Asemptomatik primer hiperparatiroidi vakalarında hastalık seyrini belirleyen faktörler
Primer hiperparatiroidi (PHPT), paratiroid bezinden aşırı paratiroid hormon (PTH) salınımı sonucu gelişen hiperkalsemi ile karakterize klinik bir tablodur. Günümüzde PHPT saptanan hastaların çoğu asemptomatik seyretmektedir. PHPT’in asemptomatik alt grubunun doğal gelişimi ile ilgili veri eksikliği bulunmaktadır. PHPT öncelikle kemik metabolizmasını etkileyen bir durumdur. Bununla birlikte deneysel çalışmalar, klasik PHPT hastalarının uzun dönemde insülin direncine (ID) sahip olabileceğini, hastalık ilerledikçe, göreceli insülin yetmezliği ve aşikar diabetes mellitus (DM) geliştirebileceğini düşündürmüştür. Biz bu çalışmamızda, asemptomatik PHPT vakalarında ID’nin ve kemik metabolizma değişikliklerinin 18 aylık süre zarfındaki doğal seyrini irdelemeyi amaçladık.
Polikliniğimize başvuran asemptomatik PHPT tanısı almış 61 hastada 75 g oral glukoz tolerans testi ile glukoz toleransı ve ID indeksi değerledirildi. Bu parametrelerin doğal seyri, 18 ay boyunca 6 aylık dönemlerde testler tekrarlanarak gözden geçirildi. Bu vakaların, ayrıca kemik metabolizmaları kemik mineral yoğunluklarının (KMY), zaman içinde değişimi değerlendirilerek yapıldı. Veriler sağlıklı kontrol grubu (n=80) ile karşılaştırıldı. Grupların demografik, klinik ve laboratuvar özellikleri benzer olduğu halde; asemptomatik PHPT grubunda açlık insülin düzeylerinde yükseklik (p=0.031) ve ID’de artış saptadık (p=0.035). PHPT grubundaki açlık insülin düzeyi ve ID’deki istatistiksel olarak anlamlı yükseklikler 18. ayda da devam etti ama, 0. ay değerleri ile aralarında anlamlı fark yoktu. İstatistiksel olarak anlamlı olmamakla beraber PHPT grubunda (bozulmuş açlık glukozu) IFG %11.5, kontrol grubunda IFG %9.8 idi. DM yüzdesi ise her iki grupta aynıydı. Ailede DM, IFG ve IGT bulunması yönünden hastaları sorguladık; PHPT grubunda kontrol grubuna göre anlamlı düşüklük vardı (p=0.032). Bu durumda, ailevi yatkınlık olmamasına rağmen, asemptomatik PHPT grubunda ID riskinin artmış olduğunu bulduk. Asemptomatik PHPT grubunda, 0.-18. ay PTH düzeylerini karşılaştırdığımızda anlamlı artış saptarken (p=0.043), serum Ca ve P düzeyleri ile 24 saatlik idrar Ca ve P seviyelerinde değişiklik olmadı. Altı ay aralarla yapılan KMY’na bakıldığında 18. ay sonunda, PHPT grubunda femur boyun ve trokanter yoğunluklarında istatistiksel olarak anlamlı artış (femur boyun p=0.020, femur trokanter p=0.028) saptanırken diğer bölgelerde (lumber vertebra ve radius distal 1/3) anlamlı değişiklik saptanmadı. Asemptomatik PHPT hastalarının, özellikle metabolik komplikasyonlarının belirlenmesi için uzun dönemli daha geniş araştırmalara ihtiyaç vardır ve bu araştırmaların sonucunda bir görüş birliği oluşturulup, bu grup hastaların nasıl takip ve tedavi edileceğine karar verilmesi gereklidir. Bu sorunlar yanıt buluncaya kadar, cerrahi tedavi uygulanmayıp takip edilen hastaların, sadece KMY kaybı yönünden değil, ID ve DM gelişimi yönünden de riskli grup içinde değerlendirilmesinin uygun olacağını düşünmekteyi
Ötiroid bireylerde metabolik sendrom komponentleri ile tiroid fonksiyon, volüm ve nodül ilişkisi
Metabolik Sendrom (MS) insülin direncinin belirgin rol oynadığı metabolik
anormallikerin bir kümelenmesidir. Son zamanlarda MS ile tiroidin fonksiyonel/morfolojik
anormallikleri arasında olabilecek bir ilişki sorgulanmaktadır. Bizim bu vaka kontrol
çalışmasındaki amacımız, MS bulunan hastalarda tiroid volüm ve nodül prevalansını
incelemekti.
Metabolik sendrom bulunan 278 hasta ile randomizasyon yöntemi kullanılarak yaş,
cinsiyet ve sigara alışkanlığı yönünden eşleştirilmiş 261 kontrol vakası eşleştirildi. MS
parametrelerinin yanı sıra TSH, sT3, sT4 ve homeostasis model assessment- IR (HOMAIR)
ile hesaplanan İD seviyeleri değerlendirildi. Bütün katılımcılara tiroid ultrasonografisi
yapıldı. Tiroid nodüllerinden 1cm den büyük olanlara tiroid ince iğne aspirasyon biyopsisi
(TİİAB) uygulandı.
