    A Neutrosophic Clinical Decision-Making System for Cardiovascular Diseases Risk Analysis

    Cardiovascular diseases are the leading cause of death worldwide. Early diagnosis of heart disease can reduce this large number of deaths so that treatment can be carried out. Many decision-making systems have been developed, but they are too complex for medical professionals. To target these objectives, we develop an explainable neutrosophic clinical decision-making system for the timely diagnose of cardiovascular disease risk. We make our system transparent and easy to understand with the help of explainable artificial intelligence techniques so that medical professionals can easily adopt this system. Our system is taking thirtyfive symptoms as input parameters, which are, gender, age, genetic disposition, smoking, blood pressure, cholesterol, diabetes, body mass index, depression, unhealthy diet, metabolic disorder, physical inactivity, pre-eclampsia, rheumatoid arthritis, coffee consumption, pregnancy, rubella, drugs, tobacco, alcohol, heart defect, previous surgery/injury, thyroid, sleep apnea, atrial fibrillation, heart history, infection, homocysteine level, pericardial cysts, marfan syndrome, syphilis, inflammation, clots, cancer, and electrolyte imbalance and finds out the risk of coronary artery disease, cardiomyopathy, congenital heart disease, heart attack, heart arrhythmia, peripheral artery disease, aortic disease, pericardial disease, deep vein thrombosis, heart valve disease, and heart failure. There are five main modules of the system, which are neutrosophication, knowledge base, inference engine, de-neutrosophication, and explainability. To demonstrate the complete working of our system, we design an algorithm and calculates its time complexity. We also present a new de-neutrosophication formula, and give comparison of our the results with existing methods

    A multicenter case control study of association of vitamin D with breast cancer among women in Karachi, Pakistan

    Background: The prevalence of vitamin D inadequacy and breast cancer are both high among women living in Karachi, Pakistan. Methods: A matched case control study was conducted in two hospitals of Karachi, Pakistan to evaluate the association of vitamin D (serum 25-hydroxyvitamin D) concentrations, vitamin D supplementation and sun exposure with breast cancer among Pakistani women. A total of 411 newly diagnosed histologically confirmed primary breast cancer cases were enrolled and 784 controls, free of breast and any other cancers, were matched by age (year of birth ± 5 years), residence in the same geographic area and study site. Information was collected on sociodemographic history, history of vitamin D supplementation, past medical and obstetrical history, family history of breast cancer, sun exposure history, histopathology reports and anthropometric measurement and venous blood was collected to measure serum 25-hydroxyvitamin D (25(OH)D) concentration. Results: Compared to patients with sufficient serum vitamin D (\u3e30 ng/ml), women with serum vitamin D deficiency (\u3c20ng/ml), had a higher risk of breast cancer (OR = 1.65, 95%CI: 1.10, 2.50). Women with history of vitamin D supplementation one year prior to enrollment, had significant protective effect against breast cancer (OR = 0.32, 95% CI: 0.24, 0.43). Conclusions and Recommendation: Serum vitamin D deficiency was associated with increased risk of breast cancer, while vitamin D supplementation was associated with decreased risk of breast cancer. In Pakistani women, where vitamin D deficiency is common, raising and maintaining serum vitamin D at population level is a safe and affordable strategy. It may play a role in reducing the incidence of both vitamin D deficiency and breast cancer, particularly among poor women where the breast cancer mortality is highest due to limited resources for early detection, diagnosis, and treatment. The effects of vitamin D with regard to breast cancer risk in Karachi Pakistan should be further evaluated

    Rheumatoid Arthritis with Deep Venous Thrombosis (DVT): A Case Report

    A 25-year-old lady presented with pain and swelling with restricted movement of right lower limb for one month. She had arthritis involving multiple large and small joints of hands and feet over the last 4 years and was diagnosed as a case of rheumatoid arthritis (RA) 2 years back. She had been taking disease modifying antirheumatic drugs (DMARD) - methotrexate since then and continued it for one year. Subsequent investigations revealed that she developed deep venous thrombosis (DVT) of right posterior tibial and calf veins but no other predisposing factor was found.     Comparative immunocompetence and interspecies transmission of avian orthoavulavirus-1 in feral birds originating from rural and urban settings

