    EXERCISE INDUCES HSP70 IN THE VASCUALTURE OF THE MYOCARDIUM

    Exercise (EX) and heat-shock (HS) both lead to increased myocardial content of the cardioprotective inducible isoform of the 70 kDa heat shock protein, (Hsp70). Twenty- four hours after HS, myocardial Hsp70 is localized within blood vessels, indicating a cell-type specific response to this stress. The objective of the current study was to determine whether a similar localization of myocardial Hsp70 occurs post-EX. Adult male Sprague Dawley rats were randomized into control (CON), EX (60 min treadmill running, 30 māˆ•min) or HS (42Ā°C, 15 min) groups. EX group was further divided into an acute exercise bout and a 5 day training group. Left ventricles (LV) were harvested at 30 min and 24 h post-treatment and analyzed for Hsp70 by both western blotting and confocal immunofluorescence microscopy. Both HS and EX resulted in significant increases in Hsp70 in LV homogenates. Twenty-four h post-EX and HS, confocal microscopy revealed a striking increase in localization of Hsp70 in smooth muscle of the larger blood vessels within the myocardium. Hsp70 was evident in the endothelium of hearts from all HS animals whereas it was observed in endothelium of only those EX animals that reached higher (40.5oC) exercise temperatures. These data support previous findings with HS and suggest that the myocardial increases in Hsp70 associated with exercise are also primarily limited to the vasculature. Differences in the models appear to be largely due to temperature difference, suggesting that heat may be the primary stimulus for the exercise induced response

    Eosinophilic Pleural Effusion: A Rare Manifestation of Hypereosinophilic Syndrome

    Several causes of eosinophilic pleural effusions have been described with malignancy being the commonest cause. Hypereosinophilic syndrome (HES) is a rare disease and very few cases have been reported of HES presenting as eosinophilic pleural effusion (EPE). We report a case of a 26-year-old male who presented with shortness of breath. He had bilateral pleural effusions, generalized lymphadenopathy, splenomegaly, and leukocytosis with marked peripheral blood eosinophilia. The pleural fluid was exudative, with 25%ā€“30% eosinophilis, and absence of neoplastic cells. Hypereosinophilic syndrome was diagnosed after other causes of eosinophilia were excluded. He continued to be dyspneic with persistent accumulation of eosinophilic pleural fluid, even after his peripheral eosinophil count had normalized in response to treatment. This patient represents a very unusual presentation of HES with dyspnea and pleural effusions and demonstrates that treatment based on response of peripheral eosinophil counts, as is currently recommended, may not always be clinically adequate

    The ancient southern Levantine city of Gezer is well-known from Egyptian, Biblical and Assyrian sources, associated with power struggles, conquests, and intriguing tales involving figures such as Milkilu and Amenhotep III, Merneptah, the Philistines, Solomon and his unidentified pharaonic father-in-law, and Shishak / Sheshonq I. Since the identity of Gezer with ā€œTell Jezerā€ is quite literally ā€˜set in stoneā€™ by some dozen boundary inscriptions, along with impressive Bronze and Iron Age remains, research at this site provides a unique opportunity to compare text and archaeology, as well as bring to light the undocumented everyday lives of the cityā€™s inhabitants. In this endeavour, independent scientific dating is crucial for anchoring the remains chronologically. This paper presents the first substantial radiocarbon dataset and Bayesian chronological analysis for Gezer spanning the last part of the Late Bronze Age (LBA; LB IIB) through Iron Age II. The dataset derives from an essentially continuous stratigraphic sequence exposed in recent years by the Tandy expedition along the central-southern edge of the site. The results allow us for the first time to independently determine the site chronology, test the viability (from a chronological perspective) of proposed historical correlations, and contribute to debates on Philistine and Iron Age chronology.</p

    Type Ia Supernova Distances at z > 1.5 from the Hubble Space Telescope Multi-Cycle Treasury Programs: The Early Expansion Rate

