Tuberculose multifocale chez un immunocompétent

Patient âgé de 34 ans sans antécédents notables qui consulte pour des lésions gommeuses cicatricielles au niveau du visage (A, B) remontant à 2 ans avec une gonalgie droite inflammatoire (C) concomitante et qui présente depuis 9 mois une tuméfaction scrotale gauche douloureuse et fistulisante (D), le tout évoluant dans un contexte de sueurs nocturnes et d'amaigrissement. La biopsie cutanée a objectivé un granulome épithélio-gigantocellulaire sans nécrose caséeuse. La radiographie standard du genou a montré un pincement fémoro-tibial diffus avec ostéolyse du tibia. L'échographie testiculaire a objectivé une hydrocèle de grande abondance cloisonnée et l'échographie articulaire a objectivé une synovite hypertrophique érodant l'os avec une biopsie synoviale en faveur de tuberculose (TB). Le reste du bilan a objectivé une intradermo-réaction (IDR) positive avec une sérologie VIH négative. Le diagnostic de TB multifocale associant une atteinte cutanée, ostéo-articulaire et génitale chez un immunocompétant a été retenue et le malade a été mis sous antibacillaires sous le schéma 2SRHZ/7RH avec bonne évolution clinique. Les formes multifocales de la tuberculose représentent 9 à 10 % des cas et sont l'apanage des patients immunodéprimés principalement porteurs du VIH. Leur pronostic est mauvais avec un taux de mortalité de 16 à 25 %. Elles posent en plus un véritable problème de diagnostique, vu son polymorphisme clinique et la diversité des organes pouvant être atteints, ce qui est à l'origine du retard du diagnostic.Pan African Medical Journal 2015; 2

A rare case of sporadic multiple trichoepitheliomas

A 60-year-old male reported with multiple growths facial tumors on the face and scalp for 44 years around her puberty. On examination multiple firm translucent papules and nodules on the face (A) especially in the nasolabial fold, the medial part of the eyebrows and preauricular area (B) and in the scalp (C) with varying sizes from 5 mm to 2 cm. Dermoscopy was objectived telangectasic vessel at the periphery of lesions (D). No other contributory medical history or similar cases in the family were reported by the patient. With the above features a clinical differential diagnosis of multiple basal cell carcinoma (BCC), multiples trichoepitheliomas or Bourneville's tuberous sclerosis was considered. Histopathological examination of the lesions was in favor of nodular BCC then another skin biopsy objectified Horn cysts characteristics of trichoepithelioma. The diagnosis of multiple Trichoepitheliomas was taken and laser ablation was planned. Trichoepithelioma is a rare benign skin lesion that originates from hair follicles. It usually appears as multiple lesions in the autosomal dominant type or as an individual flesh colored papule or nodule measuring 2 to 8 mm in the sporadic type. Histologically, it is similar to BCC, and although benign, with a rare risk of malignant transformation. Solitary lesions can be excised. In the case of multiple tumors, surgical approach may not be feasible. Other treatments have been proposed in the literature (dermabrasion, electro-surgery and laser surgery…), but the results of these procedures vary.Pan African Medical Journal 2015; 2

Hamartome lipomateux superficiel de Hoffmann-Zurhelle

L'hamartome lipomateux superficiel de Hoffmann-Zurhelle est une tumeur bénigne souvent congénitale. Histologiquement, il est caractérisé par la présence hétérotopique de cellules adipeuses quelquefois lipoblastiques autour des trajets vasculaires dermiques. Nous rapportons une nouvelle observation de forme multiple à révélation tardive chez une femme âgée de 31 ans sans antécédents pathologiques notables qui a été adressée à la consultation pour des papules et tumeurs asymptomatiques de couleur chaire se regroupent en placards à disposition linéaire et zostèriforme au niveau de la face externe de la cuisse droite depuis l'âge de 13 ans, augmentant progressivement de taille. L'étude histologique d'un fragment biopsique avait montré un épiderme régulier, plicaturé et kératinisant, soulevé par un tissu fibro-adipeux abondant incluant quelques vaisseaux sanguins aux dépens du derme moyen. Ces données cliniques et histologiques ont permis de retenir le diagnostic d'hamartome lipomateux superficiel. Une exérèse chirurgicale des tumeurs de grande taille a été proposée complété par le laser CO2 pour le reste de lésions cutanées. L'hamartome lipomateux superficiel est une lésion bénigne sans potentiel de malignité. L'exérèse chirurgicale peut être proposée si la lésion est gênante ou dans un but essentiellement esthétique.Keywords: Hamartome lipomateux, Hoffmann-Zurhelle, tumeur bénign

Diffuse intramuscular lipoma responsible for monomelic hypertrophy

Background: Lipomas are common benign soft tissue, however intramuscular lipoma, is an often misdiagnosed clinical entity. The aim of this case is to present an atypical infiltrating lipoma and discuss its diagnostic and therapeutic features. Clinical features: A 24 year-old patient presented to our consultation with a swelling of the right lower limb from childhood and gradually increasing in size. Clinical examination revealed a firm and extended swelling along the leg, thigh and foot giving an aspect of monomelic hypertrophy. There were no signs of vascular or nerve compression. Intervention and outcome: Pathological study showed a lipoma. Magnetic resonance imaging (MRI) of the right lower limb objectified a large infiltrating lipoma affecting muscles of the posterior compartment of the thigh and the right leg with no signs of malignancy. The diagnosis of intramuscular lipoma was done. The surgery was proposed but the patient refused. Conclusion: This case report described an adult female patient with an unusually intramuscular lipoma where magnetic resonance imaging was playing an important role in the diagnostic. Indeed the intramuscular is a benign fatty tumor with characteristic appearance on MRI. Its management should be considered in multidisciplinary collaboration
