Pilot study of the safety and efficacy of angiogenic therapy in diabetic foot syndrome
BACKGROUND: The syndrome of diabetic foot remains the main cause of non-traumatic amputation of the lower extremity in the world. Even with the provision of comprehensive medical care in the conditions of a specialized center, 10-15% of patients do not succeed in healing the ulcerative defect due to the ischemic component. AIMS: The objective of this study is evaluation of safety and efficacy of pl-VEGF165 transfer in patients with neuroischemic type of diabetic foot syndrome. METHODS: The pilot study included 35 diabetic patients with neuroischemic foot ulcers (Wagner stage 1-2) who were not candidates for revascularization procedures (NCT02538705). The patients were closely monitored after repeated pl-VEGF165 intramuscular gene transfer (2,4 mg) at 1, 3, and 6 months after treatment. The primary efficacy endpoint was the surface area of the ulcers (sq.cm), the secondary endpoints were transcutaneous oxygen tension (Tcp02), ankle-brachial index (ABI), neuropathy disability score (NDS), neuropathy symptoms score (NSS), and Michigan neuropathy screening instrument (MNSI). Adverse events were monitored throughout the study. RESULTS: The use of pl-VEGF165 as part of complex treatment allowed to achieve wound healing in 65,7% of patients with chronic ulcerative defects, the safety of the target limb was 84%. Carrying out therapeutic angiogenesis as a part of the combined therapy ensured a reduction in the average area of the resistant to treatment defects from 3.6 [1.0; 7.05] cm2 to 0.0 [0.0;2.0] cm2 (p=0,001), which correlated with an increase in the TcPo2 index by 15% from 35 [29.5; 40.5] to 40.5 [36.0; 46.5] mm Hg (p= p=0,005) and in the ABI by 16% from 0.96 [0.82;1.08] to 1.11 [0.85; 1.24] (p=0,062). The decrease in the signs of diabetic neuropathy was determined - the scores of NSS scales and VAT decreased from 6,5 [5.75; 8.0) to 6.0 [5.25; 7.0] (p=0,004) and from 9.0 [8.0; 13.5] to 8.0 [7.0; 12.7] (p=0,001), respectively. No adverse effects associated with the use of pl-VEGF165 were recorded. CONCLUSIONS: Thus, preliminary results of the pilot study show that the use of pl-VEGF165 gene transfer in combination therapy allows for complete healing of neuroischemic diabetic foot ulcers in the majority of patients

Coagulation Factor Activity and Hemostatic Markers of Endothelial Dysfunction in Patients with Peripheral Arterial Disease
Purpose: We aimed to evaluate the impact of intrinsic coagulation factors and hemostatic markers of endothelial dysfunction on complications in patients with atherosclerotic peripheral arterial disease (PAD). Materials and Methods : This prospective study enrolled 120 PAD patients at Fontaine stages 2b to 3 who underwent open surgical, endovascular, or conservative treatment. Coagulation factors (FVIII, FIX, and FXI) and endothelial hemostatic markers, including von Willebrand factor (vWF) activity and level, soluble endothelial protein C receptor, and plasminogen activator inhibitor-1 (PAI-1) levels, were assessed. Results : At 3 months after open bypass grafting, activity of FVIII significantly increased from a median of 175% to 233% (P<0.001). At 3 months after endovascular treatment, the activities of FVIII, FIX, and FXI significantly increased from medians of 157%, 180%, and 156% to 184%, 218%, and 181%, respectively (P<0.05). Six patients with increased FVIII activity developed bypass graft thrombosis. Four patients in the endovascular group and three patients in the conservative treatment group with increased activity of vWF developed myocardial infarction (P=0.049). The subjects who developed restenosis had increased vWF activity (P=0.023) and decreased nitric oxide metabolite levels (P=0.003). Three subjects who received conservative treatment and developed PAD progression at 12 months had increased PAI-1 activity (P=0.028). Conclusion : Patients with advanced PAD had a hypercoagulable status, and performance of open or endovascular revascularization was associated with further hypercoagulability. Increased activity of coagulation factors and altered levels of hemostatic markers of endothelial dysfunction were associated with PAD complications such as graft thrombosis, myocardial infarction, disease progression, and restenosis
ГЕОЛОГО-ГЕОХІМІЧНІ ДОСЛІДЖЕННЯ ПРИ ПРОВЕДЕННІ НАФТОГАЗОПОШУКОВИХ РОБІТ НА ПІВНІЧНО-ЗАХІДНОМУ ШЕЛЬФІ ЧОРНОГО МОРЯ
