Colemia colestatică în complicații inflamatorii ale litiazei biliare, opțiuni de tratament etapizat
Background. The rate of benign inflammatory complications in gallstones varies with the prevalence of acute cholecystitis (ALC) in 10 -15% and cholangitis (CL) in 2-4% of
cases, etc. Statistics show that in pre-existing conditions, in
10-15% of cases, cholestatic jaundice (CJ) is associated. Objective of the study. Analysis of the experience of treatment
of patients with inflammatory complications of gallstones
associated with cholestatic jaundice. Material and Methods. Options of approach of 67 patients with inflammatory
complications, selected from 191 patients with complicated
gallstones and associated cholestatic cholemia, treated in
the hospital „St. Arch. Michael” are presented. Results. It
was found: I gr. - ALC associated with choledocholithiasis
(CD) and CJ (16) - destructive form (13), simple form (3),
II gr. - ALC associated with stenosis of the sphincter Oddi
and CJ (19) - destructive form (6), simple form (13), III gr.
- Destructive ALC, CL and CJ (12), IVgr. - CD, CL and CJ (7),
V gr. - Mirizzi syndrome, CL and CJ (13). Management: in
simple ALC, compliant with drug treatment (16) - endoscopic PST in 48-72 hours, cholecystectomy over 3-4 days after
decompression. In destructive ALC (31) at the first stage
decompression was performed by cholecystectomy (laparoscopic-10, classic-21) with external drainage of the bile
ducts, in the second stage - PST if necessary. In CD and CL
- PST with litextraction - complete solution. In Mirizzi syndrome at stage I - PST with drainage of the bile ducts, in
stage II - bile duct reconstruction interventions over 3-4
days after decompression, adjusted to the type of obstruction. Mortality -0%. Morbidity - 5 cases (7.4%). Conclusions.
Individualization of the terms and type of surgery, depending on the morphological form of the disease and homeostatic liver disorders, improves the results of treatment of
patients with inflammatory complications and cholestatic
jaundice in gallstones. Regardless of the type of approach,
the primary goal in the first stage is biliary decompression.Introducere. Rata complicațiilor benigne inflamatorii în litiaza biliară variază cu prevalența colecistitei acute (CAL)
în 10 -15% și colangitei (CL) în 2-4% cazuri, etc. Statisticele arată, că în stările preexistente, în 10-15 % cazuri, se
asociază icterul colestatic (IC). Scopul. Analiza experienței
tratamentului pacienților cu complicații inflamatorii ale litiazei biliare asociate cu icter colestatic. Material și metode. Sunt expuse opțiuni de abordare a 67 pacienți cu complicații inflamatorii, selectate din 191 de bolnavi cu litiază
biliară complicată și colemie colestatică asociată, tratați în
spitalul „Sf. Arhanghel Mihail”. Rezultate. S-au constatat: I
gr. - CAL asociate cu coledocolitiază (CD) și IC (16) - formă
distructivă (13), formă simplă (3), II gr. - CAL asociate cu
stenoză a sfincterului Oddi și IC (19) - formă distructivă (6),
formă simplă (13), III gr. - CAL distructivă, CL și IC (12), IV
gr. - CD, CL și IC (7), V gr. - sindrom Mirizzi, CL și IC (13). Management: în CAL simplă, compliantă cu tratamentul medicamentos (16) – PST endoscopică în 48-72 ore, colecistectomizare peste 3- 4 zile după decompresie. În CAL distructivă
(31) la prima etapă decompresia s-a practicat prin colecistectomie (laparoscopică-10, clasică-21) cu drenare externă
a căilor biliare, în etapa a II-a – PST la necesitate. În CD și
CL – PST cu litextracție – rezolvare completă. În sindromul
Mirizzi la I etapă – PST cu drenare a căilor biliare, în etapa a
II – intervenții de reconstrucție a căilor biliare peste 3- 4 zile
după decompresie, ajustate tipului de obstrucție. Mortalitate -0%. Morbiditate – 5 cazuri (7,4%). Concluzii. Individualizarea termenilor și tipului de intervenție chirurgicală,
dependent de forma morfologică a afectării și dereglările
homeostatice hepatice, ameliorează rezultatele tratamentului bolnavilor cu complicații inflamatorii și icter colestatic
în litiaza biliară. Indiferent de tipul de abordare, obiectivul
primar în prima etapă este decompresia biliară

Delayed intestinal anastomosis in acute mesenterial thrombosis
Scopul lucrării. Tratamentul trombozei mezenterice acute (TMA) reprezintă o problemă majoră a chirurgiei contemporane. Letalitatea
cunoscută constituie 54,1% în embolia arterială și 77,4% în ocluzia venoasă. Prognosticul pacienților este rezervat, majoritatea
prezentându-se la spitalizare cu necroza de intestin și peritonită, care și după rezecții extinse progresează în porțiunile restante la
fiecare al 2-a pacient. Scopul este analiza eficacității aplicării anastomozelor intestinale temporizate la pacienți cu TMA.
