Surface Defect Inspection of a Cutting Tool by Image Processing with Neural Networks
In this research, an image processing method and a system for inspection support of a rod figured cutting tool are developed. As is well known, the visual inspection of a cutting tool by image processing is not easy, because cutting blade have a helical blade structure. To cope with the problem, an experimental facility with rotation and longitudinal tool shift functions to enable acquisition of blade surface pictures along a cutting rod is developed. The type of the defect treated in this paper is the spot of coating on blade surface. To judge the quality of the processed image of blade surface, neural network with autonomous learning is used. The processed image of cutting tool is divided into 64 × 64 blocks used for the
input to the neural networks. Before input, each block data is preprocessed applying a edge detection filter and a transformation by the discrete fourier transform (DFT). Using these technologies, the experimental inspection system is built and tested to check the capabilities of the inspection algorithms. Culture, History and Applications of Medicinal and Aromatic Plants in Japan
Historically, the Japanese began to use aromatic and medicinal plants for ritual activities, food flavor, and treatment of their bodies. The exotic plants, new ideas, and culture associated with medicinal and aromatic plants were introduced to Japan from other countries, primarily via Korea. In this way, experience and knowledge of uses were accumulated, and applications of aromatic and medicinal plants were expanded. The oldest Japanese medicine “Wa ho” leads the way to folk medicine today, and traditional Japanese medicine (Kampo) has spread into modern use. The elegance tradition of “Kodo,” an incense ceremony of Japan, was developed because of the use of aromatic incensed wood in sixteenth century as recreation. Paired along with this ceremony is the Japanese sa-do tea ceremony that the spirituality and esthetic sense are inherited to Japanese today. Japanese green tea is becoming popular in many countries due to the constituent, catechins, that medically treats vascular disease, several cancers, and type II diabetes. Today, the Japanese medical system has new direction, integrating medicine with the adoption of modern western and alternative medicine. Scientific data must continue to be collected for interactions between the two medicinal systems for integrative medicine to be ideal for body, mind, and spirit of humans and nature
Role of Medicinal and Aromatic Plants: Past, Present, and Future
Before the concept of history began, humans undoubtedly acquired life benefits by discovering medicinal and aromatic plants that were food and medicine. As our early ancestors learned to recognize and consume selected plants, civilization and personal and group health could advance. Traditional medicine would become part of every civilization with medicinal and aromatic plants widely used and applied to maintain life. Undoubtedly, the variety of available plant materials would be tasted and tested to determine whether a plant was valuable as a food or medicine. Today, a variety of available herbs and spices are used and enjoyed throughout the world and continue to promote good health. As the benefits from medicinal and aromatic plants are recognized, these plants will have a special role for humans in the future
B. I. T. : Development of a dynamic visualization tool for Bayesian inference on normal distributions
Bayesian inference Tool (B. I. T.) is a software program developed to support medical professionals and students in the application of Bayesian inference to medical decision-making and education. This software program makes it possible to visualize the posterior probability distributions of Bayesian inference. In addition, its interface aims to facilitate the intuitive understanding of the operative procedures. For many cases of Bayesian inference with normal distributions, the graphical representation of the posterior probability distribution changes dynamically when the input value is assigned by the user with the use of a computer mouse; this procedure also enables the dynamic calculation of the posterior probability. When operating this software, it is desirable for the user to have basic knowledge and understanding of the presuppositions as well as applications of Bayesian inference
Safety and Effectiveness of Perospirone in Comparison to Risperidone for Treatment of Delirium in Patients with Advanced Cancer: A Multicenter Prospective Observational Study in Real-World Psycho-Oncology Settings
The clinical benefit of perospirone for treatment of delirium in patients with advanced cancer is not sufficiently clear. The objective of this study was to compare the safety and effectiveness of perospirone to those of risperidone for the treatment of delirium in patients with advanced cancer. This is a secondary analysis of a multicenter prospective observational study in nine psycho-oncology consultation services in Japan. The study used the Delirium Rating Scale (DRS) Revised-98 to measure effectiveness and the CTCAE (Common Terminology Criteria for Adverse Events) version 4 to assess safety. Data from 16 patients who received perospirone and 53 patients who received risperidone were analyzed. The mean age was 70 years in the perospirone group and 73 years in the risperidone group. Both groups showed a significant decrease in the total score of DRS-R-98 after three days of treatment (perospirone: 11.7 (7.9-15.4) to 7.0 (3.3-10.7), difference −4.7, effect size=0.72, p=0.003; risperidone: 15.5 (13.6-17.4) to 12.2 (10.1-14.2), difference −3.3, effect size=0.55, p=0.00). The risperidone group showed significant improvements in sleep-wake cycle disturbance, orientation, attention, and visuospatial ability. In the perospirone group, there was a significant improvement of sleep-wake cycle disturbance. The median daily dose of perospirone was 4 mg/day. There were fewer episodes of somnolence as an adverse event in the perospirone group. Low-dose perospirone was thus found to be effective for the treatment of delirium in patients with advanced cancer and may be associated with fewer episodes of over-sedation as an adverse event
