    Updated Systematic Review and Meta-Analysis of the Performance of Risk Prediction Rules in Children and Young People with Febrile Neutropenia

    Introduction: Febrile neutropenia is a common and potentially life-threatening complication of treatment for childhood cancer, which has increasingly been subject to targeted treatment based on clinical risk stratification. Our previous meta-analysis demonstrated 16 rules had been described and 2 of them subject to validation in more than one study. We aimed to advance our knowledge of evidence on the discriminatory ability and predictive accuracy of such risk stratification clinical decision rules (CDR) for children and young people with cancer by updating our systematic review. Methods: The review was conducted in accordance with Centre for Reviews and Dissemination methods, searching multiple electronic databases, using two independent reviewers, formal critical appraisal with QUADAS and meta-analysis with random effects models where appropriate. It was registered with PROSPERO: CRD42011001685. Results: We found 9 new publications describing a further 7 new CDR, and validations of 7 rules. Six CDR have now been subject to testing across more than two data sets. Most validations demonstrated the rule to be less efficient than when initially proposed; geographical differences appeared to be one explanation for this. Conclusion: The use of clinical decision rules will require local validation before widespread use. Considerable uncertainty remains over the most effective rule to use in each population, and an ongoing individual-patient-data meta-analysis should develop and test a more reliable CDR to improve stratification and optimise therapy. Despite current challenges, we believe it will be possible to define an internationally effective CDR to harmonise the treatment of children with febrile neutropenia

    The Effects of Printed Lattice Cell Structure Superstrates on Printed Patch Antennas

    This work demonstrates a fully printed patch antenna consisting of a three‐dimensional (3D) printed Ultem 9085 substrate and a 3D printed body‐centered cubic lattice cell structure (LCS) superstrate made of Verowhite Plus. The radiating patch was fabricated by manual screen‐printing method using commercially available silver pastes. The superstrate was affixed to the top of the patch to mitigate shock‐induced damage to the patch. The antenna, which operates close to 5 GHz (an alternative frequency band to 2.4 GHz for data link applications) was designed as a test platform to quantify the effects of a printed superstrate on the resonant frequency and bandwidth. The addition of the superstrate shifted the resonant frequency by 0.1 GHz; and while this is not insignificant it still provides a promising strategy for adding vibration mitigation to radio frequency (RF) structures. Further, it was used to assess a less computationally expensive scheme for modeling of RF antennas involving cellular structures. In this scheme, the LCS superstrate is treated as a solid with dielectric properties that resemble that of a porous medium. Comparisons of measured and simulated S11 before and after adding the LCS superstrate revealed that the scheme yields results that are in good agreement with the experiment. Results from this work can provide guidance in the fabrication of low‐cost fully printed patch antennas with LCS superstrate for specific frequency application

    Enterococcus faecalis promotes innate immune suppression and polymicrobial catheter-associated urinary tract infection

    International audienceEnterococcus faecalis, a member of the human gastrointestinal microbiota, is an opportunistic pathogen associated with hospital-acquired wound, bloodstream, and urinary tract infections. E. faecalis can subvert or evade immune-mediated clearance, although the mechanisms are poorly understood. In this study, we examined E. faecalis-mediated subversion of macrophage activation. We observed that E. faecalis actively prevents NF-κB signaling in mouse RAW264.7 macrophages in the presence of Toll-like receptor agonists and during polymicrobial infection with Escherichia coliE. faecalis and E. coli co-infection in a mouse model of catheter-associated urinary tract infection (CAUTI) resulted in a suppressed macrophage transcriptional response in the bladder compared to E. coli infection alone. Finally, we demonstrated that co-inoculation of E. faecalis with a commensal strain of E. coli into catheterized bladders significantly augmented E. coli CAUTI. Taken together, these results support that E. faecalis suppression of NF-κB-driven responses in macrophages promotes polymicrobial CAUTI pathogenesis especially when co-infected with less virulent or commensal E. coli strains

    Short-term suppression of A315T mutant human TDP-43 expression improves functional deficits in a novel inducible transgenic mouse model of FTLD-TDP and ALS

    The nuclear transactive response DNA-binding protein 43 (TDP-43) undergoes relocalization to the cytoplasm with formation of cytoplasmic deposits in neurons in amyotrophic lateral sclerosis (ALS) and frontotemporal lobar degeneration (FTLD). Pathogenic mutations in the TDP-43-encoding TARDBP gene in familial ALS as well as non-mutant human TDP-43 have been utilized to model FTD/ALS in cell culture and animals, including mice. Here, we report novel A315T mutant TDP-43 transgenic mice, iTDP-43, with controlled neuronal over-expression. Constitutive expression of human TDP-43 resulted in pronounced early-onset and progressive neurodegeneration, which was associated with compromised motor performance, spatial memory and disinhibition. Muscle atrophy resulted in reduced grip strength. Cortical degeneration presented with pronounced astrocyte activation. Using differential protein extraction from iTDP-43 brains, we found cytoplasmic localization, fragmentation, phosphorylation and ubiquitination and insolubility of TDP-43. Surprisingly, suppression of human TDP-43 expression in mice with overt neurodegeneration for only 1\ua0week was sufficient to significantly improve motor and behavioral deficits, and reduce astrogliosis. Our data suggest that functional deficits in iTDP-43 mice are at least in part a direct and transient effect of the presence of TDP-43. Furthermore, it illustrates the compensatory capacity of compromised neurons once transgenic TDP-43 is removed, with implications for future treatments