TSH metabolik sendrom varlığı ile anlamlı olarak pozitif koreleydi. Serbest tiroid
hormon seviyeleri ile MS ve komponentleri arasında ilişki yoktu. MS hastalarının ortalama
tiroid volümleri kontrol grubuna göre anlamlı olarak artmıştı (P<0.0001). Tiroid nodül
yüzdesi MS hastalarında daha yüksekti (sırasıyla, %50.4, %14.6, P<0.0001). Çalışmaya
alınan vakalar İD varlığına göre de iki gruba ayrıldı. İnsülin direnci olan bireylerde tiroid
volüm ve nodül formasyonunun artmış olduğu görüldü. İD varlığında tiroid nodül gelişimi
için tahmini rölatif riskin 3,2 olduğu saptandı. TSH’nın yanı sıra MS komponentlerinin
tiroid volüm artışı için bağımsız risk faktörü olduğu bulundu.Tiroid nodül formasyonu İD
ile korele iken TSH ile korelasyon tespit edilmedi. Tiroid nodülü bulunan ve TİİAB
yapılan 38 MS hastasının 3’ünde (%7,9) tiroid kanseri saptanırken kontrol vakalarında
(n=22) saptanmadı.
Sonuçlar MS bulunan hastaların anlamlı olarak artmış tiroid volüm ve nodül
prevalansına sahip olduklarını göstermektedir. Multivariate modelde İD bu risk artışına
anlamlı olarak katkıda bulunmaktadır. Bizim verilerimiz nodül formasyonu için İD’nin
bağımsız bir risk faktörü olduğunu göstermektedir.
The metabolic syndrome (MS) is a cluster of metabolic abnormalities with insulin
resistance (IR) as a major characteristic. It has been recently questioned that MetS and its
related components are associated with functional and morphological alterations of thyroid
gland. Aim of our study is to examine thyroid volume and nodule prevalence in a case
control study of patients with MS.
Two hundred seventy eight patients with MS were randomly matched for age,
gender,and smoking status with 261 subjects without MetS. Serum TSH, free T3 and T4,
and the level of İR, estimated by the HOMA-IR, as well as other MS parameters were
evaluated. Thyroid ultrasonography was performed in all participants. Thyroid nodules
greater than 1cm underwent fine needle aspiration biopsy.
TSH was significantly positively correlated with the presence of MS diagnosis.
There was no association between free thyroid hormone levels and MS and its related
components. Mean thyroid volume was higher in patients with MS than in controls
(p<0.0001). Percentage of patients with thyroid nodules was also higher in patients with
MS (50.4% vs. 14.6%, p<0.0001). Participants were also divided into two groups
according to the presence of IR. Subjects with IR have also increased thyroid volume and
nodule formation. The odds ratio for the development of thyroid nodule in the presence of
IR was 3.2. TSH as well as all MS components were found to be independent risk factors
for thyroid volume increase. IR but not TSH was found to be correlated with thyroid
nodule formation. Thyroid cancer was diagnosed in 3/38 patients with MS (7.9%). No
cancer cases were found in control subjects.
The results suggest that patients with MS have significantly increased thyroid
volume and nodule prevalence. In multivariate model, the presence of IR contributed
substantially to this increased risk. Our data provide evidence that IR is an independent
risk factor for nodule formation
Prevalence of Metabolic Syndrome in Primary Hyperparathyroidism
Aim: In this study, we aimed to evaluate the prevalence of metabolic syndrome (MetS), metabolic abnormalities and independent predictors of MetS in primary hyperparathyroidism and to contribute to long-term follow-up and management of these patients.
Methods: Seventy-eight non-pregnant patients aged 18 years and over without renal or hepatic failure and other systemic diseases, who were diagnosed with primary hyperparathyroidism, between January 2005 and December 2014 were included in the study.
Results: Sixty-two (79.5%) subjects were female and 16 (20.5%) were male. The mean age of the patients was 55.3±12.6 years. Fifty eight patients were classified as symptomatic and 20 as asymptomatic. Thirty-two patients (41%) were found to have primary hyperparathyroidism in accordance with the modified National Cholesterol Education Program-Adult Treatment Panel III criteria. The frequency of urinary tract stone disease was significantly higher in patients with hyperparathyroidism with MetS than in those with primary hyperparathyroidism without MetS (p=0.018).
Conclusion: In our study, the prevalence of MetS in patients with primary hyperparathyroidism was found to be similar to that demonstrated in epidemiological studies performed in the general population of the same age. However, in these patients higher prevalence of hypertension, increased waist circumference, lipid disorders such as some syndrome abnormalities that may lead to the increased cardiovascular morbidity and mortality were observed
TERT Expression in Pituitary Adenomas
Objective: Although pituitary adenomas have benign histomorphological features, some of them may present in an aggressive manner. To predict the behaviour of these tumours, telomerase reverse transcriptase (TERT) activity in pituitary adenomas has been the subject of a few studies with contradictory results. This study aims to investigate whether immunohistochemical expression of TERT differs in neoplastic and nonneoplastic pituitary tissues and aims to investigate whether TERT expression is related to clinicopathological features of pituitary adenomas.
Material and Method: The study included 48 patients who had been diagnosed with pituitary adenomas and had clinical follow-ups. Nonneoplastic pituitary tissues were obtained from autopsy specimens (n=20). Immunohistochemistry for TERT antibody was performed. Both the nuclear and cytoplasmic expression of TERT antibody was noted, and total combined TERT staining was evaluated according to nuclear and cytoplasmic stainings.
Results: TERT expression did not differ between neoplastic and nonneoplastic pituitary tissues. Neither total (combined nuclear and cytoplasmic) TERT nor nuclear TERT expression revealed any statistically significant relationship with any of the clinicopathological features. Higher cytoplasmic TERT expression was observed in adenomas with recurrence than adenomas without recurrence (p=0.035).
Conclusion: This study introduces the notion that immunohistochemical expression of TERT does not differ in neoplastic and nonneoplastic pituitary tissues. Pituitary adenomas with cytoplasmic immunohistochemical expression of TERT have significantly higher rates of recurrence. Further studies, including combined methods of immunohistochemistry and molecular analyses in larger groups, may reveal applicable results for the clinical significance of TERT in pituitary adenomas
