Kontrowersje, jakie w ostatnich latach wywołało
pojęcie zespołu metabolicznego, sprawiają, że związki hiperglikemii, dyslipidemii, otyłości, nadciśnienia
i miażdżycy stały się obszarem szczególnie interesujących badań. Postęp wiedzy o patomechanizmach
rządzących elementami zespołu metabolicznego
może przybliżyć wyjaśnienie przyczyn głównych chorób cywilizacyjnych. Coraz większą uwagę zwraca
się na endokrynną czynność trzewnej tkanki tłuszczowej. Jednym z wielu hormonów produkowanych
przez tkankę tłuszczową jest omentyna -białko, którego funkcją prawdopodobnie jest zwiększanie wrażliwości na insulinę. Wyniki dalszych badań roli tej adipokiny mogą dostarczyć kolejnych dowodów na
związki między składowymi zespołu metabolicznego.The diagnosis of metabolic syndrome became recently a controversial conception. Thus the associations
between hyperglycaemia, dyslipidaemia, obesity,
hypertension and atherosclerosis are the field of especially interesting research. The progress of our understanding into the pathogenesis of elements of the
metabolic syndrome may help to explain the background of many diseases of westernised and developing societies. The endocrine role of the visceral adipose tissue is the focus of a number of scientific
investigations. One of the mediators secreted by adipose tissue is omentin, a protein that functions as
a factor increasing insulin sensitivity. Further research
of the role of this adipokine may help to dissect the
pathogenic background of the metabolic syndrome
and consequently clarify its diagnosis

W niniejszej pracy przedstawiono problem przyrostu masy ciała u chorych na cukrzycę typu 1 i 2 leczonych insuliną. Opisano możliwe przyczyny tego
zjawiska, takie jak zwiększenie kaloryczności posiłków w obawie przed hipoglikemią, anaboliczne działanie insuliny oraz inna niż fizjologiczna droga podawania leku. Omówiono także zalety stosowania insuliny detemir, długodziałającego analogu insuliny,
oraz potencjalne mechanizmy tłumaczące jego korzystny wpływIn this review we describe the problem of excessive
weight gain during insulin treatment in type 1 and
type 2 diabetes. We explain possible causes of this
phenomenon, such as hypoglycemia and defensive
snacking, anabolic action of insulin, and results of
non-physiologic mode of insulin administration. We
explain advantages of use of detemir, a long-acting
insulin analog, in patients with diabetes and illustrate
mechanisms of its beneficial action in respect to
weight gain

Cukrzyca typu 2 stanowi jeden z najważniejszych
problemów epidemiologicznych ostatnich kilkudziesięciu lat. Według szacunków liczba chorych podwaja
się co 30 lat. Częstość cukrzycy gwałtownie wzrasta
wraz z wiekiem, dochodząc nawet do ponad 30%
u osób po 60. roku życia. W ciągu ostatnich lat częstość cukrzycy zwiększała się szybciej u mężczyzn niż
u kobiet. To sprawia, że mężczyzna w starszym wieku jest szczególnie narażony na tę chorobę. W niniejszej pracy omówiono patogenezę cukrzycy typu
2 u osób w starszym wieku, z uwzględnieniem zaburzeń swoistych dla płci męskiej. Przedstawiono też
powikłania i problemy terapeutyczne występujące
u osób w starszym wieku, a także preferowane metody leczenia tych chorych.Type 2 diabetes mellitus is one of the most severe
epidemiologic problems during last several dozens
of years. According to the recent estimations the
prevalence of diabetes doubles every 30 years. Diabetes prevalence quickly increases with age, achieving over 30% in the group of people over 60. Recently diabetes have displayed more rapid rise in males
than in females. This makes an ageing male especially susceptible for diabetes mellitus. In this review
the pathogenesis of type 2 diabetes mellitus in
elderly was presented, together with specific features observed in males. In addition, the diabetic complications were described, with emphasis on therapeutic problems that are present in ageing patients,
as well as the preferable treatment options in this
group of subjects

