A Rare Tumour of the Breast: Carcinosarcoma
Carcinosarcoma of the breast, also known as metaplastic carcinoma, is rare with very few cases reported in the literature. A 46-year old female patient presented with a mass in her left breast. Physical examination, ultrasonography and mammography findings were consistent with malignancy. The mass was totally removed. Histopathological examination revealed carcinosarcoma of the breast. Histologic grade of the tumour was III. Ki67 proliferation index was found 40% positive. Tumour cells were positive for p53 (70% positive), c-erb-B2 (5% positive), pancytokeratin and EMA in carcinomatous areas, and vimentin in sarcomatous areas. There was no metastasis in axillary lymph node and distant metastasis. The patient is receiving chemotherapy and is under follow-up in the 54th month. Along with a review of the literature, we present the information regarding the clinical and histological findings and treatment of the patient who was operated due to breast carcinosarcoma
Giant renal oncocytoma: a case report and review of the literature
Abstract Introduction Renal oncocytomas are benign neoplasms derived from cells of the distal renal tubule, and comprise 5% to 7% of primary renal neoplasms. Oncocytomas are mostly asymptomatic, and the majority of tumors are discovered incidentally. In this case report, we present a case of a patient with a giant oncocytoma arising from her left kidney. Case presentation We describe a 25-year-old Turkish woman who was admitted to our hospital with abdominal pain and a 3-year palpable abdominal mass, which was found present since her second pregnancy. Examination revealed a 15 × 20-cm mass in her abdominal cavity. Computed tomography revealed a mass with regular outlines, measuring 18 × 11 × 12 cm, associated with the left kidney, and causing marked hydroureteronephrosis. We excised the mass and performed a left nephrectomy on our patient. The immunohistopathology of the mass was consistent with renal oncocytoma. No local or distant metastasis was seen at 6 months postoperatively. Conclusion To the best of our knowledge, this is the second largest renal oncocytoma described in the English language literature. This is also the first reported giant oncocytoma that presented during pregnancy.</p
Basaloid (Cloacogenic) Carcinoma Mimicking Intraabdominal Abscess: Report of a Case and Review of the Literature
Cloacogenic carcinoma (CC, basaloid carcinoma) generally occurs around the dentate line, rarely it can arise from the other sides of the colon. There are only 5 cases of CC located outside the anal canal in the literature. The first occurrence of a CC presents as intraabdominal abscess. We describe a 23-year-old male patient who was admitted with fever and severe abdominal pain. Computed tomography imaging showed diffuse wall thickening about 10–11 cm above the rectosigmoid junction, intraabdominal abscess and a soft tissue lession covering the pelvis with a size of 8 × 8.5 cm including cystic necrotic areas. We performed Hartman procedure since the mass was nonresectable. Histopathological examination showed CC. In total, three times radiotherapy and a concurrent three-drug regimen of irinotecan, fluorouracil and folinic acid chemotherapy were administered for 6 weeks. As a result, the patient was lost because of multiple organ dysfunction syndrome that developed 3 months after radio-chemotherapy
Castleman’s disease as cervical mass: a report of three cases and review of the literature

An unusual cause of ileal perforation: Report of a case and literature review
An ileal perforation resulting from a migrated biliary stent is a rare complication of endoscopic stent placement for benign or malignant biliary tract disease. We describe the case of a 59-year-old woman with a history of abdominal surgery in which a migrated biliary stent resulted in an ileal perforation. Patients with comorbid abdominal pathologies, including colonic diverticuli, parastomal hernia, or abdominal hernia, may be at increased risk of perforation from migrated stents
Prognostic importance of p53 expression in patients with localized gastric cancer who had adjuvant chemoradiotherapy
Amaç: Günümüzde mide kanserinde postoperatif yaklaşımda
standart tedavi 5FU bazlı kemoterapi ve eş zamanlı 45
Gy eksternal radyoterapidir. Bu tedavinin yüksek toksisite
oranları hastaların büyük çoğunluğunun tedavisin-de kesintiye
sebep olmaktadır. Tanı sırasında saptanabilen bazı
klinik ve laboratuar parametreleri kullanılarak; tedaviye
yanıtının tahmin edilmesi ile bu yan etkilerden kaçınılabilinir.
Çalışmamızda mide kanseri nedeniyle adjuvan
kemoradyoterapi uyguladığımız hastalarda p53 ekspresyonun
prognostik ve varsa prediktif rolü araştırıldı.
Gereç ve Yöntem: 2008-2011 yılları arasında Antalya Eğitim
ve Araştırma Hastanesi Tıbbi Onkoloji ve yerel Radyasyon
Onkolojisi kliniklerinde postoperatif histopatolojik olarak
tanısı doğrulanmış adjuvan kemoradyoterapi almış olan
hastalar çalışmaya dahil edildi. p53 ekspresyonu
immünohistokimya ile araştırıldı.
Bulgular: Çalışmaya 11’i (%52,4) erkek, 9’u (%42,9) kadın
olmak üzere toplam 20 hasta alındı. p53 ekspresyonu 13
hastada (%61,9) hastada pozitif olarak saptanırken 7 hastada
(%33,3) negatif olarak saptandı. Hastaların medyan
yaşam süresi p53 negatiflerde 12.2 ay, p53 pozitif olanlarda
11 ay olarak saptandı. p53 ekspresyonu ile sağkalım arasında
ilişki tespit edilemedi (P:0,781).
Sonuç: Çalışmamızda p53 ekspresyonun bu hasta grubunda
prediktif rolünü gösterememize rağmen hasta serum örneklerinde
p53’ün tespit edilmesini de içeren yeni çalışmaların
bu konudaki bilinmeyenleri aydınlatacağını düşünüyoruz.Objective: The postopreative standard therapy in gastric

external radiothearpy , nowadays. The high rates of toxicity
of this thearpy causes discontiniuty in majority of the
patients. These side effects can be prevented by predicting
the response to therapy by the help of some spesisfic
clinical and laboratory parameters determined at the time
of diagnosis. In our study the prognostic and -if there is -
the predictive role of p53 expression in patients with gastric
cancer who had adjuvant chemotherapy has been
searched.
Material and Methods: Patients with a postoperative
histopathologic diagnosis and who had adjuvant
chemoradiotherapy in Medical Oncology and local
Radiation Oncology Departments of Antalya Eucation and
Research Hospital between 2008-2011 were enrolled in this
study. p53 expression was searched by
immunohistochemical method.
Results: A total of 20 patients , (11 (%52,4) male and 9
(%42,9) female) were enrolled in the study. p53 expression
was found positive in 13 (%61,9) patients and negative in 7
(%33,3)patients. Median survival time ws found 12.2
months in p53 negaive patients and 11 months in p53
positive patients. A significant relationship was not found
between p53 expression and survival. (P:0,781).
Conclucion: Although we were not able to show the
predictive role of p53 expression in this patient group in
this study, we think that new studies including determining
p53 in blood samples will be helpful in explaining some
unknown things about this issue
