Performance of drought tolerant rice varieties in different altitudes at Duradada, Lamjung, Nepal
To evaluate high yielding drought tolerant rice varieties in different altitudes, a field experiment was carried out at Duradanda, Lamjung, Nepal during rainy season of 2016 under rainfed condition. The experiment was laid out in a factorial randomized block design with two replications. The treatments comprised of six different rice varieties viz. Sukhadhan-1, Sukhadhan-2, Sukhadhan-3, Sukhadhan-4, Sukhadhan-5 and Sukhadhan-6 and four altitudes viz. 1000, 1200, 1400 and 1600 masl. The results revealed that rice variety Sukhadhan-2 produced the highest grain yield (2.93 t/ha) followed by Sukhadhan-5 (2.92 t/ha) and Sukhadhan-3 (2.82 t/ha). The highest grain yield of Sukhadhan-2 was due to higher number of effective tillers/m2 (228), higher number of grains/panicle (83), higher panicle length (21.79 cm) and low sterility percentage (27.91%) as compared to other varieties. Regarding to altitudes, the highest grain yield (3.34 t/ha) was obtained in lower altitude (1000 masl) followed by 1200 masl (2.79 t/ha), 1400 masl (2.68 t/ha and 1600 masl (1.85 t/ha). The lower yield in higher altitude was due to higher sterility percentage, less number of effective tillers per unit area and less number of effective grains per panicle. It seems that the performance of these varieties was good up to 1400 masl
Antibiogram of Escherichia coli and Staphylococcus aureus Isolated from Milk Sold in Kathmandu District
The emergence of antibiotic resistance in microorganisms and the presence of such isolates in milk pose a great risk to public health. Therefore, this study aims to determine the antibiotic susceptibility pattern of Escherichia coli and Staphylococcus aureus isolated from milk and assess the microbial quality of milk. For this, a total of 70 milk samples were collected and the total bacterial count (TBC) was determined. E. coli and S. aureus were isolated using their respective selective media while antibiotic susceptibility testing was carried out by Kirby Bauer Disc Diffusion method. The TBC showed that the raw milk samples contained two-fold higher microbial load while the pasteurized milk samples contained four-fold higher microbial loads than the standard guidelines. A total of 62 isolates were identified from culture-positive milk samples of which 32 were E. coli and 30 were S. aureus. A significant correlation was observed between microbial load and the organism isolated (r = 0.339, p<0.01). All S. aureus isolates were susceptible to Chloramphenicol while 40% were resistant to Cefoxitin, indicating the presence of Methicillin resistant S. aureus (MRSA). Also, 12 multidrug resistant (MDR) S. aureus were identified. While for E. coli, all were susceptible to Chloramphenicol but resistant to Ampicillin. Also, 9 MDR E. coli were detected. Higher resistance was observed among isolates from the raw milk samples than the pasteurized milk. It can be concluded that the milk produced by small-scale farms and dairy industries of Kathmandu district are of poor quality. Hence, routine microbial quality assessment and antimicrobial resistance monitoring should be followed to safeguard public health
A case study of medicinal plants and their usage by the local community of Dilasaini Gaunpalika, Baitadi district, Nepal
An Ethnomedicinal survey study was conducted to get information about the usage of medicinal plants and the diversity of species found around the village. The main objective of the study was to document the plants for their medicinal and other uses. Ethnomedicinal plants are locally available used by tribal communities and local inhabitants for various medicinal purposes. Random sampling survey was conducted by selecting 50 households to access the medicinal plants and document their usage in Gokuleshwor, Baitadi. A total of 33 species of medicinal plants to treat 40 ailments was reported with their multipurpose use. During the survey, an equal proportion of males and females aged 14 to 78 were selected randomly. The literacy rate of the study area was 80% and 94% of the people were found to be involved in the collection of medicinal plants. The majority of the respondents (92%) used medicinal plants for minor diseases like cuts, wounds whereas few were found to use the medicinal plant for a long time to treat major diseases like cancer, tumor. The use of medicinal plants for skin infections, cuts and wounds, fever was found to be high followed by diarrhea, common cold, ulcer, asthma, jaundice, burns, piles, and eye inflammation. Most of the plants were found to be used for more than one disease. The conservation of Indigenous knowledge on the Ethnomedicinal plant should be promoted as most of the respondents were unaware of the conservation of medicinal plants
