    Decision making in specialist forensic psychiatric (the psychological aspect)

    The article presents the results of a study of psychological factors of decision making amongforensic psychiatric specialists. Given the survey data of10 medical psychologists and 10 psychiatrists carrying out compulsory treatment in the psychiatric hospital №5, Moscow Department of Public Health, in comparison with the control group, consisting of professionals working in the field of education, manufacturing, services and technology. Methodical complex included: 1. The Epstein questionnaire of the intuitive style (adaptation Kornilova T. V., Kornilov S. A.); A new questionnaire of tolerance/intolerance to uncertainty (Kornilova T. V.); Melbourne questionnaire of decision making (adaptation T. V.Kornilova); The questionnaire «Personal factors of decision making»(T.V. Kornilova); TheV. Smecalo and V. M. Kucher method; The Tsvetkova Method; 7. The questionnaire «Styles of thinking» by R. Bramsonand Harrison (adaptation of A. A. Alekseev); 8.The questionnaire «Scale of base convictions» (R.Yanov-Boulemane, adaptation M.A. Padun, A.V. Kotelnikov); 9. The check-list aimed to identify the type of decisions taken by the expert in professional activity, their frequency, importance, subjective evaluation is necessary for decision making qualities and to evaluate the most significant opinions of other persons in the decision-making process. There were defined the characteristics of decision making depending on gender. The relationships between type of activity and frequency of occurrence of different types of decisions in professional activities of specialists were revealed.     Analysis of the dynamics of repeated admissions of patients to inpatient compulsory treatment in moscow

    Objective. To explore the dynamics of admission of patients for repeated compulsory treatment to a medical setting providing general and specialized psychiatric care. Material and methods. The dynamics of admission of patients for repeated compulsory treatment in the «Moscow Psychiatric clinical hospital No5» for the period 1991—2019 was analyzed based on medical records of two groups of patients: the main group (n=1325) and the comparison group (n=15 100). Results and conclusion. Over the past 5 years, there is a decrease in repeated admissions of patients to compulsory treatment who have committed a new socially dangerous act. This positive dynamic is due to compliance with the principles of continuity and phasing between inpatient and outpatient units of compulsory treatment. The transition of patients from inpatient compulsory treatment to compulsory treatment in outpatient settings is an effective measure to prevent patients from committing repeated socially dangerous acts. © 2020, Media Sphera Publishing Group.     Assessment of the efficacy and compliance with Vojea® in women during the pregravid preparation and the early stages of pregnancy

Patients and Methods: this prospective multi-center observational study was performed in 2022 in four medical centers and women's health clinics of Ekaterinburg. The study included 87 women with latent iron deficiency (mean ferritin level below 15 ng/ml). The first group consisted of 37 women during the pregravid preparation period, and the second group consisted of 50 pregnant patients with up to 12 weeks of pregnancy. Patients received the Vojea® once daily for three months. Clinical and laboratory efficacy (the absence of objective anemia signs and the changes in hemoglobin level, folate and ferritin blood tests over time), as well as tolerability (the rate of adverse events, tolerability of the taken product evaluated using the visual analog scale) was assessed at days 30, 60 and 90 after the beginning of treatment. Results: beginning from the first month of treatment with the studied complex, some positive changes in clinical symptoms and laboratory findings were reported in both groups. By the third month of therapy, the increment of hemoglobin level in the pregravid preparation group was 17 (13.9%) units, the level of ferritin increased by 6.3 times (clinically significant after two months of the product intake), and the level of blood serum folates increased by 4.6 times. In group 2, the increment of hemoglobin level, as pregnancy was progressing, in three months of the product intake reached 19 units (17%), the level of ferritin increased by 5.9 times (clinically significant after two months of treatment), and the level of blood serum folates increased by 3.9 times. In group 1, adverse events were reported only in 2/37 (5.4%) patients who had dark stool color on the 2nd month of follow-up. In group 2, such adverse events as nausea were reported in 3/50 (6%) patients during the 1st month of product intake. All patients informed that the product was well tolerated which ensured high patient compliance. Conclusion: maintaining iron and folic acid homeostasis by oral intake of the Vojea® complex during three months in the preconception period and early stages of pregnancy is a feasible option based on its high efficacy and good tolerability with the minimal risk of side effects, encouraging high patient compliance.     Perspectives of the Reduction of Nutrient Export from River Watersheds through the Introduction of Best Available Technologies for Agricultural Production: Based on Modeling Results

Abstract: Estimate is made for the possible reduction of nutrient export from the drainage basins of tributaries of the Kuibyshev Reservoir through the introduction of the best available technologies of agricultural production. The activity of more than eight hundreds of agricultural enterprises in the reservoir drainage basin was analyzed. Different types of underlying surface were classified with the use of space photographs. Mathematical simulation was used to calculate first the distributed agricultural nitrogen and phosphorus load onto the drainage basin and next the export of nutrients with river runoff into the reservoir either under current conditions or after the introduction of the best available technologies (BAT) to agricultural production. The simulations show that, under the conditions of medium water abundance, the total export from the drainage basins of the Sviyaga, Kazanka, Mesha, and Bol’shoi Cheremshan will decrease by about 367 t N/year and 12 t P/year after the introduction of BAT into the agricultural practice. The application of nitrogen and phosphorus into the soil as components of mineral and organic fertilizers in most administrative territories of the region under consideration was found to be less than the nutrient demand of agricultural crops, resulting in the gradual depletion of such elements in soils and a decrease in the risk of their migration into water bodies
