Glycation. A study about regeneration

is characteristic for persons who are suffering for insipid diabetes, but also it is common in cases of a
high level of blood glucose. With reference to organism different functions degeneration, including a
bad angiogenesis, caused by glycation, it was purposed to observe how a high sugar alimentation would
influence the time of regeneration in an animal organism.
Materials and methods: For this study, was taken 40 mice and separated in 6 groups, I- 10 mice,
about 1.5 years old, high sugar diet, II- 10 mice, about 1.5 years, ordinary food, III- 5 mice, about 7
month old, high sugar diet, IV- 6 mice, about 7 month old, ordinary food, V- 3 mice, about 8 month old,
high sugar diet, VI- 6 mice, about 6 month old, high sugar diet. At 10-th day, a small incision on lower
limb was did on each mouse, after, it was observed the time of regeneration in each group. As food was
served: in groups with ordinary food, wheat, bread, carrot, beet; in groups with high sugar diet, wheat,
bread, carrot, beet, sugar and different sweets.
Discussion results: In first days of experiment, it was observed that groups of mice, which had
a ordinary diet were more active, they ran and played more than groups with a high sugar diet.Also it
was determined that groups of mice with high sugar diet like vegetables more than groups without sugar
supplement. After incisions this processes also was common. Analyzing regeneration, it may be said
that, in first days after incisions it was observed that in groups of elder mice and with ordinary
food,animals felt better, and regeneration had a higher speed than group with a high sugar diet. Anyway
at the final of experiment their results in regeneration was approximatively equal. In younger groups in
firs days also was present this phenomenon, but it continued, and in the end groups with ordinary diet
had results better with about 1-2 days than groups with high sugar diet. Also it was noticed a strange
thing, mice with high sugar diet had a strange fur, like it was wet or something like that.
Conclusion: In younger mice, the speed of regeneration is higher when alimentation is ordinary
than when alimentation is rich in sugar, in elder mice the speed of regeneration is approximatively equal.
Remain to demonstrate this not only through subjective methods, but also through objective like
histochemical methods
Sindromul Wiskott-Aldrich la copii - particularități de diagnostic
Summary.
Wiskott-Aldrich syndrome is a form of primary immune deficiency combined
with thrombocytopenia and for a long time it was thought it is a single one with such
symptoms, but now phenocopies are known. Another problem with this syndrome is a
common problem of all primary immune deficiencies — majority of medical workers
know very little about this group of diseases what leads to a very long times from the
debute of disease to the specific diagnosis. Because of that, we shortly describe criteria
for Wiskott-Aldrich and rekated disorders diagnostic and how we are performing
molecular genetic diagnostic of it.Rezumat.
Sindromul Wiskott-Aldrich este o formă de imunodeficiență primară combinată
cu trombocitopenie și multă vreme s-a crezut că este una singură cu astfel de simptome,
dar acum se cunosc fenocopii. O altă problemă cu acest sindrom, comună tuturor
deficiențelor imune primare - majoritatea lucrătorilor medicali știu foarte puțin despre
acest grup de boli, ceea ce duce la un timp foarte lung de la debutul bolii până la
diagnosticul specific. Din acest motiv, descriem pe scurt criteriile de diagnosticare a tulburărilor Wiskott-Aldrich și rekated și modul în care efectuăm diagnosticul genetic
molecular al acesteia
Проблемы диагностики митохондриальных нарушений: описание клинических случаев
Introducere. Maladiile mitocondriale sunt cele mai frecvente tulburări neurometabolice moștenite, ce pot apărea
la orice vârstă și sunt provocate de mutații ale genelor din ADN-ul mitocondrial (ADNmt), dar și ADN-ul nuclear
(ADNn) care codifică proteinele mitocondriale structurale sau cele implicate în funcția mitocondrială.
