    1/d1/d Expansion for kk-Core Percolation

    The physics of kk-core percolation pertains to those systems whose constituents require a minimum number of kk connections to each other in order to participate in any clustering phenomenon. Examples of such a phenomenon range from orientational ordering in solid ortho-para H2{\rm H}_2 mixtures to the onset of rigidity in bar-joint networks to dynamical arrest in glass-forming liquids. Unlike ordinary (k=1k=1) and biconnected (k=2k=2) percolation, the mean field k3k\ge3-core percolation transition is both continuous and discontinuous, i.e. there is a jump in the order parameter accompanied with a diverging length scale. To determine whether or not this hybrid transition survives in finite dimensions, we present a 1/d1/d expansion for kk-core percolation on the dd-dimensional hypercubic lattice. We show that to order 1/d31/d^3 the singularity in the order parameter and in the susceptibility occur at the same value of the occupation probability. This result suggests that the unusual hybrid nature of the mean field kk-core transition survives in high dimensions.Comment: 47 pages, 26 figures, revtex

    Long-term noninvasive ventilation in COPD:current evidence and future directions

    Introduction: Long-term noninvasive ventilation (NIV) is an established treatment for end-stage COPD patients suffering from chronic hypercapnic respiratory failure. This is reflected by its prominent position in national and international medical guidelines. Areas covered: In recent years, novel developments in technology such as auto-titrating machines and hybrid modes have emerged, and when combined with advances in information and communication technologies, these developments have served to improve the level of NIV-based care. Such progress has largely been instigated by the fact that healthcare systems are now confronted with an increase in the number of patients, which has led to the need for a change in current infrastructures. This article discusses the current practices and recent trends, and offers a glimpse into the future possibilities and requirements associated with this form of ventilation therapy. Expert opinion: Noninvasive ventilation is an established and increasingly used treatment option for patients with chronic hypercapnic COPD and those with persistent hypercapnia following acute hypercapnic lung failure. The main target is to augment alveolar hypoventilation by reducing PaCO2 to relieve symptoms. Nevertheless, when dealing with severely impaired patients, it appears necessary to switch the focus to patient-related outcomes such as health-related quality of life

    Transarterielle Chemoembolisation als Selektionsmarker für Patienten mit HCC vor LTX unter retrospektiver Zuhilfenahme histologischer Ergebnisse [meeting abstract]

    Einleitung: Für die meisten Patienten mit HCC ist die LTX die einzige kurative Behandlungsoption. Bei diesen Patienten scheint eine Kontrolle der Erkrankung durch lokale Verfahren im Intervall bis zur LTX zu erreichen zu sein. Als das beste Verfahren gilt die transarterielle Chemoembolisation (TACE). Die Effektivität ist jedoch umstritten. Möglicherweise kann sie aber Patienten startifizieren, die ein hohes Rezidivrisiko haben. Material und Methoden: Im Zeitraum zwischen 1995 und 2005 wurden n=27 Patienten mit HCC im Alter zwischen 22 und 69 Jahren transplantiert. Hiervon erhielten n=15 Patienten eine Vorbehandlung in Form einer alleinigen TACE oder kombiniert mit PEI [n=1] bzw. LITT [n=1]. Retrospektiv wurde das Gesamtüberleben sowie das „Event-free-survival“ (Rezidiv, Reinfektion und Tod) analysiert. Ergebnisse: Die mittlere Wartezeit betrug bei Patienten in der TACE-Gruppe 214 Tage, bei Patienten ohne Vorbehandlung 133 Tage. Bei einem mittleren Nachbeobachtungszeitraum von 1097 ± 1193 Tagen für TACE-Patienten und 1674 ± 966 Tagen für non-TACE-Patienten betrug das Überleben für Patienten, die mit TACE vorbehandelt wurden 83,3%, für Patienten, die keine TACE erhielten 86.7% (p=0,5693). Gleiches fand sich für das Event-free-survival (p=0,8823). Das Gesamtüberleben der Patienten, die auf der Warteliste einen Tumorprogress hatten lag bei 77%, während Patienten mit stabiler Tumorgröße oder Regredienz der Tumore ein Überleben von 93% aufwiesen (p=0,0153). Unter TACE-Behandlung zeigten 5/15 Patienten eine zunehmende Anzahl an Herden im histologischen Präparat verglichen mit der Ausgangsbildgebung. Nur bei einem Patienten zeigte sich der Progress der Erkrankung bereits in der präoperativen Bildgebung. Patienten mit einem Progress der Erkrankung hatten ein Gesamtüberleben von 60%, während Patienten mit „stable disease“ oder Rückgang der Herde ein Gesamtüberleben von 100% hatten (p=0,0180). Schlussfolgerung: Unseren Ergebnisse zufolge ist der Effekt der TACE als Bridgingverfahren auf das Überleben der Patienten fraglich. Allerdings scheint die TACE zur Riskostratifizierung geeignet zu sein. In unserem Patientenkollektiv hatten Patienten, die eine Progredienz der Erkrankung auf der Warteliste zeigten ein signifikant schlechteres Gesamtüberleben. Dies gilt auch bei ausschließlicher Betrachtung der Patienten mit TACE

