A III. szintű neonatális intenzív centrumokban dolgozó orvosok szakmai és családi jellemzői, nehézségei, elégedettsége, erőforrásai: Magyarországi helyzetkép
Absztrakt:
Bevezetés: Magyarországon 1997 óta még nem készült olyan
felmérés, amely a neonatális intenzív centrumokban (NIC) dolgozó orvosok
helyzetét vizsgálta volna. Célkitűzés: Átfogó helyzetkép
nyújtása a NIC III. szintű intézményekben dolgozó orvosok szociodemográfiai
jellemzőiről, magán- és szakmai jellegű nehézségeiről, családi és munkahelyi
szerepekre, az egészséges életmód gyakorlására vonatkozó elégedettségéről,
erőforrásairól. Módszer: Felmérést végeztünk 2015. április és
2016. január között a magyarországi NIC-ekben önkitöltős, anonim kérdőíves
módszerrel. Eredmények: Az orvosok (n = 111) jellemzően a
középkorosztályt képviselik, magasan kvalifikáltak, beosztottként dolgoznak,
többségüknek van másodállása. A férfiakra jellemző a vezetői és vezetőhelyettesi
pozíció. A családi állapot tekintetében jellemzően párkapcsolatban élnek.
Számukra nem a családi-magánéleti, hanem a szakmai élet és az anyagi kihívások
okoznak nehézséget. A párválasztás és gyermekvállalás, valamint a munkahelyi
nehézségek miatt családi konfliktusokkal is küzdenek. Elégedettségük leginkább a
szülői, a társkapcsolati és munkahelyi szerepükre igaz, azonban a családra mint
anyagi biztonságot nyújtó erőforrásra, valamint az egészséges életmódra
vonatkozóan ez már kevésbé jellemző. Nehéz élethelyzetekben a barátok és a
családtagok mellett a munkatársakra, a munkahelyi vezetőkre is számíthatnak.
Következtetés: Elsőként készült a NIC III. szinten dolgozó
orvosok körében átfogó helyzetkép, lehetővé téve a szakmai-magánéleti-családi

Abstract:
Introduction: A survey that investigates the situation of
physicians working at neonatal intensive care (NIC) centres has not been made
since 1997. Aim: To give an overview of the sociodemographic
characteristics; personal and professional problems; the satisfaction with their
job and family roles; their levels of healthy lifestyle; resources in the
families of physicians working at NIC. Method: We have made an
examination in Hungary at NIC among physicians from April 2015 till January
2016, with an anonym self-fill-in questionnaire. Results: The
physicians involved in the research (n = 111) are representatively middle-aged
people. They are well-qualified and hold a subordinate post. Most of them have
second jobs. The manager and deputy-manager positions are characteristic for the
men. Most of them live in family. The main problems for them are financial and
professional difficulties, not the family or personal problems. They have
conflicts in families because of finding partners and having children as well as
job problems. They are satisfied with their parenting, partnership and
professional roles but they are not contented with their family role as a
financial safety provider person and as a person living a healthy life. In
difficult situations in their life, they can count on friends and family
members, furthermore they can rely on their workmates and superiors.
Conclusion: It is the first research that focuses on the
general situation of physicians working at level 3 NIC centres. This is the
first time when a survey investigates their professional-personal-family life.

Difficulties, health problems and professional support of divorced fathers living separately from their children - experiences in Hungary
Introduction. The situation of divorced men who live separetely children is difficult in Hungary, as well as in many other countries. The effects of divorce on all family members is considerable, therefore, the topic seems worth investigating. Aim. The aim of our study was to examine the situation, difficulties and problems of divorced Hungarian fathers who live separately from their children, and their need for professional help. Material and methods. The research was conducted among men of legal age who had undergone divorce for the first time, had at least one child together with their ex-wife, and the divorce had taken place in the last 1 to 5 years. A self-made questionnaire composed of four parts (6 open and 60 closed questions) was presented to the participants. The questionnaire was available online between 21/11/2015 and 20/01/2016. 100 fathers who filled in the questionnaire correctly were included in the study. Results. Only 18.0% of the fathers had not noticed any physical, mental or behavioural symptoms after divorce. 49.0% of them noticed all three types of symptoms. Several fathers' work performance declined. One third (32.0%) of the fathers had such serious symptoms that they decided to consult a medical specialist. 80.0% of the fathers were not satisfied with financial support offered to them, 76.0% of them were not satisfied with the social allowances available to them, and 74.0% were not satisfied with the psychological and psychiatric support they are offered. Conclusions. Fathers are not prepared to face the divorce. Physicians should be increasingly aware that divorce can be the source of physical symptoms. Fathers feel the lack of financial, social and psychological support, as well as of legal support. They would like to be involved in the family care of the Hungarian health visitors' network after divorce. © 2016 Borgis
Koraszülöttek és a koraszülött ellátás történeti vonatkozásai
A neonatológiában alkalmazott új gyakorlatok és technológiák - például a világszerte alkalmazott szülés előtti kortikoszteroidok és felületaktív anyagok, valamint a nagyfrekvenciás szellőzés - rohamos fejlődésével párhuzamosan az elmúlt évtizedben jelentősen csökkent a koraszülöttek mortalitási rátája (Goldenberg és mtsai 2008, Saigal, Doyle 2008, Crump és mtsai 2011) és nőtt az 1500 gramm alattiak (VLBW), valamint a 32. hét előtt születettek (VLGA) túlélési aránya. Számos tanulmány számol be a VLBW-el születettek túlélési arányáról, amelyek ugyanakkor igen eltérő képet mutatnak. (Nayeri és mtsai 2013, El-Metwally, Vohr, Tucker 2000, Louis és mtsai 2004, Darlow, Cust, Donoghue 2003, Dmnan és mtsai 2003, Jeschke és mtsai 2016) Míg az 1960-as években az 1000 gramm születési súlyú újszülöttek esetében 95%-os mortalitással lehetett számolni, addig 2007-re ennek a csoportnak a túlélési aránya már 95% körüli volt.