Rapoarte de caz. Primul caz este cel al unui băiat, născut la termenul de 40 sa, prin operație cezariană urgentă din
cauza insuficienței secundare a forțelor de contracție cu greutatea de 2900 g, scor Apgar 6/7, în cuplu non-consangvin. Având în vedere prezența acidozei metabolice profunde, hiperalaninemie, tablou clinic manifestat prin convulsii, decompensare metabolică, hepatomegalie, deteriorarea bruscă și progresivă a stării de sănătate s-a suspectat o
eroare înnăscută de metabolism, cu predilecție o patologie mitocondrială. Pe parcursul internării starea pacientului
s-a complicat iar în pofida măsurilor de resuscitare, s-a constatat moarte clinică a nou-născutului. Post-mortem a
fost efectuată secvențierea Sanger completă a genomului mitocondrial, însă nu s-a denotat nici o mutație patologică
la nivelul ADNmt. Pacientul 2, se prezintă primar la vârsta de 15 ani, copil I, din cuplu non-consangvin, născut
la termen cu acuze la pierdere bruscă a vederii la ochiul drept de la vârsta de 14 ani, iar peste 2-3 săptâmâni și la
ochiul stâng, dureri oculare la privier, nistagm, toleranță scăzută la efort fizic, fatigabilitate, scotom centra relative
la ochiul stâng și scotom central absolut la ochiul drept. În rezultatul secvențierii Sanger a fost identificată mutația
m.11778G>A, sugestivă pentru Neuropatia optică ereditară Leber (LHON).
Discuții. În cele două cazuri ilustrate mai sus, am dorit să evidențiem că diagnosticarea patologiei mitocondriale
primare este dificilă și chiar și atunci când tabloul clinic și investigațiile biochimice sugerează prezența certă a unei
boli mitocondriale, diagnosticul molecular genetic este cel care face diferența.
Concluzii. Având în vedere că maladiile mitocondriale sunt asociate cu diverse manifestări și simptome clinice
eterogene, subliniem importanța unei abordări integrative multidisciplinare la nivel clinic, biochimic, imagistic
și genetic pentru diagnosticul, managementul și tratamentul pacienților cu simptomatologie sugestivă maladiilor
mitocondriale.Введение: Митохондриальные заболевания являются наиболее распространенными наследственными
нейрометаболическими нарушениями, которые могут появиться в любом возрасте и вызваны мутациями
в генах митохондриальной ДНК (мтДНК), а также ядерной ДНК (яДНК), которые кодируют структурные
митохондриальные белки или белки, участвующие в митохондриальной функции.
Описание клинических случаев: Первый случай – мальчик, рожденный в срок 40 недель путем экстренного
кесарева сечения по поводу вторичной недостаточности сократительной способности матки, массой 2900
г, оценка по шкале Апгар 6/7, у некровнородственной пары. Учитывая наличие глубокого метаболического
ацидоза, гипераланинемии, клинической картины, проявляющейся судорогами, метаболической
декомпенсацией, гепатомегалией, внезапным и прогрессирующим ухудшением самочувствия,
заподозрили врожденное нарушение метаболизма, преимущественно митохондриальную патологию.
При госпитализации состояние больного осложнилось и, несмотря на проведенные реанимационные мероприятия, констатирована клиническая смерть новорожденного. Было проведено посмертное полное
секвенирование митохондриального генома по Сэнгеру, но патологических мутаций мтДНК обнаружено
не было. Пациент 2, впервые обратился в возрасте 15 лет, первый ребенок от некровнородственной пары,
родился в срок, с обвинениями в внезапной потере зрения на правый глаз с 14 лет, через 2-3 нед. также в
левом глазу, боль в глазу при взгляде, нистагм, низкая переносимость физической нагрузки, повышенная
утомляемость, относительная центральная скотома левого глаза и абсолютная центральная скотома
правого глаза. В результате секвенирования по Сэнгеру была идентифицирована мутация m.11778G>A,
что свидетельствует о наследственной атрофииb зрительных нервов Лебера (LHON).
Обсуждения: В двух приведенных выше случаях мы хотели подчеркнуть, что диагностика первичной
митохондриальной патологии затруднена, и даже когда клиническая картина и биохимические
исследования указывают на определенное присутствие митохондриального заболевания, решающую роль
играет молекулярно-генетический диагноз.
