Albertet, âEn amor trob tantz de mals seignoratgesâ (BdT 16.13)
The article provides a commentary and a new critical edition of Albertetâs poem âEn amor trob tantz de mals seignoratgesâ, in which the troubadour praises some of the most courtly ladies of his time. The poem can be linked to the genre of the collective eulogy, where the enumerated ladies are praised essentially for political reasons, since the commendation is clearly addressed to the men hidden behind them: famous specimena of this genre are the âtournoiements des damesâ by Huon dâOisy and Richard de Semilli and Raimbaut de Vaqueirasâ âCarrosâ (BdT 392.32). Albertetâs song starts with a misogynic tone, since the poet declares that celebrating or loving ladies is worthless, given their immoral behaviour, and with a polemic against the strict rules of courtly love. This misogyny, as is further discovered, is a rhetorical device to praise women as well as a tool to renew the genre of the love song from a thematic point of view
Gui dâUisel, Ja non cujei qe·m desplagues amors (BdT 194.11)
This paper offers a new critical edition with commentary of Gui dâUiselâs âmala cansoâ against Love, Ja non cujei qe·m desplagues amors, a poem that is less well-known and has received less critical attention compared to the other, better known, âmala cansoâ addressed to the âmala dompnaâ by the same author, âSi be·m partetz, mala dompna, de vosâ (BdT 194.19). The paper will investigate all those aspects of the text â vocabulary, style, themes â that enable it to be classified as a song against love. As far as this is concerned, a number of features would appear to be of particular interest, such as the use of the rhyme-word âfaisâ; the equivalence of love service and feudal service that justifies abandoning Love, seen as a bad lord; the motif of ladies who have become âchamjaritzâ; the theme of the decadence of courtly values; the poetâs need to place limits on his complaint faced with the corruption of âfinâamorâ. Although the archetypes of many of the poemâs themes and motifs may be found already in the earliest troubadours, Gui dâUiselâs songs, and this one in particular, stand out because their author is well aware that he is adopting a classic genre while at the same time attempting to renew it from the point of view of content

La presente investigaciĂłn tuvo por objetivo determinar la influencia de las
estrategias arquitectĂłnicas en el incremento de ventas de un hotel en la ciudad de
Ica, 2023. Para ello, se desarrollĂł una metodologĂa de tipo bĂĄsica, no experimental,
de enfoque cuantitativo y nivel correlacional causal. La muestra estuvo conformada
por 80 usuarios de un complejo hotelero, quienes respondieron un cuestionario
relacionado a cada una de las variables. Los instrumentos fueron validados por 03
expertos, obteniendo un coeficiente de 0,896, que representa una alta confiabilidad
segĂșn el Alfa de Cronbach. AsĂ mismo, se empleĂł estadĂstica descriptiva e
inferencial, obteniendo para este Ășltimo, resultados con valores segĂșn Nagelkerke:
10,9% de incidencia de la variable estrategias arquitectĂłnicas en incremento de
ventas y Wald: 6,240 > 4,0 y p= 0,012 Ë 0,05, lo cual indica la aceptaciĂłn de la

Fasting glucose and body mass index as predictors of activity in breast cancer patients treated with everolimus-exemestane: the EverExt study
Evidence on everolimus in breast cancer has placed hyperglycemia among the most common high grade adverse events. Anthropometrics and biomarkers of glucose metabolism were investigated in a observational study of 102 postmenopausal, HRâ+âHER2- metastatic breast cancer patients treated with everolimus-exemestane in first and subsequent lines. Best overall response (BR) and clinical benefit rate (CBR) were assessed across subgroups defined upon fasting glucose (FG) and body mass index (BMI). Survival was estimated by Kaplan-Meier method and log-rank test. Survival predictors were tested in Cox models. Median follow up was 12.4 months (1.0-41.0). The overall cohort showed increasing levels of FG and decreasing BMI (pâ<â0.001). Lower FG fasting glucose at BR was more commonly associated with C/PR or SD compared with PD (pâ<â0.001). We also observed a somewhat higher BMI associated with better response (pâ=â0.052). More patients in the lowest FG category achieved clinical benefit compared to the highest (pâ<â0.001), while no relevant differences emerged for BMI. Fasting glucose at re-assessment was also predictive of PFS (pâ=â0.037), as confirmed in models including BMI and line of therapy (pâ=â0.049). Treatment discontinuation was significantly associated with changes in FG (pâ=â0.014). Further research is warranted to corroborate these findings and clarify the underlying mechanisms
Clinical Outcomes and Toxicity of CT-guided High Dose-rate Brachytherapy in Women With Locally-advanced Cervical Cancer
Background/Aim: To evaluate the outcome and toxicities in patients with locally advanced cervical cancer (LACC) treated with radiochemotherapy and intracavitary brachytherapy.
