    Prevalence of oropharyngeal beta-lactamase-producing Capnocytophaga spp. in pediatric oncology patients over a ten-year period

    BACKGROUND: The aim of this study was to evaluate the prevalence of beta-lactamase-producing Capnocytophaga isolates in young children hospitalized in the Pediatric Oncology Department of Hôpital Sud (Rennes, France) over a ten-year period (1993–2002). METHODS: In neutropenic children, a periodic survey of the oral cavity allows a predictive evaluation of the risk of systemic infections by Capnocytophaga spp. In 449 children with cancer, 3,053 samples were collected by oral swabbing and plated on TBBP agar. The susceptibility of Capnocytophaga isolates to five beta-lactams was determined. RESULTS: A total of 440 strains of Capnocytophaga spp. were isolated, 309 (70%) of which were beta-lactamase producers. The beta-lactamase-producing strains were all resistant to cefazolin, 86% to amoxicillin, and 63% to ceftazidime. The proportion of strains resistant to third-generation cephalosporins remained high throughout the ten-year study, while susceptibility to imipenem and amoxicillin combined with clavulanic acid was always conserved. CONCLUSION: These results highlight the risk of antibiotic failure in Capnocytophaga infections and the importance of monitoring immunosuppressed patients and testing for antibiotic susceptibility and beta-lactamase production

    How to make a link between Oral Health-Related Quality of Life and dentin hypersensitivity in the dental office?

    International audienceOral Health-Related Quality of Life (OHRQoL) can be considered as the scientific expression of that part of a person's well-being that is affected by his/her oral health. The aim of this paper was to evaluate how to use the data available in the field of research to make a link between OHRQoL and dentin hypersensitivity (DHS) in the dental office. Research papers in the field of OHRQoL and DHS and reviews and research papers about OHRQoL were used for analysis in this short review, with a particular insight on the instruments used to evaluate OHRQoL. Various psychometric instruments have been used to measure OHRQoL that are more or less patient- or expert-centred. Some are generic, others are adapted to specific conditions/domains or populations. The impact of DHS or exposed cervical dentin (ECD) on OHRQoL has been assessed in very few studies. It is therefore of the upmost importance that the use of the OHRQoL as a quality control tool be established in robust clinical studies. Future studies evaluating the impact of the DHS/ECD on OHQoL or evaluating the efficacy of desensitising agents should respect some key points, including study design (randomization, placebo/control group, etc.), validated specific questionnaires and trained calibrated practitioners

    L'infirmité motrice cérébrale (IMC) est un syndrome entraînant un trouble de la fonction motrice associée ou non à une déficience intellectuelle. C'est le résultat d'une lésion cérébrale non progressive et définitive survenue sur un cerveau en voie de développement. L'infirme moteur cérébral présente de nombreux troubles associés : sensoriels, du comportement, de la motricité bucco-faciale, épilepsie... Les pathologies bucco-dentaires rencontrées seront identiques à celles de la population générale (infectieuse, traumatiques et fonctionnelles), mais elles seront exacerbées par les handicaps. Chez ces patients, les soins dentaires représentent un facteur de stress et les difficultés techniques d'intervention doivent inciter, dès le plus jeune âge, au développement des méthodes de prévention (dépistage, conseil et actes de prévention) pour le patient IMC, mais aussi le milieu dans lequel il vit.RENNES1-BU Santé (352382103) / SudocSudocFranceF

    La santé bucco-dentaire des enfants lors d'une hospitalisation fait partie intégrante de leur santé générale. Sa prise en charge est parfois difficile, incomplète voire impossible. L'objectif principal de cette étude a été d'évaluer la place de l'hygiène bucco-dentaire lors d'une hospitalisation dans un service de pédiatrie. Elle a compris l'évaluation 1) des habitudes alimentaires et d'hygiène bucco-dentaire des enfants / adolescents hospitalisés, 2) des connaissances et attentes, dans le domaine de la santé bucco-dentaire, du personnel prenant en charge ces patients. Il ressort de cette étude une difficulté de suivi de l'hygiène bucco-dentaire des enfants hospitalisés et une insuffisance de formation du personnel médical à l'hygiène bucco-dentaire et alimentaire. Il ressort également la nécessité de plus d'implication des odontologistes, seniors ou en formation initiale, dans la prise en charge de ces patients.The oral health of children in a hospital is an integral part of their general health. Care is sometimes difficul, incomplete even impossible. The main objective of this study was to evaluate the place of oral hygiene during a hospitalization in a pediatric unit. It understood the assessment of 1) food habits and oral hygiene of hospitalized children / teenagers, 2) knowledge and waitings, in the field of oral health, of the staff taking care these patients. A difficulty of medical follow-up of the hospitalized children's oral hygiene ans an insufficiency of staff training in the oral and food hygiene emerge from this study.