Materiale și metode. 23 pacienți cu TMA tratați pe parcursul anilor 2019-2023 în IMU. Vârsta a variat 26-74 ani, cu media 54,1±2,4
ani. Raport b/f – 14/9.
Rezultate. La majoritatea pacienților TMA a fost localizată în bazinul AMS. Peritonita difuză a fost prezentă la 19 pacienți. Tratament
conservativ conform datelor TC au necesitat 5 pacienți, unul fiind supus și laparoscopiei diagnostice pentru excluderea necrozei
intestinale. Tabloului clinic și TC în 18 cazuri au prezentat indicații pentru laparotomie. Laparotomie exploratorie - 3 cazuri cu
confirmarea necrozei totale intestinale. În 3 cazuri de necroză sectorială de colon - rezecții sectoriale finisate cu colostomie. La 12
pacienți fost practicată sigilarea bonturilor intestinului subțire și montarea laparostomiei cu revizii repetate. Aplicarea anastomozelor
temporizate a fost efectuată la 8 pacienți peste 36-48 ore de la intervenția primară, criteriile fiind lipsa progresării necrozei și peritonitei.
Peritonita și necroza în evoluție în 4 cazuri au fost soluționate prin re-rezecție intestinală, lavaj și laparostomie, cu re-revizie peste 36
ore. Mortalitatea precoce - 21,7%, urmare a dehiscenței anastomotice și peritonitei.
Concluzii. Rezecțiile segmentare ale intestinului necrozat cu sigilarea bonturilor și aplicarea laparostomielor în TMA permit
monitorizarea progresării necrozei intestinale. Re-laparotomiile programate cu re-rezecții de intestin la necesitate și asanarea cavității
abdominale, în condiții de stopare a necrozei și diminuarea peritonitei, permit aplicarea anastomozelor temporizate.Aim of study. Treatment of acute mesenteric thrombosis (AMT) represents a major problem in contemporary surgery. The known
lethality is 54.1% in arterial embolism and 77.4% in venous occlusion. The prognosis of the patients is reserved, the majority presenting
to the hospital with bowel necrosis and peritonitis, which even after extensive resections progress in the outstanding portions in every
second patient. The aim is analysis of the effectiveness of the application of delayed intestinal anastomoses in patients with AMT.
Materials and methods. 23 patients with AMT treated during the years 2019-2023 in the IMU. The age ranged from 26 to 74 years,
with a mean of 54.1±2.4 years. Ratio m/f – 14/9.
Results. In most patients the AMT was located in the pool of the AMS. Diffuse peritonitis was present in 19 patients. According to
CT data, 5 patients required conservative treatment, one also underwent diagnostic laparoscopy to rule out intestinal necrosis. The
clinical signs and CT in 18 cases showed indications for laparotomy. Exploratory laparotomy - 3 cases with confirmation of total intestinal necrosis. In 3 cases of sectoral colon necrosis - partial resections finished with colostomy. In 12 patients, the sealing of the
small intestine abutments and the mounting of the laparostomy with repeated revisions were performed. The application of delayed
anastomosis was performed in 8 patients over 36-48 hours after the primary intervention, the criteria being the lack of progression of
necrosis and peritonitis. Peritonitis and developing necrosis in 4 cases were solved by intestinal re-resection, lavage and laparostomy,
with re-revision after 36 hours. Early mortality - 21.7%, due to anastomotic dehiscences and peritonitis.
