Prevalence and association of obesity with self-reported comorbidity: a cross-sectional study of 1321 adult participants in Lasbela, Balochistan
Association of fatness with chronic metabolic diseases is a well-established fact, and a high prevalence of risk factors for these disorders has increasingly been reported in the third world. In order to incorporate any preventive strategies for such risk factors into clinical practice, decision-makers require objective evidence about the associated burden of disease. A cross-sectional study of 1321 adults from one of the districts of Balochistan, among the most economically challenged areas of Pakistan, was carried out for the measures of fatness and self-reported comorbidities. Body mass index (BMI), waist circumference (WC), and waist-to-hip ratio (WHR) were measured and demographic information and self-reported comorbidities were documented.The prevalence of obesity was 4.8% (95% CI: [3.8, 6.1]) and 21.7% (95% CI: [19.5, 24.0]), as defined by the World Health Organization (WHO) international and Asia/Asia-Pacific BMI cut-offs, respectively. The proportion exhibiting comorbidity increased with increasing levels of fatness in a dose-response relationship
Making the Most of High Inflation
The paper analyses inflationary real effects in situation where there are frequent episodes of high inflation. It is conjectured with the increase in high inflation, and when differences between the expected and output-neutral inflation become large, output stimulation through inflationary shocks is more effective than otherwise. It is shown that this conjecture is valid for most countries with high inflation episodes, where inflation is greater than 4.8% for at least 25% of quarterly observations. This leads to a simple policy prescription that anti-inflationary monetary decisions should be undertaken in periods where the expected inflation exceeds output-neutral
Interplay of the exciton and electron-hole plasma recombination on the photoluminescence dynamics in bulk GaAs
We present a systematic study of the exciton/electron-hole plasma
photoluminescence dynamics in bulk GaAs for various lattice temperatures and
excitation densities. The competition between the exciton and electron-hole
pair recombination dominates the onset of the luminescence. We show that the
metal-to-insulator transition, induced by temperature and/or excitation
density, can be directly monitored by the carrier dynamics and the
time-resolved spectral characteristics of the light emission. The dependence on
carrier density of the photoluminescence rise time is strongly modified around
a lattice temperature of 49 K, corresponding to the exciton binding energy (4.2
meV). In a similar way, the rise-time dependence on lattice temperature
undergoes a relatively abrupt change at an excitation density of 120-180x10^15
cm^-3, which is about five times greater than the calculated Mott density in
GaAs taking into account many body corrections.Comment: 15 pages, 7 figures, submitted to Phys. Rev.
Impact of supply problems of preservative-free glaucoma medications on patients and hospital staff
PURPOSE: Glaucoma is a chronic ocular disease, which is usually managed with long-term daily medical therapy, in the form of eye drops. Patients who are intolerant to preservatives in topical medicines require preservative-free versions. From early 2011 patients attending Moorfields Eye Hospital, London, UK, started to report recurring problems with the supply of the following preservative-free glaucoma medications: Timolol 0.25% (Timoptol 0.25%, MSD UK); Dorzolamide (Trusopt, MSD UK); Dorzolamide and Timolol 0.5% (Cosopt, MSD UK). This study investigates the impact of the supply problems of these medications at Moorfields Eye Hospital from a patient, administrative and clinical perspective.
METHODS: Information was sought by interviewing both patients and hospital staff, and by a retrospective case note review between April 2010 and May 2013.
RESULTS: Many hospital roles, both administrative and clinical, were involved in attempting to resolve the impact of the supply problems. All staff reported a considerable increase in their workload. At the peak of the problem, the glaucoma secretaries received about 150 enquiries per week.