Co-Circulation of Dengue Virus Serotypes 1, 2, and 3 during the 2022 Dengue Outbreak in Nepal: A Cross-Sectional Study
The largest dengue outbreak in the history of Nepal occurred in 2022, with a significant number of casualties. It affected all 77 districts, with the nation’s capital, Kathmandu (altitude 1300 m), being the hardest hit. However, the molecular epidemiology of this outbreak, including the dengue virus (DENV) serotype(s) responsible for this epidemic, remain unknown. Here, we report the epidemic trends, clinico-laboratory features, and virus serotypes and their viral load profiles that are associated with this outbreak in Nepal. Dengue-suspected febrile patients were investigated by routine laboratory, serological, and molecular tools, including a real-time quantitative polymerase chain reaction (qRT-PCR). Of the 538 dengue-suspected patients enrolled, 401 (74.5%) were diagnosed with dengue. Among these dengue cases, 129 (32.2%) patients who required hospital admission had significant associations with myalgia, rash, diarrhea, retro-orbital pain, bleeding, and abdominal pain. DENV-1, -2, and -3 were identified during the 2022 epidemic, with a predominance of DENV-1 (57.1%) and DENV-3 (32.1%), exhibiting a new serotype addition. We found that multiple serotypes circulated in 2022, with a higher frequency of hospitalizations, more severe dengue, and more deaths than in the past. Therefore, precise mapping of dengue and other related infections through integrated disease surveillance, evaluation of the dynamics of population-level immunity and virus evolution should be the urgent plans of action for evidence-based policy-making for dengue control and prevention in the country
Obstetric Characteristics and Functional Outcomes in Women with Obstetric Anal Sphincter Injury
Background: Obstetric anal sphincter injury is a complication of vaginal delivery, if left untreated, causes significant maternal morbidities; urinary problems and fecal/ flatus incontinence. The aim of this study was to determine the incidence and describe the obstetric characteristics and functional outcomes of women who had vaginal delivery at Paropakar Maternity and Women’s Hospital Nepal and sustained Obstetric anal sphincter injury.
Methods: This retrospective descriptive study included women who had vaginal delivery, irrespective of parity, in the labor room or birthing unit of Paropakar Maternity and Women’s Hospital from April 2018 to September 2020, and sustained Grade III or IV Obstetric anal sphincter injury after 28 weeks period of gestation. Maternal characteristics, obstetric details and perineal status after vaginal delivery were noted after review of hospital records. The patients were further inquired via telephone for their current status of fecal and/or urinary incontinence.
Results: The incidence of OASI was 106 (0.33%) among 31, 786 Nepalese women with vaginal birth over a 2-year period. The mean age women with Obstetric anal sphincter injury was 24.6 ± 4.3 years and 45(52.9%) cases belonged to Janajati ethnicity. Fifty two (61.2 %) were primipara and 77 (90.6%) had spontaneous vaginal deliveries. Episiotomy was not performed on most of the patients (63, 74.1%). Problems with flatus holding, stool holding and urine holding was reported by 28.3%, 13.2% and 22.6% women respectively.
Conclusions: The incidence of Obstetric anal sphincter injury among Nepalese women with vaginal birth over a 2-year period was 0.33%, which was lower than other South Asian studies. Grade III Obstetric anal sphincter injury was the frequent most type. The injuries were more common in women with Janajati ethnicity, primipara and women who did not have episiotomy. Problems with flatus holding and urine holding were present in almost one-fourth of the women with Obstetric anal sphincter injury at follow up.

Perceived stress, stigma, and social support among Nepali health care workers during COVID-19 pandemic: A cross-sectional web-based survey.