Odesa I.I. Mechnikov National University, in collaboration with the State Regional Geological Enterprise "Prichernomorgeologiia" carry out complex geological-geochemical studies of hydrocarbon perspective areas in the central and western parts of the NorthWestern Black Sea shelf. The studies are aimed at the identification of liquid and gas forms of hydrocarbons, which migrate from the areas of oil and gas accumulation and create epigenetic anomalies in the late-Quaternary sedimentary deposits as well as in near bottom seawater. The components that are studied include gaseous hydrocarbons from methane and to pentane; group composition of liquid hydrocarbons with quantitative identification of kinds of oil, neutral and oxidized tar, asphaltens and paraffin; and the forms of carbon. The main complexity of the analysis and interpretation of the results of hydrocarbon surveys is that hydrocarbons in the bottom sediments can be brought both the deep horizons of sedimentary thickets, and bottom sediments themselves produce a significant amount of hydrocarbon gases. Therefore, the basis of the analysis is the criteria for the delimitation of the syngenetic and epigenetic component of hydrocarbon gases. The delineation of the syngenetic and epigenetic hydrocarbon component of the bottom sediments allows prediction of hydrocarbon deposits in the migration of epigenetic hydrocarbons in the sedimentary layer and the formation of epigenetic abnormalities in the bottom sediments. The survey will enable the creation of the geochemical model of perspective areas for hydrocarbons; and to give recommendations for geological survey in the future. Основы методики этих исследований базируются на выявлении мигрирующих от скоплений нефти и газа в разрезе осадочного чехла жидких и газообразных углеводородов образующих эпигенетические аномалий в позднечетвертичних донных отложениях и придонных морских водах. Были изучены газовые углеводороды от метана до пентана включительно, групповая составляющая жидких углеводородов с количественным определением масел, нейтральных и окисленных смол, асфальтенов и парафинов, а также формы нахождения углерода. Основная сложность анализа и интерпретации результатов углеводородного съемки заключается в том, что с одной стороны углеводороды в донных отложениях могут быть привнесены из глубинных горизонтов осадочной толщи, а с другой донные отложения сами по себе производят значительные количества углеводородных газов. Поэтому основу анализа составляют критерии разграничения сингенетической и эпигенетической составляющей углеводородных газов. Разграничение сингенетичнои и эпигенетической углеводородной составляющей донных отложений позволяет прогнозировать залежи углеводородов по миграции углеводородов в осадочной толще и формированию в донных отложений эпигенетических аномалий. Проведение этих работ позволит провести прогнозно-геохимическое моделирование нефтегазоперспективных участков и дать рекомендации относительно дальнейших геологоразведочных работ Одеський національний університет імені І.І. Мечникова спільно з ДРГП «Причорноморгеологія» проводить комплексні геолого-гідрохімічні дослідження нафтогазоперспективних ділянок центральної та західної частин північно-західного шельфу Чорного моря. Основи методики цих досліджень базуються на виявленні мігруючих від скупчень нафти та газу в розрізі осадового чохла епігенетичних аномалій рідких і газоподібних вуглеводнів у пiзньочетвертинних донних відкладах та придонних морських водах. Були вивчені газові вуглеводні від метану до пентану включно, групова складова рідких вуглеводнів з кількісним визначенням масел, нейтральних та окислених смол, асфальтенів і парафінів, а також форми знаходження вуглецю. Основна складність аналізу та інтерпретації результатів вуглеводневої зйомки полягає в тому, що з одного боку вуглеводні в донних відкладах можуть бути привнесенi з глибинних горизонтів осадової товщi, а з іншого донні відклади самі по собі продукують значні кількості вуглеводневих газів. Тому основу аналізу складають критерії розмежування сингенетичної та епігенетичної складової вуглеводневих газів. Розмежування сінгенетичної і епігенетичної вуглеводневої складової донних відкладів дозволяє прогнозувати поклади вуглеводнів по міграції епігенетичних вуглеводнів в осадовій товщі та формуванню в донних відкладів епігенетичних аномалій. Проведення цих робіт дозволить провести прогнозно-геохімічне моделювання нафтогазоперспективних ділянок та дати рекомендації щодо подальших геологорозвідувальних робіт
