Technique originale de relèvement enclouage à foyer fermé d’une fracture thalamique du calcanéum

Le relèvement enclouage à foyer fermé (R.E.F.F) est une technique originale de traitement des fractures thalamiques du calcanéum. Il s'agit d'un patient âgé de 40 ans sans antécédents, victime d'une chute domestique sur le talon droit, d'une hauteur de 3 mètres. Le patient présentait un 'dème et une ecchymose du talon droit, sans ouverture cutanée. L'examen vasculo-nerveux était normal. Le bilan radiologique a objectivé une fracture thalamique du calcanéum droit type IV (Duparc) avec un angle de Böhler négatif; fracture à 4 fragments avec un enfoncement mixte horizontal et vertical. Nous avons réalisé la technique de relèvement enclouage à foyer fermé (R.E.F.F) sous contrôle fluoroscopique: patient installé en décubitus latéral gauche, sous rachianesthésie. Réduction et ostéosynthèse percutanée à l'aide de 2 clous de Steinmann (A, B): 1 clou relevant la surface thalamique et restituant l'angle de Böhler et le 2ème clou corrigeant le varus de la grosse tubérosité (C, D). Traitement chirurgical complété par une immobilisation plâtrée pendant 6 semaines. Les Suites opératoires ont été simples; ablation du plâtre et des clous de Steinmann à 6 semaines, et une rééducation a été entreprise. L'appui partiel a été autorisé à 8 semaines, appui total à 3 mois. Le résultat fonctionnel était bon avec un recul de 12 mois.Pan African Medical Journal 2015; 2

Proximal clavicle physeal fracture-separation mimicking an anterior sterno-clavicular dislocation

SummaryProximal physeal fracture-separation of the clavicle is a very rare injury occurring in the adolescent and in the young adult which involves a contact loss with fracture between the clavicle and its cartilaginous ossification center similar in appearance to a sternoclavicular dislocation. The authors report an unusual case of a proximal physeal fracture-separation of the clavicle with avulsion of sternoclavicular periosteal and ligamentous structures without vasculonervous injury in a 16-year-old young person. A CT scan examination is essential. Surgical management consisted in costoclavicular ligament and periosteum reattachment associated with reduction of the fracture-separation and pin fixation.This repair demonstrated a successful outcome at 8-month follow-up

Zona survenu lors d’un traitement par Infliximab pour une Rectocolite hémorragique: Herpes Zoster following Infliximab therapy for Ulcerative colitis

The advent of biotherapy in particular tumor necrosis factor antagonists has revolutionized inflammatory bowel disease treatment. A risk of tuberculosis and opportunistic infections following treatment with anti-TNF-a has been well reported. However, little is known about the association between TNF-a blockers and viral infection. We report a case of an Ulcerative Colitis patient under azathioprine and infliximab who developed intercostal hyperalgic Herpes zoster without severity signs, having evolved well after association of analgesic with antiviral. Through our case, we explain physiopathological mechanisms. Indeed, Interferon-? and TNF-a play critical roles in the control of viral infection. Depletion of TNF-a by antiTNF-a treatment can facilitate reactivation of a viral infection, including herpes zoster. The risk of herpes zoster in patients with Ulcerative Colitis and treated with anti-TNF-a is frequent. Thus, the interest of a systematic and prophylactic treatment is raised. L’avènement de la biothérapie en particulier, des anti- TNF-a a révolutionné la prise en charge des maladies inflammatoires de l’intestin. Un risque de tuberculose et d’infections opportunistes a été bien signalé. Cependant, peu de données ont été rapportés sur l’association anti-TNF-a et infections virales. Nous rapportons le cas d’une rectocolite hémorragique sous azathioprine et infliximab atteint d’un zona intercostal hyperalgique sans signes de gravités ayant bien évolué après traitement antalgique et antiviral. A travers ce cas, les mécanismes physiopathologiques sont discutés. En effet, l’interféron-? et le TNF-a jouent un rôle critique dans le contrôle de l’infection virale. Ainsi, Leur épuisement par un blocage anti TNF peut faciliter la réactivation d’une infection virale notamment un risque accru de l’herpès zona. Le risque de zona chez les patients atteints d’une RCH et traités par anti-TNF est fréquent. Ce qui soulève l’intérêt d’un traitement prophylactique systématique

Carbimazol and acenocoumarol, where is the problem?