Conclusions. Segmental resections of the necrotic bowel with sealing of the abutments and the application of laparostomies in the
AMT allow monitoring of the progression of bowel necrosis. Scheduled relaparotomies with bowel resections when necessary and
the drying of the abdominal cavity, in conditions of stopping necrosis and diminishing peritonitis, allow the application of delayed
anastomoses

Analiza efectuării tratamentului artropatiilor la 102 cazuri de artrite secundare ce afectează calitatea vieții (cu edem, durere,
incapacitate funcțională articulară) prin utilizarea în 32 cazuri a preparatelor de sânge (trombină 250 UA) pe parcursul anilor
2015-2020. Din numărul total aflat în studiu: Femei 53,2%, bărbați 46,8%. Pacienții în 78,8 vârsta aptă de muncă. Ulterior
medicației cu preparate hematice readresări pentru asistență medicală artrologică nu au fost.Summary.
Analysis of arthropathy treatment performance in 102 cases of secondary arthritis affecting the quality of life (with edema,
pain, joint functional disability ) by using blood products in 32 cases (thrombin 250 UA) over 2015-2020. Ditribution of the

The diagnosis and surgical treatment in biliary lithiasis associated with hepatitis satellite
USMF „N.Testemițanu”, Chişinau, Republica Moldova, Al XI-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova și cea de-a XXXIII-a Reuniune a Chirurgilor din Moldova „Iacomi-Răzeșu” 27-30 septembrie 2011Introducere. Alegerea momentului operator şi volumul de intervenție chirurgicală, rămîn probleme de discuție în litiaza biliară, în prezența unui
teren hepatic alterat. Scopul. Analiza experienței tratamentului pacienților cu litiază biliară asociată cu hepatită satelit. Material şi metode. Din 256
pacienți tratați în 1995-2010 pentru sindrom icteric în litiaza biliară, hepatita satelit s-a constatat la 74 (28,2%). Suferințe hepatice au relatat 59 pacienți:
ciroză hepatică – 5(8,5%), hepatită cronică postvirală – 31(52,5%), hepatită cronică persistentă idiopatică – 23(40%). În 15 cazuri reacția colestatică
a fost cauzată de componenta infecțioasă severă. Aprecierea severității hepatitei s-a bazat pe aspecte clinice, biochimice şi investigații paraclinice.
Rezultate. Am remarcat evoluări bruşte a fermenților de citoliză cu involuție durabilă postdecompresie biliară. Hipoproteinemia s-a instalat precoce şi
a necesitat corecție pre- şi postoperatorie. Procesele de lipoperoxidare cu formare de radicali liberi sunt exprimate: DAM marcată cu 28%, SOD – cu
12,5%. Hipocoagularea precoce necesită o medicație terapeutică apreciată. Din aceste considerente au fost utilizate intervenții chirurgicale în două
etape. PSTE în I etapă a fost efectuată în 51 cazuri. La pacienții cu componentă infecțioasă severă s-a efectuat drenarea externă a căii biliare. Asocierea
medicației de corecție a dismetabolismului hepatic a permis în timp rezolvarea chirurgicală definitivă. Morbiditatea (6,9%) şi mortalitatea (2,7%) sunt
în concordanță cu gravitatea şi durata suferinței hepatice, durata icterului şi apariția mai precoce a insuficienței hepatice – factor de risc fundamental.
Concluzie: Folosirea criteriilor de apreciere a severității maladiei şi utilizarea tehnicilor chirurgicale moderne cresc acuratețea diagnosticului şi rezultatele
tratamentului chirurgical. Decompresia biliară preoperatorie reprezintă metoda de elecție în rezolvarea sindromului icteric la aceşti pacienți.Introduction.To time the operative intervention and to select the operation procedure is a problem for discussion in biliary stones associated with

Of 256 patients treated in 1995-2010 for jaundice syndrome in biliary lithiasis, hepatitis satellite was found in 74 (28.9%). History of liver disease
reported 59 patients: liver cirrhosis - 5 (8.5%), chronic hepatitis viral etiology- 31 (52.5%), chronic persistent hepatitis, idiopathic etiology - 23 (40%).
