Effects of Dexmedetomidine on Serum Interleukin-6, Hemodynamic Stability, and Postoperative Pain Relief in Elderly Patients under Spinal Anesthesia
The beneficial effects of dexmedetomidine (DEX) have not been extensively investigated in elderly patients receiving spinal anesthesia. This study evaluated the effects of intravenous DEX infusion on stress and hemodynamic response, as well as on postoperative analgesia in elderly patients undergoing total knee arthroplasty (TKA). We randomly allocated 45 adult patients to 3 patient groups (n=15 each): uni-saline group patients underwent unilateral TKA with saline administration, uni-DEX group patients underwent unilateral TKA with DEX administration, and bilateral-DEX group patients underwent bilateral TKA with DEX administration. Serum interleukin-6 (IL-6) levels were significantly lower in the bilateral-DEX group than in the uni-saline group 6 and 24h postoperatively, and were negatively correlated with total DEX dosage 24h postoperatively. Bradycardia occurred more frequently in the uni-DEX and bilateral-DEX groups than in the uni-saline group. The total dose of required supplementary analgesics was significantly higher in the uni-saline group than in the uni-DEX and bilateral-DEX groups 6h postoperatively. The results indicate that perioperative intravenous DEX administration decreases postoperative serum IL-6 levels in patients undergoing bilateral TKA, and has a postoperative analgesic effect in patients undergoing unilateral or bilateral TKA
Application of motion correction using 3D autoregressive model in kinect-based telemedicine
In telemedicine, where the convergence of different types of medical treatment occurs, it is very important to establish credibility regarding the mutual communication between patients and medical workers by acquiring and sharing more accurate data. For rehabilitation treatment in particular, where motion data are required, auxiliary equipment such as a Kinect sensor is being more widely used.This study proposes a methodology for improving the motion recognition rate by compensating the noise from a Kinect sensor using a 3D auto regressive model. Moreover, this study investigates the methods applied for vitalizing the area of telemedicine under this particular trend
The Effects of Design Attributes on Other Attributes and Product Evaluation
This study examines how consumers perceptions of a products design attributes influence their perceptions on other product attributes, and attitudes toward the focal product. This study explored image-related adjectives in order to analyze product attributes, which are categorized into design, symbolic, and functional factors. The empirical test results confirmed the mediating role of attitudes toward design, which indicate the overall preference about product design, between design attributes and attitudes toward product, while the hypothesis of the moderating role of design sensitivity was not supported. These findings revealed that design attributes have influences beyond the aesthetic value, as product design rouses symbolic value and delivers information about functions. Therefore, in the context of product development process, it would be effective to distinguish design elements that can strengthen advantages and supplement weaknesses and to measure attitudes toward design that can be an important index of product evaluation

Strong charge-spin coupling is found in a layered transition-metal
trichalcogenide NiPS3, a van derWaals antiferromagnet, from our study of the
electronic structure using several experimental and theoretical tools:
spectroscopic ellipsometry, x-ray absorption and photoemission spectroscopy,
and density-functional calculations. NiPS3 displays an anomalous shift in the
optical spectral weight at the magnetic ordering temperature, reflecting a
strong coupling between the electronic and magnetic structures. X-ray
absorption, photoemission and optical spectra support a self-doped ground state
in NiPS3. Our work demonstrates that layered transition-metal trichalcogenide
magnets are a useful candidate for the study of correlated-electron physics in

Influence of Long-term Climate on Fatigue Life of Bridge Pier Concrete and a Reinforcement Method
This paper quantitatively evaluated the fatigue life of concrete around the air-water boundary layer of bridge piers located in inland rivers, considering the long-term climate. The paper suggests a method to predict the low-cycle fatigue life by demonstrating a thermal-fluid-structural analysis of bridge pier concrete according to long-term climate such as temperature, velocity and pressure of air and water in the process of freezing and thawing in winter. In addition, it proposes a reinforcing method to increase the life of damaged piers and proves the feasibility of the proposed method with numerical comparison experiment
Visfatin exerts angiogenic effects on human umbilical vein endothelial cells through the mTOR signaling pathway
AbstractThe biologically active factors known as adipocytokines are secreted primarily by adipose tissues and can act as modulators of angiogenesis. Visfatin, an adipocytokine that has recently been reported to have angiogenic properties, is upregulated in diabetes, cancer, and inflammatory diseases. Because maintenance of an angiogenic balance is critically important in the management of these diseases, understanding the molecular mechanism by which visfatin promotes angiogenesis is very important. In this report, we describe our findings demonstrating that visfatin stimulates the mammalian target of the rapamycin (mTOR) pathway, which plays important roles in angiogenesis. Visfatin induced the expression of hypoxia-inducible factor 1α (HIF1α) and vascular endothelial growth factor (VEGF) in human endothelial cells. Inhibition of the mTOR pathway by rapamycin eliminated the angiogenic and proliferative effects of visfatin. The visfatin-induced increase in VEGF expression was also eliminated by RNA interference-mediated knockdown of the 70-kDa ribosomal protein S6 kinase (p70S6K), a downstream target of mTOR. Visfatin inactivated glycogen synthase kinase 3β (GSK3β) by phosphorylating it at Ser-9, leading to the nuclear translocation of β-catenin. Both rapamycin co-treatment and p70S6K knockdown inhibited visfatin-induced GSK3β phosphorylation at Ser-9 and nuclear translocation of β-catenin. Taken together, these results indicate that mTOR signaling is involved in visfatin-induced angiogenesis, and that this signaling leads to visfatin-induced VEGF expression and nuclear translocation of β-catenin
A case of back pain caused by Salmonella spondylitis -A case report-
Salmonella spondylitis is a rare illness, and it generally occurs in patients who have already had sickle cell anemia, and it is even rarer in patients who are without sickle cell anemia. A 61-year-old male patient was hospitalized for the evaluation of his renal function and then treatment was started for his back pain. His back pain had developed about 2 months previously without any specific trauma. Only a bulging disc was detected on the initial lumbar MRI. Regarding his fever, it was diagnosed as possible atypical pneumonia, scrub typhus, etc., and multiple antibiotic therapy was administered. At the time of transfer, the leucocytes and hs-CRP were normal and the ESR was elevated. A diagnostic epidural block was performed for his back pain, but his symptoms were not improved. Lumbar MRI was performed again and it showed findings of infective spondylitis. Salmonella D was identified on the abscess culture and so he was diagnosed as suffering from Salmonella spondylitis. After antibiotic treatment, his back pain was improved and the patient was able to walk
Idiopathic severe hypermagnesemia in an extremely low birth weight infant on the first day of life
A preterm female infant born at 27 weeks of gestation with a birth weight of 990 g developed acute hypotonia, apnea, hypotension and bradycardia mimicking septic shock syndrome at 14h after birth. Laboratory tests indicated a severe hypermagnesemia of 45 mg/dL. The renal function, complete blood count and maternal blood concentrations of magnesium were normal, and the blood cultures were negative. The patient recovered with treatment including exchange transfusion. However, the etiology of the severe hypermagnesemia remains unknown
