Хирургическое лечение больных вентральными грыжами с сопутствующим ожирением

The work is based on the analysis of hernia repair in 164 patients with postoperative, recurrent and primary ventral hernias obese. The patients were divided into two groups: a control group and the main group. Patients in the control group performed classical hernia repair with local tissues and prosthetic materials indicated. In the study group patients underwent hernia repair using mesh implants with the addition dermatolipidectomу. The majority of patients after surgery preserves the normal function of the gastrointestinal tract, only 3 patients in the control and in 1 patient in the study group underwent hernia repair over a giant ventral hernia was observed intestinal paresis, docked medication. The reliability of plastics hernia ring is provided by a polypropylene prosthesis and perform no compression hernioalloplastic and combined technique allowed to avoid increasing intra-abdominal pressure due to the increase in the abdominal cavity. As a result of dermatolipidektomу managed to reduce the number of complications in the near (9.8 %) and late postoperative period. There were not disease recurrence and death.Работа основана на анализе результатов герниопластики у 164 больных послеоперационными, рецидивными и первичными вентральными грыжами, страдающих ожирением. Больные были разделены на две группы: контрольная и основная. Пациентам контрольной группы производилась классическая герниопластика местными тканями и протезирующими материалами по показаниям. В основной группе пациентам выполнялась герниопластика с использованием сетчатых имплантатов с дополнением дерматолипидэктомии. У большинства пациентов после операции сохранялась нормальная функция ЖКТ, лишь у 3 больных в контрольной и у 1 больной в основной группе, перенесших герниопластику по поводу гигантской вентральной грыжи, отмечался парез кишечника, купированный медикаментозно. Надежность пластики грыжевых ворот обеспечивается за счет полипропиленового протеза, а выполнение ненатяжной герниоаллопластики и комбинированной методики позволило избежать повышения внутрибрюшного давления за счет увеличения объема брюшной полости. В результате применения дерматолипидэктомии удалось снизить количество осложнений в ближайшем (9,8 %) и отдаленном послеоперационном периоде. Рецидивов заболевания и летальных исходов не было

Modeling for the Spread of Water Pollution under Hydraulic Structure with a Rabbet

In industry and agriculture, along with the increasing influence of various technogenic factors on the environment, there is an important problem of protecting water resources from pollution and salinization, as well as the problem of protecting natural water bodies from pollutants. The solution of these problems essentially depends on the results of a mathematical study of the mass transfer processes of migrating substances in the filtration of groundwater. In this paper, we simulate the process of spreading water pollution under the hydraulic structure of a complex architecture. The unsteady equation of the process of mass transfer of pollutants during the filtration of groundwater was approximated by the finite difference method, while the procedure "weighting upstream" was used to approximate the convective terms. The results of calculations showed that the pollution of the downstream of a hydraulic structure depends on the number of rabbets and their location along the underground circuit. The obtained results can be used in monitoring and building predictive estimates of the migration regime of pollution under a hydraulic structure in real time

ЛЕЧЕНИЕ ДЕТЕЙ С ХРОНИЧЕСКИМ БРОНХИТОМ

Summary. For the improvement of the treatment results of children with chronic bronchitis 27 children with chronic bronchitis and 22 healthy children from control group have been investigated. Children with chronic bronchitis during bronchoscopy have unaergone autotransplantation of macrophage tissues of the respiratory tract with stimulation by polyoxydоnine. The effectiveness of such method has been proved by the results of the improvement of the indexes of the lavage fuid and immunological status of children before and after treatment. Key words: endobronchitis, polyoxydony, alveolar macrophages immunoglobulins, lymphocytes. Резюме. Для улучшения результатов лечения детей с хроническим бронхитом было обследовано 27 детей с этой патологией и 22 здоровых ребенка для контрольной группы. Детям с хроническим бронхитом во время бронхоскопии произведена аутотрансплантация макрофагальных клеток респираторного тракта со стимуляцией полиоксидонием. Эффективность данного метода была доказана улучшением показателей лаважной жидкости и иммунологического статуса детей до и после лечения. Ключевые слова: эндобронхит, полиоксидоний, альвеолярные макрофаги, иммуноглобулины, лимфоциты

Hydrodynamic studies of horizontal wells

A new technology is suggested for performance of hydrodynamic studies in horizontal wells using comprehensive off-line instrumentation. The interpretation procedure for the outcome of such studies has been developed based on the regularizalion methods