In 15 cases cholestatic liver reaction occurred neagravat field, caused by severe infectious component. Assessing the severity of hepatitis was based
on clinical aspects, biochemical and laboratory investigations. Results. I noticed evolving more pronounced cytolysis of fermented with involution
sustainable post biliary tract decompression . The decrease of the protein level was installed earlier and required pre-and postoperative correction.
Lipidic peroxidation processes with formation of free radicals are expressed: DAM marked 28%, SOD - by 12.5%. A medication regimen requires early
hypocoagulation appreciated. For these reasons surgery was used in two stages. Primary biliary decompression path through PSTE was made in 51
cases. In patients with severe infectious component was originally made to track external biliary drainage. Combination drug correction of the liver
disturbances, surgery allows for definitive resolution. Morbidity (6.75%) and mortality (2.7%) are consistent with the severity and duration of liver
disease, during early development of jaundice and liver failure - key risk factor. Conclusion: Using criteria for assessing the severity of disease and use
of modern surgical techniques increase the accuracy of diagnosis and results of surgical treatment. Preoperative biliary decompression by means is
method of choice in resolving jaundice
Thrombembolic complications in the biliary lithiasis surgery in aged patients, the preventive measures

Boala trombembolică este complicația severă ce poate interveni în chirurgie, necesitînd permanent perfecționarea diagnosticului şi
tratamentului. Scopul. Analiza tratamentului pacienților vîrstici cu litiază biliară , evidențierea factorilor de risc şi exercitatea metodelor de profilacsie
în boala tromboembolică (BTE) .Material şi metode. Din 1314 pacienți tratați chirurgical în 2005 - 2010 pentru litiază biliară, pacienții cu vârsta de
peste 60 ani au fost în 562 cazuri (42,8%). Operați pentru colecistită cronică - 419, pentru colecistită acută - 143 bolnavi. Colecistectomii clasice - 436 ,
laporoscopice - 126 cazuri. Durata intervenției chirurgicale de la 45 min – 2 ore 30 min. Evaluați factorii de risc ai BTE. S-au determinat: indicele protrombinic,
fibrionogenul, TTPA (timpul trombinei parțial active), TAR (timpul activ de recalcificare), proba cu etanol, timpul de sângerare şi coagulare,
s-a efectuat examinarea ecografică periferică Doppler în caz de afecțiuni vasculare.Rezultate. S-a constatat nivelul înalt de risc la toți pacienții cu vârsta
de peste 60 ani. . Necătând profilaxiei s-au constatat următoarele complicații trombembolice: embolia arterelor pulmonare de calibru mare – 1 caz,
embolia arterelor pulmonare de calibru mediu şi mic – 3 cazuri, tromboza venelor profunde – 2 cazuri, tromboflebită a venelor profunde – 2 cazuri,
tromboflebită a venelor superficiale – 3 cazuri, sindrom dolor in molet cu marcarea timpului de coagulare – 26 cazuri. Complicațiile au survenit în lotul
de pacienți operați în urgență, la indicații vitale, atât clasic cât şi laparoscopic. Profilaxia s-a efectuat cu heparină şi cu fraxiparină 2850ME - 3-4 zile.
În cazul emboliei ramurilor masive a a. pulmonare a survenit subit un deces, celelalte au fost rezolvate prin tratament cu anticoagulante şi externare
la a 15 - 23 zi, cu evaluare dinamică timp de trei luni.Introduction. Thrombembolic disease is a severe complications in surgery and need permanent improuvment of diagnosis and treatment..