A review of 83 sets of patient notes, retrieved from a random sample of 125 patients, showed that 22% encountered a supply problem. Of these, more than one-third attended Moorfields Eye Hospital Accident & Emergency (A&E) for repeat supplies and 89% eventually had their medication changed. In telephone interviews with 39 of a random sample of 50 patients (a subset of the 83 notes retrieved), 59% of the interviewees reported a supply problem. Of these, one-third attended Moorfields Eye Hospital A&E for repeat supplies and half eventually required an alternative medication. Some patients reported going to considerable lengths to obtain ongoing supplies in the community.
CONCLUSIONS: This study shows that medication supply problems can have a major impact on patients and hospital services. Supply problems occur across many fields of medicine and with increasing frequency. The findings of this study highlight the importance of early communication of impending shortages between manufacturers and the Department of Health, as recommended in the best practice guidelines. In order to minimise the impact of medicine shortages on patients, clinicians and administrative staff, hospitals need immediate notification of potential supply problems and clear updates on supply resolution. In addition, hospitals should consider nominating an individual as a contact point for patient enquiries regarding medicine supply problems
Cement substitution with secondary materials can reduce annual global CO2 emissions by up to 1.3 gigatons
Population and development megatrends will drive growth in cement production, which is already one of the most challenging-to-mitigate sources of CO2 emissions. However, availabilities of conventional secondary cementitious materials (CMs) like fly ash are declining. Here, we present detailed generation rates of secondary CMs worldwide between 2002 and 2018, showing the potential for 3.5 Gt to be generated in 2018. Maximal substitution of Portland cement clinker with these materials could have avoided up to 1.3 Gt CO2-eq. emissions (~44% of cement production and ~2.8% of anthropogenic CO2-eq. emissions) in 2018. We also show that nearly all of the highest cement producing nations can locally generate and use secondary CMs to substitute up to 50% domestic Portland cement clinker, with many countries able to potentially substitute 100% Portland cement clinker. Our results highlight the importance of pursuing regionally optimized CM mix designs and systemic approaches to decarbonizing the global CMs cycle
Association of Epstein Barr Virus (EBV) with nasopharyngeal carcinoma (NPC)
Aim: To observe the frequency of nasopharyngeal carcinoma (NPC) and its association with Epstein Barr Virus (EBV) infection. Setting: This study included consecutive cases of nasopharyngeal carcinoma, which were diagnosed in the Department of Pathology at the Aga Khan University Hospital, Karachi in the period of two years (1996-97). Methods: These tumors were initially evaluated on H&E stained sections. The tumors showing evidence of keratinization were excluded from the study. The Epstein Barr Virus was detected with the help of Polymerase chain reaction in formalin fixed, paraffin embedded tissue sections. Results: During the study period, seventeen cases of nasopharyngeal carcinoma were diagnosed which comprised 0.3% of all malignant tumors. The age ranged from 5 years to 70 years with male to female ratio of 2.4:1. The NPC was more prevalent in adults (71%) as compared to children (29%) under 15 years. Six cases (35%) exhibited positive signal for Epstein Barr Virus. Conclusion: Nasopharyngeal carcinoma is an infrequent tumor. The prevalence of Epstein Barr virus infection in nasopharyngeal carcinoma is quite low as compared to other regions of the world
Fully quantum-mechanical treatment of proton-hydrogen scattering

Cross sections for target ionisation and electron capture by the projectile have been calculated in the energy range from 20 keV to 1 MeV. Calculated electron capture cross sections are in good agreement with the experiment, however for ionisation discrepancies between theory and experiment at intermediate energies still remain
Dimer Decimation and Intricately Nested Localized-Ballistic Phases of Kicked Harper
Dimer decimation scheme is introduced in order to study the kicked quantum
systems exhibiting localization transition. The tight-binding representation of
the model is mapped to a vectorized dimer where an asymptotic dissociation of
the dimer is shown to correspond to the vanishing of the transmission
coefficient thru the system. The method unveils an intricate nesting of
extended and localized phases in two-dimensional parameter space. In addition
to computing transport characteristics with extremely high precision, the
renormalization tools also provide a new method to compute quasienergy