Заключение: Учитывая, что митохондриальные заболевания связаны с различными гетерогенными
клиническими проявлениями и симптомами, мы подчеркиваем важность междисциплинарного
интегративного подхода на клиническом, биохимическом, инструментальном и генетическом уровне
для диагностики, ведения и лечения пациентов с симптоматикой, указывающей на митохондриальные
заболевания
Достижения молекулярно-генетической диагностики в Институте Матери и Ребенка за 40 лет
The first group of specialists in molecular genetics was created in frame of the department for hereditary pathologies of the Scientific Research Institute of Mother and Child healthcare in 1989-1990. First molecular genetics tests
in Moldova (since 1992) were realized to detect mutations in monogenic pathologies – Duchenne myodystrophy
and hemophilia A and B, phenylketonuria and cystic fibrosis, spinal muscular atrophy. In frame of the National
Center of Reproductive Health and Medical Genetics, the laboratory of Human Molecular Genetics appeared in
2009. From 2010, there started the development of candidate genes allelic variants detection methods. Hereditary
predisposition to frequent chronic diseases is determined by various genes, target ones being: MTHFR, MTRR,
MTR, CBS, FV, FII, FXIII, VKORCI, GpIIIaL33P, PAI-1, GSTM, GSTT, GSTP, ACE, eNOS, ApoB, ApoE, HLA
DQA1 and DQB1. Currently, the laboratory can perform molecular diagnosis for the following diseases: PKU
(Phenylketonuria), CFTR (Cystic Fibrosis), AZF (azospermia), GALT (galactosemia), MECP2 (Rett Syndrome),
ALDOB (fructosemia), WD (The Wilson), AKU (Alkaptonuria), DMD (Duchenne Muscular Dystrophy), Celiac
Disease, mitochondrial diseases (MELAS, Leigh, LHON, NARP and MERRF Syndromes), SMA (Spinal muscular
atrophy), CLCN1 (Thomsen mytonia), SCA (Friedreich’s ataxia), FMR1 (Fragile X syndrome), AFF2 (X-efra syndrome), PMP22 (CharcoMarie Touth 1a), AZF (azospermia).
Research investigations were supported through the governmental grants, provided through ASM [SCREENGEN,
Cipher: 20.800009.8007.22, Contract 22-PS, 03.01.2022)] , and international grants provided by MRDA, BMBF,
DAAD and other funds. This support contributed to the skill development and experience gain in leading laboratories in Europe and USA.Первая молекулярно-генетическая группа в рамках научного отдела наследственных патологий НИИ
Охраны Здоровья Матери и Ребёнка была создана в 1998-1990 гг. Первые молекулярно-генетические
анализы (1992г.), проводимые в Молдове, были направлены на идентификацию мутаций частых моногенных
патологиях – миодистрофии Дюшенна и гемофилии А и Б, фенилкетонурии и муковисцидоза, спинальной
мышечной атрофии. В 2009 году была организована научная лаборатория Молекулярной Генетики
Человека как самостоятельная единица в рамках Национального Центра Репродуктивного Здоровья
и Медицинской Генетики. С 2010 года в лаборатории начаты разработки ДНК методов по определению
аллельных вариантов генов-кандидатов, ответственных за наследственную предрасположенность
человека к частым хроническим заболеваниям: MTHFR, MTRR, MTR, CBS, FV, FII, FXIII, VKORCI, GpI-IIaL33P, PAI-1, GSTM, GSTT, GSTP, ACE, eNOS, ApoB, ApoE, HLA DQA1 и DQB1. На данный момент в
лаборатории возможно проведение молекулярно-генетической диагностики на следующие патологии:
фенилкетонурия, муковисцидоз, галактоземия, фруктоземия, алкаптонурия, Синдром Ретта, болезнь
Вильсона, Миодистрофия Дюшенна, спинальная амиотрофия, миотония Томсона, ряд СЦА, Синдром
Мартена-Белла, Синдром ДиДжоржа, синдром Вистота-Олдрижа, митохондриальные болезни (Синдром
MELAS, Ли, Лебера, NARP, MERRF).
Научные исследования лаборатории поддержаны государственными контрактами [SCREENGEN, Cipher:
20.800009.8007.22, Contract 22-PS, 03.01.2022] и грантами международных фондов MRDA, BMBF, DAAD и

Distrofia musculară Duchenne: protocol clinic național (ediția I) PCN-403
Acest protocol a fost elaborat de grupul de lucru al Ministerului Sănătății al Republicii Moldova
(MS RM), constituit din specialiștii geneticieni, specialiști din Clinică Pediatrie cadrul IMSP
Institutului Mamei și Copilului (Clinica Neurologie, Cardiologie, Pulmonologie), specialiști din
Departamentul Pediatrie al USMF „Nicolae Testemițanu”.
