    Antibiotic prescribing pattern in paediatric in patients with first time wheezing

    <p>Abstract</p> <p>Background</p> <p>Acute wheezers for the first time in life are an important target group for efforts aimed at reducing unnecessary antibiotic use.</p> <p>Objective</p> <p>To evaluate the effect of clinical, laboratory and radiological data on the decision to prescribe antibiotics to paediatric patients with first time wheezing as well as to seek criteria that would justify antibiotic use.</p> <p>Methods</p> <p>A prospective study was made of 47 previous healthy children admitted to our hospital with first time wheezing in life between October 2008- March 2009. All the patients were treated as per the treating unit's protocol with oxygen, bronchodilators with or without antibiotics. The cases were analyzed after discharge and the characteristics of those treated with antibiotics (n = 23) were compared with those who were not (n = 24) and analyzed statistically to find the predictors for antibiotic usage.</p> <p>Results</p> <p>The mean age of the study groups was 5.8 (+/- 5.1) months. Among the clinical and investigational parameters, presence of predominant crackles and abnormalities on radiograph were the major determinants for antibiotic usage. There were no significant differences in final outcome between these groups.</p> <p>Conclusion</p> <p>Antibiotic usage in first time wheezers is still quite prevalent. Presence of crackles and radiological abnormalities often prompt the usage of antibiotics in such cases.</p

    Models for Antitubercular Activity of 5′-O-[(N-Acyl)sulfamoyl]adenosines

    The relationship between topological indices and antitubercular activity of 5′-O-[(N-Acyl)sulfamoyl]adenosines has been investigated. A data set consisting of 31 analogues of 5′-O-[(N-Acyl)sulfamoyl]adenosines was selected for the present study. The values of numerous topostructural and topochemical indices for each of 31 differently substituted analogues of the data set were computed using an in-house computer program. Resulting data was analyzed and suitable models were developed through decision tree, random forest and moving average analysis (MAA). The goodness of the models was assessed by calculating overall accuracy of prediction, sensitivity, specificity and Mathews correlation coefficient. Pendentic eccentricity index – a novel highly discriminating, non-correlating pendenticity based topochemical descriptor – was also conceptualized and successfully utilized for the development of a model for antitubercular activity of 5′-O-[(N-Acyl)sulfamoyl]adenosines. The proposed index exhibited not only high sensitivity towards both the presence as well as relative position(s) of pendent/heteroatom(s) but also led to significant reduction in degeneracy. Random forest correctly classified the analogues into active and inactive with an accuracy of 67.74%. A decision tree was also employed for determining the importance of molecular descriptors. The decision tree learned the information from the input data with an accuracy of 100% and correctly predicted the cross-validated (10 fold) data with accuracy up to 77.4%. Statistical significance of proposed models was also investigated using intercorrelation analysis. Accuracy of prediction of proposed MAA models ranged from 90.4 to 91.6%

    Subject mapping of Punjabi language books: a bibliometric study

    The study presents the subject analysis of Punjabi language books written in ‘Gurmukhi’ script from 2004-2013 covering different fields of arts and humanities, social sciences, science, engineering, and technology. The distribution of major subjects and their subdivisions were based on the Dewey Decimal Classification (DDC) 22nd edition. The information centres, and academic institutions such as universities from the different parts of Northern India, having a good collection of Punjabi language books were the focus for data collection. The Punjabi language books publishers and several websites/resources were also consulted to get the required information. A total number of 7997 Punjabi books on different subjects published during the period 2004-2013 have been studied. The largest number of books in Punjabi [5587 (69.9%)] is published in the field of literature.Subject analysis, decadal growth, analysis of books translations from different Indic and foreign languages to the Punjabi language are also included in the study. A growth rate of 29.37% in the publication of Punjabi books is seen during the decade

    Enhanced anomalous Nernst effects in ferromagnetic materials driven by Weyl nodes

    Based on high-throughput first-principles calculations, we evaluated the anomalous Hall and anomalous Nernst conductivities of 266 transition-metal-based ferromagnetic compounds. Detailed analysis based on the symmetries and Berry curvatures reveals that the origin of singular-like behaviour of anomalous Hall/Nernst conductivities can be mostly attributed to the appearance of Weyl nodes or nodal lines located in the proximity of the Fermi energy, which can be further tailored by external stimuli such as biaxial strains and magnetic fields. Moreover, such calculations are enabled by the automated construction of Wannier functions with a success rate of 92%, which paves the way to perform accurate high-throughput evaluation of the physical properties such as the transport properties using the Wannier interpolationComment: 9 pages, 4 figure

    The present paper deals with 38 ethnomedicinal plant species belonging to 37 genera and 24 families used for prevention and treatment of malaria by the tribal and indigenous non-tribal communities of Sundargarh, Mayurbhanj, Angul, Balangir, Deogarh and Nuapada districts of Odisha. This first-hand ethnomedicinal information for their therapeutic use against malaria was collected directly from a field survey of 404 remote tribal villages and forest areas of these 6 districts during 2006-2017. Additionally, uses of other 6 plants for the treatment of the same diseases were also recorded from the already published literature on these districts.     Percutaneous transolecranon pinning and lateral pinning Vs lateral pinning in displaced supracondylar fractures of humerus in children: an observative study

    Background: Supracondylar fracture (humerus) is type of extra-articular fracture occurring in the distal metaphyseal site of humerus. It is almost exclusively a fracture of the immature skeleton, seen in children and young teenagers. Fractures around the elbow are a great challenge to orthopaedic surgeons. Clinical diagnosis may be difficult due to noncooperative patient and massive swelling around the elbow. Displaced type of supracondylar fractures poses problem not only in reduction but also in maintenance of reduced fracture   and   rapid   inclusion   of nerves and vessels.Methods: The present study was conducted on 30 cases of displaced supracondylar fracture humerus in children, aged 2-14 years, who were treated by CRPP with either lateral entry of k-wires or a lateral wire and a vertical wire through olecranon (transolecranon).Results: Both the Groups achieved 90% satisfactory results, but 10% unsatisfactory results recorded in Group A only rather than in Group B.Conclusions: Although the transolecranon wire has the disadvantage of limiting the flexion and extension of the elbow, this does not influence the final-outcome much as the elbow is fixed in a POP splint for minimum 3 weeks-in all patients in both groups
