Aplicação das técnicas de HPLC e FIE na triagem neonatal de α-talassemia em amostras de sangue seco em papel filtro

Frente Ă dificuldade, baixa sensibilidade e especificidade das tĂ©cnicas disponĂveis para o diagnĂłstico de α-talassemia na triagem neonatal Ă© essencial o aperfeiçoamento e o conhecimento da capacidade de detecção de α-talassemia das tĂ©cnicas já existentes. Objetivos: Avaliar o mĂ©todo de HPLC e FIE na triagem neonatal de α-talassemia em amostras de sangue seco em papel filtro. MĂ©todos: Foram analisados os resultados dos exames de recĂ©m-nascidos (RN). As amostras de sangue seco capilar em papel filtro dos RNs para triagem neonatal foram analisadas atravĂ©s dos mĂ©todos de HPLC e FIE. ApĂłs triagem, os RNs foram atendidos no ambulatĂłrio e submetidos a análises confirmatĂłrias em sangue total, atravĂ©s das seguintes metodologias: hemograma, HPLC e estudo molecular.O teste molecular foi utilizado como padrĂŁo ouro na detectacao de α-talassemia para todas as amostras analisadas. Foram estabelecidos dois grupos com e sem α-talassemia. Os rresultados obtidos foram expressos atravĂ©s de análises estatĂsticas. Resultados: No hemograma os parâmetros que apresentaram diferença significativaentre os gruposforam os valores de eritrĂłcitos, HCM, CHCM e VCM. Entre as trĂŞs tĂ©cnicas avaliadas, as trĂŞs apresentaram diferença significa entre os dois grupos. Os valores de área sobre a curva foram de: HPLC PF 0,862; FIE (0,855)e HPLC ST (0,811). Os valores de sensibilidade e especificidade foram de: HPLC PF 91,7% e 70,6%; FIE 100% e 76,5% e HPLC ST 95,8% e 64,7%, respectivamente. Na análise do Ăndice kappa os valores obtidos foram: quando trĂŞs testes positivos o Ăndice kappa foi de 0,718, dois testes positivos 0,604 e quando somente um teste positivo foi de 0,595. Conclusões: Os mĂ©todos analisados apresentaram Ăłtimos valores de acurácia, sensibilidade e especificidade. O HPLC nĂŁo pode ser utilizado como diagnĂłstico definitivo, porĂ©m, pode auxiliar na triagem neonatal. Para futuros estudos, um aumento no valor do n amostral deve ser levado em consideração.Introduction:The α-thalassemia encompasses all conditions in which there is a deficit in the production of α globinchains of hemoglobin. For neonatal screening, the most frequent used methods the isoelectric focusing (IEF) and the HPLC. In face of the difficulty, low sensitivity and specificity of the techniques available for the diagnosis of α-thalassemia in neonatal screening is essential for improvement and the knowledge of the detection capability of α-thalassemia of existing techniques. Objectives: Evaluate the HPLC method and IEF in neonatal screening of α-thalassemia in samples of dried blood on filter paper. Methods: Were analyzed the results of examinations of newborns (NB). Capillary blood samples of the NB’s for neonatal screening were analyzed through HPLC methods and FIE. After, the NB were treated in the outpatient clinic and submitted to confirm analyzes in whole blood, through following methodologies: hemogram and HPLC. Molecular test was used as the gold standard for all the samples tested. The results obtained were expressed through statistical analyzes. Results:The hemogram parameters which presented significant difference were the values of erythrocytes, HCM, CHC Mand VCM. Among the three techniques evaluated the three-showed significant difference between the two groups. The values of area under the curve were: 0.862; IEF HPLC PF (0.855); HPLC ST (0.811). The sensitivity and specificity values were: 91.7% PF HPLC and 70.6%; 100% FIE and 76.5% and 95.8% HPLC ST and 64.7%, respectively.In the analysis of the Kappa index values obtained were: when three positive tests the kappa index was of 0, 718, two positive tests 0, 604 and only when a positive test was 0, 595. Conclusions: The methods analyzed showed great values of accuracy, sensitivity and specificity. The HPLC cannot be used as a definitive diagnosis, however, can assist in neonatal screening. For future studies, an increase in the value of the small sample size should be taken into consideration