Jednak dopiero w XX wieku nastąpił przełom w leczeniu tej śmiertelnej wcześniej choroby, poznano
jej różnorodne postacie oraz lepiej zrozumiano ich
złożoną patogenezę. W 1999 roku przyjęto powszechnie akceptowany, zaproponowany przez
komitet ekspertów WHO podział uwzględniający
etiologię poszczególnych typów choroby. Efektem
klasyfikacji etiologicznej było szybkie poszerzenie podziału o nowe jednostki w miarę postępu wiedzy,
zwłaszcza z kolejnymi sukcesami badaczy podłoża
genetycznego cukrzycy. Odkrywane od kilkunastu
lat kolejne monogenowe formy tej choroby, takie
jak MODY, cukrzyca mitochondrialna, cukrzyca noworodkowa i cukrzyce lipoatroficzne, charakteryzują
się unikatowymi cechami klinicznymi oraz możliwością zastosowania swoistego leczenia, zapewniającego optymalną korektę genetycznie uwarunkowanego
defektu metabolicznego. Diagnostyka molekularna
odgrywa zatem coraz większą rolę w diabetologii
i w przyszłości może stać się rutynową procedurą
w niektórych sytuacjach klinicznych.Diabetes mellitus is a disorder, which was known
since the antiquity. Not before the 20th century the
breakthrough in management of this formerly lethal condition was achieved. The last century brought a better understanding of the pathogenesis and
the heterogeneity of multiple disorders constituting
distinct types of diabetes. In 1999 The Expert Committee of the World Health Organization has adopted the etiology-based classification of diabetes
mellitus, which is commonly accepted nowadays.
With the recent progression in understanding of the
molecular basis of diabetes, especially the genetics
of its rare forms, the etiologic classification expands
rapidly. Newly described monogenic forms of diabetes mellitus, such as MODY, mitochondrial diabetes, neonatal diabetes, and lipoatrophic types of diabetes are characterized by certain distinct clinical
features. In many cases, it is possible to adjust a specific therapeutic intervention to optimally manage
the genetic defect and subsequent metabolic abnormalities. Molecular diagnosis plays an increasing role
in contemporary diabetology, and in future it may
become a standard procedure in certain clinical situations
Analysis of gene expression to predict dynamics of future hypertension incidence in type 2 diabetic patients
BACKGROUND: The main focus of the Genetic Analysis Workshop 19 (GAW19) is identification of genes related to the occurrence of hypertension in the cohort of patients with type 2 diabetes mellitus (T2DM). The aim of our study was to predict dynamics of the future hypertension incidence, based on gene expression profiles, systolic and diastolic blood pressure changes in time, sex, baseline age, and cigarette smoking status. We analyzed data made available to GAW19 participants, which included gene expression profiles of peripheral blood mononuclear cells (PBMCs) from the diabetic members of 20 Mexican American families. METHODS: On the basis of mid blood pressure measurements at several time points, the coefficient of regression (slope) was calculated for each individual. We corrected the slope value in patients treated with antihypertensive medications. Feature preprocessing methods were used to remove highly correlated probes and linear dependencies between them. Subsequently, multiple linear regression model was used to associate gene expression with the regression coefficient calculated for each T2DM patient. Tenfold cross-validation was used to validate the model. We used linear mixed effects model and kinship coefficients to account for the family structure. All calculations were performed in R. RESULTS: This analysis allowed us to identify 6 well-annotated genes: RTP4, FXYD6, GDF11, IFNAR1, NOX3, and HLA-DQ2, associated with dynamics of future hypertension incidence. Two of them, IFNAR1 and NOX3 were previously implicated in pathogenesis of hypertension. CONCLUSIONS: There is no obvious mechanism that links all detected genes with dynamics of hypertension incidence. Identification of possible connection with hypertension needs further investigation

masy ciała u chorych na cukrzycę typu 1 i 2 leczonych
insuliną. Opisano możliwe przyczyny tego
zjawiska, takie jak zwiększenie kaloryczności posiłków
w obawie przed hipoglikemią, anaboliczne działanie
insuliny oraz inna niż fizjologiczna droga podawania
leku. Omówiono także zalety stosowania insuliny
detemir, długodziałającego analogu insuliny,
oraz potencjalne mechanizmy tłumaczące jego korzystny

powikłania i problemy terapeutyczne występujące
u osób w starszym wieku, a także preferowane metody
leczenia tych chorych

Obraz kliniczny cukrzycy typu 2 jest uwarunkowany
współistnieniem zaburzeń wydzielania insuliny i insulinooporności. Dzięki wysiłkom naukowców z całego świata wykazano związki między mutacjami
i polimorfizmami niektórych genów a wieloczynnikowymi i monogenowymi formami cukrzycy typu 2.
Formy monogenowe, mimo ich rzadkości, stanowią
obszar, w którym dokonał się znaczący postęp w zrozumieniu molekularnego podłoża cukrzycy typu 2.
Monogenowe formy cukrzycy typu 2 obejmują cukrzycę
MODY (maturity onset diabetes of the young), cukrzycę
mitochondrialną (MIDD, maternally inherited diabetes
with deafness) oraz inne rzadkie zespoły związane głównie ze skrajnie nasiloną insulinoopornością. Prace nad
identyfikacją genów odpowiedzialnych za częstszy,
uwarunkowany wieloczynnikowo typ cukrzycy typu 2
były mało owocne. Złożona cukrzyca typu 2 wiąże się
z interakcją wielu czynników środowiskowych i genetycznych, takich jak pewne częste polimorfizmy niektórych genów. Jak dotąd różnicom w sekwencji zaledwie
kilku genów przypisano związek z patogenezą wielogenowej formy cukrzycy typu 2. Są to na przykład geny
kalpainy 10, PPARg, KCNJ11 i insuliny. Należy mieć nadzieję, że w związku z coraz szerszym zakresem wiedzy
o strukturze genomu ludzkiego i doskonaleniem metod biologii molekularnej kolejne lata przyniosą istotny
postęp dotyczący poznania podłoża cukrzycy typu 2.Clinical picture of the type 2 diabetes (T2DM) is influenced by two major, coexisting conditions: impaired insulin secretion and insulin resistance. As
a result of efforts of the scientists around the world
mutations and polymorphisms in a number of genes were linked with monogenic and polygenic
forms of T2DM. Monogenic forms, despite their rarity, were the field where a substantial progress has
been achieved in recognising the molecular background of T2DM. The monogenic forms comprise
MODY (maturity onset diabetes of the young), mitochondrial diabetes (MIDD - maternally inherited
diabetes with deafness) and certain rare forms resulting mainly from severe insulin resistance. Efforts
aiming to identify genes responsible for more common, polygenic form of T2DM were not so fruitful.
The complex, polygenic form of T2DM occurs as an
effect of interaction of many environmental and
genetic factors, such as certain common gene polymorphisms. So far, only several sequence differences have been verified to play role in the pathogenesis of polygenic T2DM. These are the polymorphisms in genes of calpain 10, PPARg, KCNJ11 and insulin. It is expected that in the nearest future more
T2DM susceptibility genes will be identified
