Improving Prediction of Real-Time Loneliness and Companionship Type Using Geosocial Features of Personal Smartphone Data
Loneliness is a widely affecting mental health symptom and can be mediated by and co-vary with patterns of social exposure. Using momentary survey and smartphone sensing data collected from 129 Android-using college student participants over three weeks, we (1) investigate and uncover the relations between momentary loneliness experience and companionship type and (2) propose and validate novel geosocial features of smartphone-based Bluetooth and GPS data for predicting loneliness and companionship type in real time. We base our features on intuitions characterizing the quantity and spatiotemporal predictability of an individual's Bluetooth encounters and GPS location clusters to capture personal significance of social exposure scenarios conditional on their temporal distribution and geographic patterns. We examine our features' statistical correlation with momentary loneliness through regression analyses and evaluate their predictive power using a sliding window prediction procedure. Our features achieved significant performance improvement compared to baseline for predicting both momentary loneliness and companionship type, with the effect stronger for the loneliness prediction task. As such we recommend incorporation and further evaluation of our geosocial features proposed in this study in future mental health sensing and context-aware computing applications. This work was supported by Whole Communities—Whole Health, a research

Multi-Modal Data Collection for Measuring Health, Behavior, and Living Environment of Large-Scale Participant Cohorts: Conceptual Framework and Findings from Deployments
As mobile technologies become ever more sensor-rich, portable, and ubiquitous, data captured by smart devices are lending rich insights into users' daily lives with unprecedented comprehensiveness, unobtrusiveness, and ecological validity. A number of human-subject studies have been conducted in the past decade to examine the use of mobile sensing to uncover individual behavioral patterns and health outcomes. While understanding health and behavior is the focus for most of these studies, we find that minimal attention has been placed on measuring personal environments, especially together with other human-centric data modalities. Moreover, the participant cohort size in most existing studies falls well below a few hundred, leaving questions open about the reliability of findings on the relations between mobile sensing signals and human outcomes. To address these limitations, we developed a home environment sensor kit for continuous indoor air quality tracking and deployed it in conjunction with established mobile sensing and experience sampling techniques in a cohort study of up to 1584 student participants per data type for 3 weeks at a major research university in the United States. In this paper, we begin by proposing a conceptual framework that systematically organizes human-centric data modalities by their temporal coverage and spatial freedom. Then we report our study design and procedure, technologies and methods deployed, descriptive statistics of the collected data, and results from our extensive exploratory analyses. Our novel data, conceptual development, and analytical findings provide important guidance for data collection and hypothesis generation in future human-centric sensing studies. This work was supported by Whole Communities—Whole Health, a research

Effects of chronic stress and high-fat diet on metabolic and nutritional parameters in Wistar rats Efeitos do estresse crônico e da dieta rica em gordura sobre os parâmetros metabólicos e nutricionais em ratos Wistar
ABSTRACT Objective: The aim of this study was assess the role of chronic stress on the metabolic and nutritional profile of rats exposed to a high-fat diet. Materials and methods: Thirty-day-old male Wistar rats (70-100 g) were distributed into four groups: normal-diet (NC), chronic stress (St), high-fat diet (HD), and chronic stress/high-fat diet (HD/St). Stress consisted at immobilization during 15 weeks, 5 times per week, 1h per day; and exposure to the high-fat diet lasted 15 weeks. Nutritional and metabolic parameters were assessed. The level of significance was 5%. Results: The HD group had final body weight, total fat, as well as insulin and leptin increased, and they were insulin resistant. The St and HD/St had arterial hypertension and increased levels of corticosterone. Stress blocked the effects of the high-fat diet. Conclusion: Chronic stress prevented the appearance of obesity. Our results help to clarify the mechanisms involved in metabolic and nutritional dysfunction, and contribute to clinical cases linked to stress and high-fat diet.
Analysis of the treatment protocol for sternotomy wound infection after cardiac surgery
INTRODUCTION: Surgical wound complications after sternotomy in cardiovascular procedures include small dehiscences, mediastinitis, and sternal osteomyelitis. Mediastinitis and sternal osteomyelitis are high-risk complications associated with high rates of morbidity, mortality, and recurrence. Treatment of the most severe cases involves prolonged hospitalization. Moreover, the long-term use of antibiotics during hospitalization and after discharge significantly increases the overall cost of treatment. A recent treatment option involves extensive surgical debridement of the surgical wound, treatment of the wound bed with negative pressure therapy, and closure of the wound with myocutaneous or fasciocutaneous flaps. The advantages of this strategy include improvement of the patient's quality of life, less manipulation and less discomfort for the patient, less burden on staff involved in care, lower rate of infection recurrence, and an overall reduction of treatment morbidity.
METHODS: The objective of this study was to conduct a retrospective survey of patients with surgical wound complications who were treated according to a protocol developed and improved by the Heart Institute of the Clinic Hospital of the School of Medicine of the University of São Paulo (Instituto do Coração do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo-InCor/HC-FM-USP), and who underwent surgery by the same plastic surgeon. The epidemiological profiles and possible indicators of worse prognosis were determined from routine examinations performed on these patients. The medical records of patients treated at InCor/HC-FM-USP who presented with sternotomy infection in 2014 were assessed retrospectively. The analyzed variables included comorbidities, interval between surgical procedures, C-reactive protein (CRP) levels, wound closure procedures, and treatment complications, among others.
RESULTS: The data are predominantly descriptive and epidemiological. At least one comorbidity was present in 84% of cases. The mean number of surgical procedures per patient was 2.5, ranging from 1 to 7. The CRP levels decreased in 75% of patients after the first surgical procedure, and the mortality rate was 17%.
CONCLUSION: The standardized method adopted significantly decreased the overall mortality and the rates of recurrence and readmission. Indicators of worse prognosis, including CRP levels and the leukogram, were identified at the time of diagnosis and initiation of the treatment protocol
