Accretion Disc Theory: From the Standard Model Until Advection
Accretion disc theory was first developed as a theory with the local heat
balance, where the whole energy produced by a viscous heating was emitted to
the sides of the disc. One of the most important new invention of this theory
was a phenomenological treatment of the turbulent viscosity, known as ''alpha''
prescription, when the (r) component of the stress tensor was
approximated by ( P) with a unknown constant . This
prescription played the role in the accretion disc theory as well important as
the mixing-length theory of convection for stellar evolution. Sources of
turbulence in the accretion disc are discussed, including nonlinear
hydrodynamical turbulence, convection and magnetic field role. In parallel to
the optically thick geometrically thin accretion disc models, a new branch of
the optically thin accretion disc models was discovered, with a larger
thickness for the same total luminosity. The choice between these solutions
should be done of the base of a stability analysis. The ideas underlying the
necessity to include advection into the accretion disc theory are presented and
first models with advection are reviewed. The present status of the solution
for a low-luminous optically thin accretion disc model with advection is
discussed and the limits for an advection dominated accretion flows (ADAF)

Inferring processes underlying B-cell repertoire diversity
We quantify the VDJ recombination and somatic hypermutation processes in
human B-cells using probabilistic inference methods on high-throughput DNA
sequence repertoires of human B-cell receptor heavy chains. Our analysis
captures the statistical properties of the naive repertoire, first after its
initial generation via VDJ recombination and then after selection for
functionality. We also infer statistical properties of the somatic
hypermutation machinery (exclusive of subsequent effects of selection). Our
main results are the following: the B-cell repertoire is substantially more
diverse than T-cell repertoires, due to longer junctional insertions; sequences
that pass initial selection are distinguished by having a higher probability of
being generated in a VDJ recombination event; somatic hypermutations have a
non-uniform distribution along the V gene that is well explained by an
independent site model for the sequence context around the hypermutation site.Comment: acknowledgement adde
Field Theory for a Deuteron Quantum Liquid
Based on general symmetry principles we study an effective Lagrangian for a
neutral system of condensed spin-1 deuteron nuclei and electrons, at
greater-than-atomic but less-than-nuclear densities. We expect such matter to
be present in thin layers within certain low-mass brown dwarfs. It may also be
produced in future shock-wave-compression experiments as an effective fuel for
laser induced nuclear fusion. We find a background solution of the effective
theory describing a net spin zero condensate of deuterons with their spins
aligned and anti-aligned in a certain spontaneously emerged preferred
direction. The spectrum of low energy collective excitations contains two spin
waves with linear dispersions -- like in antiferromagnets -- as well as gapped
longitudinal and transverse modes related to the Meissner effect -- like in
superconductors. We show that counting of the Nambu-Goldstone modes of
spontaneously broken internal and space-time symmetries obeys, in a nontrivial
way, the rules of the Goldstone theorem for Lorentz non-invariant systems. We
discuss thermodynamic properties of the condensate, and its potential

Intra-articular knee haemangioma originating from the anterior cruciate ligament: a case report
<p>Abstract</p> <p>Introduction</p> <p>Synovial haemangioma is a rare intra-articular benign tumour, which may arise from any synovium-lined surface, but particularly in the knee joint. Synovial haemangioma originating from the anterior cruciate ligament has not been reported previously.</p> <p>Case presentation</p> <p>A 34-year-old man presented with a history of intermittent knee pain, locking and swelling.</p> <p>Conclusion</p> <p>Knee intra-articular haemangioma, a very rare benign tumour, is often misdiagnosed. Magnetic resonance imaging is effective in detecting this lesion and should be performed in cases of persistent knee swelling and pain.</p
Why is there no queer international theory?
Over the last decade, Queer Studies have become Global Queer Studies, generating significant insights into key international political processes. Yet, the transformation from Queer to Global Queer has left the discipline of International Relations largely unaffected, which begs the question: if Queer Studies has gone global, why has the discipline of International Relations not gone somewhat queer? Or, to put it in Martin Wight’s provocative terms, why is there no Queer International Theory? This article claims that the presumed non-existence of Queer International Theory is an effect of how the discipline of International Relations combines homologization, figuration, and gentrification to code various types of theory as failures in order to manage the conduct of international theorizing in all its forms. This means there are generalizable lessons to be drawn from how the discipline categorizes Queer International Theory out of existence to bring a specific understanding of International Relations into existence
Interpretation of Binary Pulsar Observations
The nature, dynamics and evolution of the three known radio pulsar binaries are discussed. The system containing 1913+16 appears to comprise two ~1.4 M⊙ components, and to undergo orbital decay as predicted by general relativity. It is proposed that 1913+16 has a neutron star companion and that 0655+64 and 0820+02 have white dwarf companions which should be observable optically
Perioperative Quality Initiative (POQI) consensus statement on fundamental concepts in perioperative fluid management: fluid responsiveness and venous capacitance
Background:
Optimal fluid therapy in the perioperative and critical care settings depends on understanding the underlying cardiovascular physiology and individualizing assessment of the dynamic patient state.