    N-Acetylcysteine as modulator of the essential trace elements copper and zinc

    N-acetylcysteine (NAC) is a frequently prescribed drug and known for its metal chelating capability. However, to date it is not well characterized whether NAC intake affects the homeostasis of essential trace elements. As a precursor of glutathione (GSH), NAC also has the potential to modulate the cellular redox homeostasis. Thus, we aimed to analyze effects of acute and chronic NAC treatment on the homeostasis of copper (Cu) and zinc (Zn) and on the activity of the redox-sensitive transcription factor Nrf2. Cells were exposed to 1 mM NAC and were co-treated with 50 µM Cu or Zn. We showed that NAC treatment reduced the cellular concentration of Zn and Cu. In addition, NAC inhibited the Zn-induced Nrf2 activation and limited the concomitant upregulation of cellular GSH concentrations. In contrast, mice chronically received NAC via drinking water (1 g NAC/100 mL). Cu and Zn concentrations were decreased in liver and spleen. In the duodenum, NQO1, TXNRD, and SOD activities were upregulated by NAC. All of them can be induced by Nrf2, thus indicating a putative Nrf2 activation. Overall, NAC modulates the homeostasis of Cu and Zn both in vitro and in vivo and accordingly affects the cellular redox balance

Apresenta revisão de literatura sobre arquivos abertos e discute as questões de interoperabilidade e requisitos de organização dos conteúdos para o efetivo uso educacional. A pesquisa exploratória faz uma descrição das instituições que trabalham com recursos educacionais abertos e faz uma análise sobre a interoperabilidade e as possibilidades de uso em cursos a distância e presenciais. Apresenta os diversos tipos de licença e discute os requisitos de organização dos conteúdos para o efetivo uso educacional. Conclui, apresentando recomendações para a organização dos arquivos abertos para o uso na educação presencial e a distância

    Home noninvasive ventilatory support for patients with chronic obstructive pulmonary disease:patient selection and perspectives

    Long-term or home mechanical noninvasive ventilation (Home-NIV) has become a well-established form of therapy over the last few decades for chronic hypercapnic COPD patients in European countries. However, meta-analyses and clinical guidelines do not recommend Home-NIV for COPD patients on a routine basis. In particular, there is ongoing debate about Home-NIV in chronic hypercapnic COPD regarding the overall effects, the most favorable treatment strategy, the selection of eligible patients, and the time point at which it is prescribed. The current review focuses on specific aspects of patient selection and discusses the various scientific as well as clinical-guided perspectives on Home-NIV in patients suffering from chronic hypercapnic COPD. In addition, special attention will be given to the topic of ventilator settings and interfaces

