma. All in-house cases of follicular adenomas, follicular carcinomas and adjacent normal thyroid tissue showed positive immunostaining with anti-DDIT3 and anti-STT3A. Anti-ARG2 and anti-FAM129A polyclonal antibodies showed positive staining in 20 and 60% of in-house follicular adenomas, and 40 and 87% of in-house follicular carcinomas, respectively. Monoclonal anti-FAM129A demonstrated positive staining in 13 and 33% of in-house follicular adenomas and follicular carcinomas, respectively. Polyclonal anti-DDIT3, -STT3A and -FAM129A antibodies showed positive staining in all tissue microarray slides of follicular carcinoma and in 76, 85 and 81% of the follicular adenomas, respectively. Monoclonal anti-STT3A stained 81% of the follicular adenoma cores. Anti-ARG2 stained positive in 13% of follicular carcinomas and 10% of follicular adenomas on the tissue microarray slides. In conclusion, DDIT3, STT3A, ARG2 and FAM129A immunohistochemistry does not appear to be useful in the diagnosis of thyroid follicular neoplasias, as they do not reliably distinguish follicular thyroid carcinoma from follicular thyroid adenoma

    Patient empowerment in long-term conditions: development and preliminary testing of a new measure

    BACKGROUND: Patient empowerment is viewed by policy makers and health care practitioners as a mechanism to help patients with long-term conditions better manage their health and achieve better outcomes. However, assessing the role of empowerment is dependent on effective measures of empowerment. Although many measures of empowerment exist, no measure has been developed specifically for patients with long-term conditions in the primary care setting. This study presents preliminary data on the development and validation of such a measure. METHODS: We conducted two empirical studies. Study one was an interview study to understand empowerment from the perspective of patients living with long-term conditions. Qualitative analysis identified dimensions of empowerment, and the qualitative data were used to generate items relating to these dimensions. Study two was a cross-sectional postal study involving patients with different types of long-term conditions recruited from general practices. The survey was conducted to test and validate our new measure of empowerment. Factor analysis and regression were performed to test scale structure, internal consistency and construct validity. RESULTS: Sixteen predominately elderly patients with different types of long-term conditions described empowerment in terms of 5 dimensions (identity, knowledge and understanding, personal control, personal decision-making, and enabling other patients). One hundred and ninety seven survey responses were received from mainly older white females, with relatively low levels of formal education, with the majority retired from paid work. Almost half of the sample reported cardiovascular, joint or diabetes long-term conditions. Factor analysis identified a three factor solution (positive attitude and sense of control, knowledge and confidence in decision making and enabling others), although the structure lacked clarity. A total empowerment score across all items showed acceptable levels of internal consistency and relationships with other measures were generally supportive of its construct validity. CONCLUSION: Initial analyses suggest that the new empowerment measure meets basic psychometric criteria. Reasons concerning the failure to confirm the hypothesized factor structure are discussed alongside further developments of the scale

    Oral health of Hong Kong homeless people

    Session 22: P2Conference Theme: We are the FuturePoster PresentationObjectives: To study the caries experience, periodontal status and toothbrushing habit of the homeless people in Hong Kong. Methods: A convenient sample of 40 generally healthy and dentate homeless people attending one of the service homes in Hong Kong Island was invited in this study. They were asked about their toothbrushing habits. Clinical examination was performed with a disposable front surface dental mirror attached to an intra-oral LED light and ball-ended community periodontal index (CPI) probe. Caries experience and periodontal status were assessed with DMFT index and CPI. T-test and X2- test were used to assess association of toothbrushing habit and gender with untreated caries and periodontal pockets. Results : A total of 37 people (93%) joined this pilot study. Their mean age (SD) was 71±14 years, and more than half (59%) of them were female. All the participants had caries experience and their mean DMFT (SD) was 21.1 (9.4). Two third (66%) of the caries experience were missing teeth, and 22% were untreated decay. None of the participants had healthy gums. About half (54%) of them had periodontal pockets with at least 4mm. About a quarter (27%) of them did not have daily tooth brushing habit. No correlation was found between their daily toothbrushing habit and untreated caries or periodontal pockets. Male had higher DT scores (p=0.001). Conclusions: According to this pilot study, more than a quarter of Hong Kong homeless people had no daily toothbrushing. Their dental caries prevalence and experience was high, and many caries were left untreated. Their periodontal status was poor and about half of them had advanced periodontal disease

    Operations Analysis of Behavioral Observation Procedures: A Taxonomy for Modeling in an Expert Training System

    This article introduces a taxonomy based on a procedural operations analysis (Verplanck, 1996) of various method descriptions found in the behavior observation research literature. How these alternative proceduresimpact the recording and subsequent analysis of behavioral events on the basis of the type of time and behavior recordings made is also discussed. The taxonomy was generated as a foundation for the continuing development of an expert training system called Train-to-Code (TTC; J. M. Ray & Ray, (Behavior Research Methods 40:673-693, 2008)). Presently in its second version, TTC V2.0 is software designed for errorlesstraining (Terrace, (Journal of the Experimental Analysis of Behavior 6:1-27, 1963)) of student accuracy and fluency in the direct observation and coding of behavioral or verbal events depicted via digital video. Two of16 alternative procedures classified by the taxonomy are presently modeled in TTC\u27s structural interface and functional services. These two models are presented as illustrations of how the taxonomy guides software user interface and algorithm development. The remaining 14 procedures are described in sufficient operational detail to allow similar model-oriented translation
