Which standard biomarkers are useful for the evaluation of myocardial injury after pulmonary vein isolation with cryoballoon?
Wstęp: W dotychczas opublikowanych badaniach dotyczących oceny uszkodzenia mięśnia sercowego po zabiegach ablacji
oznaczano aktywność kinazy kreatynowej (CK), izoenzymu sercowego CK (CK-MB) i stężenie sercowych troponin I (cTnI)
oraz T (cTnT). Dokonano pomiarów koncentracji kardio-specyficznych biomarkerów jako odpowiednika masy uszkodzonych
komórek mięśnia sercowego.
Cel: Celem pracy było wyjaśnienie, który ze standardowo dostępnych biomarkerów jest użyteczny w ocenie uszkodzenia
komórek mięśnia sercowego po krio-balonowej izolacji żył płucnych (CBA).
Metody: U 33 pacjentów z migotaniem przedsionków wykonano CBA. Próbki krwi pobrano przed CBA oraz w 1., 6. oraz
24. godzinie po CBA. Analizie poddano CK, CK-MB i cTnI.
Wyniki: W próbkach pobranych po CBA zaobserwowano istotny wzrost koncentracji wszystkich badanych biomarkerów
w stosunku do poziomu wyjściowego. Maksymalny wzrost zanotowano w 6. godzinie; CK, CK-MB i cTnI osiągnęły wartości
patologiczne u, odpowiednio, 94%, 100% i 100% pacjentów. Maksymalne wartości CK i CK-MB korelowały (p < 0.05)
z medianą temperatury osiągniętej w czasie CBA.
Wnioski: Okazało się, że CK-MB jest najlepszym standardowym biomarkerem do oceny uszkodzenia mięśnia sercowego po
CBA. Sercowa troponina I może być użyteczna jako dodatkowy parametr oceny uszkodzenia po CBA.

(cTnI) and T (cTnT) to evaluate myocardial cells injury after ablation. We applied measurements of the blood concentration
of cardio-specific biomarkers as surrogates for the injured cell mass.
Aim: To clarify which of the standard biomarkers are useful in the evaluation and quantification of lesions produced by
cryoballoon ablation (CBA) during pulmonary vein isolation.
Methods: The CBA was performed in 33 patients with atrial fibrillation. Blood samples were obtained before CBA and one,
six, and 24 h after CBA. We analysed CK, CK-MB and cTnI.
Results: A significant increase of all biomarkers was observed at each hour of collection as compared to the baseline measurement.
Maximum median peak levels occurred at 6 h. Pathological values of CK, CK-MB and cTnI were observed in 94%,
100% and 100% of patients, respectively. Both maximum CK and CK-MB values correlated with median temperature
(p < 0.05) reached during CBA. Additionally, CK-MB correlated with total cryo-time (p < 0.03).
Conclusions: The CK-MB is the best biochemical marker for the evaluation of myocardial injury after CBA. The cTnI can be
useful as an additional parameter of myocardial injury after CBA.

015 Impact of discharge heart rate on 30 day mortality in patients with acute myocardial infarction
BackgroundIn patients with acute myocardial infarction (MI), beta-blockers are recommended and contribute to control the heart rate (HR). The factors associated with HR and outcome of patients discharged with HR>70 beats per minute (bpm) is poorly documented.MethodsProspective registry including patients with acute MI. Recorded variables corresponded to the CARDS dataset. The proportion of patients discharged with HR>70bpm was assessed. Multivariate regression was used to determine factors associated with HR at discharge and logistic regression was used to determine the prognostic value of high HR at discharge on 30 day mortality.ResultsAmong the 837 patients, 592 survived and were discharged with a beta blocker. Discharge HR was ≤70bpm in 61% when a betablocker was given and in 31% without beta blocker (p<0.001). Discharge HR was associated with older age, admission HR, admission systolic blood pressure, BNP level, diabetes, use of beta blockers and ACEI. Patients discharged with HR>70bpm (n=227, 39%) had a threefold higher mortality as compared with those with HR ≤70 (figure). Multivariate analysis showed that a HR ≥70 at discharge was an independent predictor of 30 day mortality on top of the GRACE risk score and use of beta blockers.ConclusionsAmong patients discharged after acute MI, those with HR>70 have higher 30 day mortality, independently of the GRACE risk score and use of beta blockers.Mortality by discharge heart rat
Does playing O.zen improve our well-being?