The COVID-19 pandemic has caused immense psychological distress among Health Care Workers (HCWs). HCWs have been stigmatized by people at their workplace and community, fearing that health care workers are the sources of spreading the virus. This study aimed to assess the perceived stress, stigma, and social support of Nepali health care workers during the COVID-19 pandemic. A web-based cross-sectional survey was conducted among 380 Nepali HCWs. Perceived stress was measured using Perceived Stress Scale (PSS-10). The questionnaire related to stigma was derived from a study conducted to measure stigma among HCWs during the Middle East respiratory syndrome coronavirus (MERS-CoV) outbreak. Furthermore, perceived social support was measured by Oslo Social Support Scale (OSSS-3). Associated factors were examined using Chi-square tests followed by multivariate logistic regression analyses at the significance level of 0.05. This study illustrated that nearly half (44.2%) of the respondents perceived poor social support. Similarly, almost 3% of the HCWs experienced high perceived stress, whereas 87.6% perceived moderate stress, and 35% of the HCWs felt stigmatized. Nepali healthcare workers experienced perceived stress, social stigma, and social support in various severity levels during the COVID-19 pandemic. COVID-19 emergency is emotionally difficult and where psychological distress can jeopardize outcomes and affect work performance. Appropriate psychological interventions are needed to promote the mental well-being of the healthcare workers
Effect of Different Organic and Inorganic Fertilizers on Spring Rice Var. (Hardinath 1) Production in Rural Gorkha, Nepal
The effect of different organic and inorganic fertilizers had a significant effect on the yield of spring rice. A field experiment was carried out to determine the effects of different organic and inorganic fertilizers on the yield of spring rice var Hardinath-1 at Siranchowk rural municipality-4, Gorkha during the spring season of 2021 with four replications and five treatments: T1[Recommended inorganic fertilizer Nitrogen, Phosphorous, and Potassium (@100:30:30 kg/ha)], T2[75% recommended inorganic fertilizer + 25% recommended organic fertilizer: i.e. (Farm Yard Manure (6 ton /ha)], T3(50% recommended inorganic fertilizer + 50% recommended organic fertilizer), T4(25% recommended inorganic fertilizer + 75% recommended organic fertilizer ), and T5 (Recommended organic fertilizer) using simple Randomized Complete Block Design with 3*3 m of individual plot size. A biometrical observation like plant height (95.33 cm), and the number of tillers (340.75/ m2) were found significantly highest on T2. Similarly, yield attributing characteristics like the number of effective tillers (315.75/m2), several filled grains per panicle (150.25), and highest panicle length (25.06 cm) were also significantly highest on T2. The highest grain yield (8.27 ton/ha), straw yield (12.14 ton/ha), and harvest index (0.40) were also observed on T2. Total net return was found highest on T2: 287330 Nepalese Rupees(NPR) incurring the cost of fertilizer Rs NPR 18940 per hectare returning the gross return of NPR 268390 per hectare. In conclusion, T2 was the best combination of other biofertilizer combinations. So, the spring rice productivity can be enhanced via the adoption of 75% recommended inorganic fertilizer + 25% recommended organic fertilizer in fertilizer management practice at Chorkate, Gorkha
Health literacy and associated factors among undergraduates: A university-based cross-sectional study in Nepal.
Health literacy is one of the most critical aspects of health promotion. Limited health literacy is also accounted for adverse health outcomes and a huge financial burden on society. However, a gap exists in the level of health literacy, especially among undergraduates. This study aimed to assess the levels of health literacy and its socio-demographic determinants among undergraduate students of Tribhuvan University, Nepal. A web-based cross-sectional survey was conducted among 469 undergraduate students from five institutes of Tribhuvan University, Nepal. The 16-item short version of the European Health Literacy Survey Questionnaire (HLS-EU-Q16) was used to measure students' health literacy levels. Associated factors were examined using Chi-square tests followed by multivariate logistic regression analyses at the level of significance of 0.05. Nearly 61% of students were found to have limited health literacy (24.5% had "inadequate" and 36.3% had "problematic" health literacy). Female students (aOR = 1.6, 95% CI: 1.1-2.5), students from non-health related majors (aOR = 1.9, 95% CI: 1.2-3.0), students with unsatisfactory health status (aOR = 2.8, 95% CI: 1.7-4.5), students with poor financial status (aOR = 2.9, 95% CI: 1.2-6.8) and students with low self-esteem (aOR = 2.5, 95% CI: 1.5-4.1) were significantly more likely to have limited health literacy. The majority of the undergraduates were found to have limited health literacy. Gender, sector of study, self-rated health status, self-rated financial status, and self-esteem were significantly associated with limited health literacy. This study indicates university students should not be assumed to be health-literate and interventions to improve students' health literacy especially for those whose majors are not health-related should be implemented. Further studies using a longer version of the health literacy survey questionnaire and qualitative methods to explore more on determinants of health literacy are recommended