    We study a class of subdivision invariant lattice models based on the gauge group ZpZ_{p}, with particular emphasis on the four dimensional example. This model is based upon the assignment of field variables to both the 11- and 22-dimensional simplices of the simplicial complex. The property of subdivision invariance is achieved when the coupling parameter is quantized and the field configurations are restricted to satisfy a type of mod-pp flatness condition.     Ecologically grown medicinal plants containing bioactive compounds hold great poten-tial as high-value niche crops for farmers. However, the way to grow these plants differs from traditional crops. Growing techniques, harvest methods and postharvest handling of the raw material plays a crucial role regarding the quality of the raw material that the farmers can offer. The purpose of a new research project financed by EU-Interreg IIIA programme is among other things to carry out production, harvest and processing experiments with plants containing bioactive plant compounds that hold a preventive effect toward diabetes II. One of the project´s goals is to draw up cultivation instructions for the primary producers to use when cultivating the plants in question. Examples of the plants that are being examined in the project are Goat´s Rue (Galega officinalis) and Fenugreek (Trigonella foenum-graecum)

    Obesity hypoventilation syndrome treated with non-invasive ventilation:Is a switch to CPAP therapy feasible?

    Background and objective: Obesity hypoventilation syndrome (OHS) can be treated with either continuous positive airway pressure (CPAP) or non-invasive ventilation (NIV) therapy; the device choice has important economic and operational implications. Methods: This multicentre interventional trial investigated the safety and short-term efficacy of switching stable OHS patients who were on successful NIV therapy for ≥3 months to CPAP therapy. Patients underwent an autotitrating CPAP night under polysomnography (PSG); if the ensuing parameters were acceptable, they were sent home on a fixed CPAP for a 4–6-week period. It was hypothesized that blood gas analysis, PSG parameters and lung function tests would remain unchanged. Results: A total of 42 OHS patients were recruited, of whom 37 patients were switched to CPAP therapy. All patients had a history of severe obstructive sleep apnoea syndrome; chronic obstructive pulmonary disease (COPD) (Global Initiative for Obstructive Lung Disease (GOLD) I/II) was present in 52%. Regarding the primary outcome, 30 of 42 patients (71%, 95% CI: 55–84%) maintained daytime partial pressure of carbon dioxide (PaCO2) levels ≤45 mm Hg after the home CPAP period. There was no further impairment in quality of life, sleep parameters or lung function. Interestingly, 24 patients (65%) preferred CPAP as their long-term therapy, despite the high pressure levels used (mean: 13.8 ± 1.8 mbar). After the CPAP period, 7 of 37 patients were categorized as CPAP failure, albeit only due to mild hypercapnia (mean: 47.9 ± 2.7 mm Hg). Conclusion: It is feasible to switch most stable OHS patients from NIV to CPAP therapy, a step that could significantly reduce health-related costs. The auto-adjusted CPAP device, used in combination with the analysis of the PSG and capnometry, is a valid titration method in OHS patients

    E_{7(7)} Duality, BPS Black-Hole Evolution and Fixed Scalars

    We study the general equations determining BPS Black Holes by using a Solvable Lie Algebra representation for the homogenous scalar manifold U/H of extended supergravity. In particular we focus on the N=8 case and we perform a general group theoretical analysis of the Killing spinor equation enforcing the BPS condition. Its solutions parametrize the U-duality orbits of BPS solutions that are characterized by having 40 of the 70 scalars fixed to constant values. These scalars belong to hypermultiplets in the N=2 decomposition of the N=8 theory. Indeed it is shown that those decompositions of the Solvable Lie algebra into appropriate subalgebras which are enforced by the existence of BPS black holes are the same that single out consistent truncations of the N=8 theory to intereacting theories with lower supersymmetry. As an exemplification of the method we consider the simplified case where the only non-zero fields are in the Cartan subalgebra H of Solv(U/H) and correspond to the radii of string toroidal compactification. Here we derive and solve the mixed system of first and second order non linear differential equations obeyed by the metric and by the scalar fields. So doing we retrieve the generating solutions of heterotic black holes with two charges.