    Feral birds residing close to urban settings exhibit higher immunocompetence against environmental pathogens than their counterparts in rural areas. In this study, we comprehensively evaluated the immunocompetence of five specific feral bird species and investigated the potential for interspecies transmission and pathogenicity of Avian orthoavulavirus-1 (AOAV-1) originating from the Anseriformes order. The immunocompetence assessment involved administering the phytohemagglutinin (PHA) test to individual groups of birds from rural and urban settings, measuring patagium thickness at specific time intervals (12, 24, 36, 48, and 60 h) following the administration of 0.1 mL (1 mg/mL) of PHA. Urban birds displayed significantly enhanced mean swelling responses, particularly urban pigeons, which exhibited a significant difference in patagium thickness at all-time intervals except for 24 h (p = 0.000, p = 0.12). Similarly, rural and urban quails and crows showed substantial differences in patagium thickness at all-time intervals except for 12 h (p = 0.542, p = 0.29). For the assessment of interspecies transmission potential and pathogenicity, each feral bird group was separately housed with naive broiler birds (n = 10 each) and challenged with a velogenic strain of AOAV-1 isolate (Mallard-II/UVAS/Pak/2016) at a dose of 1 mL (10 EID /mL). Urban birds demonstrated higher resistance to the virus compared to their rural counterparts. These findings highlight the specific immunocompetence of feral bird species and their potential contributions to AOAV-1 transmission and pathogenicity. Continuous monitoring, surveillance, and strict implementation of biosafety and biosecurity measures are crucial for effectively controlling AOAV-1 spillover to the environment and wild bird populations in resource-limited settings, particularly Pakistan

    Trichogranuloma of the external auditory canal mimicking aural polyp: A rare case report

    We report a case of an 18-year-old male who presented to us with complaints of decreased hearing, ear discharge, and fullness in the right ear for last 2 months. Examination revealed a swelling of 0.5 cm × 0.5 cm in the inferior part of the external acoustic canal, tympanic membrane was found to be normal. Patient was advised antibiotic and steroids ear drops after making a provisional diagnosis of aural polyp. Surgical excision of the swelling was done when conservative treatment was unsuccessful. The excised polyp was sent for histopathological examination which revealed presence of foreign body giant cells and mixed inflammatory infiltrate around the fragments of hair shafts which confirmed the diagnosis of trichogranuloma in the external auditory canal. Owing to the rarity of such location of trichogranuloma, this case is being reported here

    Adaptive Color Image Encryption Scheme Based on Multiple Distinct Chaotic Maps and DNA Computing

    In this paper, we propose an adaptive encryption scheme for color images using Multiple Distinct Chaotic Maps (MDCM) and DNA computing. We have chosen three distinct chaotic maps, including a 2D-Henon map, a Tent map, and a Logistic map, to separately encrypt the red, green, and blue channels of the original image. The proposed scheme adaptively modifies the parameters of the maps, utilizing various statistical characteristics such as mean, variance, and median of the image to be encrypted. Thus, whenever there is a change in the plain image, the secret keys also change. This makes the proposed scheme robust against the chosen and known plaintext attacks. DNA encoding has also been used to add another layer of security. The experimental analysis of the proposed scheme shows that the average value of entropy is approximately eight, the Number of Pixels Change Rate (NPCR) and Unified Average Changing Intensity (UACI) are 99.61% and 33%, respectively, and correlation coefficients close to zero, making the scheme not only reliable but also resilient against many attacks. Moreover, the use of low-dimensional maps reduces the computational costs of the scheme to a large extent

    The 21st century has seen rapid changes in technology, industry, and social patterns. Most industries have moved towards automation, and human intervention has decreased, which has led to a revolution in industries, named the fourth industrial revolution (Industry 4.0). Industry 4.0 or the fourth industrial revolution (IR 4.0) relies heavily on the Internet of Things (IoT) and wireless sensor networks (WSN). IoT and WSN are used in various control systems, including environmental monitoring, home automation, and chemical/biological attack detection. IoT devices and applications are used to process extracted data from WSN devices and transmit them to remote locations. This systematic literature review offers a wide range of information on Industry 4.0, finds research gaps, and recommends future directions. Seven research questions are addressed in this article: (i) What are the contributions of WSN in IR 4.0? (ii) What are the contributions of IoT in IR 4.0? (iii) What are the types of WSN coverage areas for IR 4.0? (iv) What are the major types of network intruders in WSN and IoT systems? (v) What are the prominent network security attacks in WSN and IoT? (vi) What are the significant issues in IoT and WSN frameworks? and (vii) What are the limitations and research gaps in the existing work? This study mainly focuses on research solutions and new techniques to automate Industry 4.0. In this research, we analyzed over 130 articles from 2014 until 2021. This paper covers several aspects of Industry 4.0, from the designing phase to security needs, from the deployment stage to the classification of the network, the difficulties, challenges, and future directions