Acenocoumarol and carbimazole are two drugs widely prescribed, they can sometimes be used in the same time. There is no known drug interaction between the two drugs but we report a case of a serious hemorrhagic complication following the concomitant use of the acenocoumarol and carbimazole. A 70-year old man treated by acenocoumarol for an aortic and mitral valve replacement. For a clinical and biological hyperthyroidism, he began treatment with carbimazole, ten days before admission. Three days later, he developed a mucocutaneous icterus with major hemorrhagic syndrome. The outcome was favourable after stopping medication and the use of vitamin K

Serum vitamin D and vitamin D receptor gene polymorphism in Moroccan patients with systemic lupus erythematosus

Background: Vitamin D plays an important role in the immunomodulation and could be involved in the development of autoimmune diseases such as systemic lupus erythematous (SLE). The study of the polymorphism of the Vitamin D Receptor (VDR) gene may be of interest in explaining the pathophysiology of SLE.Methods: In this study, we aimed to examine the characteristics of VDR gene BsmI polymorphism for the first time in Moroccan patients with SLE and their relationship with clinical manifestations of the disease. We also measured the serum level of 25-hyroxyvitamin D3 to assess its relation to such polymorphism.Results: The study included 66 SLE patients and 91 healthy controls. Our results showed that there were no differences observed in VDR genotypes and allelic distribution within the two groups. Both groups were in Hardy-Weinberg equilibrium, with no significant P values for the observed and expected genotype frequencies. 25-hyroxyvitamin D3 serum levels were the same in the two groups.Conclusions: Based on the results of the present study. We cannot verify any association between VDR gene BsmI polymorphism and SLE. This polymorphism could not be regarded as a genetic marker of the SLE. A larger study examining BsmI and other VDR gene polymorphisms is needed

Thrombophlébite cérébrale inhabituelle du post-partum

La thrombophlébite cérébrale du post-partum immédiat constitue un événement rare et gravissime pouvant mettre en jeu le pronostic vital à court terme. Celle-ci doit être systématiquement évoquée devant la persistance d'une fièvre dans les suites de couches. La prise en charge associera le plus souvent, des  antibiotiques à large spectre et des anticoagulants. Le suivi évolutif est indispensable, afin d'apprécier  l'efficacité thérapeutique. A travers une observation singulière et à présentation inhabituelle, nous insistons sur le grand intérêt des moyens d'imagerie dont nous disposons afin de porter le diagnostic et de choisir le traitement le mieux adapté.Key words: Anticoagulants, IRM, post-partum, thrombophlébite cerebral

Tétraparésie révélant un adénome de Conn chez une femme enceinte

Nous rapportons le cas d’un adénome de Conn révélé par une tétraparésie chez une femme de 33 ans, enceinte à 16 semaines d’aménorhées. La patiente a présenté une tension artérielle à 147/87 mmHg qui paraissait normale haute avec une hypokaliémie à 1,1 mmo/l. Le diagnostic a été confirmé par le dosage hormonal qui a montré une élévation de l’aldostérone plasmatique et une baisse de l’activité rénine plasmatique. L’imagerie par résonance magnétique a mis en évidence un nodule surrénalien gauche de 1,5 centimètre de diamètre pouvant être compatible avec un adénome surrénalien. Une surrénalectomie gauche a été pratiquée avec des suites opératoires simples, normalisation de la kaliémie et de la tension artérielle.The Pan African Medical Journal 2016;2