Rho-associated kinase inhibitor eye drop (Ripasudil) transiently alters the morphology of corneal endothelial cells
PURPOSE: Ripasudil (Glanatec), a selective Rho-associated coiled coil-containing protein kinase (ROCK) inhibitor, was approved in Japan in September 2014 for the treatment of glaucoma and ocular hypertension. The purpose of this study was to investigate the effect of ripasudil eye drops on corneal endothelial morphology, as ROCK signaling is known to modulate the actin cytoskeleton. METHODS: Morphological changes in the corneal endothelium were evaluated in human subjects by specular and slit-lamp microscopy, following topical administration of ripasudil. We also used a rabbit model to evaluate the effect of ripasudil on clinical parameters of the corneal endothelium. Twenty-four hours after ripasudil application, corneal specimens were evaluated by phalloidin staining, immunohistochemical analysis, and electron microscopy. RESULTS: Specular microscopy revealed morphological changes in human eyes, and slit-lamp microscopy showed guttae-like findings. The rabbit model showed morphological changes similar to those seen in human eyes after ripasudil administration. Electron microscopy demonstrated that these alterations are due to the formation of protrusions along the cell-cell borders, but this formation is transient. Expression of corneal endothelial function-related markers was not disrupted; corneal thickness and corneal volume were not changed; and no cell death was observed following ripasudil administration. CONCLUSIONS: Ripasudil induces transient guttae-like findings in humans, most likely due to protrusion formation along intracellular borders caused by the reduction in actomyosin contractility of the corneal endothelial cells. No severe adverse effects were observed. Physicians should be aware that ROCK inhibitors can cause these guttae-like findings, to avoid misdiagnosing patients as having Fuchs endothelial corneal dystrophy. Treatment of Thoracolumbar Spinal Fracture Accompanied by Diffuse Idiopathic Skeletal Hyperostosis Using Transdiscal Screws for Diffuse Idiopathic Skeletal Hyperostosis: Preliminary Results
Study Design This retrospective case series enrolled 13 patients who underwent posterior fixation with both transdiscal screws for diffuse idiopathic skeletal hyperostosis (TSDs) and pedicle screws (PSs) to treat spinal injury accompanied by diffuse idiopathic skeletal hyperostosis (DISH). Purpose To describe the usefulness, feasibility, and biomechanics of TSD. Overview of Literature Vertebral bodies accompanied by DISH generally have lower bone mineral density than normal vertebral bodies because of the stress shielding effect. This phenomenon tends to makes screw fixation challenging. To our knowledge, solutions for this issue have not previously been reported. Methods Patients were assessed using the data on surgical time, estimated intraoperative blood loss, mean number of stabilized intervertebral segments, number of screws used, perioperative complications, union rate, and the three-level EuroQol five-dimensional questionnaire (EQ5D-3L) score at the final follow-up. The Hounsfield unit (HU) values of the screw trajectory area, and the actual intraoperative screw insertion torque of TSDs and PSs were also analyzed and compared. Results The surgical time and estimated intraoperative blood loss were 165.9±45.5 minutes and 71.0±53.4 mL, respectively. The mean number of stabilized intervertebral segments was 4.6±1.0. The number of screws used was 4.9±1.3 for TSDs and 3.0±1.4 for PSs. One death occurred after surgery. The union rate and EQ5D-3L scores were 100% and 0.608±0.128, respectively. The HU value and actual intraoperative screw insertion torque of TSDs were significantly better than those of PSs (p<0.001, p=0.033). Conclusions We were able to achieve stable surgical outcomes using the combination of TSDs and PSs. The HU value and actual intraoperative screw insertion torque were significantly higher for TSDs than for PSs. Based on these results, when treating thoracolumbar spinal fractures accompanied by DISH in elderly populations, the TSD could be a stronger anchor than the PS
A Multidisciplinary Approach to the Management of Chronic Pain through a Self-managed Behavioral Exercise Program : A Pilot Study in Japan
We conducted this study to determine the short-term treatment outcomes of multidisciplinary approaches to chronic pain management for outpatients in Japan. We evaluated pain reduction and improvement in quality of life (QOL) after treatment. We analyzed 32 patients who had experienced intractable chronic pain for > 3 months. The patients received multidisciplinary therapeutic self-managed exercise instructions and then underwent evaluations 1 and 3 months after the treatment. We used the Pain Disability Short Form-36 (SF-36), Pain Catastrophizing Scale (PCS), and Pain Disability Assessment Scale (PDAS) to evaluate QOL. Although the pain levels were the same before and after the physical exercise program, the patients showed significant improvements in physical function on the SF-36 (48.5 vs. 54.5, 3 months vs. 1 month; p=0.0124), the magnification subscale on the PCS (6.8 vs. 5.9, 1 month vs. before; p=0.0164) and the PDAS (29.2 vs. 23.4, 3 months vs. before; p=0.0055). Chronic pain should be treated with a biopsychosocial approach, but time constraints and costs have limited the implementation of multidisciplinary and behavioral approaches to chronic pain management. Our findings demonstrate that clinical improvements are possible for patients with chronic pain, using multidisciplinary team resources widely available in Japanese clinical practice
