Medical science says that ischemia is a common complication in Diabetes, especially in the lower limb but this is a case presenting ischemic changes in upper limb which is unique and also in such type of acute ischemia amputation is the only choice of treatment to save the life of patient from septicaemia. Such a rare and interesting case was taken at Sri Sri Ayurveda Hospital and handled by proper Ayurvedic line of treatment by which circulation is re-established which saved both limb and life of patient. A patient by name Sundaramma, 62yr Female, visited Sri Sri Ayurveda Hospital on 29/2/2016 with a diagnosis of Right upper limb Ischemia with Diabetes Mellitus and Active TB. The main symptoms were severe pain and burning sensation in Right hand and forearm, discolouration of fingers with ulcers, Bluish black discolouration over right thumb, necrosis, pus dischargeand stiffness, associated with difficulty in movement of right arm, shoulder joint, elbow joint, palm and fingers. Diagnosed as Right Upper Limb Ischemia with Diabetes Mellitus and active TB. Therapeutic interventions like Right upper limb Doppler and CT angiogram were done. Such a complicated case was taken at Sri Sri Ayurveda Hospital, and carefully handled by Ayurvedic methods of management, which was ended up by preventing amputation of the limb and also achieving the re-establishment of arterial circulatio

Management of PCOD - A Case Report

Woman being the root cause of progeny, utmost care should be given to protect her from any ailments that affect her motherhood. PCOD is one of the conditions affecting this unique capacity of woman. The disorder is probably the most common hormonal abnormality in women of reproductive age and certainly a leading cause of infertility. In Ayurvedic classics there is no direct mentioning of this disease rather, symptoms are found under various diseased conditions at various references i.e. menstrual irregularities are described under the broad heading of Ashtoartavadushti, while the description of infertility due to Anovulation is scattered. Pushpaghni, Jataharini mentioned in Kashyapa Samhita, Revati Kalpadhyaya bears similitude to the symptom of hyperandrogenism. But features of metabolic dysfunction and polycystic ovarian morphology are not evident from any description

Adaptive Learning Based Whale Optimization and Convolutional Neural Network Algorithm for Distributed Denial of Service Attack Detection in Software Defined Network Environment

SDNs (Software Defined Networks) have emerged as a game-changing network concept. It can fulfill the ever-increasing needs of future networks and is increasingly being employed in data centres and operator networks. It does, however, confront certain fundamental security concerns, such as DDoS (Distributed Denial of Service) assaults. To address the aforementioned concerns, the ALWO+CNN method, which combines ALWOs (Adaptive Learning based Whale Optimizations) with CNNs (Convolution Neural Networks), is suggested in this paper. Initially, preprocessing is performed using the KMC (K-Means Clustering) algorithm, which is used to significantly reduce noise data. The preprocessed data is then used in the feature selection process, which is carried out by ALWOs. Its purpose is to pick out important and superfluous characteristics from the dataset. It enhances DDoS classification accuracy by using the best algorithms.  The selected characteristics are then used in the classification step, where CNNs are used to identify and categorize DDoS assaults efficiently. Finally, the ALWO+CNN algorithm is used to leverage the rate and asymmetry properties of the flows in order to detect suspicious flows specified by the detection trigger mechanism. The controller will next take the necessary steps to defend against DDoS assaults. The ALWO+CNN algorithm greatly improves detection accuracy and efficiency, as well as preventing DDoS assaults on SDNs. Based on the experimental results, it was determined that the suggested ALWO+CNN method outperforms current algorithms in terms of better accuracies, precisions, recalls, f-measures, and computational complexities

A Timely Referral to Palliative Care Team Improves Quality of Life

    No full text
In the trajectory of disease progress and treatment plan, patients and the family members are confronted with challenging situations like unsurmountable physical distress, inadequate coping patterns, unanswered spiritual issues in the background of serious threat to very existence of life leads to a debilitating Quality of life. The Palliative Care team approach addresses all the issues and also sees the patient to go through the protocols of Palliative care management as well as Oncological treatment plan. Further, this fecilitates a smooth transition from the hospital to home and hospice care. Various studies conducted globally revealed that patients received palliative care intervention along with oncological treatments had higher scores of Quality of life compared to patients received onlyoncology care alone. This article discusses the various factors contributing to late referrals to palliative care team and also care giver’s views pertaining to need for early referral. Timely referral to palliative care minimises the patient’s and care giver’s distress, ensures modest Quality of life and appropriate measures at the end of life care

Quality of life in cancer patients with disfigurement due to cancer and its treatments

Aim: The aim of this study was to evaluate the effect of disfigurement due to cancer and its treatments on quality of life. Materials andMethods: A total of 120 patients from the inpatient/outpatient department of oncology who had undergone various forms of treatment for cancer were included in this study. The WHOQuality of Life BREF (WHOQL-BREF) version was administered to the patients to assess their quality of life. Results: Patients′ overall quality of life score ranged from 34 to 79 with an average of 53.18 (SD 11.94) and a large number of patients had scored from 40 to 54 on the WHOQOL-BREF.The study showed a significant difference between gender groups (t = 3.899, P< 0.05), with a significant difference in the mean quality of life between different categories of the prominent stigma (f = 4.018, P< 0.05) and the nature of stigma. Disfigurement clearly was a stressful experience for both sexes, but substantially more distressing for women. Majority of the patients experienced poor quality of life in all dimensions, namely, physical health, psychological health, social relationships, environmental health, and other sociodemographic variables. Conclusion: Living with a disfiguring body which is visibly different is not always easy. A sudden change either due to cancer or its treatment or due to side effects leads to significant social maladjustment, elevated anxiety, depression, and poor quality of life among the cancer survivors with body disfigurement which calls for multiprofessional involvement in addressing various psychosocial issues

    No full text
In this article we establish the existence of at least three positive solutions for 3n-th order three-point boundary value problem by using five functional fixed point theorem. We also establish the existence of at least 2m − 1 positive solutions of the problem for arbitrary positive integer m