Methods:
The Perioperative Quality Initiative (POQI-5) consensus conference brought together an international team of multidisciplinary experts to survey and evaluate the literature on the physiology of volume responsiveness and perioperative fluid management. The group used a modified Delphi method to develop consensus statements applicable to the physiologically based management of intravenous fluid therapy in the perioperative setting.
Discussion:
We discussed the clinical and physiological evidence underlying fluid responsiveness and venous capacitance as relevant factors in fluid management and developed consensus statements with clinical implications for a broad group of clinicians involved in intravenous fluid therapy. Two key concepts emerged as follows: (1) The ultimate goal of fluid therapy and hemodynamic management is to support the conditions that enable normal cellular metabolic function in order to produce optimal patient outcomes, and (2) optimal fluid and hemodynamic management is dependent on an understanding of the relationship between pressure, volume, and flow in a dynamic system which is distensible with variable elastance and capacitance properties
Incidence, patterns and severity of reported unintentional injuries in Pakistan for persons five years and older: results of the National Health Survey of Pakistan 1990–94
<p>Abstract</p> <p>Background</p> <p>National level estimates of injuries are not readily available for developing countries. This study estimated the annual incidence, patterns and severity of unintentional injuries among persons over five years of age in Pakistan.</p> <p>Methods</p> <p>National Health Survey of Pakistan (NHSP 1990–94) is a nationally representative survey of the household. Through a two-stage stratified design, 18, 315 persons over 5 years of age were interviewed to estimate the overall annual incidence, patterns and severity of unintentional injuries for males and females in urban and rural areas over the preceding one year. Weighted estimates were computed adjusting for complex survey design using <it>surveyfreq </it>and <it>surveylogistic </it>option of SAS 9.1 software.</p> <p>Results</p> <p>The overall annual incidence of all unintentional injuries was 45.9 (CI: 39.3–52.5) per 1000 per year; 59.2 (CI: 49.2–69.2) and 33.2 (CI: 27.0–39.4) per 1000 per year among males and females over five years of age, respectively. An estimated 6.16 million unintentional injuries occur in Pakistan annually among persons over five years of age. Urban and rural injuries were 55.9 (95% CI: 48.1–63.7) and 41.2 (95% CI: 32.2–50.0) per 1000 per year, respectively. The annual incidence of injuries due to falls were 22.2 (95% CI: 18.0–26.4), poisoning 3.3 (95%CI: 0.5–6.1) and burn was 1.5 (95%CI: 0.9–2.1) per 1000 per year. The majority of injuries occurred at home 19.2 (95%CI: 16.0–22.4) or on the roads 17.0 (95%CI: 13.8–20.2). Road traffic/street, school and urban injuries were more likely to result in handicap.</p> <p>Conclusion</p> <p>There is high burden of unintentional injuries among persons over five years of age in Pakistan. These results are useful to plan further studies and prioritizing prevention programs on injuries nationally and other developing countries with similar situation.</p
From food to pest: Conversion factors determine switches between ecosystem services and disservices
Ecosystem research focuses on goods and services, thereby ascribing beneficial values to the ecosystems. Depending on the context, however, outputs from ecosystems can be both positive and negative. We examined how provisioning services of wild animals and plants can switch between being services and disservices. We studied agricultural communities in Laos to illustrate when and why these switches take place. Government restrictions on land use combined with economic and cultural changes have created perceptions of rodents and plants as problem species in some communities. In other communities that are maintaining shifting cultivation practices, the very same taxa were perceived as beneficial. We propose conversion factors that in a given context can determine where an individual taxon is located along a spectrum from ecosystem service to disservice, when, and for whom. We argue that the omission of disservices in ecosystem service accounts may lead governments to direct investments at inappropriate targets
