Repurposing of Rutan showed effective treatment for COVID-19 disease
Previously, from the tannic sumac plant (Rhus coriaria), we developed the Rutan 25βmg oral drug tablets with antiviral activity against influenza A and B viruses, adenoviruses, paramyxoviruses, herpes virus, and cytomegalovirus. Here, our re-purposing study demonstrated that Rutan at 25, 50, and 100βmg/kg provided a very effective and safe treatment for COVID-19 infection, simultaneously inhibiting two vital enzyme systems of the SARS-CoV-2 virus: 3C-like proteinase (3CLpro) and RNA-dependent RNA polymerase (RdRp). There was no drug accumulation in experimental animalsβ organs and tissues. A clinical study demonstrated a statistically significant decrease in the C-reactive protein and a reduction of the viremia period. In patients receiving Rutan 25βmg (children) and 100βmg (adults), the frequency of post-COVID-19 manifestations was significantly less than in the control groups not treated with Rutan tablets. Experience in using Ketorol in osteoarthrosis patients with significant pain syndrome
Objective - to study the clinical efficacy and tolerability of two dosage forms of Ketorol (tablets and solution for injections) versus the similar formulations of diclofenac sodium in patients with osteoarthrosis (OA) in an open-labeled randomized study.
Patients and methods. The study covered 109 patients with OA (by the criteria developed by R. Altman et al.) with the knee joints being predominantly involved from 4 rheumatological centers of the Russian Federation. A study group comprised 51 female patients with gonarthrosis, treated with Ketorol (25 patients received the drug as tablets and 26 patients had it as injections) and a control group included 58 patients given diclofenac sodium (29 patients received it as tablets and 29 had it as injections). The duration of Ketorol therapy was 5 days. Both formulations for injection were used only for 2 days and then the patients were switched to tablets.
Results. Ketorol was found to be superior to diclofenac in short-term effectiveness (in relieving the pain syndrome) by 25-30%. Comparison of the results of treatment in the study and control groups showed a significantly more effective relief of the pain syndrome in those who received the drug as tablets. Both Ketorol formulations reduced the severity index by 25% whereas diclofenac did only by 15-18%. The same picture was also observed when the general condition of patients was evaluated by a physician. Analysis of WOMAK index changes indicated that by the end of the study, the percent of improvement with Ketorol use was much higher for both formulations (31% for tablets and 33% for injections) than that with diclofenac (18% and 6%). Pairwise comparison showed that the efficiency of Ketorol tablets was, in physician's opinion, much higher than that of diclofenac and the effectiveness of injections was comparable. However, the mean duration of Ketorol injections was twice greater (p = 0,003) than the similar formulation of diclofenac (426,3+153,2 min versus 288,3+173,1 min). There were no side effects of Ketorol on renal and hepatic function; no occult blood loss was found. Conclusion. The short-term use of Ketorol in severe gonarthrosis-caused pains results in a more marked improvement than that of diclofenac

EXPERIENCE WITH RITUXIMAB IN PATIENTS WITH ANKYLOSING SPONDYLITIS
Objective: to analyze the efficacy of rituximab (RTM) in patients with active ankylosing spondylitis (AS) resistant to conventional therapy. Subjects and methods. The trial enrolled 10 male patients with the reliable and valid diagnosis of AS who received therapy with RTM used in cases of steadily high AS activity, resistance to standard therapy, and contraindications to the use of tumor necrosis factor-Ξ± inhibitors. The number of patients meeting the Assessment of Spondyloarthritis International Society (ASAS) criteria for 20% improvements 24 weeks after treatment initiation was the main indicator of therapeutic effectiveness. Results. After 24-week therapy, 7 and 4 of the 10 patients showed 20 and 40% improvements, respectively; and 2 patients had partial remission according to the ASAS criteria. During the treatment, the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) significantly decreased (p = 0.046), the Bath Ankylosing Spondylitis Functional Index (BASFI) remained substantially unchanged. Treatment response did not depend on the development of an early (at 2 weeks) complete depletion of CD20 lymphocytes. No significant effect could be achieved in patients with high baseline BASDAI and BASFI scores. Conclusion. RTM used patients with AS can ensure clinical improvement and even remission in a number of cases; however, it is unpromising in cases of high disease activity and severe functional failure
Chemical composition and biological activity of seed oil of amaranth varieties
The work is devoted to study of seed oil composition of amaranth varieties: Kharkov, Lera, Andijan and Helios, acclimatized in Uzbekistan. We demonstrated the possibility of using reversed-phase HPLC using a refractometric detector, which allows simultaneous determination of squalene and triacylglycerides in plant seeds and determining the authenticity of amaranth oils. Established seed oiliness ranged from 6.39 to 7.81 % of the initial mass. Amaranth oil samples contained quite large amount of unsaturated fatty acids 72.72 β 73.28 %, 1.17 % of which is omega-3-alpha-linolenic acid. The squalene content in the seeds ranged from 0.35 % to 0.55 %. It was established that the squalene content in oils obtained by extraction is greater than the one obtained by cold pressing. In the triacylglyceride composition of the investigated cold-pressed and extracted oils, no significant differences were found