Stress is considered to be an individual’s response to different pressures and demands in a particular environment (Pottier et al., 2011). In an ever more demanding and stressful world, many options for people to learn how to relax and how to deal with a challenging lifestyle are becoming available. O.zen is a great alternative, since it is a ludic and not expensive device, which goal is to help people to relax with the help of breathing games that can easily be accomplished no matter what time of the day or context. The goal of this study was to test O.zen’s efficiency. In order to do so, neuropsychological tests were held throughout the experiment to 28 participants that were divided in experimental (played O.zen) and control (watched videos equivalent to O.zen) groups. Also, cortisol levels and participants’ skin conductance was measured. Both groups showed overall improvements, with participants feeling significantly less anxious after playing O.zen, and showing a lower level of skin conductance. Background: Radiofrequency (RF) current is used as a common energy source to perform pulmonary vein isolation (PVI) in
patients with atrial fibrillation. We applied measurements of the blood concentration of cTnI as a surrogate parameter for the
injured cell mass.
Aim: To clarify which parameters are major determinants of myocardial injury, estimated by cTnI, after PVI with RF ablation.
Methods: The study population consisted of 82 consecutive patients in whom PVI with RF ablation was performed. In 41 patients,
additional linear lesions (LL) were needed. Blood samples were obtained during venous puncture before a procedure
and a further one, six and 24 hours after ablation.
Results: Pathological cTnI values were observed in all patients in the first hour and further increased in time. The median of
peak cTnI value in the LL group was significantly (p < 0.05) higher than the respective value in patients without LL made:
1.16 (0.85;1.98) and 0.94 (0.65;1.14) ng/mL, respectively. Significantly higher cTnI values (p = 0.043) were observed in
patients who maintained sinus rhythm in long term follow-up.
Conclusions: The only independent predictor of myocardial injury after PVI with RF ablation, expressed as an increase in
cTnI level, is cumulative energy applied. The larger the myocardial injury, the greater the PVI effectiveness.Wstęp: Izolacja elektryczna ujścia żył płucnych (PVI), przy użyciu prądu o wysokiej częstotliwości (RF), jest coraz częściej
stosowaną metodą inwazyjnego leczenia pacjentów z migotaniem przedsionków (AF). W badaniu zmierzono koncentrację
troponiny I (cTnI) jako odpowiednika masy uszkodzonych komórek mięśnia sercowego.
Cel: Celem pracy było wyjaśnienie, jakie parametry są głównymi wyznacznikami uszkodzenia mięśnia sercowego po ablacji
RF PVI.
Metody: Badaniem objęto 82 kolejnych chorych z AF, u których wykonano ablację RF PVI. W 41 przypadkach, oprócz PVI,
przeprowadzono dodatkowe linie (LL) w dachu i cieśni mitralnej lewego przedsionka. Próbki krwi żylnej pobrano przed
procedurą oraz w 1. ,6. i 24. godzinie po zabiegu.
Wyniki: Patologiczne wartości cTnI obserwowano u wszystkich pacjentów, zarówno w 1. godzinie, jak i ich narastające
wartości w godzinach kolejnych. Mediana maksymalnych wartości cTnI była istotnie wyższa (p < 0,05) w grupie chorych z LL
w porównaniu z pacjentami bez LL, odpowiednio: 1,16 (0,85; 1,98) i 0,94 (0,65; 1,14) ng/ml. Istotnie wyższe wartości cTnI
(p = 0,043) zanotowano u chorych, którzy pozostawali na rytmie zatokowym w długim okresie obserwacji.
Wnioski: Jedynym niezależnym czynnikiem uszkodzenia mięśnia sercowego po ablacji RF PVI, wyrażonym jako wzrost
stężenia cTnI, jest łączna wartość zastosowanego prądu RF. Im większy jest uraz mięśnia sercowego, tym większa jest
skuteczność ablacji RF PVI