Protocolul Clinic Național (PCN) este elaborat în conformitate cu ghidurile internaționale actuale
privind distrofia musculară Duchenne (DMD), cu elucidarea particularităților regionale de
management al patologiei la diverse etape de acordare a asistenței medicale. Acest protocol va servi
drept bază pentru elaborarea protocoalelor instituționale. La recomandarea MS RM, pentru
monitorizarea protocoalelor instituționale pot fi folosite formulare suplimentare, care nu sunt
incluse în protocolul clinic național
MOLECULAR TESTING STRATEGIES OF MITOCHONDRIAL DNA DISORDERS
Universitatea de Stat de Medicină şi Farmacie „Nicolae Testemiţanu”, Chişinău, Republica MoldovaIntroducere. Maladiile mitocondriale cauzate de mutațiile ADN-ului mitocondrial (ADNmt) sunt cel mai frecvent grup de tulburări metabolice ereditare, cauzate de defecte biochimice al lanțului respirator și al sistemului de fosforilare oxidativă. Diagnosticul acestor tulburări poate fi o provocare din cauza eterogenității clinice și a complexității genetice. Scopul lucrării. Prezentarea strategiilor actuale de diagnostic molecular-genetic a pacienților suspecți pentru tulburări mitocondriale utilizate de Laboratorul de Genetică Moleculară Umană al Institutului Mamei și Copilului. Material și metode. Prezentul studiu a fost realizat pe un grup de 43 pacienți cu fenotipul caracteristic bolilor mitocondriale. Strategia pentru diagnosticul molecular-genetic al tulburărilor mitocondriale cauzate de mutațiile ADNmt a implicat efectuarea analizei qPCR-HRM (quantitative PCR-High Resolution Melting) în scopul testării pacienților pentru cele mai frecvente mutații patogene punctiforme, urmată de secvențierea Sanger a ADNmt. Rezultate. În urma testării a 43 pacienți pentru 11 mutații punctiforme patogene comune ale ADNmt asociate cu boli mitocondriale prin tehnica qPCR-HRM, au fost identificați 3 pacienți (7%) cu mutațiile patogene m.8344 A>G, m.8993 T>G și m.11778 G>A. Conform strategiei de diagnostic molecular-genetic, în cazul în care nu au fost identificate abnormalități la nivelul ADNmt prin tehnica qPCR-HRM, pasul următor a implicat analiza spectrului de variante genetice prin secvențierea Sanger a ADNmt. Secvențierea Sanger a 21 de gene mitocondriale a fost realizată la 29 pacienți, iar în rezultat, la 12 pacienți au fost identificate mutații patogene sau potențial patogene asociate cu patologia mitocondrială (41%). Concluzii. În lotul de 43 de pacienți investigați, s-au putut identifica variante patogene și potențial patogene asociate cu fenotipul pacienților în 34% din cazuri.Background. Mitochondrial DNA (mtDNA) diseases are the commonest group of heritable metabolic disorders, caused by a biochemical defect of the respiratory chain and oxidative phosphorylation system. Diagnosis of these disorders can be challenging due to the heterogeneity of clinical features and the complexity of the underlying genetic abnormalities. Objective of the study is to present the current molecular diagnostic strategies in patients suspected for mtDNA disorders used by the Laboratory of Human Molecular Genetics of Institute of Mother and Child. Material and methods. We analyzed mtDNA from 43 patients with common clinical features of mitochondrial disorders. The strategy for the molecular-genetic diagnosis of mitochondrial disorders caused by mtDNA mutations involved performing qPCR-HRM (quantitative PCR-High Resolution Melting) analysis in order to test patients for the most common pathogenic point mutations, followed by mtDNA Sanger sequencing. Results. After testing 43 patients for 11 common pathogenic mtDNA point mutations associated with mitochondrial diseases by the qPCR-HRM technique, 3 patients with the pathogenic mutations m.8344 A>G, m.8993 T>G and m.11778 G>A were identified (7%). Our molecular genetic diagnosis strategy provides that if no abnormalities are identified in the mtDNA analyzed by the qPCR-HRM method, the subsequent step involves the analysis of the spectrum of mtDNA genetic variants through Sanger sequencing. The Sanger sequencing technique of 21 mtDNA genes was performed in 29 patients, and as a result, pathogenic or potentially pathogenic mutations associated with mitochondrial pathology were identified in 12 patients (41%). Conclusion. In the group of 43 investigated patients, it was possible to identify pathogenic and potentially pathogenic variants associated with the patients’ phenotype in 34% of cases