Purpose. Analysis of risk factors and the prevention measures in thrombembolic complications (TEC) in treatment experience of aged patients
with biliary stones .Material and methods: Of 1314 patients treated in 2005 - 2010 for biliary lithiasis, the aged patients were in 562 cases
(42,8%). The cronic cholecystitis was in 419 cases, the acute cholecystitis was in 143 cases. The classic cholecystectomy was been done in 436
cases, the laparoscopic cholecistectomy - in 126 cases. The operations time was from 45 min to 2 hour 30 min. The analysis of risk factors of
thrombembolic complications was been done. The laboratory investigations included : blood prothrombin, blood coagulation, the test with
ethanol, hemoconcentration, Doplerography.Results. It was noticed the high risk of TEC in all aged patients. But in post operation period we
determined some complications: the pulmonary large artery thrombembolism -1case, the little bronchus of pulmonary artery thrombembolism
–3 cases, the thrombosis of deep veins-2 cases, the thrombophlebitis of deep veins-2 cases, the thrombophlebitis of varicose veins-3 cases, the
arching pain syndrome with the increasing of coagulation time in 26 cases. These complications had appeared in group with urgent surgical
treatment and was in classic and laparoscopic cholecystectomy. We notice that these patients were administrated heparin, fragmin and fraxiparin
for 3-4 days. One case of pulmonary embolism had a fatal outcomes.The treatment of thrombembolic complications was anticoagulant therapy.
The patients were discharged from the hospital on the 15 – 23 days of the treatment. We follow-up of this patients for 3 months
Hemoperitoneum in close abdominal trauma - diagnostic and medico-surgical options
Scopul lucrării. Abordarea pacientului cu traumatism abdominal inchis(TAI) confruntă chirurgul cu două dileme majore: stabilirea
rapidă a diagnosticului si conduita terapeutică. Prezența în traumatismele abdominale închise a lichidului liber intraabdominal
reprezintă criteriul de bază, uneori și unicul semn de leziune a organelor intraperitoneale. Scopul lucrării este evaluarea rezultatelor
tratamentului pacienților cu TAI și hemoperitoneu.
Materiale și metode. Studiul include 49 pacienți cu TAI tratați în perioada 2021-2023. Prezența lichidului liber a fost stabilit prin USG
și TC. Vârsta a variat 18-82 ani, cu media 39,3±6,8 ani. Raport b/f – 38/11. Mecanismele traumatismelor au fost diverse. Traumatism
asociat - 46,9% cazuri.
Rezultate. Pacienții în cunoștință, hemodinamic stabili, în lipsa hemoragiei active și peritonitei, lichid liber <500 ml conform USG și TC,
au fost supuși tratamentului conservativ cu evaluare dinamică - 22 pacienți cu leziuni ale organelor parenchimatoase. Inconștiența,
hemodinamica instabilă, volum lichidian >1000 ml, cât și <1000 ml cu creșterea ulterioară a volumului lichidian au servit drept criterii
pentru laparoscopie diagnostică - 4 pacienți. Indicații pentru laparotomie au fost: eșecul hemostazei laparoscopice - 3 bolnavi și
prezența peritonitei și instabilitatea hematoamelor retroperitoneale confirmate clinic și instrumental în 23 cazuri. Letalitatea a constituit
14,3%, cu excepția pacienților tratați non-operator, cauzele fiind șocul hemoragic gr. II-III(7,6%), traumatic(2,1%), complicațiile
septice(4,1%) și TCC(2,1%).
Concluzii. Elementele cheie ce determină tactica chirurgicală în TAI cu hemoperitoneu sunt – activitatea hemoragiei și volumul
lichidului evaluat în dinamică. Diferențierea tacticilor de diagnostic și tratament cu implementarea tehnologiilor miniinvazive în
hemoperitoneul traumatic permite reducerea considerabilă a letalității, complicațiilor și laparotomiilor neargumentate.Aim of study. The approach to the patient with closed abdominal trauma (CAT) confronts the surgeon with two major dilemmas: the
rapid establishment of the diagnosis and the therapeutic attitude. In closed abdominal trauma, the presence of free intra-abdominal
fluid is the basic criterion, sometimes the only sign of injury to the intraperitoneal organs. The aim is evaluation of treatment outcomes
of patients with CAT and hemoperitoneum.
Materials and methods. The study includes 49 patients with CAT treated during 2021-2023. The presence of free fluid was established
by USG and CT. The age ranged from 18 to 82 years, with a mean of 39.3±6.8 years. Ratio m/f – 38/11. The mechanisms of trauma
were diverse. Associated trauma - 46.9% of cases.