    We present an analysis of 15 Type Ia supernovae (SNe Ia) at redshift z > 1 (9 at 1.5 < z < 2.3) recently discovered in the CANDELS and CLASH Multi-Cycle Treasury programs using WFC3 on the Hubble Space Telescope. We combine these SNe Ia with a new compilation of 1050 SNe Ia, jointly calibrated and corrected for simulated survey biases to produce accurate distance measurements. We present unbiased constraints on the expansion rate at six redshifts in the range 0.07 < z < 1.5 based only on this combined SN Ia sample. The added leverage of our new sample at z > 1.5 leads to a factor of ~3 improvement in the determination of the expansion rate at z = 1.5, reducing its uncertainty to ~20%, a measurement of H(z=1.5)/H0=2.67 (+0.83,-0.52). We then demonstrate that these six measurements alone provide a nearly identical characterization of dark energy as the full SN sample, making them an efficient compression of the SN Ia data. The new sample of SNe Ia at z > 1 usefully distinguishes between alternative cosmological models and unmodeled evolution of the SN Ia distance indicators, placing empirical limits on the latter. Finally, employing a realistic simulation of a potential WFIRST SN survey observing strategy, we forecast optimistic future constraints on the expansion rate from SNe Ia.Comment: 14 pages, 5 figures, 7 tables; submitted to Ap

    Increased adherence to perioperative safety guidelines associated with improved patient safety outcomes:a stepped-wedge, cluster-randomised multicentre trial

    Background: National Dutch guidelines have been introduced to improve suboptimal perioperative care. A multifaceted implementation programme (IMPlementatie Richtlijnen Operatieve VEiligheid [IMPROVE]) has been developed to support hospitals in applying these guidelines. This study evaluated the effectiveness of IMPROVE on guideline adherence and the association between guideline adherence and patient safety. Methods: Nine hospitals participated in this unblinded, superiority, stepped-wedge, cluster RCT in patients with major noncardiac surgery (mortality risk >= 1%). IMPROVE consisted of educational activities, audit and feedback, reminders, organisational, team-directed, and patient-mediated activities. The primary outcome of the study was guideline adherence measured by nine patient safety indicators on the process (stop moments from the composite STOP bundle, and timely administration of antibiotics) and on the structure of perioperative care. Secondary safety outcomes included in-hospital complications, postoperative wound infections, mortality, length of hospital stay, and unplanned care. Results: Data were analysed for 1934 patients. The IMPROVE programme improved one stop moment: 'discharge from recovery room' (+16%; 95% confidence interval [CI], 9-23%). This stop moment was related to decreased mortality (-3%; 95% CI, -4% to -1%), fewer complications (-8%; 95% CI, -13% to -3%), and fewer unscheduled transfers to the ICU (-6%; 95% CI, -9% to -3%). IMPROVE negatively affected one other stop moment - 'discharge from the hospital' - possibly because of the limited resources of hospitals to improve all stop moments together. Conclusions: Mixed implementation effects of IMPROVE were found. We found some positive associations between guideline adherence and patient safety (i.e. mortality, complications, and unscheduled transfers to the ICU) except for the timely administration of antibiotics

    Type Ia Supernova Rate Measurements To Redshift 2.5 From CANDELS: Searching For Prompt Explosions In The Early Universe

    dThe Cosmic Assembly Near-infrared Deep Extragalactic Legacy Survey (CANDELS) was a multi-cycle treasury program on the Hubble Space Telescope (HST) that surveyed a total area of -0.25 deg2 with -900 HST orbits spread across five fields over three years. Within these survey images we discovered 65 supernovae (SNe) of all types, out to z 2.5. We classify -24 of these as Type Ia SNe (SNe Ia) based on host galaxy redshifts and SN photometry (supplemented by grism spectroscopy of six SNe). Here we present a measurement of the volumetric SN Ia rate as a function of redshift, reaching for the first time beyond z =- 2 and putting new constraints on SN Ia progenitor models. Our highest redshift bin includes detections of SNe that exploded when the universe was only -3 Gyr old and near the peak of the cosmic star formation history. This gives the CANDELS high redshift sample unique leverage for evaluating the fraction of SNe Ia that explode promptly after formation ( 40 Myr. However, mild tension is apparent between ground-based low-z surveys and space-based high-z surveys. In both CANDELS and the sister HST program CLASH (Cluster Lensing And Supernova Survey with Hubble), we find a low rate of SNe Ia at z > 1. This could be a hint that prompt progenitors are in fact relatively rare, accounting for only 20% of all SN Ia explosions-though further analysis and larger samples will be needed to examine that suggestion.
