    Radical radiotherapy for paediatric solid tumour metastases:An overview of current European protocols and outcomes of a SIOPE multicenter survey

    Purpose/objective: About 20% of children with solid tumours (ST) present with distant metastases (DM). Evidence regarding the use of radical radiotherapy of these DM is sparse and open for personal interpretation. The aim of this survey was to review European protocols and to map current practice regarding the irradiation of DM across SIOPE-affiliated countries. Materials/methods: Radiotherapy guidelines for metastatic sites (bone, brain, distant lymph nodes, lung and liver) in eight European protocols for rhabdomyosarcoma, non-rhabdomyosarcoma soft-tissue sarcoma, Ewing sarcoma, neuroblastoma and renal tumours were reviewed. SIOPE centres irradiating >= 50 children annually were invited to participate in an online survey. Results: Radiotherapy to at least one metastatic site was recommended in all protocols, except for high-risk neuroblastoma. Per protocol, dose prescription varied per site, and information on delineation and treatment planning/delivery was generally missing. Between July and September 2019, 20/27 centres completed the survey. Around 14% of patients were deemed to have DM from ST at diagnosis, of which half were treated with curative intent. A clear cut-off for a maximum number of DM was not used in half of the centres. Regardless of the tumour type and site, conventional radiotherapy regimens were most commonly used to treat DM. When stereotactic radiotherapy was used, a wide range of fractionation regimens were applied. Conclusion: Current radiotherapy guidelines for DM do not allow a consistent approach in a multicentre setting.     Parkour, Counter-Conducts and the Government of Difference in Post-industrial Turin

    The following paper aims to offer a critical discussion of the unfolding politics of belonging and exclusion taking place in Turin's regenerating cityscape as a way to illuminate the paradoxes, tensions and daily negotiations of emerging forms of social and spatial restructuring in the post-industrial city. In developing this analysis, we engage with an integrated methodological approach that privileges the voices and experiences of about 30 young men, mostly of migrant origins and aged 16-21, practicing parkour in the city's public spaces. In addressing these issues, we focus on the participants' engagement with one of the symbols of Turin's (multi)cultural, community-oriented and creative renewal, the post-industrial urban park of Parco Dora in order to unpack the processes of inclusion/exclusion and the conduct of conduct (Rose 2000) enacted in the creation, management and use of the city's regenerating areas. Our discussion of the participants' ambivalent and contested practices in Turin's cityscape enabled us to address how these young men re-inscribe tensions, instabilities and fault-lines relational to the “selective story-telling” (Vanolo 2015, 2) characterizing Turin's narratives of consensual transformation, post-industrial renaissance and (multi)cultural vitality. In particular, by engaging with the participants' bodily and spatial negotiations in Turin's public spaces through the lens of counter-conduct (Foucault 2007[1978]), we highlight the significance of recognising and examining partial, but productive forms of urban contestation within contemporary, pacified scenarios of urban regeneration

    The determinants of stroke phenotypes were different from the predictors (CHADS2 and CHA2DS2-VASc) of stroke in patients with atrial fibrillation: a comprehensive approach

    <p>Abstract</p> <p>Background</p> <p>Atrial fibrillation (AF) is a leading cause of fatal ischemic stroke. It was recently reported that international normalized ratio (INR) levels were associated with infarct volumes. However, factors other than INR levels that affect stroke phenotypes are largely unknown. Therefore, we evaluated the determinants of stroke phenotypes (pattern and volume) among patients with AF who were not adequately anticoagulated.</p> <p>Methods</p> <p>We analyzed data pertaining to consecutive AF patients admitted over a 6-year period with acute MCA territory infarcts. We divided the patients according to DWI (diffusion-weighted imaging) lesion volumes and patterns, and the relationship between stroke predictors (the CHADS<sub>2 </sub>and CHA<sub>2</sub>DS<sub>2</sub>-VASc score), systemic, and local factors and each stroke phenotype were then evaluated.</p> <p>Results</p> <p>The stroke phenotypes varied among 231 patients (admission INR median 1.06, interquartile range (IQR) 1.00-1.14). Specifically, (1) the DWI lesion volumes ranged from 0.04-338.62 ml (median 11.86 ml; IQR, 3.07-44.20 ml) and (2) 46 patients had a territorial infarct pattern, 118 had a lobar/deep pattern and 67 had a small scattered pattern. Multivariate testing revealed that the CHADS<sub>2 </sub>and CHA<sub>2</sub>DS<sub>2</sub>-VASc score were not related to either stroke phenotype. Additionally, the prior use of antiplatelet agents was not related to the stroke phenotypes. Congestive heart failure and diastolic dysfunction were not associated with stroke phenotypes.</p> <p>Conclusions</p> <p>The results of this study indicated that the determinants of stroke phenotypes were different from the predictors (i.e., CHADS2 and CHA<sub>2</sub>DS<sub>2</sub>-VASc score) of stroke in patients with AF.</p