Results. Conscious, hemodynamically stable patients, in the absence of active hemorrhage and peritonitis, free fluid <500 ml
according to USG and CT, underwent conservative treatment with dynamic evaluation - 22 patients with lesions of parenchymal
organs. Unconsciousness, unstable hemodynamics, fluid volume >1000 ml, as well as <1000 ml with subsequent increase in fluid
volume served as criteria for diagnostic laparoscopy - 4 patients. Indications for laparotomy were: failure of laparoscopic hemostasis
- 3 patients and the presence of peritonitis and instability of retroperitoneal hematomas confirmed clinically and instrumentally in 23
cases. The lethality was 14.3%, with the exception of patients treated non-operatively, the causes being hemorrhagic shock gr. IIIII(7.6%), traumatic(2.1%), septic complications(4.1%) and TCC(2.1%).
Conclusions. The key elements that determine the surgical tactics in CAT with hemoperitoneum are – the activity of the hemorrhage
and the fluid volume evaluated dynamically. The differentiation of diagnostic and treatment tactics with the implementation of miniinvasive technologies in traumatic hemoperitoneum allows the considerable reduction of lethality, complications and unproven
laparotomies

complications in gallstone disease.
Material and methods
The analysis of 14 cases of GC Conclusions
clinically diagnosed and treated in
2010-2019. 9 patients had anamnesis
of gallstones for more than 10 years.
Women- 10 men- 4. The diagnosis
included: clinical aspects, laboratory
tests, USG, contrast CT, ERSP, MRI with
cholangiography, laparoscopy.
Results
Increased efficacy in diagnosis had MRI
with cholangiography and CT. Treatment
methods: laparoscopic cholecystectomy
with enlarged diathermocoagulation of the
vesicular lodge(2), extended classical
cholecystectomy with enlarged resection
of the area of destruction and ablation of
regional lymph nodes, Kerh drainage (2),
classic cholecystectomy with external
tumor drilling and Champeau drainage(1),
cholecystectomy, tumor drilling and
internal drainage (3), cholecisectomy,
tumor resection, bihepaticojejunostomy a
la Roux and ablation of lymph nodes (6)
Morbidity (50%).
Conclusions
The volume of surgery in CVB correlates
with the tumor stage. Radical surgery with
enlarged tumor removal contributes to a

Association of two severe surgical emergencies - solution variant
Background.
With an incidence of 0.7-4.4%, Mirizzi
syndrome is considered a rare and severe
complication of gallstones. Pathology is
even less frequently reported in the
literature in conjunction with other
medical-surgical emergencies.
Purpose.
Analysis of the treatment experience of
the elderly patient with associated severe
surgical emergencies.
Material and methods. Treatment opportunity in a 76-year-old patient with
multiple severe concomitants is presented. Clinical
diagnosis: Mirizzi II syndrome, with progressive
mechanical jaundice; giant antral peptic ulcer,
complicated with repeated digestive bleeding, grade III
anemia; ischemic heart disease, angina pectoris, CI III
NIHA, taxisystolic permanent atrial fibrillation; drug
coagulopathy; hypertension gr. III.
Results.
Hospitalized for progressive mechanical jaundice syndrome. MRI
cholangiography over 24 hours - complicated gallstones with
Mirizzi II syndrome. FGDS - bile absent in the duodenum, at the
same time - in the gastric antral region presence of giant peptic
ulcer. On the 3rd day of hospitalization, against the background of
hypocoagulability, caused by the permanent use of anticoagulants
for cardiac pathology, the peptic ulcer was complicated by a spurt of
digestive hemorrhage, stopped endoscopically. Despite the
administration of a complex pathogenetic treatment, he had
repeated hemorrhage over 24 hours with indications for
emergency treatment without delay. Surgery: gastric antral
resection with short-loop gastro-jejunal anastomosis, anterograde
partial cholecystectomy, choledochotomy with removal of stones,
choledocho-duodenoanastomosis with transcystic drainage of the
bile ducts. Simple postoperative evolution. Patient discharged on
the 14th day after surgery. Biliary drain removed in 3 months.
Monitored 2 years. Evolution without complications.
