    Primary small-cell neuroendocrine carcinoma of the duodenum – a case report and review of literature

    BACKGROUND: Small-cell neuroendocrine carcinoma in the duodenum is an extremely rare neoplasm with poor prognosis. CASE PRESENTATION: A 57-year-old man presented with sudden onset gastrointestinal bleeding and fainting attacks. Duodenoscopy and hypotonic duodenography revealed a 3 × 3 cm protruding tumor with ulcerations situated opposite the ampulla of Vater in the second part of the duodenum. Local excision of the tumor was performed, followed by adjuvant chemotherapy with 5-fluoro uracil and leucovorin. Examination of the tumor by immunohistochemistry and electron microscopy indicated it to be neuroendocrine in nature, expressing synaptophysin and AE1/AE3, and containing dense core granules. The patient showed no sign of recurrence and has been disease-free for more than 48 months after surgery. CONCLUSIONS: Most cases of small-cell neuroendocrine carcinoma in the duodenum show rapid progression of the disease, and even radical surgery with or without chemotherapy do not prevent death. We report a rare subtype of small-cell neuroendocrine carcinoma. This subtype appears to have a much better prognosis, and may be amenable to local excision, if the lesion is away from the ampulla of Vater

    CNVs in Three Psychiatric Disorders

    BACKGROUND: We aimed to determine the similarities and differences in the roles of genic and regulatory copy number variations (CNVs) in bipolar disorder (BD), schizophrenia (SCZ), and autism spectrum disorder (ASD). METHODS: Based on high-resolution CNV data from 8708 Japanese samples, we performed to our knowledge the largest cross-disorder analysis of genic and regulatory CNVs in BD, SCZ, and ASD. RESULTS: In genic CNVs, we found an increased burden of smaller (500 kb) exonic CNVs in SCZ/ASD. Pathogenic CNVs linked to neurodevelopmental disorders were significantly associated with the risk for each disorder, but BD and SCZ/ASD differed in terms of the effect size (smaller in BD) and subtype distribution of CNVs linked to neurodevelopmental disorders. We identified 3 synaptic genes (DLG2, PCDH15, and ASTN2) as risk factors for BD. Whereas gene set analysis showed that BD-associated pathways were restricted to chromatin biology, SCZ and ASD involved more extensive and similar pathways. Nevertheless, a correlation analysis of gene set results indicated weak but significant pathway similarities between BD and SCZ or ASD (r = 0.25–0.31). In SCZ and ASD, but not BD, CNVs were significantly enriched in enhancers and promoters in brain tissue. CONCLUSIONS: BD and SCZ/ASD differ in terms of CNV burden, characteristics of CNVs linked to neurodevelopmental disorders, and regulatory CNVs. On the other hand, they have shared molecular mechanisms, including chromatin biology. The BD risk genes identified here could provide insight into the pathogenesis of BD

    Role of Neuron–Glia Signaling in Regulation of Retinal Vascular Tone in Rats

    The interactions between neuronal, glial, and vascular cells play a key role in regulating blood flow in the retina. In the present study, we examined the role of the interactions between neuronal and glial cells in regulating the retinal vascular tone in rats upon stimulation of retinal neuronal cells by intravitreal injection of N-methyl-d-aspartic acid (NMDA). The retinal vascular response was assessed by measuring the diameter of the retinal arterioles in the in vivo fundus images. Intravitreal injection of NMDA produced retinal vasodilation that was significantly diminished following the pharmacological inhibition of nitric oxide (NO) synthase (nNOS), loss of inner retinal neurons, or intravitreal injection of glial toxins. Immunohistochemistry revealed the expression of nNOS in ganglion and calretinin-positive amacrine cells. Moreover, glial toxins significantly prevented the retinal vasodilator response induced by intravitreal injection of NOR3, an NO donor. Mechanistic analysis revealed that NO enhanced the production of vasodilatory prostanoids and epoxyeicosatrienoic acids in glial cells in a ryanodine receptor type 1-dependent manner, subsequently inducing the retinal vasodilator response. These results suggest that the NO released from stimulated neuronal cells acts as a key messenger in neuron&#8211;glia signaling, thereby causing neuronal activity-dependent and glial cell-mediated vasodilation in the retina

    Activated Carbon Impregnated with Elementary Iodine: Applications against Virus- and Bacteria-Related Issues

    An iodine-doped activated carbon (named IodAC) was developed by adsorbing molecular iodine (I2) on commercially available activated carbon (AC). Iodine was selected with the purpose to add its well-known antibacterial and antiviral properties to the AC and in order to produce an innovative material for environmental pathogens control and for healthcare-related applications. The impregnation method achieved the goal of strongly adsorbing iodine on the AC surface, since both volatility and water solubility resulted to be negligible, and therefore it did not affect the stability of the material. An antibacterial test (on Escherichia coli) and an antiviral test (on an avian influenza strain) were conducted, showing the effectiveness of IodAC against the pathogens. In addition, IodAC was also compared to slaked lime (a material widely used for disinfection of outdoor spaces and livestock farming areas). The data proved the performance of IodAC against virus and bacteria and also evidenced a more stable and long-lasting disinfecting power of IodAC compared to slaked lime, the later reacting with carbon dioxide and suffering a gradually decrease of its disinfectant power; such drawback does not affect IodAC. Overall, the presented results show that IodAC can be used for a wide range of applications, including as a granular disinfectant for public spaces, for water disinfection, zoonotic diseases countermeasures (e.g., as an animal feed additive for avian influenza control), post-harvest food storage, and sanitization. Its characteristics also indicate its potential to be used for medical treatments, such as for blood, intestinal (for HIV, sepsis, irritable syndrome, ulcerative colitis therapy), and medical supplies (antibacterial bandages, gauze, cotton, etc.) sterilization
