    The Effect of Military Load Carriage on Postural Sway, Forward Trunk Lean, and Pelvic Girdle Motion

    International Journal of Exercise Science 10(1): 25-36, 2017. Musculoskeletal injuries are a common occurrence in military service members. It is believed that the load carried by the service member impedes stability and alters back and pelvis kinematics, increasing their susceptibility to musculoskeletal injuries, specifically in the lower extremities. The purpose of this study was to examine the effects of two different loads on postural sway, forward trunk lean, and pelvic girdle motion in United States Army Cadets. Twenty male Army Reserve Officers’ Training Corps Cadets participated in this study. Each participant performed the Modified Clinical Testing of Sensory Interaction (mCTSIB) Protocol and the Unilateral Stance (ULS) Protocol under three different rucksack load conditions (unloaded, 16.0 kg, and 20.5 kg loads). Mean postural sway velocity was recorded along with 2-D kinematics of the trunk in the sagittal plane and the pelvis in the frontal and sagittal planes. External loads of 16.0 kg (p \u3c 0.001) and 20.5 kg (p ≤ 0.003) significantly increased mean sway velocity by 16% to 52% depending on stance and visual condition, but did not produce significant changes in trunk and pelvic kinematics

    Impact of Two Types of Fitness Programs on Soldier Physical Fitness

This study compared an expert supervised, fully resourced physical training (PT) program compared to a traditional physical training PT plan on Army Officer Candidate School (OSC) soldier fitness outcomes. This retrospective cohort study compared 228 OCS soldiers (179 male [26.74±3.78 years] and 49 female [26.55±4.18 years]) in two companies for 12 weeks. One company participated in a fully resourced PT program designed by fitness experts to improve overall fitness and mobility (TAP-C). One company participated in traditional physical training designed to excel on the Army combat fitness test (ACFT, includes deadlift, power throw, push up, sprint-drag-carry, core strength, run) developed and led by OCS soldiers with standard resources. We assessed performance on the ACFT events, and grip strength, standing broad jump, overhead squat, and 90/90 switch assessment. Analysis of covariance was used to compare main effects of company on ACFT measures, controlling for covariates of pretest score differences and sex. Results included a significant effect of group on ACFT performance (N=228), F(1, 223) = 12.8, p\u3c0.001 and on performance of five of the six ACFT events: MDL, F(1, 223) = 5.44, p = 0.021; HRP, F(1, 223) = 11.67, p \u3c 0.001; SDC, F(1, 223) = 20.06, p \u3c 0.001; LTK, F(1, 223) = 16.95, p \u3c 0.001; and 2MR, F(1, 223) = 23.76, p \u3c 0.001. The traditional company performed significantly better on ACFT muscular, anerobic and aerobic endurance focused events; the TAP-C company performed significantly better on muscular strength/explosive power events and mobility assessments

    Massage Therapy Produces Short-term Improvements in Balance, Neurological, and Cardiovascular Measures in Older Persons

    Background: Falls are the primary cause of accidental death in older persons, producing increased morbidity, decreased independence, and billions in medical costs annually. Massage therapy (MT) may produce adaptations that decrease risk of falling. If MT can improve stability in older persons, it may provide a new intervention for this issue. Purpose: Determine the acute effects of a 60-minute MT treatment on static and functional balance, neurological measures, heart rate, and blood pressure in healthy, older individuals. Setting: Laboratory Research Design: A 2 by 4 (treatment by time) mixed factorial experimental design for the cardiovascular and postural control variables; independent variables were treatment with two levels (control, MT) and time with four levels (pretreatment baseline, immediate post-treatment, 20-minute post-treatment, 60-minute post-treatment). Neurological measures utilized a 2 by 2 mixed design, with testing conducted pre- and 60-minutes post-treatment. Participants: Thirty-five healthy, older volunteers (19 male and 16 female; ages 62.9 ± 4.6). Intervention: A 60-minute full-body therapeutic massage. The control group rested quietly in the treatment room. Main Outcome Measures: Static (double-legged) and functional (single-legged) postural control with eyes-open and eyes-closed; Hoffmann-reflex measures; heart rate, and systolic and diastolic blood pressure. Results: MT significantly decreased rectangular displacement area in both the eyes-open and eyesclosed, double-legged stance conditions (p < 0.05); displacement velocity in both eyes-open conditions (p < .05); and systolic and diastolic blood pressure (p < .05), while increasing heart rate (p < .05). MT also significantly lowered Hmax/Mmax ratios compared to controls (p = .002). Decreased Hmax/Mmax measures were correlated to improved stability. Conclusions: A single, 60-minute, full-body massage therapy treatment was shown to have a stabilizing effect on measures of static and dynamic balance and physiological factors related to stability in older adults. MT should be investigated as a potential intervention to decrease falls in older individuals