Conclusions. The individualized selection of the
type and terms of surgery in an

Diagnostic and medico-surgical options in Mirizzi syndrome
Scopul lucrării. Prezentându-se cu o incidență rară la pacienții cu litiază biliară de 1 la 1000, sindromul Mirizzi(SM) rămâne o
provocare serioasă atât pentru un chirurg începător, cât și pentru un bun practician. În pofida modernizării tehnicilor de abordare,
subiectul rămâne unul dificil în chirurgia biliară. Scopul lucrării este evaluarea rezultatelor tratamentului pacienților cu sindrom Mirizzi
Materiale și metode. Experienţa diagnostică şi terapeutică a 13 pacienţi cu SM, tratați în anii 2006 - 2019. Vârsta a variat 54-81 ani,
cu media 69,9±2,6 ani. Raport f/b – 8/5. Anamnestic de litiază biliară -10,1±0,43 ani
Rezultate. Analiza a confirmat la toți pacienții prezența durerilor colicative, icter obstructiv asociat și angiocolită recurentă, cu o suferință
litiazică de 7- 14 ani. 11 pacienţi au manifestat un icter recurent tranzitoriu, icterul continuu a fost în 2 cazuri. Conform clasificării
Csendez: tip I - 2 cazuri, tip II - 4, tip III - 5, tip IV - 2 cazuri. Metode elective sugestive de diagnostic preoperator: colangiografie
RMN, CPGR endoscopică asociată cu PST și drenarea căilor biliare în sindrom icteric persistent. În 10 cazuri a fost utilizată
colangiografia intraoperatorie. Volumul operator a fost selectat dependent de tipul leziunii. Înlăturarea calculului și lichidarea defectului
fistular prin utilizarea de preferinţă a unui lambou vezicular menajat(4) cu drenare Kehr - 6 cazuri; colecistectomia, coledocolitotomia
coledocoduodenostomie în 4 cazuri; colecistectomia, coledocolitotomia coledochojejunostomie Roux-în-Y în 3 cazuri. Morbiditate în
3 cazuri(27,3%), mortalitate - 0%
Concluzii. Preoperator diagnosticul sugestiv de sindrom Mirizzi se axează pe colangiografia RMN şi CPGRE, diagnosticul de certitudine
fiind dat de colangiografia intraoperatorie premergătoare disecției veziculei biliare. Obiectivele intervenţiei necesită adaptate tipului și
particularităţilor morfologice, în funcţie de topografia fistulei.Aim of study. Presenting with a rare incidence in patients with biliary lithiasis 1 in 1000, Mirizzi syndrome(MS) remains a serious
challenge for both the novice surgeon and the good practitioner. Despite the modernization of approach techniques, the subject
remains a difficult one in biliary surgery. The aim of study was the evaluation of treatment outcomes of patients with Mirizzi syndrome.
Materials and methods. The diagnostic and therapeutic experience of 13 patients with MS, treated between 2006 and 2019. The age
ranged from 54 to 81 years, with an average of 69.9±2.6 years. f/m ratio – 8/5. History of gallstones -10.1±0.43 years.
Results. The analysis confirmed in all patients the presence of colic pains, signs of associated obstructive jaundice and recurrent
angiocolitis, on the background of 7-14 years of biliary lithiasis. 11 patients showed transient recurrent jaundice, continuous jaundice
was in 2 cases. According to the Csendez classification : type I - 2 cases, type II - 4, type III - 5, type IV - 2 cases. Suggestive
elective methods of preoperative diagnosis: MRI cholangiography, endoscopic CPGR associated with PST and drainage of bile
ducts in persistent jaundice syndrome. Intraoperative cholangiography was used in 10 cases. The operative volume was selected
depending on the type of lesion. Removal of the calculus and liquidation of the fistula defect by preferably using a managed vesical
flap with Kehr drainage - 6 cases; cholecystectomy, choledocholithotomy choledochoduodenostomy in 4 cases; cholecystectomy,
choledocholithotomy, Roux-in-Y choledochojejunostomy in 3 cases. Morbidity in 3 cases (27.3%), mortality - 0%.
Conclusions. Preoperatively, the suggestive diagnosis of Mirizzi syndrome focuses on MRI cholangiography and CPGRE, the certainty diagnosis being given by the intraoperative cholangiography preceding the dissection of the gallbladder. The objectives of
the intervention need to be adapted to the type and morphological particularities, depending on the topography of the fistula