Patients and Methods: This study included 67 patients with LACC treated between 2010 and 2018. The most represented stage was FIGO IIB. The patients were treated with external beam radiotherapy (EBRT) to the pelvis and boost to the cervix and parametrials. Concomitant chemotherapy (CHT) with cisplatin (CDDP) 40 mg/mq was planned. Subsequently, the patients underwent CT-based endouterine brachytherapy (BT). The response was evaluated at 3 months with PET-CT and/or pelvic magnetic resonance imaging (MRI). Since then, the patients have been followed with clinical instrumental controls every 4 months for the first 2 years and every 6 months for the following 3 years. Local response was assessed with pelvic MRI and/or PET-CT scan at the end of intracavitary BT) according to RECIST 1.1 criteria.
Results: The median duration of treatment was 55 days (range=40-73 days). The prescription dose to the planning target volume (PTV) was delivered in 25 to 30 (median 28) daily fractions. The EBRT median dose to the pelvis and gross tumor volume were 50.4 Gy (range=45-56.25) and 61.6 Gy (range=45-70.4), respectively. The 1-year, 2-year, 3-year, and 5-year overall survival rates were 92.44%, 80.81%, 78.84%, and 76.45% respectively. The actuarial 1-year, 2-year, 3-year, and 5-year disease-free survival rates were 89.5%, 83.6%, 81%, and 78.2% respectively.
Conclusion: This study analyzed acute and chronic toxicity, survival, and local control in cervical cancer patients treated with IMRT followed by CT-planned high dose rate-brachytherapy. Patients demonstrated satisfactory outcomes and incidence of acute and late toxicities
The impact of the Hippo pathway and cell metabolism on pathological complete response in locally advanced Her2+ breast cancer: the TRISKELE multicenter prospective study
The Hippo pathway and its two key effectors, Yes-associated protein (YAP) and transcriptional coactivator with PDZ-binding motif (TAZ), are consistently altered in breast cancer. Pivotal regulators of cell metabolism such as the AMP-activated protein kinase (AMPK), Stearoyl-CoA-desaturase 1 (SCD1), and HMG-CoA reductase (HMGCR) are relevant modulators of TAZ/YAP activity. In this prospective study, we measured the tumor expression of TAZ, YAP, AMPK, SCD1, and HMGCR by immunohistochemistry in 65 Her2+ breast cancer patients who underwent trastuzumab-based neoadjuvant treatment. The aim of the study was to assess the impact of the immunohistochemical expression of the Hippo pathway transducers and cell metabolism regulators on pathological complete response. Low expression of cytoplasmic TAZ, both alone and in the context of a composite signature identified by machine learning including also low nuclear levels of YAP and HMGCR and high cytoplasmic levels of SCD1, was a predictor of residual disease in the univariate logistic regression. This finding was not confirmed in the multivariate model including estrogen receptor > 70% and body mass index > 20. However, our findings were concordant with overall survival data from the TCGA cohort. Our results, possibly affected by the relatively small sample size of this study population, deserve further investigation in adequately sized, ad hoc prospective studies
Transcatheter heart valve commissural alignment: an updated review
Transcatheter aortic valve replacement (TAVR) indications recently extended to lower surgical risk patients with longer life expectancy. Commissural alignment (CA) is one of the emerging concepts and is becoming one of the cornerstones of the TAVR procedure in a patient with increased longevity. Indeed, CA may improve transcatheter heart valve (THV) hemodynamics, future coronary access, and repeatability. The definition of CA has been recently standardized by the ALIGN-TAVR consortium using a four-tier scale based on CT analysis. Progress has been made during the index TAVR procedure to optimize CA, especially with self-expandable platforms. Indeed, specific delivery catheter orientation, THV rotation, and computed-tomography-derived views have been proposed to achieve a reasonable degree of CA. Recent data demonstrate feasibility, safety, and a significant reduction in coronary overlap using these techniques, especially with self-expandable platforms. This review provides an overview of THV CA including assessment methods, alignment techniques during the index TAVR procedure with different THV platforms, the clinical impact of commissural misalignment, and challenging situations for CA
Head and neck radiotherapy amid the COVIDâ19 pandemic: practice recommendations of the Italian Association of Radiotherapy and Clinical Oncology (AIRO)
Abstract
Management of patients with head and neck cancers (HNCs) is challenging for the Radiation Oncologist, especially in the COVID-19 era. The Italian Society of Radiotherapy and Clinical Oncology (AIRO) identified the need of practice recommendations on logistic issues, treatment delivery and healthcare personnelâs protection in a time of limited resources. A panel of 15 national experts on HNCs completed a modified Delphi process. A five-point Likert scale was used; the chosen cut-offs for strong agreement and agreement were 75% and 66%, respectively. Items were organized into two sections:
(1) general recommendations (10 items) and (2) special recommendations (45 items), detailing a set of procedures to be applied to all specific phases of the Radiation Oncology workflow. The distribution of facilities across the country was as follows: 47% Northern, 33% Central and 20% Southern regions. There was agreement or strong agreement across the majority (93%) of proposed items including treatment strategies, use of personal protection devices, set-up modifications and follow-up re-scheduling. Guaranteeing treatment delivery for HNC patients is well-recognized in Radiation Oncology. Our recommendations provide a flexible tool for management both in the pandemic and post-pandemic phase of the COVID-19 outbreak