    Six Weeks of Massage Therapy Produces Changes in Balance, Neurological and Cardiovascular Measures in Older Persons

    Objectives: Falls in older adults represent a primary cause of decreased mobility and independence, increased morbidity, and accidental death. Research and clinical reports indicate that therapeutic massage (TM) may positively influence suggested causative factors. The second in a two-part study, this project assessed the effects of six weeks of TM treatment on balance, nervous system, and cardiovascular measures in older adults. Design: A randomized controlled trial assessed the effects of six weekly 60-minute sessions of TM on balance, cardiovascular, and nervous system measures. Thirty-five volunteers (19 male and 16 female; ages 62.9 ± 4.6) were randomly assigned to relaxation control or TM groups. A 2 × 4 [treatment condition X time (week 1 and 6)] mixed factorial experimental design was utilized for cardiovascular/balance variables assessed at pretreatment baseline, immediate post-treatment, and 20- and 60-minutes post-treatment; nervous system measures were assessed only at pretreatment and at 60-minute follow-up (2 × 2 mixed design). Longterm benefits were assessed by comparing the TM and control groups on pretreatment baseline measures at week six and a follow-up assessment at week seven (2 × 3 mixed design). Setting: Laboratory Intervention: Six weekly 60-minute, full-body TM. Outcome Measures: Postural control/cardiovascular measures were assessed weeks one, six, and seven; pretreatment and immediate, 20- and 60-minutes post-treatment. Motoneuron pool excitability was assessed pretreatment and 60 minutes post-treatment. Results: The TM group showed significant differences relative to controls in cardiovascular and displacement area/velocity after the week six session, with decreasing blood pressure and increasing stability over time from immediate post-TM to 60 minutes post-TM. The TM group revealed lower H-max/M-max ratios 60-minutes post-treatment. Long-term differences between the groups were detected at week seven in displacement area/velocity and systolic blood pressure. Conclusions: Results suggest six weeks of TM resulted in immediate and long-term improvements in postural stability and blood pressure, compared to a controlled condition

    Developing, Maintaining, and Using a Body of Knowledge for the Massage Therapy Profession

    Background: The diverse field of massage therapy has lacked a formal body of knowledge to serve as a practice and educational foundation and to guide future development. This deficit has hampered the growth of the profession and its acceptance and recognition by the medical and allied health care community. Purpose: To provide massage therapists, bodyworkers, physicians, educators, and associated allied health care professionals in the United States with a description of the purpose and development of the massage therapy body of knowledge (MTBOK) and recommendations for its future development and utilization. Methods: Professional groups in the massage therapy community came together and established a task force to develop a body of knowledge for the profession. Five groups became the stewards for this effort. A nationwide search produced a task force of eight volunteers from diverse areas of the profession charged with the responsibility of researching and developing the MTBOK document. Review of documents, curricula, state laws and regulations, certification exam content, interviews, and public comment resulted in the development of the MTBOK. During development multiple opportunities for comment and discussion by stakeholders (public) were provided in an effort to create a professional consensus. Results: The resulting MTBOK document establishes professional descriptions of the field; scope of practice; knowledge, skills, and abilities for entry-level massage therapists; and definitions for terminology to insure standardization, in order to provide a foundation for future discussion and growth. Conclusions: The MTBOK fulfills the goal for which it was developed, to serve as a foundation for the growth and development of the massage therapy profession as a whole. A living document, it should continue to evolve and grow with the profession. Maintenance and continued stewardship of this document by the massage therapy community is vital for continued professional progress

    Vertical and Side-Alternating Whole Body Vibration Platform Parameters Influence Lower Extremity Blood Flow and Muscle Oxygenation

    This study directly compared blood flow and oxygenation during six treatment parameters used with vertical and side alternating whole body vibration (WBV). Twenty-seven healthy adults were randomized into the vertical or side-alternating (vibration type) WBV group. Participants completed three WBV sessions a week apart, 5 sets of 1 min on/off, at 3 conditions (Vertical: 30 Hz and 4 mm, 40 Hz and 2 mm, 45 Hz and 4 mm; Side-alternating: 10 Hz and 4 mm, 18 Hz and 3 mm and 26 Hz and 2 mm). Blood flow velocity and popliteal artery diameter, muscle oxygenation, skin temperature, heart rate and blood pressure were assessed. Muscle oxygenation was significantly increased for all vibration frequencies and types following two minutes of WBV (14.78%, p = 0.02) and continued until immediately after the cessation of WBV (24.7%, p p p < 0.001) regardless of frequency and vibration type. Side-alternating and vertical WBV increased muscle oxygenation and heart rate in healthy participants completing an isometric squat. Muscle oxygenation was not increased until the second vibration set indicating the amount of time spent on the platform may have a significant effect on increases in blood flow
