    Surgical Site Infection Reduction Through Nasal Decolonization Prior to Surgery

    Surgical site infections (SSIs) are the most common and expensive healthcare-acquired infection. The objective of this nurse-driven process improvement project was to implement a cost-effective and practical decolonization protocol to improve outcomes for high-risk veteran populations undergoing surgery at a large veteran’s hospital in the Pacific Northwest. Prior to the implementation, a decolonization protocol was used for cardiac and orthopedic preoperative patients. The process involved screening for methicillin-resistant Staphylococcus aureus (MRSA) colonization via nasal swab preoperatively and treating positive patients with mupirocin ointment to nares BID and chlorhexidine showers for five days prior to surgery. Adherence to the protocol was problematic and SSI rates increased in these populations. New evidence-based practice recommendations published in 2105 described a 50% reduction of SSI with screening for MRSA in the pre-operative clinic, followed by treating each preoperative patient with chlorhexidine washcloths, oral chlorhexidine rinse, and intranasal povidone-iodine. Starting May 15, 2017, all surgical patients now receive intranasal povidone-iodine treatment before surgery with the aim of reducing MRSA SSIs by 10% in one fiscal year (FY). For the past three years, the hospital has averaged 11 SSIs per year, and 5 SSIs for the current FY, all prior to initiation of the new evidence-based protocol

    Initial Plant Growth in Sand Mine Spoil Amended with Peat Moss and Fertilizer Under Greenhouse Conditions: Potential Species for Use in Reclamation

    The Great Lakes Basin exhibits the largest collection of freshwater sand dunes in the world. Sand dunes are ecologically important and support a unique assemblage of flora and fauna. Sand dunes are also economically valuable. However, when sand dunes are mined, soil quality is drastically reduced. Therefore, soil quality improvements followed by revegetation maybe necessary for successful reclamation. This study evaluates the germination and initial growth of 2 legume species, sundial lupine (Lupinus perennis) and Illinois bundleflower (Desmanthus illinoensis), and 2 warm-season grass species, Indian grass (Sorghastrum nutans) and little bluestem (Schizachyrium scoparium), in the presence of 2 soil amendments (inorganic fertilizer and sphagnum peat moss) added to spoil from a local sand mine. We sowed species in pots and propagated them under greenhouse conditions. Results indicate that sundial lupine and Illinois bundleflower exhibited the greatest germination and growth among species. Peat moss had the greatest overall impact on germination and growth while the addition of fertilizer positively affected initial growth. Based on these results, sundial lupine is recognized as a primary candidate for sand mine reclamation, while Illinois bundleflower is also recommended as an appropriate species for revegetation efforts. We recommend using soil amendments that are functionally equivalent to peat in increasing soil water holding capacity. We further suggest that fertilization may be accomplished by including legumes in plant species mixes used for revegetation. Results presented here may help to identify appropriate species and soil amendments for the reclamation of former sand mines or restoration of freshwater sand dunes

    The hippocampus and the flexible use and processing of language

    Fundamental to all human languages is an unlimited expressive capacity and creative flexibility that allow speakers to rapidly generate novel and complex utterances. In turn, listeners interpret language “on-line,” incrementally integrating multiple sources of information as words unfold over time. A challenge for theories of language processing has been to understand how speakers and listeners generate, gather, integrate, and maintain representations in service of language processing. We propose that many of the processes by which we use language place high demands on and receive contributions from the hippocampal declarative memory system. The hippocampal declarative memory system is long known to support relational binding and representational flexibility. Recent findings demonstrate that these same functions are engaged during the real-time processes that support behavior in-the-moment. Such findings point to the hippocampus as a potentially key contributor to cognitive functions that require on-line integration of multiple sources of information, such as on-line language processing. Evidence supporting this view comes from findings that individuals with hippocampal amnesia show deficits in the use of language flexibly and on-line. We conclude that the relational binding and representational flexibility afforded by the hippocampal declarative memory system positions the hippocampus as a key contributor to language use and processing

In August 2015, the University of Vermont (UVM) adopted a tobacco-free policy that bans all forms of tobacco use on university property.     Evidence of the Boyer Model of Scholarship in Counselor Education

    This study delineated how the Boyer (1990) model of scholarship has been implemented in counselor education. The taxonomy of scholarship was delineated: Scholarship of Discovery, Scholarship of Teaching, Scholarship of Application, and Scholarship of Integration. The Journal of Counseling and Development and Counselor Education and Supervision were examined from 2000 to 2008. Examples of scholarship from each journal were identified. Evidence of the Boyer model in premier counseling journals suggests a consistency of approach between this model and scholarly activities in counselor education. An expanded view of scholarship may have positive implications for scholarly activity in graduate education and pretenured faculty

    In vitro transactivation of Bacillus subtilis RNase P RNA

    AbstractDeletion of the ‘signature’ PL5.1 stem-loop structure of a Type II RNase P RNA diminished its catalytic activity. Addition of PL5.1 in trans increased catalytic efficiency (kcat/KM) rather than kcat. Transactivation was due to the binding of a single PL5.1 species per ribozyme with an apparent Kd near 600 nM. The results are consistent with the role of PL5.1 being to position the substrate near the active site of the ribozyme, and with the hypothesis that ribozymes can evolve by accretion of preformed smaller structures
